| 1 | Non-invasive assessment of choledocholithiasis in patients with gallstones and abnormal liver function显示文摘AIM:To find a non-invasive strategy for detecting choledocholithiasis before cholecystectomy,with an acceptable negative rate of endoscopic retrograde cholangiopancreatography.METHODS:All patients with symptomatic gallstones were included in the study.Patients with abnormal liver functions and common bile duct abnormalities on ultrasound were referred for endoscopic retrograde cholangiopancreatography.Patients with normal ultrasoundwere referred to magnetic resonance cholangiopancreatography.All those who had a negative magnetic resonance or endoscopic retrograde cholangiopancreatography underwent laparoscopic cholecystectomy with intraoperative cholangiography.RESULTS:Seventy-eight point five percent of patients had laparoscopic cholecystectomy directly with no further investigations.Twenty-one point five percent had abnormal liver function tests,of which 52.8%had normal ultrasound results.This strategy avoided unnecessary magnetic resonance cholangiopancreatography in 47.2%of patients with abnormal liver function tests with a negative endoscopic retrograde cholangiopancreatography rate of 10%.It also avoided un-necessary endoscopic retrograde cholangiopancreatography in 35.2%of patients with abnormal liver function.CONCLUSION:This strategy reduces the cost of the routine use of magnetic resonance cholangiopancreatography,in the diagnosis and treatment of common bile duct stones before laparoscopic cholecystectomy. | Bilal O Al-Jiffry Abdeen Elfateh Tariq Chundrigar Bassem Othman Owaid AlMalki Fares Rayza Hashem Niyaz Hesham Elmakhzangy Mohammed Hatem | 2013 | World Journal of Gastroenterology2013,19,35: | 6 |
| 2 | Diagnostic non-invasive model of large risky esophageal varices in cirrhotic hepatitis C virus patients显示文摘AIM To build a diagnostic non-invasive model for screening of large varices in cirrhotic hepatitis C virus(HCV) patients. METHODS This study was conducted on 124 post-HCV cirrhotic patients presenting to the clinics of the Endemic Medicine Department at Mansoura University Hospital for evaluation before HCV antiviral therapy: 78 were Child A and 46 were Child B(score ≤ 8). Inclusion criteria for patients enrolled in this study was presence of cirrhotic HCV(diagnosed by either biopsy or fulfillment of clinical basis). Exclusion criteria consisted of patients with other etiologies of liver cirrhosis, e.g., hepatitis B virus and patients with high MELD score on transplant list. All patients were subjected to full medical record, full basic investigations, endoscopy, and computed tomography(CT), and then divided into groups with no varices, small varices, or large risky varices. In addition, values of Fibrosis-4 score(FIB-4), aminotransferase-to-platelet ratio index(APRI), and platelet count/splenic diameter ratio(PC/SD) were also calculated.RESULTS Detection of large varies is a multi-factorial process, affected by many variables. Choosing binary logistic regression, dependent factors were either large or small varices while independent factors included CT variables such coronary vein diameter, portal vein(PV) diameter, lieno-renal shunt and other laboratory noninvasive variables namely FIB-4, APRI, and platelet count/splenic diameter. Receiver operating characteristic(ROC) curve was plotted to determine the accuracy of non-invasive parameters for predicting the presence of large esophageal varices and the area under the ROC curve for each one of these parameters was obtained. A model was established and the best model for prediction of large risky esophageal varices used both PC/SD and PV diameter(75% accuracy), while the logistic model equation was shown to be(PV diameter ×-0.256) plus(PC/SD ×-0.006) plus(8.155). | 7 | A simplified two-dimensional boundary element method with arbitrary uniform mean flow显示文摘To reduce computational costs, an improved form of the frequency domain boundary element method(BEM) is proposed for two-dimensional radiation and propagation acoustic problems in a subsonic uniform flow with arbitrary orientation. The boundary integral equation(BIE) representation solves the two-dimensional convected Helmholtz equation(CHE) and its fundamental solution, which must satisfy a new Sommerfeld radiation condition(SRC) in the physical space. In order to facilitate conventional formulations, the variables of the advanced form are expressed only in terms of the acoustic pressure as well as its normal and tangential derivatives, and their multiplication operators are based on the convected Green's kernel and its modified derivative. The proposed approach significantly reduces the CPU times of classical computational codes for modeling acoustic domains with arbitrary mean flow.
