5Corneal re-innervation following refractive surgery treatments显示文摘Laser refractive surgery is one of the most performed surgical procedures in the world. Although regarded safe and efficient, it has side effects. All of the laser based refractive surgical procedures invoke corneal nerve injury to some degree. The impact of this denervation can range from mild discomfort to neurotrophic corneas. Currently, three techniques are widely used for laser vision correction: small incision lenticule extraction, laser-assisted keratomileusis in situ and photorefractive keratotomy. Each of these techniques affects corneal innervation differently and has a different pattern of nerve regeneration. 7Non-invasive versus invasive mechanical ventilation for respiratory failure in severe acute respiratory syndrome显示文摘Background Severe acute respiratory syndrome is frequently complicated by respiratory failure requiringventilatory support.We aimed to compare the efficacy of non-invasive ventilation against invasive mechanicalventilation treating respiratory failure in this disease.Methods Retrospective analysis was conducted on all respiratory failure patients identified from the HongKong Hospital Authority Severe Acute Respiratory Syndrome Database.Intubation rate, mortality and secondaryoutcome of a hospital utilizing non-invasive ventilation under standard infection control conditions ( NIVHospital) were compared against13 hospitals using solely invasive ventilation (IMVHospitals).Multiple logisticregression analyses with adjustments for confounding variables were performed to test for association betweenoutcomes and hospital groups.Results Both hospital groups had comparable demographics and clinical profiles, but NIV Hospital (42patients) had higher lactate dehydrogenase ratio and worse radiographic score on admission and ribavirin-corticosteroid commencement.Compared to IMV Hospitals (451 patients), NIV Hospital had lower adjustedodds ratios for intubation (0.36, 95% CI 0.164 -0.791,P=0.011) and death (0.235, 95% CI 0.077 -0.716,P=0.011), and improved earlier after pulsed steroid rescue.There were no instances of transmission ofsevere acute respiratory syndrome among health care workers due to the use of non-invasive ventilation.Conclusion Compared to invasive mechanical ventilation, non-invasive ventilation as initial ventilatory supportfor acute respiratory failure in the presence of severe acute respiratory syndrome appeared to be associated withreduced intubation need and mortality.Loretta YC Yam Alfred YF Chan Thomas MT Cheung Eva LH Tsui Jane CK Chan Vivian CW Wong 2005Chinese Medical Journal2005,,17:3

9Decomposition and Reformulation of Banxia Baizhu Tianma Decoction:A Vasodilatory Approach显示文摘Objective To decompose and reformulate Banxia Baizhu Tianma Decoction(BBT) in order to optimize its ratio for vasodilatory effect.The fingerprints of these six component herbs were also studied via Tri-step FTIR analysis.Methods The herbal components of BBT were extracted separately with different solvents,such as 95%ethanol,50%ethanol,and distilled water.The vasodilatory effect of the extracts was studied using the in vitro pre-contracted aortic ring model.Among these extracts,the water extracts of Pinellia ternata[PT(W)]and Citri reticulatae[CR(W)],50%ethanol extracts of Castrodia elata[GE(50)]and Clycyrrhiza uralensis[GU(50)],and 95%ethanol extracts of Atractylodes macrocephala[AM(E)],and Poria cocos[PC(E)]were selected for the orthogonal stimulus-response compatibility group study due to their high effectiveness in vasodilation.Results The best combination ratio(G2) was GE(50),GU(50),AM(E),PC(E),CR(W),and PT(W) at EC_0,EC_5,EC_5,EC_5,EC_5,and EC_5,with EC_(50) of(0.06 ± 0.01) mg/mL.Comparison between the vasodilatory effects of extracts derived from G2 and BBT were also studied.From the results,the 50%ethanol extract following G2 ratio was found to be most potent extract with the highest vasodilatory effect compared with other extracts derived from G2 or BBT,with EC_(50) of(0.05 ±0.005) mg/mL.Conclusion The optimum ratio and appropriate extraction method for the new BBT formulae are obtained.G2 is determined as the best formulae with the highest potency in producing the vasodilatory effect on isolated rat aortic rings.The results obtained suggest that 50%ethanol is found to be the best solvent to extract the vasoactive compounds of G2 in order to exhibit the highest efficacy of vasodilatory effect.Chu Shan Tan Yean Chun Loh Yung Sing Ch'ng Mariam Ahmad Mohd.Zaini Asmawi Mun Fei Yam 2017Chinese Herbal Medicines2017,9,2:3

