| 1 | Differing coping mechanisms, stress level and anorectal physiology in patients with functional constipation显示文摘AIM: To investigate coping mechanisms, constipation symptoms and anorectal physiology in 80 constipated subjects and 18 controls.METHODS: Constipation was diagnosed by Rome Ⅱ criteria.Coping ability and anxiety/depression were assessed by validated questionnaires. Transit time and balloon distension test were performed.RESULTS: 34.5% patients were classified as slow transit type of constipation. The total colonic transit time (56 h vs 10 h, P<0.0001) and rectal sensation including urge sensation (79 mL vs 63 mL, P = 0.019) and maximum tolerable volume (110 mL vs95 mL, P = 0.03) differed in patients and controls. Constipated subjects had significantly higher anxiety and depression scores and lower SF-36 scores in all categories. They also demonstrated higher scores of'monitoring' coping strategy (14+6 vs9+3, P = 0.001),which correlated with the rectal distension sensation (P = 0.005), urge sensation (P=0.002), and maximum tolerable volume (P = 0.035). | 5 | Macrophage migration inhibitory factor stimulated by He/icobacter py/oriincreases proliferation of gastric epithelial cells显示文摘AIM: Helicobacter pylori (H pylori) is associated with increased gastric inflammatory and epithelial expression of macrophage migration inhibitory factor (MIF) and gastric epithelial cell proliferation. This study aimed at determining whether H pylori directly stimulates release of MIF in monocytes, whether the cay pathogenicity island (PAI) is involved for this function, and whether MIF stimulated by H pylori increases gastric epithelial cell proliferation in vitro.METHODS: A cytotoxic wild-type H pylori strain (TN2),its three isogenic mutants (TN2△cag, TN2△cagA and TN2△cagE) were co-cultured with cells of a human monocyte cell line, THP-1, for 24 h at different organism/cell ratios. MIF in the supernatants was measured by an ELISA. Cells of a human gastric cancer cell line, MKN45,were then co-cultured with the supernatants, with and without monoclonal anti-MIF antibody for 24 h. The cells were further incubated for 12 h after addition of 3H-thymidine, and the levels of incorporation of 3H-thymidine were measured with a liquid scintillation counter.RESULTS: The wild-type strain and the isogenic mutants,TN2△cagA and TN2△cagE, increased MIF release at organism/cell ratios of 200/1 and 400/1, but not at the ratios of 50/1 and 100/1. However, the mutant TN2△cag did not increase the release of MIF at any of the four ratios.3H-thymidine readings for MKN-45 cells were significantly increased with supernatants derived from the wild-type strain and the mutants TN2△cagA and TN2△cagE, but not from the mutant TN2△cag. Moreover, in the presence of monoclonal anti-MIF antibody, the stimulatory effects of the wild-type strain on cell proliferation disappeared.CONCLUSION: H pylori stimulates MIF release in monocytes, likely through its cag PAI, but not related to cagA or cagE. | 12 | Hepatocellular carcinoma in the elderly:Meta-analysis and systematic literature review显示文摘AIM: To conduct a meta-analysis to investigate theclinical outcomes of surgical resection and locoregional treatments for hepatocellular carcinoma(HCC) in elderly patients defined as aged 70 years or more. METHODS: Literature documenting a comparison of clinical outcomes for elderly and non elderly patients with hepatocellular carcinoma was identified by searching Pub Med, Ovid, Cochrane Library, and Web of Science databases, for those from inception to March 2015 with no limits. Dichotomous outcomes and standard meta-analysis techniques were used. Heterogeneity was tested by the Cochrane Q statistic. Pooled estimates were measured using the fixed or random effect model.RESULTS: Twenty three studies were included with a total of 12482 patients. Of these patients, 6341 were treated with surgical resection, 3138 were treated with radiofrequency ablation(RFA), and 3003 were treated with transarterial chemoembolization(TACE). Of the patients who underwent surgical resection, the elderly had significantly more respiratory co-morbidities than the younger group, with both groups having a similar proportion of cardiovascular co-morbidities and diabetes. After 1 year, the elderly group had significantly increased survival rates after surgical resection compared to the younger group(OR = 0.762, 95%CI: 0.583-0.994, P = 0.045). However, the 3-year and 5-year survival outcomes with surgical resection between the two groups were similar(OR = 0.947, 95%CI: 0.777-1.154, P = 0.67 for the third year; and OR = 1.131, 95%CI: 0.895-1.430, P = 0.304 for the fifth year). Postoperative treatment complications were similar between the elderly and younger group. The elderly group and younger group had similar survival outcomes for the first and third year after RFA(OR = 1.5, 95%CI: 0.788-2.885, P = 0.217 and OR = 1.352, 95%CI: 0.940-1.944, P = 0.104). For the fifth year, the elderly group had significantly worse survival rates compared to the younger group after RFA(OR = 1.379, 95%CI: 1.079-1.763, P = 0.01). For patients who underwent TACE, the elderly group had significantlyincreased survival compared to the younger group for the first and third year(OR = 0.664, 95%CI: 0.548-0.805, P = 0.00 and OR = 0.795, 95%CI: 0.663-0.953, P = 0.013). At the fifth year, there