| 1 | Clinical Validation of Global Coagulation Tests to Guide Blood Component Transfusions in Cirrhosis and ACLF显示文摘Background and Aims:Patients with cirrhosis and acuteon-chronic liver failure(ACLF)may have bleeding complications and need for invasive procedures.Point-of-care(POC)coagulation tests like thromboelastography(TEG)and Sonoclot may be better for guiding patient management than the standard coagulation tests(SCTs),like prothrombin time,platelet count and international normalized ratio.Methods:We prospectively compared and validated the POC tests and SCTs in 70 persons with ACLF and 72 persons with decompensated cirrhosis who had clinical bleeding and checked for episodes of re-bleeding and transfusion requirements.We assessed pre-procedure requirement of blood components when correction was done based on an SCT or POC strategy.Results:Episodes of bleeding were seen in 45%and 28%of ACLF and cirrhosis patient,respectively(p=0.036),with the major site of bleeding being gastrointestinal(31%and 16%,respectively).Platelet counts correlated with TEG-maximum amplitude in cirrhosis(p=0.045)and prothrombin time correlated positively with TEG-reaction(R)time(p=0.032),TEG-Clot kinetics(K)time(p=0.042),Son-activated clotting time(p=0.038)and negatively with clot rate(p=0.043)in ACLF,making these correctable target variables in POC transfusion algorithms.Of 223 procedures,transfusion of fresh frozen plasma and platelet concentrate was reduced by 25%(p=0.035)and 20.8%(p=0.045)by using a POC strategy in 76 patients.Correction of deranged Son-activated clotting time and TEG-reaction time was noted in 68%and 72%after 24 h of fresh frozen plasma transfusion in ACLF and 85%and 80%in cirrhosis,respectively.Conclusions:Our study clinically validates that POC tests can better detect coagulation defects and transfusion thresholds in ACLF and cirrhosis,whereas use of conventional tests appear to be less suitable in patients with clinical bleeding.Trial Registration:NCT04332484. | Madhumita Premkumar Rohit Mehtani Smita Divyaveer Kamal Kajal Anand VKulkarni Syed Ahmed Harmanpreet Kaur Harpreet Kaur Radhakrishna Dhiman Ajay Duseja Arka De | 2021 | Journal of Clinical and Translational Hepatology2021,9,2: | 7 |

| 3 | Extraction of locked-up coal by strengthening of rib pillars with FRP--A comparative study through numerical modelling显示文摘In some of the coalfields in India, coal seams are only developed but no extraction of pillars is possible due to the presence of surface or sub-surface structures and also non-availability of stowing materials which leads to huge amounts of coal being locked-up underground. Spontaneous heating and fire, accumulation of poisonous gases, severe stability issues leading to unsafe workings and environmental hazards are the major problems associated with the developed coal pillars. So, there is a pressing need for a technology for the mining industry to extract the huge amount of coal locked-up under different constraints. In this study, the locked-up coal is proposed to be extracted by artificially strengthening the rib pillars. The detailed comparative study is carried out to know the increase of extraction percentage of locked-up coal by strengthening the rib pillars with FRP. | 4 | Free-space optical neural network based on thermal atomic nonlinearity显示文摘As artificial neural networks(ANNs)continue to make strides in wide-ranging and diverse fields of technology,the search for more efficient hardware implementations beyond conventional electronics is gaining traction.In particular,optical implementations potentially offer extraordinary gains in terms of speed and reduced energy consumption due to the intrinsic parallelism of free-space optics.At the same time,a physical nonlinearity—a crucial ingredient of an ANN—is not easy to realize in free-space optics,which restricts the potential of this platform.This problem is further exacerbated by the need to also perform the nonlinear activation in parallel for each data point to preserve the benefit of linear free-space optics.Here,we present a free-space optical ANN with diffraction-based linear weight summation and nonlinear activation enabled by the saturable absorption of thermal atoms.We demonstrate,via both simulation and experiment,image classification of handwritten digits using only a single layer and observed 6% improvement in classification accuracy due to the optical nonlinearity compared to a linear model.Our platform preserves the massive parallelism of free-space optics even with physical nonlinearity,and thus opens the way for novel designs and wider deployment of optical ANNs. | Albert Ryou James Whitehead Maksym Zhelyeznyakov Paul Anderson Cem Keskin Michal Bajcsy Arka Majumdar | 2021 | Photonics Research2021,9,4: | 2 |

| 10 | Evaluation of load transfer mechanism under axial loads in a novel coupler of dual height rock bolts显示文摘The effective reinforcement of two or more overlying layers of mine openings in a single installation is usually done by coupling of two standard rock bolts mainly during the extraction of medium-thick coal seams.However,field observations show that the couplers of multiple bolts often degrade or break mostly at their connections.These types of failures can be avoided by strengthening the couplers of such multi-bolts assemblies.To achieve this,a novel threaded coupler system with an expansion shell was suggested in this paper.The newly designed coupler consists of a threaded tapered-plug-cumconnector with an expansion shell for connecting and tightening two standard rock bolts.An analytical model for evaluating the load distribution along the coupler subject to axial load was derived.Numerical analysis was performed to analyse the load transfer,deformation,and strains across the coupler including the factor of safety for the bolt-coupler-resin and bolt-coupler-expansion shell.The results validated the analytical model of the proposed coupler design,which provides better anchorage near the interface of the host rock mass.Thus,the developed coupler design would reduce the failures of the proposed coupler and stabilize laminated roof strata above the medium-thick coal seams in underground mines. | Ranjan Kumar Prabhat Kumar Mandal Ashish Narayan Arka Jyoti Das | 2021 | International Journal of Mining Science and Technology2021,31,2: | 1 |
| 11 | Transcatheter pulmonic valve implantation: Techniques, current roles, and future implications显示文摘Right ventricular outflow tract(RVOT)obstruction is present in a variety of congenital heart disease states including tetralogy of Fallot,pulmonary atresia/stenosis and other conotruncal abnormalities etc.After surgical repair,these patients develop RVOT residual abnormalities of pulmonic stenosis and/or insufficiency of their native outflow tract or right ventricle to pulmonary artery conduit.There are also sequelae of other surgeries like the Ross operation for aortic valve disease that lead to right ventricle to pulmonary artery conduit dysfunction.Surgical pulmonic valve replacement(SPVR)has been the mainstay for these patients and is considered standard of care.Transcatheter pulmonic valve implantation(TPVI)was first reported in 2000 and has made strides as a comparable alternative to SPVR,being approved in the United States in 2010.We provide a comprehensive review in this space–indications for TPVI,detailed procedural facets and up-to-date review of the literature regarding outcomes of TPVI.TPVI has been shown to have favorable medium-term outcomes free of reinterventions especially after the adoption of the practice of pre-stenting the RVOT.Procedural mortality and complications are uncommon.With more experience,recognition of risk of dreaded outcomes like coronary compression has improved.Also,conduit rupture is increasingly being managed with transcatheter tools.Questions over endocarditis risk still prevail in the TPVI population.Head-to-head comparisons to SPVR are still limited but available data suggests equivalence.We also discuss newer valve technologies that have limited data currently and may have more applicability for treatment of native dysfunctional RVOT substrates. | Mark Aaron Law Arka Chatterjee | 2021 | World Journal of Cardiology2021,13,5: | 1 |
