1Hepatocellular carcinoma: current management and recent advances显示文摘BACKGROUND: Hepatocellular carcinoma (HCC) is a major health problem worldwide. It is the fifth most common cancer in the world, and the third most common cause of cancer-related death. Without specific treatment, the prognosis is very poor. The goal of management is 'cancer control' - a reduction in its incidence and mortality as well as an improvement in the quality of life of patients with HCC and their families. This article aims to review the current management of HCC and its recent advances. DATA SOURCES: A MEDLINE database search was performed to identify relevant article using the keyworks 'hepatocellular carcinoma', 'hepatectomy', 'liver transplantation', and 'local ablative therapy'. Additional papers and book chapters were identified by a manual search of the references from the key articles. RESULTS: Liver resection and liver transplantation remain the options that give the best chance of a cure. Recent evidence suggests that local ablative therapy may offer comparable survival results in patients with small HCC, and preserved liver function. Transarterial chemoembolization (TACE) is the most promising palliative modality for unresectable HCC, but other techniques, such as transarterial radioembolization (TARE), and local ablative therapy, have also shown comparable results. CONCLUSIONS: Early diagnosis of HCC remains a key goal in improving the prognosis of patients. During the last two decades, operative mortality and surgical outcome of liver resection and liver transplantation for HCC have improved. Progress also has been made in multi-modality therapy which can increase the chance of survival and improve the quality of life for patients with advanced HCC.Wan-Yee Lau Eric C. H. 2Differing coping mechanisms, stress level and anorectal physiology in patients with functional constipation显示文摘AIM: To investigate coping mechanisms, constipation symptoms and anorectal physiology in 80 constipated subjects and 18 controls.METHODS: Constipation was diagnosed by Rome Ⅱ criteria.Coping ability and anxiety/depression were assessed by validated questionnaires. Transit time and balloon distension test were performed.RESULTS: 34.5% patients were classified as slow transit type of constipation. The total colonic transit time (56 h vs 10 h, P<0.0001) and rectal sensation including urge sensation (79 mL vs 63 mL, P = 0.019) and maximum tolerable volume (110 mL vs95 mL, P = 0.03) differed in patients and controls. Constipated subjects had significantly higher anxiety and depression scores and lower SF-36 scores in all categories. They also demonstrated higher scores of'monitoring' coping strategy (14+6 vs9+3, P = 0.001),which correlated with the rectal distension sensation (P = 0.005), urge sensation (P=0.002), and maximum tolerable volume (P = 0.035). 7New geological evidence of crustal thickening in the Gangdese block prior to the Indo-Asian collision显示文摘Recent mapping in the Gangdese block has revealed many leucogranites that are similar to those in the High Himalaya. These leucogranites formed at ~140 Ma as indicated by monazite Th-Pb ion-microprobe dating and cooled at ~130 Ma as indicated by muscovite 40Ar/39Ar dating. In conjunction with previous structural and paleogeographic studies, the new data indicate that the Gangdese block underwent crustal thickening and associated exhumation during ~140-130 Ma. In this regard, the southern margin of Eurasia continent was comparable to the modern South American Altiplano-Puna plateau, the prime example of active ocean-continent subduction and associated thickened crust. Specifically, the early stages of crustal thickening and uplifting of the Gangdese block may result from subduction of the Neo-Tethyan Ocean. If the Tibetan Plateau would form by accretion of a series of blocks with thickened crust, an elevated topographic plateau similar to the Altiplano- Puna plateau had formed before collision between the Indian and Eurasian plates. Then the Tibetan Plateau would have quickly thickened, uplifted, and begun to extend soon after onset of the collision. 