| 2 | Evaluation of modifi ed Alvarado scoring system and RIPASA scoring system as diagnostic tools of acute appendicitis显示文摘BACKGROUND: Acute appendicitis is the most common surgical condition presented in emergency departments worldwide. Clinical scoring systems, such as the Alvarado and modified Alvarado scoring systems, were developed with the goal of reducing the negative appendectomy rate to 5%–10%. The Raja Isteri Pengiran Anak Saleha Appendicitis(RIPASA) scoring system was established in 2008 specif ically for Asian populations. The aim of this study was to compare the modif ied Alvarado with the RIPASA scoring system in Kuwait population.METHODS: This study included 180 patients who underwent appendectomies and were documented as having 'acute appendicitis' or 'abdominal pain' in the operating theatre logbook(unit B) from November 2014 to March 2016. The sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), diagnostic accuracy, predicted negative appendectomy and receiver operating characteristic(ROC) curve of the modified Alvarado and RIPASA scoring systems were derived using SPSS statistical software.RESULTS: A total of 136 patients were included in this study according to our criteria. The cut-off threshold point of the modif ied Alvarado score was set at 7.0, which yielded a sensitivity of 82.8% and a specif icity of 56%. The PPV was 89.3% and the NPV was 42.4%. The cut-off threshold point of the RIPASA score was set at 7.5, which yielded a 94.5% sensitivity and an 88% specif icity. The PPV was 97.2% and the NPV was 78.5%. The predicted negative appendectomy rates were 10.7% and 2.2% for the modif ied Alvarado and RIPASA scoring systems, respectively. The negative appendectomy rate decreased significantly, from 18.4% to 10.7% for the modified Alvarado, and to 2.2% for the RIPASA scoring system, which was a signif icant difference(P<0.001) for both scoring systems.CONCLUSION: Based on the results of this study, the RIPASA score is a simple scoring system with better sensitivity and specif icity than the modif ied Alvarado scoring system in Asian populations. It consists of 14 clinical parameters that can be obtained from a good patient history, clinical examination and laboratory investigations. | 5 | Landslide mapping and analysis along the China-Pakistan Karakoram Highway based on SBAS-InSAR detection in 2017显示文摘The Karakoram Highway(KKH),a part of the China–Pakistan Economic Corridor(CPEC),is a major highway connecting northern Pakistan to China.The inventorying and analysis of landslides along KKH are challenging because of poor accessibility,vast study area,limited availability of ground-based datasets,and the complexity of landslide processes in the region.In order to preserve life,property,and infrastructure,and to enable the uninterrupted and efficient operation of the KKH,it is essential to strengthen measures for the prevention and control of geological disasters.In the present study,SBASInSAR(Small Baseline Subsets-Interferometric Synthetic Aperture Radar)was used to process 150 scenes of Sentinel 1-A images in the year 2017 along the Karakoram Highway.A total of 762 landslides,including 57 complex landslides,126 rock falls,167 debris slides,and 412 unstable slopes,ranging in size between 0.0017 and 10.63 km2 were identified.Moreover,this study also gains an inventory of 40 active glacier movements in this region.Landslide categorization,displacements characteristics,spatial distribution,and their relationship with various contributing factors have been successfully investigated along the entire KKH using image interpretation and frequency-area statistics.The criteria adopted for landslides categorization is presented in the study.The results showed that the 2-D ground deformation derived in Hunza valley echoes well with the general regional landslides characteristics.The spatial distribution analysis revealed that there are clumped distributions of landslides in the Gaizi,Tashkurgan,and Khunjerab in China,as well as in Hunza valley,and north of Chilas city in Pakistan.Statistical results indicated that these landslides mainly occur on south-facing slopes with a slope angle of 20°–45°and elevation relief of 550–2,100 m.Landslide development is also related to low vegetation cover and weathering effects in mountain gullies.Overall,our study provides scientific data support and theoretical references for prevention,control,and mitigation of geological disasters in the Karakoram region. | SU Xiao-jun ZHANG Yi MENG Xing-min YUE Dong-xia MA Jin-hui GUO Fu-yun ZHOU Zi-qiang REHMAN Mohib Ur KHALID Zainab CHEN Guan ZENG Run-qiang ZHAO Fu-meng | 2021 | Journal of Mountain Science2021,18,10: | 4 |
