| 1 | A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. | 2 | Multimodality treatment in hepatocellular carcinoma patients with tumor thrombi in portal vein显示文摘AIM To compare the therapeutic effect andsignificances of multimodality treatment forhepatocellular carcinoma (HCC) with tumorthrombi in portal vein (PVTT).METHODS HCC patients (n = 147) with tumortrombi in the main portal vein or the first branchof portal vein were divided into four groups bythe several therapeutic methods. There wereconservative treatment group in 18 out ofpatients (group A); and hepatic artery ligation(HAL) and/or hepatic artery infusion (HAl)group in 18 patients (group B), in whompostoberative chemoembolization was doneperiodically; group of removal of HCC with PVTTin 79 (group C) and group of transcatheterhepatic arterial chemoembolization (TACE) orHAl and/or portal vein infusion (PVI) afteroperation in 32 (group D).RESULTS The median survival period was 12months in our series and the 1-, 3-, and 5-yearsurvival rates were 44.3%, 24.5% and 15.2%,respectively. The median survival times were 2,5, 12 and 16 months in group A, B, C and D,respectively. The 1-, 3- and 5-year survival rateswere 5.6%, 0% and 0% in group A; 22.2%,5.6% and 0% in group B; 53.9%, 26.9% and16.6% in group C; 79.3%, 38.9% and 26.8% ingroup D, respectively. | 4 | Prevalence of Thyroid Nodules and Its Relationship with Iodine Status in Shanghai:a Population-based Study显示文摘Objective This study was designed to evaluate the prevalence of thyroid nodules(TNs) and its relationship with urine iodine concentrations(UICs) after the regional rapid economic growth and lifestyle changes. Methods A cross-sectional survey was conducted in the general population aged 15-69 years. A questionnaire regarding general and personal characteristics and relevant information was administered. Ultrasonography of the thyroid was performed, and serum triiodothyronine(T3), tetraiodothyronine(T4), serum thyroid stimulating hormone(TSH), free triiodothyronine(FT3), free tetraiodothyronine(FT4), thyroglobulin antibody(Tg Ab), thyroid peroxidase antibody(TPOAb), and TSH receptor antibody(TRAb) levels were measured for each individual subject. Results The prevalence rates of TNs in the whole population, females and males were 27.76%, 34.04%, and 21.60%, respectively. The prevalence of multiple nodules increased with age, whereas the prevalence peaks differed between males and females. The median UICs in the whole population and females with non-TNs were higher than those of subjects with TNs(P=0.0035, P=0.0068). The median UICs in subjects with a single TN were higher than those in subjects with multiple TNs(P=0.0164, P=0.0127). The result showed a U-shaped curve relationship between UIC and prevalence of TNs. The prevalence of TNs was the lowest when the UIC was 140-400 μg/L. Conclusion The prevalence of TNs was nearly 30% and increased with age. The relationship between UIC and prevalence of TNs is U-shaped, with an increase in risk when the UIC was <140 μg/L and >400 μg/L. | 11 | Clinical trial with traditional Chinese medicine intervention ''tonifying the kidney to promote liver regeneration and repair by affecting stem cells and their microenvironment'' for chronic hepatitis B-associated liver failure显示文摘AIM:To study the clinical efficacy of traditional Chinese medicine(TCM)intervention'tonifying the kidney to promote liver regeneration and repair by affecting stem cells and their microenvironment'('TTK')for treating liver failure due to chronic hepatitis B.METHODS:We designed the study as a randomized controlled clinical trial.Registration number of Chinese Clinical Trial Registry is Chi CTR-TRC-12002961.A total of 144 patients with liver failure due to infection with chronic hepatitis B virus were enrolled in this randomized controlled clinical study.Participants were randomly assigned to the following three groups:(1)a modern medicine control group(MMC group,36patients);(2)a'tonifying qi and detoxification'('TQD')group(72 patients);and(3)a'tonifying the kidney to promote liver regeneration and repair by affecting stem cells and their microenvironment'('TTK')group(36patients).Patients in the MMC group received general internal medicine treatment;patients in the'TQD'group were given a TCM formula'tonifying qi and detoxification'and general internal medicine treatment;patients in the'TTK'group were given a TCM formula of'TTK'and general internal medicine treatment.All participants were treated for 8 wk and then followed at 48 wk following their final treatment.The primaryefficacy end point was the patient fatality rate in each group.Measurements of various virological and biochemical indicators served as secondary endpoints.The one-way analysis of variance and the t-test were used to compare patient outcomes in the different treatment groups.RESULTS:At the 48-wk post-treatment time point,the patient fatality rates in the MMC,'TQD',and'TTK'groups were 51.61%,35.38%,and 16.67%,respectively,and the differences between groups were statistically significant(P<0.05).However,there were no significant differences in the levels of hepatitis B virus DNA or prothrombin activity among the three groups(P>0.05).Patients in the'TTK'group had significantly higher levels of serum total bilirubin compared to MMC subjects(339.40μmol/L±270.09μmol/L vs 176.13μmol/L±185.70μmol/L,P=0.014).Serum albumin levels were significantly increased in both the'TQD'group and'TTK'group as compared with the MMC group(31.30 g/L±4.77g/L,30.72 g/L±2.89 g/L vs 28.57 g/L±4.56 g/L,P<0.05).There were no significant differences in levels of alanine transaminase among the three groups(P>0.05).Safety data showed that there was one case of stomachache in the'TQD'group and one case of gastrointestinal side effect in the'TTK'group.CONCLUSION:Treatment with'TTK'improved the survival rates of patients with liver failure due to chronic hepatitis B.Additionally,liver tissue was regenerated and liver function was restored. | Han-Min Li Zhi-Hua Ye Jun Zhang Xiang Gao Yan-Ming Chen Xin Yao Jian-Xun Gu Lei Zhan Yang Ji Jian-Liang Xu Ying-He Zeng Fan Yang Lin Xiao Guo-Guang Sheng Wei Xin Qi Long Qing-Jing Zhu Zhao-Hong Shi Lian-Guo Ruan Jia-Yao Yang Chang-Chun Li Hong-Bin Wu Sheng-Duo Chen Xin-La Luo | 2014 | World Journal of Gastroenterology2014,20,48: | 20 |

