| 1 | Association of CagA and VacA presence with ulcer and non-ulcer dyspepsia in a Turkish population显示文摘AIM: The mostly known genotypic virulence features, of H. pyloriare cytotoxin associated gene A (ragA) and Vacuolating cytotoxin gene A (VacA). We investigated the association of these major virulence factors with ulcer and non-ulcer dyspepsia in our region.METHODS: One hundred and forty two dyspeptic patients were studied (average age 44.8±15.9 years, range 15-87years, 64 males and 78 females). Antral and corpus biopsies were taken for detecting and genotyping of H. pylori. 107patients who were H. pylori positive by histological assessment were divided into three groups according to endoscopic findings: Duodenal ulcer (DU), gastric ulcer (GU)and non-ulcer dyspepsia (NUD). The polymerase chain reaction (PCR) was used to detect CagA and VacA genes of H.pylori using specific primers.RESULTS: H.pyloriwas isolated from 75.4 % (107/142) of the patients. Of the 107 patients, 66 (61.7 %) were cagApositive and 82 (76.6 %) were Vacl-positive. CagA gene was positively associated with DU and GU (P<0.01, P<0.02),but not with NUD (P>0.05). Although VacA positivity in ulcer patients was higher than that in NUD group, the difference was not statistically significant (P>0.05).CONCLUSION: There is a significantly positive association between CagA genes and DU and GU. | 2 | Meshless Method for Analysis of Permeable Breakwaters in the Proximity of A Vertical Wall显示文摘In the present work, the improved version of the meshless singular boundary method(ISBM) is developed for analyzing the performance of bottom standing submerged permeable breakwaters in regular normally incident waves and in the proximity of a vertical wall. Both single and dual prismatic breakwaters of rectangular and trapezoidal forms are examined. The physical problem is cast in terms of the Laplace equation governing an irrotational flow and incompressible fluid motion with appropriate mixed type boundary conditions, and solved numerically using the ISBM. To model the permeability of the breakwaters fully absorbing boundary conditions are assumed. Numerical results are presented in terms of hydrodynamic quantities of the reflection coefficients. These are firstly validated against the results of a multi-domain boundary element method(BEM) developed independently for a previous study. The agreement between the results of the two methods is excellent. The coefficients of reflection are then computed and discussed for a variety of structural conditions including the breakwaters height, width, spacing, and absorbing permeability. Effects of the proximity of the vertical plane wall are also investigated. The breakwater's width is found to have only marginal effects compared with its height. Permeability tends to decrease the minimum reflections. These coefficients show periodic variations with the spacing relative to the wavelength. Trapezoidal breakwaters are found to be more cost-effective than the rectangular breakwaters. | 3 | Reflection and Transmission of Regular Waves from/Through Single and Double Perforated Thin Walls显示文摘In this paper, reflection and transmission coefficients of regular waves from/through perforated thin walls are investigated. Small scale laboratory tests have been performed in a wave flume firstly with single perforated thin Plexiglas plates of various porosities. The plate is placed perpendicular to the flume with the height from the flume bottom to the position above water surface. With this thin wall in the flume wave overtopping is prohibited and incident waves are able to transmit. The porosities of the walls are achieved by perforating the plates with circular holes.Model settings with double perforated walls parallel to each other forming so called chamber system, have been also examined. Several parameters have been used for correlating the laboratory tests' results. Experimental data are also compared with results from the numerical model by applying the multi-domain boundary element method(MDBEM)with linear wave theory. Wave energy dissipation due to the perforations of the thin wall has been represented by a simple yet effective porosity parameter in the model. | 4 | Comparison of pre-treatment and post-treatment use of selenium in retinal ischemia reperfusion injury显示文摘AIM: To investigate the effects of selenium in rat retinal ischemia reperfusion(IR) model and compare pretreatment and post-treatment use.METHODS: Selenium pre-treatment group(n =8) was treated with intraperitoneal(i.p.) selenium 0.5 mg/kg for7 d and terminated 24 h after the IR injury. Selenium posttreatment group( n = 8) was treated with i. p. selenium0.5 mg/kg for 7d after the IR injury with termination at the end of the 7d period. Sham group(n =8) received i.p.saline injections identical to the selenium volume for 7d with termination 24 h after the IR injury. Control group(n =8) received no intervention. Main outcome measures were retina superoxide dismutase(SOD), glutathione(GSH),total antioxidant status(TAS), malondialdehyde(MDA),DNA fragmentation levels, and immunohistological apoptosis evaluation.RESULTS: Compared to the Sham group, selenium pre-treatment had a statistical difference in all parameters except SOD. Post-treatment selenium also resulted in statistical differences in all parameters except the MDA levels. When comparing selenium groups, the pre-treatment selenium group had a statistically higher success in reduction of markers of cell damage such as MDA and DNA fragmentation. In contrast, the post-selenium treatment group had resulted in statisticallyhigher levels of GSH. Histologically both selenium groups succeeded to limit retinal thickening and apoptosis. Pre-treatment use was statistically more successful in decreasing apoptosis in ganglion cell layer compared to post-treatment use.CONCLUSION: Selenium was successful in retinal protection in IR injuries. | 5 | Delaying surgery after neoadjuvant chemoradiotherapy improves prognosis of rectal cancer显示文摘AIM To investigate the prognostic effect of a delayed interval between neoadjuvant chemoradiotherapy(CRT) and surgery in locally advanced rectal cancer.METHODS We evaluated 87 patients with locally advanced mid-or distal rectal cancer undergoing total mesorectal excision following an interval period after neoadjuvant CRT at ?i?li Hamidiye Etfal Training and Research Hospital,Istanbul between January 2009 and January 2014.Patients were divided into two groups according to the intervalbefore surgery: < 8 wk(group Ⅰ) and ≥ 8 wk(group Ⅱ).Data related to patients,cancer characteristics and pathological examination were collected and analyzed.RESULTS When the distribution of timing between group Ⅰ(n = 45) and group Ⅱ(n = 42) was viewed,comparison of interval periods(median ± SD) of groups showed a significant difference of as 5 ± 1.28 wk in group Ⅰ and 10.1 ± 2.2 wk in group Ⅱ(P < 0.001).The median follow-up period for all patients was 34.5(9.9-81) mo.group Ⅱ had significantly higher rates of pathological complete response(p CR) than group Ⅰ had(19% vs 8.9%,P = 0.002).Rate of tumor regression grade(TRG) poor response was 44.4% in group Ⅰ and 9.5% in group Ⅱ(P < 0.002).A poor pathological response was associated with worse disease-free survival(P = 0.009).The interval time did not show any association with local recurrence(P = 0.79).CONCLUSION Delaying the neoadjuvant CRT-surgery interval may provide nodal down-staging,improve p CR rate,and decrease the rate of TRG poor response. | Mehmet Mihmanli Esin Kabul Gürbulak Ismail Ethem Akgün Mustafa Fevzi Celayir Pinar Yazici Deniz Tuncel Tuba Tülin Bek Ayhan Oz Sinan Omeroglu | 2016 | World Journal of Gastrointestinal Oncology2016,8,9: | 3 |
