2A typical presentation of a rare cause of obscure gastrointestinal bleeding显示文摘A 52-year-old white woman had suffered from intermittent gastrointestinal (GI) bleeding for one year. Upper GI endoscopy, colonoscopy and peroral doubleballoon enteroscopy (DBE) did not detect any bleeding sour ce, suggesting obscure GI bleeding. However, in video capsule endoscopy a jejunal ulceration without blee ding signs was suspected and this was endoscopically conf irmed by another peroral DBE. After transfusion of packed red blood cells, the patient was discharged from our hospital in good general condition.Two weeks later she was readmitted because of another episode of acute bleeding. Multi-detector row computed tomography with 3D reconstruction was performed revealing a jejunal tumor causing lower gastrointestinal bleeding. The patient underwent exploratory laparotomy with partial jejunal resection and end-to-end jejunostomy for reconstruction. Histological examination of the specimen confirmed the diagnosis of a low risk gastrointestinal stromal tumor (GIST). Nine days after surgery the patient was discharged in good health. No signs of gastrointestinal rebleeding occurred in a followup of eight months. 3Personalized Computer Simulation of Diastolic Function in Heart Failure显示文摘The search for a parameter representing left ventricular relaxation from non-invasive and invasive diagnostic tools has been extensive, since heart failure(HF) with preserved ejection fraction(HF-p EF) is a global health problem. We explore here the feasibility using patient-speci?c cardiac computer modeling to capture diastolic parameters in patients suffering from different degrees of systolic HF. Fifty eight patients with idiopathic dilated cardiomyopathy have undergone thorough clinical evaluation, including cardiac magnetic resonance imaging(MRI), heart catheterization,echocardiography, and cardiac biomarker assessment. A previously-introduced framework forcreating multi-scale patient-speci?c cardiac models has been applied on all these patients. Novel parameters, such as global stiffness factor and maximum left ventricular active stress, representing cardiac active and passive tissue properties have been computed for all patients. Invasive pressure measurements from heart catheterization were then used to evaluate ventricular relaxation using the time constant of isovolumic relaxation Tau(s). Parameters from heart catheterization and the multi-scale model have been evaluated and compared to patient clinical presentation. The model parameter global stiffness factor, representing diastolic passive tissue properties, is correlated significantly across the patient population with s. 4Continuous size fractionation of magnetic nanoparticles by using simulated moving bed chromatography显示文摘The size fractionation of magnetic nanoparticles is a technical problem,which until today can only be solved with great effort.Nevertheless,there is an important demand for nanoparticles with sharp size distributions,for example for medical technology or sensor technology.Using magnetic chromatography,we show a promising method for fractionation of magnetic nanoparticles with respect to their size and/or magnetic properties.This was achieved by passing magnetic nanoparticles through a packed bed of fine steel spheres with which they interact magnetically because single domain ferro-/ferrimagnetic nanoparticles show a spontaneous magnetization.Since the strength of this interaction is related to particle size,the principle is suitable for size fractionation.This concept was transferred into a continuous process in this work using a so-called simulated moving bed chromatography.Applying a suspension of magnetic nanoparticles within a size range from 20 to 120 nm,the process showed a separation sharpness of up to 0.52 with recovery rates of 100%.The continuous feed stream of magnetic nanoparticles could be fractionated with a space-time-yield of up to 5 mg/(L∙min).Due to the easy scalability of continuous chromatography,the process is a promising approach for the efficient fractionation of industrially relevant amounts of magnetic nanoparticles.Carsten-Rene Arlt Dominik Brekel Stefan Neumann David Rafaja Matthias Franzreb 2021Frontiers of Chemical Science and Engineering2021,15,5:0