| 6 | Micro RNAs and clinical implications in hepatocellular carcinoma显示文摘AIM To assess the role of some circulating miRNAs(miR-23a, miR-203, miR338, miR-34, and miR-16) as tumor markers for diagnosis of hepatocellular carcinoma(HCC).METHODS One hundred and seventy-one subjects were enrolled, 57 patients with HCC, 57 patients with liver cirrhosis(LC) and 57 healthy subjects as control group. Severity of liver disease was assessed by Child Pugh score. Tumor staging was done using Okuda staging system. Quantification of Micro RNA(miR-23a, miR-203, miR338, miR-34, and miR-16) was performed.RESULTS All studied miRNA showed significant difference between HCC and cirrhotic patients in comparison tohealthy control. miR-23a showed statistically significant difference between HCC and cirrhotic patients being higher in HCC group than cirrhotic. miR-23a is significantly higher in HCC patients with focal lesion size equal or more than 5cm, patients with multiple focal lesions and Okuda stage Ⅲ. At cutoff value ≥ 210, miR-23a showed accuracy 79.3% to diagnose HCC patients with sensitivity 89.47% and specificity about 64.91%. At cut off level ≥ 200 ng/mL, serum alpha fetoprotein had 73.68% sensitivity, 52.63% specificity, 43.75% PPV, 80% NPV for diagnosis of HCC.CONCLUSION MicroR NA 23a can be used as a screening test for early detection of HCC. | 7 | Mutagenic and cytotoxic potential of Endosulfan and Lambda-cyhalothrin—In vitro study describing individual and combined effects of pesticides显示文摘Excessive use of pesticides poses increased risks to non target species including humans. In the developing countries, lack of proper awareness about the toxic potential of pesticides makes the farmer more vulnerable to pesticide linked toxicities, which could lead to diverse pathological conditions. The toxic potential of a pesticide could be determined by their ability to induce genetic mutations and cytotoxicity. Hence, determination of genetic mutation and cytotoxicity of each pesticide is unavoidable to legislate health and safety appraisal about pesticides. The objective of current investigation was to determine the genotoxic and cytotoxic potential of Endosulfan(EN) and Lambda-cyhalothrin(LC); individually and in combination. 3-(4,5-Dimethylthiazol-2-yl)-2,5-Diphenyltetrazolium Bromide(MTT) assay was utilized to determine cytotoxicity, while two mutant histidine dependent Salmonella strains(TA98, TA100) were used to determine the mutagenicity of EN and LC.Moreover, mutagenicity assay was conducted with and without S9 to evaluate the effects of metabolic activation on mutagenicity. Even though a dose dependent increase in the number of revertant colonies was detected with EN against both bacterial strains, a highly significant(p < 0.05) increase in the mutagenicity was detected in TA98 with S9. In comparison, data obtained from LC revealed less mutagenic potential than EN. Surprisingly,the non-mutagenic individual-concentrations of EN and LC showed dose dependent mutagenicity when combined. Combination of EN and LC synergistically induced mutagenicity both in TA98 and TA100. MTT assay spotlighted comparable dose dependent cytotoxicity effects of both pesticides. | 8 | Cytogenetic and Gene Expression Study in Radiation Workers Occupationally Exposed to Low Levels of Ionizing Radiation显示文摘Background:Exposure to IR(ionizing radiation)causes damage to living cells,especially to DNA,the degree of cellular damage depends on the radiation amount.Humans are naturally exposed to IR from cosmic rays,and artificially through diagnostic procedures,medical treatments or occupationally during work shifts.Cytogenetic assay of peripheral blood lymphocytes can provide a biological estimation of the dose received in IR exposures.The CBMN(cytokinesis-blocked micronucleus)assay is a widely used,since it represents a reliable test to detect radiation-induced chromosome aberration and it is a valuable biomarker in many biomonitoring studies on human populations occupationally exposed to IR.The NDI(nuclear division index)is a cell proliferation marker in cultures which is considered a measure of general cytotoxicity,the relative frequencies of the cells may be used to define cell cycles progression of the lymphocyte after mitogenic stimulation.Nevertheless the assay is frequently employed as a useful research tool for understanding the cell cycling kinetics of the cultures.Many gene expression studies demonstrated an up-regulation of genes involved in the processes of signal transduction,control of cell cycle,DNA repair and apoptosis after exposure of IR in different mammalian cell types.Altered expression of a few genes plays specific roles in DNA repair/cell cycle control such as CDKN1A,XPC,GADD45A,DDB2 and PCNA and cell cycle regulation/proliferation such as IL16,CABLES2,TGFB2 and RHOA.Objective:The present study aims to use the MN,NDI and gene expression analysis as biomarkers for investigation of the effects of IR exposure in some radiation workers occupationally exposed to low IR in Al-Tuwaitha site,Baghdad,Iraq.Also,authors assessed the effect of IR on the expression of some gene marker such as:RHOA,CDKN1A,GADD45A and RAD52.Methods:This study was carried out on thirty Iraqi male radiation workers occupationally exposed to low levels of IR at Al-Tuwaitha site,Baghdad,non-smokers and non-alcoholic,aged 30-59 years,as well as thirty apparently