2Steroid hyperglycemia: Prevalence, early detection and therapeutic recommendations: A narrative review显示文摘Steroids are drugs that have been used extensively in a variety of conditions. Although widely prescribed for their anti-inflammatory and immunosuppressive properties, glucocorticoids have several side effects, being hyperglycemia one of the most common and representative. In the present review, we discuss the main epidemiologic characteristics associated with steroid use, with emphasis on the identification of high risk populations. Additionally we present the pathophysiology of corticosteroid induced hyperglycemia as well as the pharmacokinetics and pharmacodynamics associated with steroid use. We propose a treatment strategy based on previous reports and the understanding of the mechanism of action of both, the different types of glucocorticoids and the treatment options, in both the ambulatory and the hospital setting. 3Novel genetic markers in inflammatory bowel disease显示文摘Genetic factors play a significant role in determining inflammatory bowel disease(IBD)susceptibility.Epidemiologic data support genetic contribution to the pathogenesis of IBD,which include familial aggregation,twin studies,racial and ethnic differences in disease prevalence.Linkage studies have identified several susceptibility genes contained in different genomic regions named IBD1 to IBD9.Nucleotide oligomerization domain(NOD2)and human leukocyte antigen(HLA)genes are the most extensively studied genetic regions(IBD1 and IBD3 respectively)in IBD.Mutations of the NOD2 gene are associated with Crohn's disease(CD)and several HLA genes are associated with ulcerative colitis(UC)and CD.Toll like receptors(TLRs)have an important role in the innate immune response against infections by mediating recognition of pathogen-associated microbial patterns.Studying single-nucleotide polymorphisms(SNPs)in molecules involved in bacterial recognition seems to be essential to define genetic backgrounds at risk of IBD.Recently,numerous new genes have been identified to be involved in the genetic susceptibility to IBD:NOD1/Caspase-activation recruitment domains 4(CARD4),Chemokine ligand 20(CCL20),IL-11,and IL-18 among others.The characterization of these novel genes potentially will lead to the identification of therapeutic agents and clinical assessment of phenotype and prognosis in patients with IBD.Lorena Rodríguez-Bores Gabriela C Fonseca Marco A Villeda Jesús K Yamamoto-Furusho 2007World Journal of Gastroenterology2007,13,42:8
4Speckle tracking echocardiography to assess regional ventricular function in patients with apical hypertrophic cardiomyopathy显示文摘AIM To explore regional systolic strain of midwall and endocardial segments using speckle tracking echocardiography in patients with apical hypertrophic cardiomyopathy(HCM).METHODS We prospectively assessed 20 patients(mean age 53 ± 16 years,range:18-81 years,10 were male),with apical HCM. We measured global longitudinal peak systolic strain(GLPSS) in the midwall and endocardium of the left ventricle. RESULTS The diastolic thickness of the 4 apical segments was 16.25 ± 2.75 mm. All patients had a normal global systolicfunction with a fractional shortening of 50% ± 8%. In spite of supernormal left ventricular(LV) systolic function,midwall GLPSS was decreased in all patients,more in the apical(-7.3% ±-8.8%) than in basal segments(-15.5% ±-6.93%),while endocardial GLPPS was significantly greater and reached normal values(apical:-22.8% ±-7.8%,basal:-17.9% ±-7.5%). CONCLUSION This study shows that two-dimensional strain was decreased mainly confined to the mesocardium,while endocardium myocardial deformation was preserved in HCM and allowed to identify subclinical LV dysfunction. This transmural heterogeneity in systolic strain had not been previously described in HCM and could be explained by the distribution of myofibrillar disarray in deep myocardial areas. 