1Update on pathogenesis and clinical management of acute pancreatitis显示文摘Acute pancreatitis (AP), defined as the acute nonbacterial inflammatory condition of the pancreas, is derived from the early activation of digestive enzymes found inside the acinar cells, with variable compromise of the gland itself, nearby tissues and other organs. So, it is an event that begins with pancreatic injury, elicits an acute inflammatory response, encompasses a variety of complications and generally resolves over time. Different conditions are known to induce this disorder, although the innermost mechanisms and how they act to develop the disease are still unknown. We summarize some well established aspects. A phase sequence has been proposed: etiology factors generate other conditions inside acinar cells that favor the AP development with some systemic events; genetic factors could be involved as susceptibility and modifying elements. AP is a disease with extremely different clinical expressions. Most patients suffer a mild and limited disease, but about one fifth of cases develop multi organ failure, accompanied by high mortality. This great variability in presentation, clinical course and complications has given rise to the confusion related to AP related terminology. However, consensus meetings have provided uniform definitions, including the severity of the illness. The clinical management is mainly based on the disease′s severity and must be directed to correct the underlying predisposing factors and control the inflammatory process itself. The first step is to determine if it is mild or severe. We review the principal aspects to be considered in this treatment, as reflected in several clinical practice guidelines. For the last 25 years, there has been a global increase in incidence of AP, along with many advances in diagnosis and treatment. 2黄芪多糖对过氧化氢刺激皮肤成纤维细胞中线粒体和溶酶体的保护作用(英文)显示文摘目的:证明黄芪多糖对氧化应激皮肤细胞的保护作用。方法:原代培养皮肤成纤维细胞在过氧化氢0.5 mmol/L中,孵育30 min造成急性氧化损伤的模型。MTT法测黄芪多糖的剂量反应,DAPI染色法检测黄芪多糖对细胞死亡的作用;激光共聚焦法检测黄芪多糖对线粒体、线粒体膜电位以及溶酶体稳定性的保护作用。结果:经过黄芪多糖的治疗,能够明显提高细胞存活率,其作用浓度从0.5 mg/L开始,到1 mg/L达最大效果,呈剂量依赖性。而且,黄芪多糖能够抑制线粒体膜电位崩解保护线粒体形态,保护溶酶体膜。结论:这些结果证明了黄芪多糖在线粒体和溶酶体水平存在1个新的调节凋亡的途径。这个途径可以成为一些氧化应激以及溶酶体缺陷疾病的1个新的治疗靶点。岳晓莉 李萍 刘欣 黄启福 BARBIER-CHASSEFIERE Veronique MORIN Christophe PAPY-GARCIA Dulce 2008中国病理生理杂志2008,24,4:9
3Obstructive Sleep Apnea Hypopnea Syndrome and Pulmonary Embolism: Warfarin Versus Positive Airway Pressure as Determinant显示文摘To the Editor:Xie et al.analyzed warfarin doses in pulmonary embolism (PE) and its adverse effects in patients with obstructive sleep apnea hypopnea syndrome (OSAHS).The key findings were that OSAHS patients required a significantly higher dose of warfarin than their non-OSAHS counterparts (4.73 mg vs.3.61 mg,P < 0.001) and that PE recurrence was higher in OSAHS than in non-OSAHS groups after withdrawal of warfarin (21.43% vs.6.78%,P =0.047).They conclude that OSAHS patients may present with hypercoagulation and relatively high risk of recurrence of PE after cessation of 6-month warfarin treatment.We have read this with great interest.[1]Filipa Ramalho Fernandes Dulce Apolinario Antonio M Esquinas 2016Chinese Medical Journal2016,,2:3

5Which is the best choice for gastroesophageal disorders:Melatonin or proton pump inhibitors?显示文摘Melatonin is used in many countries to improve sleep disorders.Melatonin is a hormone produced by the pineal gland and enterochromaff in cells which control sleep and gastrointestinal motility.Low levels of melatonin lead to gastroesophageal reflux disease(GERD).Most of patients with GERD have a sleep disorder.So,low melatonin levels is the main cause of insomnia.Beyond this,it has an inhibitory action on gastric acid secretion and seems to control the lower esophageal sphincter.Proton pump inhibitors(PPIs) are a group of drugs whose main action is a pronounced and long-lasting reduction of gastric acid production.They are the most potent inhibitors of acid secretion available today.Omeprazole(one of the PPIs) and melatonin have similarities in their chemical structures.Therefore,we could consider omeprazole as a rough copy of melatonin.In this paper,we compare the advantages and disadvantages of the clinical use of melatonin and PPIs.Joanna Dulce Favacho de Oliveira Torres Ricardo de Souza Pereira 2010World Journal of Gastrointestinal Pharmacology and Therapeutics2010,1,5:2
