| 3 | Expression of vascular endothelial growth factor and its role in oncogenesis of human gastric carcinoma显示文摘AIM To establish the role of vascular endothelial growth factor (VEGF) in the oncogenesisof human gastric carcinoma more directly.METHODS The expression of VEGF and its receptor kinase-domain insert containing receptor (KDR) in human gastric cancer tissue were observed by immunohistochemical staining. VEGF levels were manipulated in human gastric cancer cell using eukaryotic expression constructs designed to express the complete VEGF165 complimentary DNA in either the sense or antisense orientation. The biological changes of the cells were observed in which VEGF was up-regulated or downregulated.RESULTS VEGF-positive rate was 50%, and VEGF was mainly localized in the cytoplasm and membrane of the tumor cells, while KDR was mainly located in the membrane of vascular endothelial cells in gastric cancer tissues and peri-cancerous tissue. In 2 cases of 50 specimens, the gastric cancer cells expressed KDR,localized in both the cytoplasm and membrane.Introduction of VEGF165 antisense into human gastric cancer cells ( SGC-7901, immunofluorescence intensity,31.6%)) resulted in a significant reduction in VEGFspecific messenger RNA and total and cell surface VEGF protein ( immunofluorescence intensity, 8.9%)(P<0.05). Conversely, stable integration of VEGF165 in the sense orientation resulted in an increase in cellular and cell surface VEGF (immunofluorescence intensity,75.4%) (P<0.05). | 5 | Acupuncture's Role in Solving the Opioid Epidemic: Evidence, Cost-Effectiveness, and Care Availability for Acupuncture as a Primary, Non-Pharmacologic Method for Pain Relief and Management-White Paper 2017显示文摘The United States(U.S.) is facing a national opioid epidemic, and medical systems are in need of nonpharmacologic strategies that can be employed to decrease the public's opioid dependence. Acupuncture has emerged as a powerful, evidence-based, safe, cost-effective, and available treatment modality suitable to meeting this need. Acupuncture has been shown to be effective for the management of numerous types of pain conditions, and mechanisms of action for acupuncture have been described and are understandable from biomedical, physiologic perspectives. Further, acupuncture's cost-effectiveness can dramatically decrease health care expenditures, both from the standpoint of treating acute pain and through avoiding addiction to opioids that requires costly care, destroys quality of life, and can lead to fatal overdose. Numerous federal regulatory agencies have advised or mandated that healthcare systems and providers offer non-pharmacologic treatment options for pain. Acupuncture stands out as the most evidence-based, immediately available choice to fulfil these calls. Acupuncture can safely, easily, and cost-effectively be incorporated into hospital settings as diverse as the emergency department, labor and delivery suites, andneonatal intensive care units to treat a variety of commonly seen pain conditions. | 6 | TIPS improves liver transplantation-free survival in cirrhotic patients with refractory ascites:An updated meta-analysis显示文摘AIM:To compare the liver transplantation-free(LTF)survival rates between patients who underwent transjugular intrahepatic portosystemic shunts(TIPS)and those who underwent paracentesis by an updated meta-analysis that pools the effects of both number of deaths and time to death.METHODS:MEDLINE,EMBASE,and the Cochrane Library were searched from the inception to October2012.LTF survival,liver transplantation,liver diseaserelated death,non-liver disease-related death,recurrent ascites,hepatic encephalopathy(HE)and severe HE,and hepatorenal syndrome were assessed as outcomes.LTF survival was estimated using a HR with a95%CI.Other outcomes were estimated using OR with95%CIs.Sensitivity analyses were performed to assess the effects of potential outliers in the studies according to the risk of bias and the study characteristics.RESULTS:Six randomized controlled trials with 390patients were included.In comparison to paracentesis,TIPS significantly improved LTF survival(HR=0.61,95%CI:0.46-0.82,P<0.001).TIPS also significantly decreased liver disease-related death(OR=0.62,95%CI:0.39-0.98,P=0.04),recurrent ascites(OR=0.15,95%CI:0.09-0.24,P<0.001)and hepatorenal syndrome(OR=0.32,95%CI:0.12-0.86,P=0.02).However,TIPS increased the risk of HE(OR=2.95,95%CI:1.87-4.66,P=0.02)and severe HE(OR=2.18,95%CI:1.27-3.76,P=0.005).CONCLUSION:TIPS significantly improved the LTF survival of cirrhotic patients with refractory ascites and decreased the risk of recurrent ascites and hepatorenal syndrome with the cost of increased risk of HE compared with paracentesis.Further studies are warranted to validate the survival benefit of TIPS in clinical practice settings. | Ming Bai Xing-Shun Qi Zhi-Ping Yang Man Yang Dai-Ming Fan Guo-Hong Han | 2014 | World Journal of Gastroenterology2014,20,10: | 29 |
