1Model combining pre-transplant tumor biomarkers and tumor size shows more utility in predicting hepatocellular carcinoma recurrence and survival than the BALAD models显示文摘AIM To assess the performance of BALAD, BALAD-2 and their component biomarkers in predicting outcome of hepatocellular carcinoma(HCC) patients after liver transplant.METHODS BALAD score and BALAD-2 class are derived from bilirubin, albumin, alpha-fetoprotein(AFP), Lens culinaris agglutinin-reactive AFP(AFP-L3), and des-gammacarboxyprothrombin(DCP). Pre-transplant AFP, AFP-L3 and DCP were measured in 113 patients transplanted for HCC from 2000 to 2008. Hazard ratios(HR) for recurrence and death were calculated. Univariate and multivariate regression analyses were conducted. C-statistics were used to compare biomarker-based to predictive models. RESULTS During a median follow-up of 12.2 years, 38 patients recurred and 87 died. The HRs for recurrence in patients with elevated AFP, AFP-L3, and DCP defined by BALAD cut-off values were 2.42(1.18-5.00), 1.86(0.98-3.52), and 2.83(1.42-5.61), respectively. For BALAD, the HRs for recurrence and death per unit increased score were 1.48(1.15-1.91) and 1.59(1.28-1.97). For BALAD-2, the HRs for recurrence and death per unit increased class were 1.45(1.06-1.98) and 1.38(1.09-1.76). For recurrence prediction, the combination of three biomarkers had the highest c-statistic of 0.66 vs. 0.64, 0.61, 0.53, and 0.53 for BALAD, BALAD-2, Milan, and UCSF, respectively. Similarly, for death prediction, the combination of three biomarkers had the highest c-statistic of 0.66 vs 0.65,0.61, 0.52, and 0.50 for BALAD, BALAD-2, Milan, and UCSF. A new model combining biomarkers with tumor size at the time of transplant(S-LAD) demonstrated the highest predictive capability with c-statistics of 0.71 and 0.69 for recurrence and death. 2Aminoguanidine impedes human pancreatic tumor growth and metastasis development in nude mice显示文摘AIM:To study the action of aminoguanidine on pancreatic cancer xenografts in relation to cell proliferation,apoptosis,redox status and vascularization.METHODS:Xenografts of PANC-1 cells were developed in nude mice. The animals were separated into two groups:control and aminoguanidine treated. Tumor growth,survival and appearance of metastases were determined in vivo in both groups. Tumors were excised and ex vivo histochemical studies were performed. Cell growth was assessed by Ki-67 expression. Apoptosis was studied by intratumoral expression of B cell lymphoma-2 protein (Bcl-2) family proteins and Terminal deoxynucleotidyl transferase biotin-dUTP Nick End Labeling (Tunel). Redox status was evaluated by the expression of endothelial nitric oxide synthase (eNOS),catalase,copper-zinc superoxide dismutase (CuZnSOD),manganese superoxide dismutase (MnSOD) and glutathione peroxidase (GPx). Finally,vascularization was determined by Massons trichromic staining,and by VEGF and CD34 expression.RESULTS:Tumor volumes after 32 d of treatment by aminoguanidine (AG) were significantly lower than in control mice (P < 0.01). Median survival of AG mice was significantly greater than control animals (P < 0.01). The appearance of both homolateral and contralateral palpable metastases was significantly delayed in AG group. 6Association of homocysteine and smoking with cerebral microemboli in patients with mechanical heart valves:a transcranial Doppler study显示文摘Objectives Microembolic signals(MES)on transcranial Doppler(TCD)predict stroke and cognitive decline.Plasma levels of total homocysteine(tHcy),a prothrombotic factor,are higher in patients with microemboli in carotid stenosis and in patients with paradoxical embolism.In this study we assessed the association between the level of tHcy and the number of MES in patients with mechanical heart valves(MHVs).Methods TCD monitoring was performed to detect MES before and after breathing 100% oxygen and repeated every 2-4 weeks up to six times.results Twenty-five patients with MHVs(mean age:63.60±10.15 years)participated in this study;15 were men(66.47±7.25 years)and 10 were women(59.30±12.60 years).In total,there were 126 study visits.In multiple regression,higher tHcy was associated with more MES in both preoxygenation(OR 1.34(95%CI 1.07 to 1.68,P=0.009))and postoxygenation(OR 1.40(95% CI 1.07 to 1.83,P=0.01))phases.Current smoking and the length of time between the operation and monitoring also correlated with a higher number of MES before and after breathing oxygen,particularly in women.conclusions Higher tHcy and smoking were associated with a higher MES count in both preoxygenation and postoxygenation phases.Because smoking can be stopped and hyperhomocysteinaemia is treatable,these are clinically important findings.Alicia Mattia M Reza Azarpazhooh Claudio Munoz Chrysi Bogiatzi Mackenzie A Quantz J David Spence 2017Stroke & Vascular Neurology2017,2,4:2