11Role of Small Extracellular Vesicles in Liver Diseases:Pathogenesis,Diagnosis,and Treatment显示文摘Extracellular vesicles(EVs)are vesicular bodies that bud off from the cell membrane or are secreted virtually by all cell types.Small EVs(sEVs or exosomes)are key mediators of cell-cell communication by delivering their cargo,including proteins,lipids,or RNAs,to the recipient cells where they induce changes in signaling pathways and phenotypic prop-erties.Tangible findings have revealed the pivotal involve-ment of sEVs in the pathogenesis of various diseases.On the bright side,they are rich sources of biomarkers for diag-nosis,prognosis,treatment response,and disease monitor-ing.sEVs have high stability,biocompatibility,targetability,low toxicity,and are immunogenic in nature.Their intrinsic properties make sEVs an ideal delivery vehicle to be loaded with cargo for therapeutic interventions.Liver diseases are a major global health problem.This review aims to focus on the roles and mechanisms of sEVs in the pathogenesis of liver diseases,liver injury,liver failure,and liver can-cer.sEVs are released not only by hepatocytes but also by stromal and immune cells in the microenvironment.Early detection of liver disease determines the chance for cura-tive treatment and high survival of patients.This review focuses on the potential of circulating sEV cargo as specific and sensitive noninvasive biomarkers for the early detection and prognosis of liver diseases.In addition,the therapeutic use of sEVs derived from various cell types is discussed.Al-though sEVs hold promise for clinical applications,there are still challenges to be overcome by further research to bring utilization of sEVs into clinical practice.Tingmao Xue Judy Wai Ping Yam 2022Journal of Clinical and Translational Hepatology2022,10,6:2

16Unburned Carbon Loss in Fly Ash of CFB Boilers Burning Hard Coal显示文摘The unburned carbon loss in fly ash of circulating fluidized bed (CFB) boilers, most of which are burning active fuels such as lignite or peat, is normally very low. However, most CFB boilers in China usually burn hard coals such as anthracite and bituminous coal and coal wastes, so the carbon content in the fly ash from these boilers is higher than expected. This paper investigates the source of unburned carbon in the fly ash of CFB boilers burning hard coal through a series of field tests and laboratory investigations. The char behavior during combustion, including fragmentation and deactivation, which is related to the parent coal, has an important impact on the carbon burnout in CFB boilers. The research shows that char deactivation occurs during char burnout in fluidized bed combustion, especially for large particles of low rank coal. The uneven mixing of solids and air in the core region of the furnace also causes poor burnout of carbon in CFB fly ash. An index describing the volatile content (as dry ash free basis) over the heating value is proposed to present the coal rank. The coal combustion efficiency is shown to be strongly connected with this coal index. 17Comparison of Dextran Perfusion and GSI-B4 Isolectin Staining in a Mouse Model of Oxygen-induced Retinopathy显示文摘Purpose: Oxygen-induced retinopathy(OIR) is a robust and widely used animal model for the study of retinal neovascularization(NV). Dextran perfusion and Griffonia simplicifolia isolectin B4(GSI-B4) staining are two common methods for examining the occurrence and extent of OIR. This study provides a quantitative comparison of the two for OIR detection.Methods: At postnatal day 7(PN7), fifteen C57 BL / 6J mice were exposed to a 75% hyperoxic condition for 5 days and then returned to room air conditions. At PN17, the mice received intravitreal injection of GSI-B4 Alexa Fluor 568 conjugate. After 10 hours, they were infused with FITC-dextran conjugate via the left ventricle. Retinal flat mounts were photographed by confocal microscopy. Areas with fluorescent signals and the total retinal areas were quantified by Image J software.Results:Both GSI-B4 and dextran detected the peripheral neovascular area. The mean hyper fluorescence area was 0.33 ±0.14% of whole retinal area determined by GSI-B4 staining and 0.25±0.28% determined by dextran perfusion. The difference between the two measures was 0.08%(95% CI:-0.59%,0.43%)..The Pearson correlation coefficient between the two methods was 0.386,P =0.035..The mean coincidence rates were 14.3 ±13.4% and 24.9 ±18.5% for GSI-B4 and dextran staining, respectively.Conclusion:.Both methods can complement each other indemonstrating and quantitatively evaluating retinal NV.