9Natural history of hepatitis-related hepatocellular carcinoma显示文摘Hepatocellular carcinoma(HCC)is an important cause of cancer death in the world.It has great regional differences in the pathology and epidemiology.The variation is greatly influenced by the aetiologies of the disease.Hepatitis B and C infection are the most important risk factors.HCC incidence rates are higher but in decreasing trend in developing countries. However,the figures in the developed countries are contrary.Successful hepatitis B virus(HBV)vaccination programs,better food hygiene,increased global hepatitis C virus(HCV)prevalence and population migration are the possible explanations.A number of clinical and pathogenic differences exist between HBV- and HCV- related HCC.HBV infection leads to the development of HCC through direct and indirect pathways as it has the ability to integrate into the host genome affecting cellular signaling and growth control.HCV causes HCC mainly through indirect pathways:chronic inflammation, cell deaths and proliferation.As a result,HCC is almost exclusively found in cirrhotic HCV patients while HCC is sometimes found in HBV patients without significant liver cirrhosis.Due to the different severities of liver cirrhosis and HCC extent,therapeutic strategies from resection, liver transplantation to symptoms palliation are available. 14Anticoagulation therapy prevents portal-splenic vein thrombosis after splenectomy with gastroesophageal devascularization显示文摘AIM:To compare the incidence of early portal or splenic vein thrombosis(PSVT) in patients treated with irregular and regular anticoagulantion after splenectomy with gastroesophageal devascularization.METHODS:We retrospectively analyzed 301 patients who underwent splenectomy with gastroesophageal devascularization for portal hypertension due to cirrhosis between April 2004 and July 2010.Patients were categorized into group A with irregular anticoagulation and group B with regular anticoagulation,respectively.Group A(153 patients) received anticoagulant monotherapy for an undesignated time period or with aspirin or warfarin without low-molecular-weight heparin(LMWH) irregularly.Group B(148 patients) received subcutaneous injection of LMWH routinely within the first 5 d after surgery,followed by oral warfarin and aspirin for one month regularly.The target prothrombin time/international normalized ratio(PT/INR) was 1.25-1.50.Platelet and PT/INR were monitored.Color Doppler imaging was performed to monitor PSVT as well as the effectiveness of thrombolytic therapy.RESULTS:The patients' data were collected and analyzed retrospectively.Among the patients,94 developed early postoperative mural PSVT,including 63 patients in group A(63/153,41.17%) and 31 patients in group B(31/148,20.94%).There were 50(32.67%) patients in group A and 27(18.24%) in group B with mural PSVT in the main trunk of portal vein.After the administration of thrombolytic,anticoagulant and antiaggregation therapy,complete or partial thrombus dissolution achieved in 50(79.37%) in group A and 26(83.87%) in group B.CONCLUSION:Regular anticoagulation therapy can reduce the incidence of PSVT in patients who undergo splenectomy with gastroesophageal devascularization,and regular anticoagulant therapy is safer and more effective than irregular anticoagulant therapy.Early and timely thrombolytic therapy is imperative and feasible for the prevention of PSVT.Wei Lai Shi-Chun Lu Guan-Yin Li Chuan-Yun Li Ju-Shan Wu Qing-Liang Guo Meng-Long Wang Ning Li 2012World Journal of Gastroenterology2012,18,26:46
15A preliminary study of rare-metal mineralization in the Himalayan leucogranite belts, South Tibet显示文摘The Himalayan leucogranite occurs as two extensive(>1000 km) E-W trending belts on the Tibetan Plateau with the unique features. The leucogranite comprised biotite granite, two-mica/muscovite granite, tourmaline granite and garnet granite, which have been identified in previous studies, as well as albite granite and granitic pegmatite that were identified in this investigation. Fifteen leucogranite plutons were studied and 12 were found to contain rare-metal bearing minerals such as beryl(the representative of Be mineralization), columbite-group minerals, tapiolite, pyrochlore-microlite, fergusonite, Nb-Ta rutile(the representative of Nb-Ta mineralization), and cassiterite(the representative of Sn mineralization) mainly based on the field trip,microscope observation and microprobe analysis. The preliminary result shows that the Himalayan leucogranite is commonly related to the rare-metal mineralization and warrants future investigation. 