| 10 | FOLFOXIRI vs FOLFIRINOX as first-line chemotherapy in patients with advanced pancreatic cancer: A population-based cohort study显示文摘BACKGROUND FOLFIRINOX regimen is the first-line reference chemotherapy(L1)in advanced pancreatic ductal adenocarcinoma(aPDAC).FOLFOXIRI,a schedule with a lower dose of irinotecan and no bolus 5-fluorouracil,has demonstrated efficacy and feasibility in colorectal cancer.AIM To investigate the potential clinical value of FOLFOXIRI in patients with aPDAC in routine clinical practice.METHODS Analyses were derived from all consecutive aPDAC patients treated in L1 between January 2011 and December 2017 in two French institutions,with either FOLFOXIRI(n=165)or FOLFIRINOX(n=124)regimens.FOLFOXIRI consisted of irinotecan(165 mg/m2),oxaliplatin(85 mg/m2),leucovorin(200 mg/m2)and 5-fluorouracil(3200 mg/m2 as a 48-h continuous infusion)every 2 wk.Ninety-six pairs of patients were selected through propensity score matching,and clinical outcomes of the two treatment regimens were compared.RESULTS Median overall survival was 11.1 mo in the FOLFOXIRI and 11.6 mo in the FOLFIRINOX cohorts,respectively.After propensity score matching,survival rates remained similar between the two regimens in terms of overall survival(hazard ratio=1.22;P=0.219)and progression-free survival(hazard ratio=1.27;P=0.120).The objective response rate was 37.1%in the FOLFOXIRI group vs 47.8%in the FOLFIRINOX group(P=0.187).Grade 3/4 toxicities occurred in 28.7%of patients in the FOLFOXIRI cohort vs 19.5%in the FOLFIRINOX cohort(P=0.079).FOLFOXIRI was associated with a higher incidence of grade 3/4 digestive adverse events.Hematopoietic growth factors were used after each chemotherapy cycle and the low hematological toxicity rates were below 5%with both regimens.CONCLUSION FOLFOXIRI is feasible in L1 in patients with aPDAC but does not confer any therapeutic benefit as compared with FOLFIRINOX.The low hematological toxicity rates strengthened the relevance of primary prophylaxis with hematopoietic growth factors. | Angélique Vienot Hortense Chevalier Clément Bolognini Elisabeta Gherga Elodie Klajer Aurélia Meurisse Marine Jary Stefano Kim Christelle d’Engremont Thierry Nguyen Fabien Calcagno Hamadi Almotlak Francine Fein Meher Nasri Syrine Abdeljaoued Anthony Turpin Christophe Borg Dewi Vernerey | 2020 | World Journal of Gastrointestinal Oncology2020,12,3: | 0 |
| 11 | Hydrological variability and long-term floristic-structural modifications in different habitats of a tropical semi-deciduous forest显示文摘Shifts in hydrological regimes alter river flow rates and flood pulses, decrease environmental heterogeneity and the floristic-structural complexity of associated plant communities. We tested the hypothesis that drought events affect plant community composition and structure at a small-scale within a riparian fragment towards a reduction in floristic-structural complexity. The tree community was sampled in three habitats (wet, transitional and dry) and monitored in seven inventories carried out between 1991 and 2018. Hydrological variations were evaluated through annual rainfalls, river flow rates and water level data. The species richness and the detrended correspondence analysis axes were used to characterise the temporal modifications in floristic composition. Community structure was described in terms of biomass: accumulated, growth of survivors, mortality and recruitment. Generalised linear mixed models were fitted to evaluate the effects of time and environment in community. It was concluded that the climate has become drier in recent years due to declining precipitation that has affected flow rates and water levels. The floristic-structural complexity of the study fragment was maintained during the monitoring period. | 12 | Can small-scale altitudinal gradients predict spatial and temporal patterns in tropical forests?显示文摘In tropical montane forests,compositional and structural changes are commonly driven by broad-scale altitudinal variation.Here,given the lack of knowledge on small-scale vegetation changes and temporal dynamics,we address the effects of small-scale variations in soil and altitude on tree community structure,temporal dynamics and phylogenetic diversity in a semi-deciduous tropical forest of the Atlantic Forest Domain,southeastern Brazil.In 2010 and 2015 we sampled thirty plots of 400 m^(2),set up along an altitudinal gradient between 1000 and 1500 m a.s.I..In each plot,we collected soil samples for chemical and textural analyses.We fitted linear models to test the effects of altitude and soil on community dynamics and phylogenetic parameters.Altitude and soil explained the spatial variation in number of individuals and phylogenetic diversity metrics.From lower to higher altitudes,we found decreasing fertility,increasing tree density and decreasing phylogenetic diversity.Altitude significantly influenced the increases in total biomass(from 240.9 to 255.4 t ha^(-1))and individual biomass(from 0.15 to 0.17 t)recorded in the interval.And while community temporal dynamics had rates of 1.96%for mortality,1.02%for recruitment,1.61%for biomass loss and 2.81%for biomass gain,none of them were explained by altitude or soil.Temporal species substitution averaged0.1 in the interval.Altogether,these results suggest that the small-scale variations in altitude and soil likely determine the conditions and resources that drive community assembly and structure,which are expressed by spatial variations along the altitudinal gradient.At the same time,temporal patterns were not influenced by altitude-related environmental variation,resulting in a similar dynamic behaviour across the gradient,suggesting that broad-scale factors may play a more important role than local ones. | Mariana Caroline Moreira Morelli Cléber Rodrigo de Souza Jean Daniel Morel Vinícius Andrade Maia Alisson Borges Miranda Santos Kaline Fernandes Miranda Rubens Manoel dos Santos | 2021 | Journal of Forestry Research2021,32,5: | 0 |