| 14 | Identification and differentiation of Panax ginseng,Panax quinquefolium, and Panax notoginseng by monitoring multiple diagnostic chemical markers显示文摘To differentiate traditional Chinese medicines(TCM) derived from congeneric species in TCM compound preparations is usually challenging. The roots of Panax ginseng(PG), Panax quinquefolium(PQ) and Panax notoginseng(PN) are used as popular TCM. They contain similar triterpenoid saponins(ginsenosides) as the major bioactive constituents. Thus far, only a few chemical markers have been discovered to differentiate these three species. Herein we present a multiple marker detection approach to effectively differentiate the three Panax species, and to identify them in compound preparations. Firstly, 85 batches of crude drug samples(including 32 PG, 30 PQ, and 23 PN) were analyzed by monitoring 40 major ginsenosides in the extracted ion chromatograms(EICs) using a validated LC–MS fingerprinting method. Secondly, the samples were clustered into different groups by pattern recognition chemometric approaches using PLS-DA and OPLS-DA models, and17 diagnostic chemical markers were discovered. Aside from the previously known Rf and p-F11, ginsenoside Rs1 could be a new marker to differentiate PG from PQ. | 16 | Chromosomal level assembly and population sequencing of the Chinese tree shrew genome显示文摘Chinese tree shrews (Tupaia belangeri chinensis) have become an increasingly important experimental animal in biomedical research due to their close relationship to primates. An accurately sequenced and assembled genome is essential for understanding the genetic features and biology of this animal. In this study, we used long-read single-molecule sequencing and high-throughput chromosome conformation capture (Hi-C) technology to obtain a high-qualitychromosome-scale scaffolding of the Chinese tree shrew genome. The new reference genome (KIZ version 2: TS_2.0) resolved problems in presently available tree shrew genomes and enabled accurate identification of large and complex repeat regions, gene structures, and species-specific genomic structural variants. In addition, by sequencing the genomes of six Chinese tree shrew individuals, we produced a comprehensive map of 12.8 M single nucleotide polymorphisms and confirmed that the major histocompatibility complex (MHC) loci and immunoglobulin gene family exhibited high nucleotide diversity in the tree shrew genome. We updated the tree shrew genome database (TreeshrewDB v2.0: http://gffzzb70c77447da74c53hxvfb0b09wc9n6q9c.ffgz.tsg.suse.edu.cn) to include the genome annotation information and genetic variations. This study was undertaken to investigate the key role of transforming growth factor-β(TGF-β)signalling in the cartilage and subchondral bone of the TMJ using a temporomandibular joint disorder(TMD) rat model, an ageing mouse model and a Camurati–Engelmann disease(CED) mouse model. In the three animal models, the subchondral bone phenotypes in the mandibular condyles were evaluated by μCT, and changes in TMJ condyles were examined by TRAP staining and immunohistochemical analysis of Osterix and p-Smad2/3. Condyle degradation was confirmed by Safranin O staining, the Mankin and OARSI scoring systems and type X collagen(Col X), p-Smad2/3 a and Osterix immunohistochemical analyses. We found apparent histological phenotypes of TMJ-OA in the TMD, ageing and CED animal models, with abnormal activation of TGF-βsignalling in the condylar cartilage and subchondral bone. Moreover, inhibition of TGF-β receptor I attenuated TMJ-OA progression in the TMD models. | 20 | APPLICATIONS OF CFD TECHNIQUE IN THE DESIGN AND FLOW ANALYSIS OF IMPLANTABLE AXIAL FLOW BLOOD PUMP显示文摘Based on established numerical methods and hydrodynamic performance testing facilities,CFD technique are applied to improve the design of the implantable axial flow blood pump and the flow analysis.(1) Applying brushless machine magnet steel,reducing its thickness while increasing the length,the flow channel's cross-section is increased,with no space connection between the large and the small rotators,and with a cone transition segment from the bearing to the principal axis,the flow is made smoother.The rotating speed is lowered by 1000 rpm-1200 rpm under the same flow rate and pressure head,and thus the hemolysis can be avoided.(2) Different outlet stator guiding vanes are selected for the same blood pump for analyses of hydrodynamic performances and flow fields.An excellent design not only can regulate the rotating flow field into an axial one,reduce the circulation loss and improve the pump efficiency,but also can avoid backflow,vortex and secondary flow at the pump outlet,and thus the thrombus can be prevented.(3) The calf live tests show that some residual clots exist at the inner wall of the outlet connection bends,which are analyzed and explained by CFD techniques and the corresponding improvements are proposed.All results are verified by hydrodynamic performance tests and PIV flow field tests,and consistent conclusions are obtained. | FAN Hui-min HONG Fang-wen ZHANG Guo-ping YE Liang LIU Zhong-min | 2010 | Journal of Hydrodynamics2010,22,4: | 13 |