5Bone marrow-derived monocyte infusion improves hepatic fibrosis by decreasing osteopontin,TGF-β1,IL-13 and oxidative stress显示文摘AIM To evaluate the therapeutic effects of bone marrowderived CD11b+CD14+ monocytes in a murine model of chronic liver damage. METHODS Chronic liver damage was induced in C57BL/6 mice by administration of carbon tetrachloride and ethanol for 6 mo. Bone marrow-derived monocytes isolated by immunomagnetic separation were used for therapy. The cell transplantation effects were evaluated by morphometry,biochemical assessment,immunohistochemistry and enzyme-linked immunosorbent assay. RESULTS CD11b+CD14+ monocyte therapy significantly reduced liver fibrosis and increased hepatic glutathione levels. Levels of pro-inflammatory cytokines,including tumor necrosis factor-α,interleukin(IL)-6 and IL-1β,in addition to pro-fibrotic factors,such as IL-13,transforming growth factor-β1 and tissue inhibitor of metalloproteinase-1 also decreased,while IL-10 and matrix metalloproteinase-9 increased in the monocytetreated group. CD11b+CD14+ monocyte transplantation caused significant changes in the hepatic expression of α-smooth muscle actin and osteopontin. 7Glutamine transporters as pharmacological targets: From function to drug design显示文摘Among the different targets of administered drugs,there are membrane transporters that play also a role in drug delivery and disposition.Moreover,drug-transporter interactions are responsible for off-target effects of drugs underlying their toxicity.The improvement of the drug design process is subjected to the identification of those membrane transporters mostly relevant for drug absorption,delivery and side effect production.A peculiar group of proteins with great relevance to pharmacology is constituted by the membrane transporters responsible for managing glutamine traffic in different body districts.The interest around glutamine metabolism lies in its physio-pathological role;glutamine is considered a conditionally essential amino acid because highly proliferative cells have an increased request of glutamine that cannot be satisfied only by endogenous synthesis.Then,glutamine transporters provide cells with this special nutrient.Among the glutamine transporters,SLC1A5,SLC6A14,SLC6A19,SLC7A5,SLC7A8 and some members of SLC38 family are the best characterized,so far,in both physiological and pathological conditions.Few 3D structures have been solved by CryoEM;other structural data on these transporters have been obtained by computational analysis.Interactions with drugs have been described for several transporters of this group.For some of them,the studies are at an advanced stage,for others,the studies are still in nuce and novel biochemical findings open intriguing perspectives.Mariafrancesca Scalise Lorena Pochini Michele Galluccio Lara Console Cesare Indiveri 2020Asian Journal of Pharmaceutical Sciences2020,15,2:5

9Dynamic and reversibility of heterochromatic gene silencing in human dis-ease显示文摘In eukaryotic organisms cellular fate and tissue specific gene expression are regulated by the activity of proteins known as transcription factors that by interacting with specific DNA sequences direct the activation or repression of target genes. The post genomic era has shown that transcription factors are not the unique key regulators of gene expression. Epigenetic mechanisms such as DNA methylation, post-translational modifications of histone proteins, remodeling of nucleosomes and expression of small regulatory RNAs also contribute to regulation of gene expression, determination of cell and tissue specificity and assurance of inheritance of gene expression levels. 10Diffuse intestinal ganglioneuromatosis an uncommon manifestation of Cowden syndrome显示文摘Diffuse intestinal ganglioneuromatosis is a hamartomatous polyposis characterized by a disseminated, intramural or transmural proliferation of neural elements involving the enteric plexuses. It has been associated with MEN Ⅱ, neurofibromatosis type 1 and hamartomatous polyposis associated with phosphatase and tensin homolog mutation. We report the case of a female patient with a history of a breast and endometrial tumor who presented in a colonoscopy performed for rectal bleeding diffuse ganglioneuromatosis, which oriented the search for other characteristic findings of Cowden syndrome given the personal history of the patient. The presence of an esophagogastric polyposis was also noted. Cowden syndrome is characterized by skin lesions, but it is rarely diagnosed by these lesions, because they are usually overlooked. Intestinal polyposis is not a major diagnostic criterion but it is very useful for early diagnosis. 