| 6 | Free-breathing radial volumetric interpolated breathhold examination vs breath-hold cartesian volumetric interpolated breath-hold examination magnetic resonance imaging of the liver at 1.5T显示文摘AIM: To compare breath-hold cartesian volumetric interpolated breath-hold examination(cVIBE) and freebreathing radial VIBE(rVIBE) and determine whether rVIBE could replace cVIBE in routine liver magnetic resonance imaging(MRI).METHODS: In this prospective study, 15 consecutive patients scheduled for routine MRI of the abdomen underwent pre- and post-contrast breath-hold cVIBE imaging(19 s acquisition time) and free-breathing rVIBE imaging(111 s acquisition time) on a 1.5T Siemens scanner. Three radiologists with 2, 4, and 8 years post-fellowship experience in abdominal imaging evaluated all images. The radiologists were blinded to the sequence types, which were presented in a random order for each patient. For each sequence, the radiologists scored the cVIBE and rVIBE images for liver edge sharpness, hepatic vessel clarity, presence of artifacts, lesion conspicuity, fat saturation, and overall image quality using a five-point scale. RESULTS: Compared to rVIBE, cVIBE yielded significantly(P < 0.001) higher scores for liver edge sharpness(mean score, 3.87 vs 3.37), hepatic-vessel clarity(3.71 vs 3.18), artifacts(3.74 vs 3.06), lesion conspicuity(3.81 vs 3.2), and overall image quality(3.91 vs 3.24). cVIBE and rVIBE did not significantly differ in quality of fat saturation(4.12 vs 4.03, P = 0.17). The inter-observer variability with respect to differences between rVIBE and cVIBE scores was close to zero compared to random error and inter-patient variation. Quality of rVIBE images was rated as acceptable for all parameters. CONCLUSION: rVIBE cannot replace cVIBE in routine liver MRI. | 7 | Port Integration in China: Temporal Pathways, Spatial Patterns and Dynamics显示文摘Over the past two decades, numerous ports located in China have participated in port integration strategies, thus influencing the entire port system. The current research is initiated in order to examine the nature of port integration in China, including associated temporal pathways, spatial patterns and dynamics. Results indicate that port integration in China has been characterized by a significant increase at the turn of the 21 st century, comprising thirteen distinguishable pathways typified by differing dynamics, particularly between the northern and southern ports. Pathways were found to include 44 seaports and river ports, chiefly concentrated in the Bohai Rim, Yangtze(Changjiang) River Delta, Beibu Gulf and the southeastern Fujian, thus representing significant spatial regions. Categorically larger seaports have become the primary beneficiaries of port integration. Integration cases were divided into four categories based upon quantified dynamic magnitude including the government-driven mode, market-driven mode, government/market-driven mode and strategic alliance, and into five further categories based upon spatial extent including port internal integration, jurisdictional port integration, port integration across neighbor region, regional port integration and hub-feeder port integration. | 8 | Pulmonary infection control window in treatment of severe respiratory failure of chronic obstructive pulmonary diseases: a prospective, randomized controlled, multi-centred study显示文摘Background Early withdraw from invasive mechanical ventilation (MV) followed by noninvasive MV is a new strategy for changing modes of treatment. This study was conducted to estimate the feasibility and the efficacy of early extubation and sequential noninvasive MV commenced at beginning of pulmonary infection control window in patients with exacerbated hypercapnic respiratory failure caused by chronic obstructive pulmonary diseases (COPD). Methods A prospective, randomized controlled study was conducted in eleven teaching hospitals' respiratory or medical intensive care units in China. Ninety intubated COPD patients with severe hypercapnic respiratory failure triggered by pulmonary infection (pneumonia or purulent bronchitis) were involved in the study. When the pulmonary infection had been controlled by antibiotics and comprehensive therapy, the 'pulmonary infection control window (PIC window)' has been reached. Each case was randomly assigned