| 2 | Efficacy and safety of anti-hepatic fibrosis drugs显示文摘Recent progress in our understanding of the pathways linked to progression from hepatic insult to cirrhosis has led to numerous novel therapies being investigated as potential cures and inhibitors of hepatic fibrogenesis. Liver cirrhosis is the final result of prolonged fibrosis, which is an intimate balance between fibrogenesis and fibrinolysis. A number of these complex mechanisms are shared across the various etiologies of liver disease. Thankfully, investigation has yielded some promising results in regard to reversal of fibrosis, particularly the indirect benefits associated with antiviral therapy for the treatment of hepatitis B and C and the farnesoid receptor agonist for the treatment of primary biliary cholangitis and metabolic associated fatty liver disease. A majority of current clinical research is focused on targeting metabolic associated fatty liver disease and its progression to metabolic steatohepatitis and ultimately cirrhosis, with some hope of potential standardized therapeutics in the near future. With our ever-evolving understanding of the underlying pathophysiology, these therapeutics focus on either controlling the primary disease(the initial trigger of fibrogenesis), interrupting receptor ligand interactions and other intracellular communications, inhibiting fibrogenesis, or even promoting resolution of fibrosis. It is imperative to thoroughly test these potential therapies with the rigorous standards of clinical therapeutic trials in order to ensure the highest standards of patient safety. | 6 | The efficacy of tucatinib-based therapeutic approaches for HER2-positive breast cancer显示文摘Overexpression of human epidermal growth factor receptor 2 (HER2) occurs in approximately 15–20% of breast cancer cases. HER2 is a member of the epidermal growth factor receptor (EGFR) family with tyrosinase kinase activity, and its overexpression is linked to poor prognosis and shorter progression-free survival (PFS) and overall survival (OS). Among various treatment options, HER2-targeting monoclonal antibodies and tyrosine kinase inhibitors (TKIs) have mostly been applied in recent decades to treat HER2-positive (HER2^(+)) breast cancer patients. Although positive clinical outcomes were documented in both advanced disease and neoadjuvant settings, the development of resistance mechanisms to such approaches has been one of the major challenges with the continuous usage of these drugs. In addition, patients who experience disease progression after treatment with multiple HER2-targeted therapies often have limited treatment options. The Food and Drug Administration (FDA) has recently approved a new TKI (i.e., tucatinib) for use in combination with immunotherapy and/or chemotherapeutic agents for the treatment of advanced-stage/metastatic HER2^(+) breast cancer. | 11 | Therapeutic efficiency of bone marrow-derived mesenchymal stem cells for liver fibrosis:A systematic review of in vivo studies显示文摘Although multiple drugs are accessible for recovering liver function in patients,none are considered efficient.Liver transplantation is the mainstay therapy for end-stage liver fibrosis.However,the worldwide shortage of healthy liver donors,organ rejection,complex surgery,and high costs are prompting researchers to develop novel approaches to deal with the overwhelming liver fibrosis cases.Mesenchymal stem cell(MSC)therapy is an emerging alternative method for treating patients with liver fibrosis.However,many aspects of this therapy remain unclear,such as the efficiency compared to conventional treatment,the ideal MSC sources,and the most effective way to use it.Because bone marrow(BM)is the largest source for MSCs,this paper used a systematic review approach to study the therapeutic efficiency of MSCs against liver fibrosis and related factors.We systematically searched multiple published articles to identify studies involving liver fibrosis and BM-MSC-based therapy.Analyzing the selected studies showed that compared with conventional treatment BM-MSC therapy may be more efficient for liver fibrosis in some cases.In contrast,the cotreatment presented a more efficient way.Nevertheless,BM-MSCs are lacking as a therapy for liver fibrosis;thus,this paper also reviews factors that affect BM-MSC efficiency,such as the implementation routes and strategies employed to enhance the potential in alleviating liver fibrosis.Ultimately,our review summarizes the recent advances in the BM-MSC therapy for liver fibrosis.It is grounded in recent developments underlying the efficiency of BM-MSCs as therapy,focusing on the preclinical in vivo experiments,and comparing to other treatments or sources and the strategies used to enhance its potential while mentioning the research gaps. | Zaid Al-Dhamin Ling-Di Liu Dong-Dong Li Si-Yu Zhang Shi-Ming Dong Yue-Min Nan | 2020 | World Journal of Gastroenterology2020,26,47: | 2 |