11Effect of psychological stress on gastric motility assessed by electrical bio-impedance显示文摘AIM:To evaluate gastric motility using electrical bioimpedance(EBI) and gastric changes as a result of stress induced by psychological tests.METHODS:A group of 57 healthy women,aged 40-60 years,was recruited,and a clinical history and physical examination were performed.The women were free from severe anxiety,chronic or acute stress,severe depression,mental diseases and conditions that affect gastric activity.The women were evaluated under fasting conditions,and using a four-electrode configuration,the gastric signals were obtained through a BIOPAC MP-150 system.The volunteers were evaluated using the following paradigm:basal state,recording during the Stroop Test,intermediate resting period,recording during the Raven Test,and a final resting period.We analyzed the relative areas of the frequency spectrum:A1(1-2 cpm),A2(2-4 cpm),A3(4-8 cpm),and A4(8-12 cpm),as well as the median of area A2 + A3.The data were analyzed by an autoregressive method using a Butterworth filter with MatLab and Origin.Analysis of variance(ANOVA) and Friedman ANOVA(for nonparametric variables) were performed;in addition,pairs of groups were compared using the T dependent and Wilcoxon T tests.RESULTS:The results of the main values of area A2 were not significantly different comparing the five steps of the experimental paradigm.Nevertheless,there was a tendency of this A2 region to decrease during the stress tests,with recuperation at the final resting step.When an extended gastric region was considered(1-4 cpm),significant differences with the psychological stress tests were present(F = 3.85,P = 0.005).The A3 region also showed significant changes when the stress psychological tests were administered(F = 7.25,P < 0.001).These differences were influenced by the changes in the adjacent gastric region of A2.The parameter that we proposed in previous studies for the evaluation of gastric motility by electrical bio-impedance(EBI) was the median of the area under the region from 2 to 8 cpm(A2 + A3).The mean values of these frequencies(median of the A2 + A3 area) with the stress test showed significant changes(F = 5.5,P < 0.001).The results of the WilcoxonT test for the A4 area parameter,which is influenced by the breathing response,changed significantly during the Raven stress test(P < 0.05).CONCLUSION:We confirm that the gastric response to acute psychological stress can be evaluated by short-term EBI.María Raquel Huerta-Franco Miguel Vargas-Luna Juana Berenice Montes-Frausto Ismael Morales-Mata Lorena Ramirez-Padilla 2012World Journal of Gastroenterology2012,18,36:5

13Tenascin C upregulates interleukin-6 expression in human cardiac myofibroblasts via toll-like receptor 4显示文摘AIM:To investigate the effect of Tenascin C(TNC)on the expression of pro-inflammatory cytokines and matrixmetalloproteinases in human cardiac myofibroblasts(CMF).METHODS:CMF were isolated and cultured from patients undergoing coronary artery bypass grafting.Cultured cells were treated with either TNC(0.1μmol/L,24 h)or a recombinant protein corresponding to different domains of the TNC protein;fibrinogen-like globe(FBG)and fibronectin typeⅢ-like repeats(TNⅢ5-7)(both 1μmol/L,24 h).The expression of the proinflammatory cytokines;interleukin(IL)-6,IL-1β,TNFαand the matrix metalloproteinases;MMPs(MMP1,2,3,9,10,MT1-MMP)was assessed using real time RT-PCR and western blot analysis.RESULTS:TNC increased both IL-6 and MMP3(P<0.01)mR NA levels in cultured human CMF but had no significant effect on the other markers studied.The increase in IL-6 mR NA expression was mirrored by an increase in protein secretion as assessed by enzymelinked immunosorbant assay(P<0.01).Treating CMF with the recombinant protein FBG increased IL-6mR NA and protein(P<0.01)whereas the recombinant protein TNⅢ5-7 had no effect.Neither FBG nor TNⅢ5-7 had any significant effect on MMP3 expression.The expression of toll-like receptor 4(TLR4)in human CMF was confirmed by real time RT-PCR,western blot and immunohistochemistry.Pre-incubation of cells with TLR4neutralising antisera attenuated the effect of both TNC and FBG on IL-6 mR NA and protein expression.CONCLUSION:TNC up-regulates IL-6 expression in human CMF,an effect mediated through the FBG domain of TNC and via the TLR4 receptor.Azhar Maqbool Emma J Spary Iain W Manfield Michaela Ruhmann Lorena Zuliani-Alvarez Filomena O Gamboa-Esteves Karen E Porter Mark J Drinkhill Kim S Midwood Neil A Turner 2016World Journal of Cardiology2016,8,5:4
