| 1 | Pentoxifylline versus prednisolone for severe alcoholic hepatitis:A randomized controlled trial显示文摘AIM:To compare the efficacy of pentoxifylline and prednisolone in the treatment of severe alcoholic hepatitis,and to evaluate the role of different liver function scores in predicting prognosis.METHODS:Sixty-eight patients with severe alcoholic hepatitis(Maddrey score≥32)received pentoxifylline(n=34,groupⅠ)or prednisolone(n=34,groupⅡ) for 28 d in a randomized double-blind controlled study,and subsequently in an open study(with a tapering dose of prednisolone)for a total of 3 mo,and were followed up over a period of 12 mo.RESULTS:Twelve patients in groupⅡdied at the end of 3 mo in contrast to five patients in groupⅠ.The probability of dying at the end of 3 mo was higher in groupⅡas compared to groupⅠ(35.29%vs 14.71%,P=0.04;log rank test).Six patients in groupⅡ developed hepatorenal syndrome as compared to none in groupⅠ.Pentoxifylline was associated with a significantly lower model for end-stage liver disease(MELD)score at the end of 28 d of therapy(15.53±3.63 vs 17.78±4.56,P=0.04).Higher baseline Maddrey score was associated with increased mortality.CONCLUSION:Reduced mortality,improved risk-benefit profile and renoprotective effects of pentoxifylline compared with prednisolone suggest that pentoxifylline is superior to prednisolone for treatment of severe alcoholic hepatitis. | Binay Krishna De Subhabrata Gangopadhyay Deep Dutta Sumanta Das Baksi Adyapad Pani Pramit Ghosh | 2009 | World Journal of Gastroenterology2009,15,13: | 26 |

| 4 | An international program on Silk Road Disaster Risk Reduction--a Belt and Road initiative (2016-2020)显示文摘Belt and Road Initiative(BRI) is a Chinese national strategy which calls for cooperative economic, political and cultural exchange at the global level along the ancient Silk Road. The overwhelming natural hazards located along the belt and road bring great challenges to the success of BRI. In this framework, a 5-year international program was launched to address issues related to hazards assessment and disaster risk reduction(DRR). The first workshop of this program was held in Beijing with international experts from over 15 countries. Risk conditions on Belt and Road Countries(BRCs) have been shared and science and technology advancements on DRR have been disseminated during the workshop. Under this program, six task forces have been setup to carry out collaborative research works and three prioritized study areas have been established. This workshop announced the launching of this program which involved partners from different countries including Pakistan, Nepal, Russia, Italy, United Kingdom, Sri Lanka and Tajikistan. The program adopted the objectives of Sendai Framework for Disaster Risk Reduction 2015-2030 and United Nation Sustainable Development Goals 2030 and was implemented to assess disaster risk in BRCs and to propose suitable measures for disaster control which can be appropriate both for an individual country and for specific sites. | 5 | Landslide susceptibility assessment of the region affected by the 25 April 2015 Gorkha earthquake of Nepal显示文摘Nepal was hit by a 7.8 magnitude earthquake on 25^(th) April,2015.The main shock and many large aftershocks generated a large number of coseismic landslips in central Nepal.We have developed a landslide susceptibility map of the affected region based on the coseismic landslides collected from remotely sensed data and fieldwork,using bivariate statistical model with different landslide causative factors.From the investigation,it is observed that most of the coseismic landslides are independent of previous landslides.Out of 3,716 mapped landslides,we used 80% of them to develop a susceptibility map and the remaining 20% were taken for validating the model.A total of 11 different landslide-influencing parameters were considered.These include slope gradient,slope aspect,plan curvature,elevation,relative relief,Peak Ground Acceleration(PGA),distance from epicenters of the mainshock and major aftershocks,lithology,distance of the landslide from the fault,fold,and drainage line.The success rate of 87.66% and the prediction rate of86.87% indicate that the model is in good agreement between the developed susceptibility map and theexisting landslides data.PGA,lithology,slope angle and elevation have played a major role in triggering the coseismic mass movements.This susceptibility map can be used for relocating the people in the affected regions as well as for future land development. | Amar Deep Regmi Megh Raj Dhital ZHANG Jian-qiang SU Li-jun CHEN Xiao-qing | 2016 | Journal of Mountain Science2016,13,11: | 7 |
| 6 | Landslide Susceptibility Mapping along Bhalubang–Shiwapur Area of Mid-Western Nepal Using Frequency Ratio and Conditional Probability Models显示文摘Roads constructed in fragile Siwaliks are prone to large number of instabilities. Bhalubang–Shiwapur section of Mahendra Highway lying in Western Nepal is one of them. To understand the landslide causative factor and to predict future occurrence of the landslides, landslide susceptibility mapping(LSM) of this region was carried out using frequency ratio(FR) and weights-of-evidence(W of E) models. These models are easy to apply and give good results. For this, landslide inventory map of the area was prepared based on the aerial photo interpretation, from previously published/unpublished reposts, and detailed field survey using GPS. About 332 landslides were identified and mapped, among which 226(70%) were randomly selected for model