16Clinical and procedural predictors of no-ref low in patients with acute myocardial infarction after primary percutaneous coronary intervention显示文摘BACKGROUND: The treatment of acute myocardial infarction(AMI) is thought to restore antegrade blood flow in the infarct-related artery(IRA) and minimize ischemic damage to the myocardium as soon as possible. The present study aimed to identify possible clinical predictors for no-refl ow in patients with AMI after primary percutaneous coronary intervention(PCI).METHODS: A total of 312 consecutive patients with AMI who had been treated from January 2008 to December 2010 at the Cardiology Department of East Hospital, Tongji University School of Medicine were enrolled in this study. Inclusion criteria were:(i) patients underwent successfully primary PCI within 12 hours after the appearance of symptoms; or(ii) patients with ischemic chest pain for more than 12 hours after a successful primary PCI within 24 hours after appearance of symptoms. Exculsion criteria were:(i) coronary artery spasm;(ii) diameter stenosis of the culprit lesion was ≤50% and coronary blood f low was normal;(iii) patients with severe left main coronary or multivessel disease, who had to require emergency revascularization. According to thrombolysis in myocardial infarction(TIMI), the patients were divided into a reflow group and a no-reflow group. The clinical data, angiography f indings and surgical data were compared between the two groups. Univariate and multivariate logistic regressions were used to determine the predictors for no-ref low.RESULTS: Fifty-four(17.3%) of the patients developed NR phenomenon after primary PCI. Univariate analysis showed that age, time from onset to reperfusion, systolic blood pressure(SBP) on admission, Killip class of myocardial infarction, intra-aortic balloon pump(IABP) use before primary PCI, TIMI flow grade before primary PCI, type of occlusion, thrombus burden on baseline angiography, target lesion length, reference luminal diameter and method of reperfusion were correlated with no-reflow(P<0.05 for all). Multiple logistic regression analysis identified that age >65 years [OR=1.470, 95% confi dence interval(CI) 1.460–1.490, P=0.007], long time from onset to reperfusion >6 hours(OR=1.270, 95%CI 1.160–1.400, P=0.001), low SBP on admission <100 mmHg(OR=1.910, 95%CI 1.018–3.896, P=0.004), IABP use before PCI(OR= 1.949, 95%CI 1.168–3.253, P=0.011), low(≤1) TIMI fl ow grade before primary PCI(OR=1.100, 95%CI 1.080–1.250, P<0.001), high thrombus burden(OR=1.600, 95%CI 1.470–2.760, P=0.030), and long target lesion(OR=1.948, 95%CI 1.908–1.990, P=0.019) on angiography were independent predictors of no-refl ow.CONCLUSION: The occurrence of no-refl ow after primary PCI for acute myocardial infarction can predict clinical, angiographic and procedural features.Hua Zhou Xiao-yan He Shao-wei Zhuang Juan Wang Yan Lai Wei-gang Qi Yi-an Yao Xue-bo Liu 2014World Journal of Emergency Medicine2014,5,2:46
17Mechanism underlying treatment of ischemic stroke using acupuncture:transmission and regulation显示文摘The inflammatory response after cerebral ischemia/reperfusion is an important cause of neurological damage and repair.After cerebral ischemia/reperfusion,microglia are activated,and a large number of circulating inflammatory cells infiltrate the affected area.This leads to the secretion of inflammatory mediators and an inflammatory cascade that eventually causes secondary brain damage,including neuron necrosis,blood-brain barrier destruction,cerebral edema,and an oxidative stress response.Activation of inflammatory signaling pathways plays a key role in the pathological process of ischemic stroke.Increasing evidence suggests that acupuncture can reduce the inflammatory response after cerebral ischemia/reperfusion and promote repair of the injured nervous system.Acupuncture can not only inhibit the activation and infiltration of inflammatory cells,but can also regulate the expression of inflammation-related cytokines,balance the effects of pro-inflammatory and anti-inflammatory factors,and interfere with inflammatory signaling pathways.Therefore,it is important to study the transmission and regulatory mechanism of inflammatory signaling pathways after acupuncture treatment for cerebral ischemia/reperfusion injury to provide a theoretical basis for clinical treatment of this type of injury using acupuncture.Our review summarizes the overall conditions of inflammatory cells,mediators,and pathways after cerebral ischemia/reperfusion,and discusses the possible synergistic intervention of acupuncture in the inflammatory signaling pathway network to provide a foundation to explore the multiple molecular mechanisms by which acupuncture promotes nerve function restoration.Bing-Qian Cao Feng Tan Jie Zhan Peng-Hui Lai 2021Neural Regeneration Research2021,16,5:43