healthy individuals males collected randomly from population living in Baghdad,aged 30-59 years who are non-smokers,non-alcoholic as control group.Three molecular genetic parameters were employed such as CBMN,NDI and gene expression assay.The MN and NDI assay were performed according to the description by IAEA,2001.Total RNA was isolated from blood for radiation worker and control group.The RNA concentration was determined by measuring their absorbance that depends on the ratio A260/A280 of the wavelength,which leads to the determination of RNA purity,which ranged from 1.79-2.1 in two groups.Using RT-PCR for study gene expression,four types of specialized primer genes were selected for the mRNA genes:RHOA,CDKN1A,GADD45A and RAD52 which have a relation with IR in addition to the primers for internal control.Housekeeping gene((β-actin)was used as a reference gene to normalize the quantity of the target genes.Results:The results of cytogenetic analysis showed that micronuclei frequencies were significantly higher(p<0.01)in the radiation workers group in Al-Tuwaithasite,Baghdadas compared with the control group,and showed significant decrease(p>0.01)in the NDI in radiation workers as compared with the control group.Total RNA was isolated from blood for the radiation workers and control groups mentioned.Using RT-PCR for study gene expression,four types of specialized primer genes were selected for the mRNA:genes RHOA,CDKN1A,GADD45A and RAD52 which have a relation with IR in addition to the primers for internal control(β-actin)gene.The products of replicated specialized primers for the genes concerned and the cDNA for the studied samples were electrophoretically separated in agarose gels.The banding profiles were visualized by ethidium bromide staining,as the molecular weight were 135 bp,165 bp,185 bp and 470 bp(nitrogen-base pair)for RHOA,CDKN1A,GADD45A and RAD52 genes,respectively.Gene expression analysis revealed statistically significant(ΔΔCt comparative Ct method)transcriptional changes in three genes RHOA,GADD45A,CDKN1A up-regulated while the RAD52 gene down-regulated.Conclusions:The results indicated that there is a possibility of using the changes in the MN,NDI and genes expression such as RHOA,CDKN1A,GADD45A and RAD52 as useful biomarkers for detection of IR in radiation workers occupationally exposed to low levels of IR. | Abdulsahib.K.Ali.Al-Ziyad Amel J.Mutar Zainab Abbas Ouda Shatha K.Khyon | 2021 | Journal of US-China Medical Science2021,18,1: | 2 |

| 20 | Application and circular economy prospects of palm oil waste for eco-friendly asphalt pavement industry:A review显示文摘During the production of palm oil,a significant amount of waste is generated.However,because of inefficient handling and utilization,these wastes are becoming a larger issue.As a result,one initiative is to use these wastes in the pavement industry as sustainable materials.However,there is still a lack of understanding about the wider incorporation of palm oil waste in asphalt pavement and its performance.This study examines existing literature on the use of various wastes in the pavement industry,including palm oil clinker(POC),palm oil fibre(POF),palm kernel shell(PKS),and palm oil fuel ash(POFA).As a result,this paper presents a systematic review and scientometric investigation of related study publications on many uses of palm oil waste in the asphalt pavement industry and its performance from 2009 to 2022.The VOS viewer application was used to conduct the sciento-metric study analysis.The relationship between interactions detected in co-authored country studies cited sources of co-citation,and the keyword of the co-occurrence and publication source enabled the identification of the research gap.According to the systematic literature review,40%–60% POC can be used to fine aggregate for optimal performance,while 0–100%PKS can be used to replace coarse aggregate.In addition,50%–80% POFA or POC fine(POCF)can be used as a filler replacement,5%–8% POCF or POFA as a bitumen modifier,and 0.3% POF as a stabilizing additive.Furthermore,the study demonstrates that the safety of utilizing wastes with more than 50% CO_(2) emissions can be curtailed with minimal heavy metal leaching and radioactivity levels.The scientometric analysis may encourage researchers to seek out gaps in the literature that will aid in the long-term,multifaceted use of palm oil wastes in the asphalt pavement industry.Furthermore,the study recommends employing and researching the enormous potential of using palm oil waste in the pavement sectors because they are more sustainable and have better performance.However,there are some barriers to using palm oil waste in the asphalt pavement industry,such as a lack of design standards and guidelines,inefficient raw material pro-cessing conversion facilities,and large-scale production equipment. | Nura Shehu Aliyu Yaro Muslich Hartadi Sutanto Noor Zainab Habib Madzlan Napiah Aliyu Usman Ahmad Hussaini Jagaba Abdulnaser M.Al-Sabaeei | 2022 | Journal of Road Engineering2022,2,4: | 1 |
