| 2 | Cost-utility analysis of transcatheter aortic valve implantation versus surgery in severe aortic stenosis patients with intermediate surgical risk in Thailand显示文摘BACKGROUND Transcatheter Aortic Valve Implantation(TAVI) has been shown to provide comparable survival benefit and improvement in quality of life to surgical aortic valve replacement(SAVR) for treating patients with severe aortic stenosis(AS) at intermediate surgical risk.This study aimed to evaluate the cost-utility of TAVI compared with SAVR for severe aortic stenosis with interme diate surgical risk in Thailand.METHODS A two-part constructed model was used to analyze lifetime costs and quality-adjusted life-years(QALYs) from societal and healthcare perspectives.The study cohort comprised severe AS patients at intermediate surgical risk with an average age of 80 years.The landmark trials were used to populate the model in terms of mortality and adverse event rates.All cost-related data and quality of life were based on Thai population.Costs and QALYs were discounted at 3% armually and presented as2021 values.Incremental cost-effectiveness ratios(ICERs) were calculated.Deterministic and probabilistic sensitivity analyses were conducted.RESULTS In comparison to SAVR,TAVI resulted in higher total cost(THB 1,717,132 [USD 52;415.51] vs.THB 893,524 [USD27,274.84]) and higher QALYs(4.88 vs.3.98) in a societal perspective.The estimated ICER was THB 906,937/QALY(USD27,684.27/QALY).From a healthcare system perspective,TAVI also had higher total cost than SAVR(THB 1,573,751 [USD48,038.79] vs.THB 726,342 [USD 22,171.63]) with similar QALYs gained to the societal perspective.The estimated ICER was THB933,145/QALY(USD 933,145/QALY).TAVI was not cost-effective at the Thai willingness to pay(WTP) threshold of THB160,000/QALY(USD 4,884/QALY).The results were sensitive to utility of either SAVR or TAVI treatment and cost of TAVI valve.CONCLUSION In patients with severe AS at intermediate surgical risk,TAVI is not a cost-effective strategy compared with SAVR at the WTP of THB 160;000/QALY(USD 4,884/QALY) from the perspectives of society and healthcare system. | Unchalee Permsuwan Voratima Yoodee Wacin Buddhari Nattawut Wongpraparut Tasalak Thonghong Sirichai Cheewatanakornkul Krissada Meemook Pranya Sakiyalak Pongsanae Duangpakdee Jirawit Yadee | 2022 | Journal of Geriatric Cardiology2022，19，11： | 1 |

| 11 | Experimental study on driver seat vibration characteristics of crawler-type combine harvester显示文摘To improve the driving comfort of combine harvesters,driver seat low-frequency vibration and related driver ride-comfort problems were investigated on a Chinese CFFL-850 crawler-type full-feed combine harvester based on ISO2631.Driver vibration and driving seat transmission characteristics were measured under the following conditions:no-load idling,driving on the road,driving in the field,and simulated harvesting.The root mean square values composite vibration under four conditions were 3.63 m/s^(2),2.35 m/s^(2),3.34 m/s^(2),and 2.67 m/s^(2),respectively.For the same condition,the maximum root mean square scores of vibration component on driver whole-body occurred in the seat support surface(test point 1)and vertical direction(Z direction),which were 3.56 m/s^(2),2.05 m/s^(2),3.15 m/s^(2),and 2.43 m/s^(2),respectively.The test point 2 to test point 1 vertical-transfer function curve trends were nearly identical.Nearly all of the transfer coefficients were greater than 1 in the range of 1-50 Hz,therefore,the seat vibration attenuation performance was poor.Based on the analysis results,the driver seat structure was altered and a verification test was performed.The test results indicated that after an X-damping mechanism was installed,vibration acceleration,on the surface of the seat support under the road-driving conditions,decreased from 2.35 m/s^(2) to 1.68 m/s^(2).Under the simulated harvesting condition,the vibration acceleration decreased from 2.56 m/s^(2) to 1.46 m/s^(2).Nearly all of the seat vertical transfer coefficients were less than 1 within the frequency range of 1-80 Hz,therefore the dynamic comfort of the seat was ameliorated after structural improvement. | Lizhang Xu Xiaoyu Chai Zhipeng Gao Yaoming Li Yading Wang | 2019 | International Journal of Agricultural and Biological Engineering2019，12，2： | 1 |

| 16 | Characteristics and in-hospital mortality of elderly patients with heart failure in Spanish hospitals显示文摘BACKGROUND The prevalence of heart failure(HF)increases with age,and it is one of the leading causes of hospitalization and death in older patients.However,there are little data on in-hospital mortality in patients with HF≥75 years in Spain.METHODS A retrospective analysis of the Spanish Minimum Basic Data Set was performed,including all HF episodes discharged from public hospitals in Spain between 2016 and 2019.Coding was performed using the International Classification of Diseases,10th Revision.Patients≥75 years with HF as the principal diagnosis were selected.We calculated:(1)the crude in-hospital mortality rate and its distribution according to age and sex;(2)the risk-standardized in-hospital mortality ratio;and(3)the association between in-hospital mortality and the availability of an intensive cardiac care unit(ICCU)in the hospital.RESULTS We included 354,792 HF episodes of patients over 75 years.The mean age was 85.2±5.5 years,and 59.2%of patients were women.The most frequent comorbidities were renal failure(46.1%),diabetes mellitus(35.5%),valvular disease(33.9%),cardiorespiratory failure(29.8%),and hypertension(26.9%).In-hospital mortality was 12.7%,and increased with age[odds ratio(OR)=1.07,95%CI:1.07–1.07,P<0.001]and was lower in women(OR=0.96,95%CI:0.92–0.97,P<0.001).The main predictors of mortality were the presence of cardiogenic shock(OR=19.5,95%CI:16.8–22.7,P<0.001),stroke(OR=3.5,95%CI:3.0–4.0,P<0.001)and advanced cancer(OR=2.6,95%CI:2.5–2.8,P<0.001).In hospitals with ICCU,the in-hospital risk-adjusted mortality tended to be lower(OR=0.85,95%CI:0.72–1.00,P=0.053).CONCLUSIONS In-hospital mortality in patients with HF≥75 years between 2016 and 2019 was 12.7%,higher in males and elderly patients.The main predictors of mortality were cardiogenic shock,stroke,and advanced cancer.There was a trend toward lower mortality in centers with an ICCU. | Alberto Esteban-Fernández Manuel Anguita-Sánchez Juan Luis Bonilla-Palomas María Anguita-Gámez María García JoséLuis Bernal Náyade Del Prado Cristina Fernández-Pérez Julián Pérez-Villacastin Juan JoséGómez-Doblas Francisco Javier Elola | 2023 | Journal of Geriatric Cardiology2023，20，4： | 0 |