| 7 | Prevalence of Thyroid Nodules and Its Relationship with Iodine Status in Shanghai:a Population-based Study显示文摘Objective This study was designed to evaluate the prevalence of thyroid nodules(TNs) and its relationship with urine iodine concentrations(UICs) after the regional rapid economic growth and lifestyle changes. Methods A cross-sectional survey was conducted in the general population aged 15-69 years. A questionnaire regarding general and personal characteristics and relevant information was administered. Ultrasonography of the thyroid was performed, and serum triiodothyronine(T3), tetraiodothyronine(T4), serum thyroid stimulating hormone(TSH), free triiodothyronine(FT3), free tetraiodothyronine(FT4), thyroglobulin antibody(Tg Ab), thyroid peroxidase antibody(TPOAb), and TSH receptor antibody(TRAb) levels were measured for each individual subject. Results The prevalence rates of TNs in the whole population, females and males were 27.76%, 34.04%, and 21.60%, respectively. The prevalence of multiple nodules increased with age, whereas the prevalence peaks differed between males and females. The median UICs in the whole population and females with non-TNs were higher than those of subjects with TNs(P=0.0035, P=0.0068). The median UICs in subjects with a single TN were higher than those in subjects with multiple TNs(P=0.0164, P=0.0127). The result showed a U-shaped curve relationship between UIC and prevalence of TNs. The prevalence of TNs was the lowest when the UIC was 140-400 μg/L. Conclusion The prevalence of TNs was nearly 30% and increased with age. The relationship between UIC and prevalence of TNs is U-shaped, with an increase in risk when the UIC was <140 μg/L and >400 μg/L. | 8 | Shunting branch of portal vein and stent position predict survival after transjugular intrahepatic portosystemic shunt显示文摘AIM:To evaluate the effect of the shunting branch of the portal vein(PV)(left or right)and the initial stent position(optimal or suboptimal)of a transjugular intrahepatic portosystemic shunt(TIPS).METHODS:We retrospectively reviewed 307 consecu5tive cirrhotic patients who underwent TIPS placement for variceal bleeding from March 2001 to July 2010 at our center.The left PV was used in 221 patients and the right PV in the remaining 86 patients.And,224 and83 patients have optimal stent position and sub-optimal stent positions,respectively.The patients were followed until October 2011 or their death.Hepatic encephalopathy,shunt dysfunction,and survival were evaluated as outcomes.The difference between the groups was compared by Kaplan-Meier analysis.A Cox regression model was employed to evaluate the predictors.RESULTS:Among the patients who underwent TIPS to the left PV,the risk of hepatic encephalopathy(P=0.002)and mortality were lower(P<0.001)compared to those to the right PV.Patients who underwent TIPS with optimal initial stent position had a higher primary patency(P<0.001)and better survival(P=0.006)than those with suboptimal initial stent position.The shunting branch of the portal vein and the initial stent position were independent predictors of hepatic encephalopathy and shunt dysfunction after TIPS,respectively.And,both were independent predictors of survival.CONCLUSION:TIPS placed to the left portal vein with optimal stent position may reduce the risk of hepatic encephalopathy and improve the primary patency rates,thereby prolonging survival. | Ming Bai Chuang-Ye He Xing-Shun Qi Zhan-Xin Yin Jian-Hong Wang Wen-Gang Guo Jing Niu Jie-Lai Xia Zhuo-Li Zhang Andrew C Larson Kai-Chun Wu Dai-Ming Fan Guo-Hong Han | 2014 | World Journal of Gastroenterology2014,20,3: | 26 |

| 13 | In-situ U-Pb dating of uraninite by fs-LA-ICP-MS显示文摘In this study,the Pb/U fractionation between zircon and uraninite during femtosecond Laser Ablation Inductively Coupled Plasma Mass Spectrometry(fs-LA-ICP-MS)analysis was studied in detail.The results show significant Pb/U fractionation between zircon and uraninite during fs-LA-ICP-MS analysis that when calibrated against the zircon standard M257,the obtained U-Pb age of the Chinese national uraninite standard GBW04420 is 17%older than the recommended value.Thus,the accurate in-situ U-Pb dating of uraninite by LA-ICP-MS requires matrix-matched external standards for calibration.Uraninite in thin sections of two U-mineralized leucogranite from the Gaudeanmus in Namibia was analyzed by a fs-LA-ICP-MS equipped with a Signal Smooth Device(SSD),using laser spot and frequency of 10μm and 1 Hz,respectively.When calibrated using GBW04420 as the external standard,two samples give weighted mean 206Pb/238U ages of 504±3 Ma(2σ,n=21)and503±3 Ma(2σ,n=22),and only one of two samples yields a concordia U-Pb age of 507±1 Ma(2σ,n=21).These results are consistent with ID-TIMS U-Pb ages of 509±1 and 508±12 Ma and are also indistinguishable from zircon U-Pb upper intercept ages of 506±33 Ma(2σ,n=29)and 501±51 Ma(2σ,n=29).The present study shows that in-situ U-Pb dating of uraninite can deliver more reliable formation ages of the deposit than dating coeval high-U zircon because the latter commonly suffer significant Pb loss after formation.Our results confirm that GBW04420 is an ideal matrix matching standard for in-situ U-Pb dating of uraninite. | ZONG KeQing CHEN JinYong HU ZhaoChu LIU YongSheng LI Ming FAN HongHai MENG YanNing | 2015 | Science China Earth Sciences2015,58,10: | 22 |
