1Predictive factors for early aspiration in liver abscess显示文摘AIM: To determine the predictive factors for early aspiration in liver abscess. METHODS: A retrospective analysis of all patients with liver abscess from 1995 to 2004 was performed. Abscess was diagnosed as amebic in 661 (68%) patients, pyogenic in 200 (21%), indeterminate in 73 (8%) and mixed in 32 (3%). Multiple logistic regression analysis was performed to determine predictive factors for aspiration of liver abscess. RESULTS: A total of 966 patients, 738 (76%) male, mean age 43 ± 17 years, were evaluated: 540 patients responded to medical therapy while adjunctive percutaneous aspiration was performed in 426 patients. Predictive factors for aspiration of liver abscess were: age ≥ 55 years, size of abscess ≥ 5 cm, involvement of both lobes of the liver and duration of symptoms ≥ 7 d. Hospital stay in the aspiration group was relatively longer than in the non aspiration group. Twelve patients died in the aspiration group and this mortality was not statistically significant when compared to the non aspiration group. 4Infectious diseases in the aftermath of monsoon flooding in Pakistan显示文摘Pakistan is ranked 9th in terms of flood-affected countries worldwide.In the summer of 2010,the northern province of Khyber-Pakhtunkhwa received more than 312 mm of rain in a 56 hour period.This resulted in over 1 600 deaths across the region.In addition,over 14 million people were directly affected by this record-breaking deluge.Flood affected regions serve as ideal breeding grounds for pathogens,leading to the spread of diseases.The poor standards of hygiene in camps set up for individuals displaced by the floods also contribute to this.It is essential that those involved in relief efforts are aware of the epidemiology of diseases that have historically seen a sudden upsurge after natural disasters.Keeping this in mind,we conducted a simple review of literature.An extensive literature search was conducted using the PubMed data base and online search engines.Articles published in the last 20 years were considered along with some historical articles where a background was required.Seven major diseases were identified to increase substantially in the aftermath of natural disasters.They were then classified into acute and sub-acute settings.Diarrhea,skin&eye infections and leptospirosis were identified in the acute setting while malaria,leishmaniasis,respiratory infections and hepatitis were identified in the sub-acute setting.Maryam Baqir Zain A Sobani Amyn Bhamani Nida Shahab Bham Sidra Abid Javeria Farook M Asim Beg 2012Asian Pacific Journal of Tropical Biomedicine2012,2,1:2
5Hydrochemistry and sediment characteristics of polar periglacial lacustrine environments on Fisher Island and Broknes Peninsula,East Antarctica显示文摘Fisher Island and Broknes Peninsula in the Larsemann Hills constitute part of a polar lowland periglacial environment between marine and glacial ecosystems.The landscape is characterized by gently rolling hills and broad valleys interspersed with lakes formed in glacially scoured basins.We analyzed the physicochemical parameters and the ionic constituents of water samples from 10 lakes in each of these two locations.Our results showed considerable differences between the two regions and demonstrated the influence of lithology and processes including weathering,evaporation,and atmospheric precipitation.All major cations and anions in the lake waters showed positive correlations indicating balanced ionic concentrations.Unconsolidated sediments were sparsely distributed and scattered over glacial deposits,valley fills,and occasional moraine ridges.The type and rate of sedimentation was mainly controlled by surface run-off and aeolian influx.The sediment samples from lake beds and the catchment area on Fisher Island were immature and poorly to very poorly sorted,consisting of gravelly sand with negligible silt and finer fractions.Sediments had a polymodal grain size distribution with the two major populations lying between-2 and-1 phi and between 0 and 1.5 phi.The sediments were lithic arenite to arkosic in composition and the microtextures imprinted over quartz grains were dominated by mechanical textures resulting from several stages of glacial crushing and grinding.The presence of deep dissolution cavities,cryptocrystalline precipitation,and euhedral crystal growth signified the effect of chemical activity after the deposition of grains in the lacustrine environment.Rajesh Asthana Prakash Kumar Shrivastava Hari Bahadur Srivastava Mirza Javed Beg Pradeep Kumar 2013Advances in Polar Science2013,24,4:2
