| 4 | Submucosal tunneling and endoscopic resection of submucosal tumors at the esophagogastric junction显示文摘AIM: To evaluate the safety and efficacy of submucosal tunneling and endoscopic resection(STER) for treating submucosal tumors(SMTs).METHODS: Between August 2012 and October 2013, 21 patients with SMTs originating from the muscularis propria(MP) layer at the esophagogastric junction were treated by STER of their tumors. Key steps of the procedure include:(1) mucosal incision: a 2-cm longitudinal mucosal incision was made 5 cm proximal to the tumor;(2) submucosal tunneling: a submucosal tunnel was created 5 cm proximal to and 1 to 2 cm distal to the tumor;(3) tumor resection: the SMT was resected under direct endoscopic viewing;(4) hemostasis: while finishing the tumor resection, careful hemostasis of the MP defect and the tunnel was performed; and(5) mucosal closure: the mucosal incision site was closed by using hemostatic clips. During the operation, equipment used included a cap-fitted endoscope, an insulatedtip knife, a hook knife, hemostatic forceps, an injection needle, a snare, an endoclip, and a high-frequency generator. Carbon dioxide(CO2) insufflation was achieved by using a CO2 insufflator.RESULTS: The median age of the patients was 46.2 years(range, 35-59 years), and the majority were male(18 male vs 3 female). Complete resection rate was 100%(21/21). Eighteen lesions were resected en bloc. Mean tumor size was 23 mm(range, 10-40 mm), and mean procedure time was 62.9 min(range, 45-90 min). Pathological diagnosis of these tumors included leiomyoma(15 out of 21) and gastrointestinal stromal tumor(6 out of 21). Full-thickness MP resection was performed in 9 of 21 patients(42.9%), with mediastinal and subcutaneous emphysema occurring in all nine. At the completion of the procedure, all patients received closure of the incision with hemoclips. One patient required percutaneous drainage. The remaining 20patients required no further endoscopic or surgical intervention. There were no incidents of massive or delayed bleeding. The median follow-up period after the procedure was 6 mo(range, 2-14 mo). | 10 | Polymorphisms of MTHFD,plasma homocysteine levels, and risk of gastric cancer in ahigh-risk Chinese population显示文摘Purpose:Accumulative evidence suggests that folate has a protective effect on gastric cancer. The methylenetetrahyd-rofolate dlehydrogenase(MTHFD) plays an important role in folate and homocysteine metabolisms, and polymorphisms of MTHFD may result in disturbance of the folate-mediated homocysteine pathway. The aim of this study is to test the hypothesis that genetic variants of MTHFD and plasma homocysteine levels are associated with risk of gastric cancer and modulated by genotypes of methylenetetrahydrofolate reductase(MTHFR). Experimental Design: We genotyped G1958A and T401C in MTHFD and C677T in MTHFR and detected total plasma homocysteine(tHcy) levels in a case-control study of 589 gastric cancer cases and 635 cancer-free controls in a high-risk Chinese population. Results:The variant genotypes of MTHFD 1958AA and 401CC were associated with a significantly increased risk of gastric canceradjusted odds ratio(OR), 2.05; 95% confidence interval(95% CI),1.34-3.13 for 1958AA; adjusted OR,1.43; 95% CI,1.14-1.80 for 401CC compared with 1958GG/GA and 401TT/TC genotypes, respectively. Both of the effects were more evident in the subjects carrying MTHFR 677CT/TT genotypes. The average tHcy level was significantly higher in gastric cancer cases than in controls(P < 0.01), and the upper quartile of tHcy (> 13.6 mu mol/L) was associated with an 82% significantly increased risk of gastric cancer, compared with the lowest quartile of tHcy(<= 8.0 pmol/L;adjusted OR,1.82; 95% CI,1.20-2.75). Conclusions:The strong associations between MTHFD variants and the plasma tHcy levels and gastric cancer risk suggest, for the first time, a possible gene-environment interaction between genetic variants of folate-metabolizing genes and high tHcy levels in gastric carcinogenesis. | Wang, L. | 16 | Evaluation of three-dimensional biofilms on antibacterial bonding agents containing novel quaternary ammonium methacrylates显示文摘Antibacterial adhesives are promising to inhibit biofilms and secondary caries.The objectives of this study were to synthesize and incorporate quaternary ammonium methacrylates into adhesives,and investigate the alkyl chain length effects on three-dimensional biofilms adherent on adhesives for the first time.Six quaternary ammonium methacrylates with chain lengths of 3,6,9,12,16 and 18were synthesized and incorporated into Scotchbond Multi-Purpose.Streptococcus mutans bacteria were cultured on resin to form biofilms.Confocal laser scanning microscopy was used to measure biofilm thickness,live/dead volumes and live-bacteria percentage vs.distance from resin surface.Biofilm thickness was the greatest for Scotchbond control;it decreased with increasing chain length,reaching a minimum at chain length 16.Live-biofilm volume had a similar trend.Dead-biofilm volume increased with increasing chain length.The adhesive with chain length 9 had 37%live bacteria near resin surface,but close to 100%live bacteria in the biofilm top section.For chain length 16,there were nearly 0%live bacteria throughout the three-dimensional biofilm.In conclusion,strong antibacterial activity was achieved by adding quaternary ammonium into adhesive,with biofilm thickness and live-biofilm volume decreasing as chain length was increased from 3 to 16.Antibacterial adhesives typically only inhibited bacteria close to its surface;however,adhesive with chain length 16 had mostly dead bacteria in the entire three-dimensional biofilm.Antibacterial adhesive with chain length 16 is promising to inhibit biofilms at the margins and combat secondary caries. | Han Zhou Michael D Weir Joseph M Antonucci Gary E Schumacher Xue-Dong Zhou Hockin H K Xu | 2014 | International Journal of Oral Science2014,6,2: | 5 |

The tumor suppressor liver kinase B1 (LKB1), also known as serine/threonine kinase 11 (STK11), is involved in cell polarity, cell detachment and adhesion, tumor metastasis, and energetic stress response. A key role of LKB1 is to negatively regulate the activity of mTOR complex 1 (mTORC1). | 20 | Improving Densification and Mechanical Properties of FeAl/TiC Composites by Addition of Ni显示文摘FeAl/TiC composites were fabricated by hot pressing blended elemental powders. The effects of Ni-doping on thedensification and mechanical properties of the composites were studied. Results show that the density of the com-posites decreases with the content of TiC increasing, and the addition of Ni significantly improves the densificationprocess by enhancing mass transfer in the bonding phase. The mechanical properties of the composites are closelyrelated with their porosity.
