| 1 | Tourette syndrome associated with attention deficit hyperactivity disorder: The impact of tics and psychopharmacological treatment options显示文摘Tourette syndrome(TS) is a neurodevelopmental disorder characterized by multiple chronic motor and vocal tics beginning in childhood. Several studies describe the association between TS and attention deficit hyperactivity disorder(ADHD). Fifty percent of children diagnosed with ADHD have comorbid tic disorder. ADHD related symptoms have been reported in 35% to 90% of children with TS. Since ADHD is the most prevalent comorbid condition with TS and those with concomitant TS and ADHD present with considerable psychosocial and behavioral impairments, it is essential for clinicians to be familiar with these diagnoses and their management. This paper highlights the association between treating ADHD with stimulants and the development of tic disorders. The two cases discussed underscore the fact that children with TS may present with ADHD symptomatology prior to the appearance of any TS related symptoms. | 7 | Successful multidisciplinary clinical approach and molecular characterization by whole transcriptome sequencing of a cardiac myxofibrosarcoma: A case report显示文摘BACKGROUND Cardiac tumors are rare and complex entities.Surgery represents the cornerstone of therapy,while the role of adjuvant treatment remains unclear and,in case of relapse or metastatic disease,the prognosis is very poor.Lack of prospective,randomized clinical trials hinders the generation of high level evidence for the optimal diagnostic workup and multimodal treatment of cardiac sarcomas.Herein,we describe the multidisciplinary clinical management and molecular characterization of a rare case of cardiac myxofibrosarcoma in an elderly woman.CASE SUMMARY A 73-year-old woman presented signs and symptoms of acute left-sided heart failure.Imaging examination revealed a large,left atrial mass.With suspicion of a myxoma,she underwent surgery,and symptoms were promptly relieved.Histology showed a cardiac myxofibrosarcoma,a rare histotype of cardiac sarcoma.Eight months later,disease unfortunately relapsed,and after a multidisciplinary discussion,a chemotherapy with doxorubicin and then gemcitabine was started,achieving partial radiologic and complete metabolic response,which was maintained up to 2 years and is still present.This report is focused on the entire clinical path of our patient from diagnosis to follow-up,through surgery and strategies adopted at relapse.Moreover,due to their rarity,very little is known about the molecular landscape of myxofibrosarcomas.Thus,we also performed and described preliminary genome analysis of the tumor tissue to get further insight on mechanisms involved in tumor growth,and to possibly unveil new clinically actionable targets.CONCLUSION We report a case of cardiac myxofibrosarcoma that achieved a very good prognosis due to an integrated surgical,cardiac and oncologic treatment strategy. | Maristella Saponara Valentina Indio Carmine Pizzi Elena-Daniela Serban Milena Urbini Annalisa Astolfi Pasquale Paolisso Sofia Martin Suarez Margherita Nannini Davide Pacini Valentina Agostini Ornella Leone Valentina Ambrosini Giuseppe Tarantino Stefano Fanti Fabio Niro Francesco Buia Domenico Attinà Maria Abbondanza Pantaleo | 2019 | World Journal of Clinical Cases2019,7,19: | 2 |
| 8 | Effects of underground explosions on soil and structures显示文摘Much effort has been dedicated to the study of underground explosions because they pose a major threat to people and structures below or above the ground.In this regard,it is especially important to model the propagation of blast waves in soil and their effects on structures.The main phenomena caused by underground explosive detonation that must be addressed are crater or camouflet formation,shock wave and elastic–plastic wave propagation in soil,and soil-structure interaction.These phenomena can be numerically simulated using hydrocodes,but much care must be taken to obtain reliable results.The objective of this study is to analyze the ability of a hydrocode and simple soil models that do not require much calibration to approximately reproduce experimental and empirical results related to different buried blast events and to provide general guidelines for the simulation of this type of phenomena.In this regard,crater formation,soil ejecta,blast wave propagation in soil,and their effects on structures below and above the ground are numerically simulated using different soil models and parameters;the results are analyzed.The properties of soil have a significant effect on structures,the ejecta,and the propagation of shock waves in soil.Thus,the model of the soil to study these phenomena must be carefully selected.However their effect on the diameter of a crater is insignificant. | Daniel Ambrosini Bibiana Luccioni | 2020 | Underground Space2020,5,4: | 2 |