to study group (extubation and noninvasive MV via facial mask immediately) or control group (invasive MV was received continuously after PIC window by using conventional weaning technique).Results Study group (n=47) and control group (n=43) had similar clinical characteristics initially and at the time of PIC window. | 13 | Environmental and Psycho-social Factors Related to Prostate Cancer Risk in the Chinese Population:a Case-control Study显示文摘Objective To study the risk environmental and psycho-social factors associated to prostate cancer(PCa) in Chinese population. Methods 250 PCa patients and 500 controls were enrolled in this case-control study. Information was collected and logistic regression analysis was used to estimate the odds ratios(OR) and 95% confidence intervals(95% CI) for relationship between lifestyle, eating habits and psycho-social factors with PCa risk. Results Green vegetables and green tea were associated with a decreased risk of PCa(OR=0.39, 95% CI: 0.28-0.53; OR=0.59, 95% CI: 0.40-0.87, respectively). Family history of PCa(OR=7.16, 95% CI: 2.01-25.49), history of prostate diseases(OR=2.28, 95% CI: 1.53-3.41), alcohol consumption(OR=1.97, 95% CI: 1.33-2.90), red meat consumption(OR=1.74, 95% CI: 1.20-2.52), barbecued(OR=2.29, 95% CI: 1.11-4.73) or fried(OR=2.35, 95% CI: 1.24-4.43) foods were related with increased PCa risk. Negative psycho-social factors including occupational setbacks(OR=1.61, 95% CI: 1.00-2.59), marital separation(OR=1.94, 95% CI: 1.29-2.91), self-contained suffering(OR=2.37, 95% CI: 1.58-3.55), and high sensitivity to the personal comments(OR=1.73, 95% CI: 1.18-2.54) were related to PCa. Conclusion Regular consumption of green vegetables and green tea may suggest protective effects on PCa. Alcohol consumption, red meat consumption and barbecued or fried foods were associated with PCa. | 18 | ^137Cs tracing dynamics of soil erosion,organic carbon,and total nitrogen in terraced fields and forestland in the Middle Mountains of Nepal显示文摘The Middle Mountains is one of the regions of Nepal most vulnerable to water erosion, where fragile geology, steep topography, anomalous climatic conditions, and intensive human activity have resulted in serious soil erosion and enhanced land degradation. Based on the 137 Cs tracing method, spatial variations in soil erosion, organic carbon, and total nitrogen(TN) in terraced fields lacking field banks and forestland were determined. Soil samples were collected at approximately 5 m and 20 m intervals along terraced field series and forestland transects respectively. Mean 137 Cs inventories of the four soil cores from the reference site was estimated at 574.33 ± 126.22 Bq m-2(1 Bq(i.e., one Becquerel) is equal to 1 disintegration per second(1 dps)). For each terrace, the 137 Cs inventory generally increased fromupper to lower slope positions, accompanied by a decrease in the soil erosion rate. Along the entire terraced toposequence, 137 Cs data showed that abrupt changes in soil erosion rates could occur between the lower part of the upper terrace and the upper part of the immediate terrace within a small distance. This result indicated that tillage erosion is also a dominant erosion type in the sloping farmland of this area. At the same time, we observed a fluctuant decrease in soil erosion rates for the whole terraced toposequence as well as a net deposition at the toe terrace. Although steep terraces(lacking banks and hedgerows) to some extent could act to limit soil sediment accumulation in catchments, soil erosion in the terraced field was determined to be serious. For forestland, with the exception of serious soil erosion that had taken place at the top of slopes due to concentrated flows from a country road situated above the forestland site, spatialvariation in soil erosion was similar to the 'standard' water erosion model. Soil organic carbon(SOC) and TN inventories showed similar spatial patterns to the 137 Cs inventory for both toposequences investigated. However, due to the different dominant erosion processes between the two, we found similar patterns between the <0.002 mm soil particle size fraction(clay sized) and 137 Cs inventories in terraced fields, while different patterns could be found between 137 Cs inventories and the <0.002 mm soil particle size fraction in the forestland site.