training and the remaining 106(30%) were used for validation purpose. A spatial database was constructed from topographic, geological, and land cover maps. The reclassified maps based on the weight values of frequency ratio and weights-of-evidence were applied to get final susceptibility maps. The resultant landslide susceptibility maps were verified andcompared with the training data, as well as with the validation data. From the analysis, it is seen that both the models were equally capable of predicting landslide susceptibility of the region(W of E model(success rate = 83.39%, prediction rate = 79.59%); FR model(success rate = 83.31%, prediction rate = 78.58%)). In addition, it was observed that the distance from highway and lithology, followed by distance from drainage, slope curvature, and slope gradient played major role in the formation of landsides. | 8 | Transarterial chemoembolization for liver metastases from solid pseudopapillary epithelial neoplasm of pancreas: A case report显示文摘Solid pseudo-papillary epithelial neoplasm(SPEN) is a rare epithelial tumor of pancreas with a low malignant potential occurs most commonly in young females. We report a case of 40 years old woman presented withextensive liver metastasis from SPEN of pancreatic body for which she was operated four years ago. Due to the extensive nature of metastatic disease she was offered Transarterial chemoembolisation(TACE) using gemcitabine as chemotherapeutic agent. Short term follow up after a month of TACE with multiphase computed tomography showed > 90% resolution in the viable tumor with significant clinical improvement. TACE ensures targeted delivery of chemotherapeutic drugs in higher doses with least systemic toxicity and is more effective and safe than systemic chemotherapy. | 13 | Hemodynamic effects of propranolol with spironolactone in patients with variceal bleeds: A randomized controlled trial显示文摘AIM: To study the hemodynamic effects of spironolactone with propranolol vs propranolol alone in the secondary prophylaxis of variceal bleeding. METHODS: Thirty-five cirrhotics with variceal bleeding randomly received propranolol (n = 17: Group A) or spironolactone plus propranolol (n = 18: Group B). Hemodynamic assessment was performed at baseline and on the eighth day. RESULTS: Spironolactone with propranolol caused a greater reduction in the hepatic venous pressure gradient than propranolol alone (26.94% vs 10.2%; P < 0.01). Fourteen out of eighteen patients on the combination treatment had a reduction in hepatic venous pressure gradient to ≤ 12 mmHg or a 20% reduction from baseline in contrast to only six out of seventeen (6/17) on propranolol alone (P < 0.05). CONCLUSION: Spironolactone with propranolol results in a better response with a greater reduction in hepatic venous pressure gradient in the secondary prophylaxis of variceal bleeding. A greater number of patients may be protected by this combination therapy than by propranolol alone. | 15 | Bipolar radiofrequency ablation of tibialchondroblastomas: A report of three cases显示文摘Chondroblastoma is a rare benign cartilaginous neoplasm of bone. The recurrence rate is high and complications are frequent following open curettage with bone grafting which is the standard treatment forchondroblastomas. We performed radiofrequency ablation in three cases of tibialchondroblastoma using the bipolar system. One patient experienced residual pain for which repeat ablation was performed. No other complications were observed during follow-up. Radiofrequency ablation may offer an effective alternative for the treatment of selected cases of chondroblastoma. | 18 | Is CT scan a predictor of instability in recurrent dislocation shoulder?显示文摘Purpose: Glenoid bone defect and the defect on the posterior-superior surface of the humerus “Hill- Sachs lesion” are the commonly seen bony lesions in patients with recurrent dislocation shoulder. Computed tomography (CT) scan is considered as the best option in assessing the bony defects in the recurrent dislocation shoulder. The aim of this study was to assess the clinical and radiological corelation in the patients with recurrent dislocation shoulder. Methods: Forty-four patients of recurrent dislocation shoulder who were evaluated between January 2015 and December 2017 at a tertiary care center, clinically and radiologically using CT scan and meeting the inclusion criteria, were included. The correlation between the clinical history of the number of dislocations and the bone loss using CT scan was evaluated. Two sided statistical tests were performed at a significance level of α=0.05. The analysis was conducted using IBM SPSS STATISTICS (version 22.0). Results: All the patients were male with mean age of 25.95 (SD ± 4.2) years were evaluated. Twenty-four patients sustained injury in sporting activities while 20 patients sustained injury in training. There were an average of 4.68 (SD ± 3.1, range 2e15, median 3) episodes of dislocation. Forty-one patients had the glenoid bone loss while 40 had the Hill-Sachs lesions. The mean glenoid width defect was 10.80%(range 0e27%) while the mean Hill-Sachs defect was 14.27 mm (range 0e26.6 mm). The mean area of bone loss of the glenoid surface was 10.81%(range 0e22.4%). The lesions were on track in 34 patients and off track in 10 patients. Conclusions: CT scan of the shoulder joint is an effective method for assessing the amount of bone loss. The number of dislocations are correlated significantly with off-track lesions and the amount of bone loss on the glenoid and Hill-Sachs lesion.