14Immunohistochemical expression of intrarenal renin angiotensin system components in response to tempol in rats fed a high salt diet显示文摘AIM To determine the effect of tempol in normal rats fed high salt on arterial pressure and the balance between antagonist components of the renal renin-angiotensin system.METHODS Sprague-Dawley rats were fed with 8% Na Cl highsalt(HS) or 0.4% Na Cl(normal-salt,NS) diet for 3 wk,with or without tempol(T)(1 mmol/L,administered in drinking water). Mean arterial pressure(MAP),glomerularfiltration rate(GFR),and urinary sodium excretion(UVNa) were measured. We evaluated angiotensin Ⅱ(Ang Ⅱ),angiotensin 1-7(Ang 1-7),angiotensin converting enzyme 2(ACE2),mas receptor(Mas R),angiotensin type 1 receptor(AT1R) and angiotensin type 2 receptor(AT2R) in renal tissues by immunohistochemistry.RESULTS The intake of high sodium produced a slight but significant increase in MAP and differentially regulated components of the renal renin-angiotensin system(RAS). This included an increase in Ang Ⅱ and AT1 R,and decrease in ACE-2 staining intensity using immunohistochemistry. Antioxidant supplementation with tempol increased natriuresis and GFR,prevented changes in blood pressure and reversed the imbalance of renal RAS components. This includes a decrease in Ang Ⅱ and AT1 R,as increase in AT2,ACE2,Ang(1-7) and Mas R staining intensity using immunohistochemistry. In addition,the natriuretic effects of tempol were observed in NS-T group,which showed an increased staining intensity of AT2,ACE2,Ang(1-7) and Mas R.CONCLUSION These findings suggest that a high salt diet leads to changes in the homeostasis and balance between opposing components of the renal RAS in hypertension to favour an increase in Ang Ⅱ. 15Hepatic flow is an intraoperative predictor of early allograft dysfunction in whole-graft deceased donor liver transplantation: An observational cohort study显示文摘BACKGROUND Early allograft dysfunction(EAD)after liver transplantation(LT)is an important cause of morbidity and mortality.To ensure adequate graft function,a critical hepatocellular mass is required in addition to an appropriate blood supply.We hypothesized that intraoperative measurement of portal venous and hepatic arterial flow may serve as a predictor in the diagnosis of EAD.AIM To study whether hepatic flow is an independent predictor of EAD following LT.METHODS This is an observational cohort study in a single institution.Hepatic arterial blood flow and portal venous blood flow were measured intraoperatively by transit flow.EAD was defined using the Olthoff criteria.Univariate and multivariate analyses were used to determine the intraoperative predictors of EAD.Survival analysis and prognostic factor analysis were performed using the Kaplan-Meier and Cox regression models.RESULTS A total of 195 liver transplant procedures were performed between January 2008 and December 2014 in 188 patients.A total of 54(27.7%)patients developed EAD.The median follow-up was 39 mo.Portal venous flow,hepatic arterial flow(HAF)and total hepatic arterial flow were associated with EAD in both the univariate and multivariate analyses.HAF is an independent prognostic factor for 30-d patient mortality.CONCLUSION Intraoperative measurement of blood flow after reperfusion appears to be a predictor of EAD;Moreover,HAF should be considered a predictor of 30-d patient mortality.Pablo Lozano Lominchar Maitane Igone Orue-Echebarria Lorena Martín Cristina Julia Lisbona María Magdalena Salcedo Luis Olmedilla Hemant Sharma Jose Manuel Asencio JoséAngel López-Baena 2019World Journal of Hepatology2019,11,9:4

18Aminoguanidine impedes human pancreatic tumor growth and metastasis development in nude mice显示文摘AIM:To study the action of aminoguanidine on pancreatic cancer xenografts in relation to cell proliferation,apoptosis,redox status and vascularization.METHODS:Xenografts of PANC-1 cells were developed in nude mice. The animals were separated into two groups:control and aminoguanidine treated. Tumor growth,survival and appearance of metastases were determined in vivo in both groups. Tumors were excised and ex vivo histochemical studies were performed. Cell growth was assessed by Ki-67 expression. Apoptosis was studied by intratumoral expression of B cell lymphoma-2 protein (Bcl-2) family proteins and Terminal deoxynucleotidyl transferase biotin-dUTP Nick End Labeling (Tunel). Redox status was evaluated by the expression of endothelial nitric oxide synthase (eNOS),catalase,copper-zinc superoxide dismutase (CuZnSOD),manganese superoxide dismutase (MnSOD) and glutathione peroxidase (GPx). Finally,vascularization was determined by Massons trichromic staining,and by VEGF and CD34 expression.RESULTS:Tumor volumes after 32 d of treatment by aminoguanidine (AG) were significantly lower than in control mice (P < 0.01). Median survival of AG mice was significantly greater than control animals (P < 0.01). The appearance of both homolateral and contralateral palpable metastases was significantly delayed in AG group. 