6Role of PTPN2/22 polymorphisms in pathophysiology of Crohn's disease显示文摘AIM To establish the relationship of protein tyrosine phosphatase non-receptor type 2 and 22(PTPN2/22) polymorphisms and mycobacterial infections in Crohn's disease(CD). METHODS All 133 subjects' blood samples were genotyped for nine single nucleotide polymorphisms(SNPs) in PTPN2/22 using TaqMan^(?) genotyping, while the effect of the SNPs on PTPN2/22 and IFN-γ gene expression was determined using RT-PCR. Detection of Mycobacterium avium subspecies paratuberculosis(MAP) IS900 gene was done by nPCR after DNA extraction from the isolated leukocytes of each subjects' blood samples. T-cells isolated from the patient samples were tested for response to phytohematoagglutonin(PHA) mitogen or mycobacterial antigens by Brd U proliferation assays for T-cell activity. RESULTS Out of the nine SNPs examined, subjects with either heterozygous(TC)/minor(CC) alleles in PTPN2:rs478582 occurred in 83% of CD subjects compared to 61% healthy controls(P-values < 0.05; OR = 3.03). Subjects with either heterozygous(GA)/minor(AA) alleles in PTPN22:rs2476601 occurred in 16% of CD compared to 6% healthy controls(OR = 2.7). Gene expression in PTPN2/22 in CD subjects was significantly decreased by 2 folds compared to healthy controls(P-values < 0.05). IFN-γ expression levels were found to be significantly increased by approxiately 2 folds in subjects when either heterozygous or minor alleles in PTPN2:rs478582 and/or PTPN22:rs2476601 were found(P-values < 0.05). MAP DNA was detected in 61% of CD compared to only 8% of healthy controls(P-values < 0.05, OR = 17.52), where subjects with either heterozygous or minor alleles in PTPN2:rs478582 and/or PTPN22:rs2476601 had more MAPbacteremia presence than subjects without SNPs did. T h e average T-cell proliferation in CD treated with PHA or mycobacteria antigens was, respectively, 1.3 folds and 1.5 folds higher than healthy controls without any significant SNP. 8Improvement in the dissolution rate and tableting properties of cefuroxime axetil by melt-granulated dispersion and surface adsorption显示文摘A combination of melt-granulated dispersion and surface adsorption techniques was used to enhance the dissolution and tableting properties of cefuroxime axetil(CA).Gelucire 50/13 was used as the melt-dispersion carrier and Sylysia 350 was used to adsorb the melt dispersion.Solubility studies showed an 8-fold increase in solubility at a ratio of 1:1.5 for CA:Gelucire 50/13.The minimum quantity of Sylysia 350 required to achieve the desired flowability and compressibility was 0.5 parts of Sylysia 350 per unit of Gelucire 50/13.Phase solubility studies showed negative ΔG_(tr)^(0) values for Gelucire 50/13 at various concentrations(2-10%,vv/v),indicating the spontaneous nature of solubilization.FT-IR and DSC spectra exhibited drug-excipient compatibility.Molecular modeling by a computational method employing energy minimization revealed entrapment of CA in Gelucire 50/13.The total potential energy of CA(70.562 keal/mol)was reduced to 33.578 keal/mol after solid dispersion with Gelucire 50/13.P-XRD studies indicated that the presence of Sylysia 350 is less likely to promote the reversion of the amorphous CA to a crystalline state.In vitro dissolution studies demonstrated an improved dissolution rate,and drug release at 15 min(Q_(15min))exhibited a 15-fold improvement.The rapidly dissolving CA tablets showed improved dissolution with improved tableting properties.Jammula Sruti Chinam Niranjan Patra Suryakanta Swain Kanhu Charan Panigrahi Allupati Premananda Patro Sarwar Beg Subas Chandra Dinda Muddana Eswara Bhanoji Rao 2013Acta Pharmaceutica Sinica B2013,3,2:2