were no significant differences in overall survival between the elderly group and younger group(OR = 1.256, 95%CI: 0.806-1.957, P = 0.313). CONCLUSION: The optimal management strategy for elderly patients with HCC is dependent on patient and tumor characteristics. Compared to patients less than 70, elderly patients have similar three year survival after resection and ablation and an improved three year survival after TACE. At five years, elderly patients had a lower survival after ablation but similar survival with resection and TACE as compared to younger patients. Heterogeneity of patient populations and selection bias can explain some of these findings. | 15 | Isolation and characteristics of autoreactive T cells specific to aggrecan G1 domain from rheumatoid arthritis patients显示文摘Our previous work showed that the cartilage proteoglycan aggrecan could induce an erosive polyarthritis and spondylitis in BALB/c mice and the GI globular domain of the aggrecan (GI) contained the arthritogenic region. To elucidate whether autoreactive T cells to G1 are expressed in rheumatoid arthritis patients, we analyzed the frequency of human G1-specific T cells in the peripheral blood of five rheumatoid arthritis patients and tried to establish G1-reactive T cell lines from these rheumatoid arthritis patients. The results showed that the G1-specific T cells in PBL were detectable at the range of 4.97 ±0.5 ×10-6 in peripheral blood lymphocytes. We have also generated 15 G1-specific T lymphocyte lines from these pateints with a standard split-well method. All these cells expressed fine specificity to human recombinant G1, but not to unrelated antigen. All the 15 lines expressed a panT cell marker and 13 of them selectively used the αβ T cell receptor. Two of them used rye T cell receptor. The 13 of these T cell lines was CD4 positive. One line expressed CD8. One line expressed both CD4 and CD8. Moreover, 14 out of 15 lines expressed the Th-1 cytokine profile, characterized by interferon-γpositivity and IL-4 negativity. No Th-2 type cell line was generated. These data provide strong evidence in favor of the presence of autoreactive T cells in the rheumatoid arthritis pateints. | 16 | Assessment of liver ablation using cone beam computed tomography显示文摘AIM: To investigate the feasibility and accuracy of cone beam computed tomography(CBCT) in assessing the ablation zone after liver tumor ablation.METHODS: Twenty-three patients(17 men and 6 women, range: 45-85 years old, mean age 65 years) with malignant liver tumors underwent ultrasoundguided percutaneous tumor ablation [radiofrequency(n = 14), microwave(n = 9)] followed by intravenous contrast-enhanced CBCT. Baseline multidetector computed tomography(MDCT) and peri-procedural CBCT images were compared. CBCT image quality was assessed as poor, good, or excellent. Image fusion was performed to assess tumor coverage, and quality of fusion was rated as bad, good, or excellent. Ablation zone volumes on peri-procedural CBCT and post-procedural MDCT were compared using the nonparametric paired Wilcoxon t-test.RESULTS: Rate of primary ablation effectiveness was 100%. There were no complications related to ablation. Local tumor recurrence and new liver tumors were found 3 mo after initial treatment in one patient(4%). The ablation zone was identified in 21/23(91.3%) patients on CBCT. The fusion of baseline MDCT and peri-procedural CBCT images was feasible in all patients and showed satisfactory tumor coverage(at least 5-mm margin). CBCT image quality was poor, good, and excellent in 2(9%), 8(35%), and 13(56%), patients respectively. Registration quality between periprocedural CBCT and post-procedural MDCT images was good to excellent in 17/23(74%) patients. The median ablation volume on peri-procedural CBCT and post-procedural MDCT was 30 cm3(range: 4-95 cm3) and 30 cm3(range: 4-124 cm3), respectively(P-value > 0.2). There was a good correlation(r = 0.79) between the volumes of the two techniques. | 20 | Polymeric foams for flexible and highly sensitive low-pressure capacitive sensors显示文摘Flexible low-pressure sensors(<10 kPa)are required in areas as diverse as blood-pressure monitoring,human–computer interactions,robotics,and object detection.For applications,it is essential that these sensors combine flexibility,high sensitivity,robustness,and low production costs.Previous works involve surface micro-patterning,electronic amplification(OFET),and hydrogels.However,these solutions are limited as they involve complex processes,large bias voltages,large energy consumption,or are sensitive to evaporation.Here,we report a major advance to solve the challenge of scalable,efficient and robust e-skin.We present an unconventional capacitive sensor based on composite foam materials filled with conductive carbon black particles.Owing to the elastic buckling of the foam pores,the sensitivity exceeds 35 kPa^(−1) for pressure<0.2 kPa.These performances are one order of magnitude higher than the ones previously reported.These materials are low-cost,easy to prepare,and display high capacitance values,which are easy to measure using low-cost electronics.These materials pave the road for the implementation of e-skin in commercialized applications. | Mickaël Pruvost Wilbert J.Smit Cécile Monteux Philippe Poulin Annie Colin | 2019 | npj Flexible Electronics2019,3,1: | 5 |