19Impacts of land use and land cover changes on hydrological processes and sediment yield determined using the SWAT model显示文摘Land use and land cover(LULC)changes strongly affect local hydrology and sediment yields.The current study focused on a basin in the Brazilian Amazon and had the following three objectives:(1)to perform an effective diagnosis of flow and sediment yield,(2)to evaluate the impacts of LULC changes over the last 40 years on the hydro-sedimentological variables,and(3)to investigate the impacts of the possible trends or breaking points in the flow,surface runoff,and sediment yield series.The Soil and Water Assessment Tool(SWAT)model validation showed that the simulated results were consistent with the data measured in the dynamic reproduction of flow seasonality.Furthermore,changes in LULC altered surface runoff,sediment yield,and flow according to the Mann-Kendall and Pettitt non-parametric tests.It was also observed that the sub-basins in which pastureland is predominant are more susceptible to increased surface runoff and sediment yield.According to trend and homogeneity tests,these sub-basins had break points in the time series of these variables.On the other hand,in the sub-basins whose land cover is predominantly forest,the time series is homogeneous and trendless.The current study highlighted the impacts that 40 years of LULC changes in an Amazonian basin had on hydro-sedimentological variables,which,in turn,can play an important role in changing the hydrological cycle,consequently these changes can also impact diverse human activities in the region,such as agribusiness,livestock,energy production,food security,and public water supply.Thus,it is concluded that the SWAT model is a good estimator of hydro-sedimentological processes in Amazonian basins and can be used by decision makers in the management of water and environmental resources.Edivaldo Afonso de Oliveira Serrao Madson Tavares Silva Thomas Rocha Ferreira Lorena Conceiçao Paiva de Ataide Cleber Assis dos Santos Aline Maria Meiguins de Lima Vicente de Paulo Rodrigues da Silva Francisco de Assis Salviano de Sousa Denis Jose Cardoso Gomes 2022International Journal of Sediment Research2022,37,1:3
20Yersinia type Ⅲ effectors perturb host innate immune responses显示文摘The innate immune system is the first line of defense against invading pathogens. Innate immune cells recognize molecular patterns from the pathogen and mount a response to resolve the infection. The production of proinflammatory cytokines and reactive oxygen species, phagocytosis, and induced programmed cell death are processes initiated by innate immune cells in order to combat invading pathogens. However, pathogens have evolved various virulence mechanisms to subvert these responses. One strategy utilized by Gram-negative bacterial pathogens is the deployment of a complex machine termed the type Ⅲ secretion system(T3SS). The T3SS is composed of a syringe-like needle structure and the effector proteins that are injected directly into a target host cell to disrupt a cellular response. The three human pathogenic Yersinia spp.(Y. pestis, Y. enterocolitica, and Y. pseudotuberculosis) are Gramnegative bacteria that share in common a 70 kb virulence plasmid which encodes the T3 SS. Translocation of the Yersinia effector proteins(YopE, YopH, YopT, YopM, YpkA/YopO, and YopP/J) into the target host cell results in disruption of the actin cytoskeleton to inhibit phagocytosis, downregulation of proinflammatory cytokine/chemokine production, and induction of cellular apoptosis of the target cell. Over the past 25 years, studies on the Yersinia effector proteins have unveiled tremendous knowledge of how the effectors enhance Yersinia virulence. Recently, the long awaited crystal structure of YpkA has been solved providing further insights into the activation of the YpkA kinase domain. Multisite autophosphorylation by YpkA to activate its kinase domain was also shown and postulated to serve as a mechanism to bypass regulation by host phosphatases. In addition, novel Yersinia effector protein targets, such as caspase-1, and signaling pathways including activation of the inflammasome were identified.
