| 1 | Efficacy and safety of liquid nitrogen cryotherapy for treatment of Barrett's esophagus显示文摘AIM To evaluate the efficacy and safety of liquid nitrogen cryotherapy as a primary or rescue treatment for BE,with and without dysplasia,or intramucosal adenocarcinoma (IMC).METHODS This was a retrospective,single-center study carried out in a tertiary care center including 45 patients with BE who was treatment-na?ve or who had persistent intestinal metaplasia(IM),dysplasia,or IMC despite prior therapy.Barrett's mucosa was resected via EMR when clinically appropriate,then patients underwent cryotherapy until eradication or until deemed to have failed treatment.Surveillance biopsies were taken at standard intervals.RESULTS From 2010 through 2014,33 patients were studied regarding the efficacy of cryotherapy.Overall,29 patients (88%) responded to cryotherapy,with 84% having complete regression of all dysplasia and cancer.Complete eradication of cancer and dysplasia was seen in 75% of subjects with IMC; the remaining two subjects did not respond to cryotherapy.Following cryotherapy,15 patients with high-grade dysplasia (HGD) had 30% complete regression,50% IM,and 7% low-grade dysplasia (LGD); one subject had persistent HGD.Complete eradication of dysplasia occurred in all 5 patients with LGD.In 5 patients with IM,complete regression occurred in 4,and IM persisted in one.In 136 cryotherapy sessions amongst 45 patients,adverse events included chest pain (1%),stricture (4%),and one gastrointestinal bleed in a patient on dual antiplatelet therapy who had previously undergone EMR.CONCLUSION Cryotherapy is an efficacious and safe treatment modality for Barrett's esophagus with and without dysplasia or intramucosal adenocarcinoma. | Kristen Suchniak-Mussari Charles E Dye Matthew T Moyer Abraham Mathew Thomas J McGarrity Eileen M Gagliardi Jennifer L Maranki John M Levenick | 2017 | World Journal of Gastrointestinal Endoscopy2017，9，9： | 4 |
| 2 | Nonlocal-Means Image Denoising Technique Using Robust M-Estimator显示文摘Edge preserved smoothing techniques have gained importance for the purpose of image processing applications. A good edge preserving filter is given by nonlocal-means filter rather than any other linear model based approaches. This paper explores a different approach of nonlocal-means filter by using robust M-estimator function rather than the exponential function for its weight calculation. Here the filter output at each pixel is the weighted average of pixels with surrounding neighborhoods using the chosen robust M-estimator function. The main direction of this paper is to identify the best robust M-estimator function for nonlocal-means denoising algorithm. In order to speed up the computation, a new patch classification method is followed to eliminate the uncorrelated patches from the weighted averaging process. This patch classification approach compares favorably to existing techniques in respect of quality versus computational time. Validations using standard test images and brain atlas images have been analyzed and the results were compared with the other known methods. | 3 | Is there a weekend effect in hip fracture patients presenting to a United Kingdom teaching hospital?显示文摘AIM To compare mortality and time-to-surgery of patients admitted with hip fracture to our teaching hospital on weekdays vs weekends. METHODS Data was prospectively collected and retrospectively analysed for 816 hip fracture patients. Multivariate logistic regression was carried out on 3 binary outcomes(timeto-surgery < 36 h; 30-d mortality; 120-d mortality), using the explanatory variables time-of-admission; age; gender; American Society of Anesthesiologist(ASA) grade; abbreviated mental test score(AMTS); fracture type; accommodation admitted from; walking ability outdoors; accompaniment outdoors and season. RESULTS Baseline characteristics were not statistically different between those admitted on weekdays vs weekends. Weekend admission was not associated with an increased time-to-surgery(P = 0.975), 30-d mortality(P = 0.842) or 120-d mortality(P = 0.425). Gender(P = 0.028), ASA grade(P < 0.001), AMTS(P = 0.041) and accompaniment outdoors(P = 0.033) were significant covariates for 30-d mortality. Furthermore, age(P < 0.001),gender(P = 0.011), ASA grade(P < 0.001), AMTS(P < 0.001) and accompaniment outdoors(P = 0.033) all significantly influenced mortality at 120 d. ASA(P < 0.001) and season(P = 0.014) had significant effect on the odds of undergoing surgery in under 36 h.CONCLUSION Weekend admission was not associated with increased time-to-surgery or mortality in hip fracture patients.
