3Role of the gut microbiota in inflammatory bowel disease pathogenesis: What have we learnt in the past 10 years?显示文摘Our understanding of the microbial involvement in inflammatory bowel disease(IBD) pathogenesis has increased exponentially over the past decade. The development of newer molecular tools for the global assessment of the gut microbiome and the identification of nucleotide-binding oligomerization domain-containing protein 2 in 2001 and other susceptibility genes for Crohn's disease in particular has led to better understanding of the aetiopathogenesis of IBD. The microbial studies have elaborated the normal composition of the gut microbiome and its perturbations in the setting of IBD. This altered microbiome or 'dysbiosis' is a key player in the protracted course of inflammation in IBD. Numerous genome-wide association studies have identified further genes involved in gastrointestinal innate immunity(including polymorphisms in genes involved in autophagy: ATG16L1 and IGRM), which have helped elucidate the relationship of the local innate immunity with the adjacent luminal bacteria. These developments have also spurred the search for specific pathogens which may have a role in the metamorphosis of the gut microbiome from a symbiotic entity to a putative pathogenic one. 6WONCA研究论文摘要汇编——建议冬季口服益生菌以减少应用抗生素治疗哮喘的随机对照试验显示文摘目的有研究表明儿童口服益生菌能预防呼吸道感染,从而可以减少抗生素的使用。本研究探讨了哮喘患者在冬季口服益生菌以减少应用抗生素治疗呼吸道感染的建议是否可行。方法本研究在英国1家社区医疗中心开展了随机对照试验。研究对象为5岁及以上的哮喘患者。干预措施是向哮喘患者发放传单,这种传单和普通的建议人们在冬季如何减少呼吸道感染或哮喘发作的传单不同,其内容为建议人们于2013年10月—2014年3月每日服用益生菌来减少这些疾病的发生。以研究对象中使用抗生素来治疗呼吸道感染的人数占总人数的比例作为评价结果的指标。结果 1 302例研究对象被随机分为对照组(n=650)和干预组(n=652)。干预组、对照组分别有27.7%、26.9%的患者应用了抗生素治疗呼吸道感染,两组间差异无统计学意义〔OR=1.04,95%CI(0.82,1.34)〕。服用益生菌量不多的患者在应用抗生素治疗呼吸道感染的人数比例上和对照组相比也相近〔调整后的OR=1.08,95%CI(0.69,1.69)〕。也未发现服用益生菌对治疗呼吸道感染或者哮喘急性发作有何作用。结论本研究中未发现有证据支持在冬季口服益生菌以减少使用抗生素来治疗呼吸道感染的建议。SMITH T D WATT H GUNN L 本刊编辑部 2016中国全科医学2016,19,34:11
7中国西南虎跳峡地区云南松林土壤侵蚀控制潜能(英文)显示文摘作为中国西南长江上游众多峡谷、深谷的典型代表 ,虎跳峡地区的土壤侵蚀是一个严重的环境问题。为了解决侵蚀问题 ,大面积的适宜荒山已经植树造林。云南松是运用最广的造林树种之一。本研究试图以虎跳峡地区1993年的情况为例 ,探讨云南松林土壤侵蚀控制的水文效应及其潜能。研究发现 ,云南松密林能够截留 31.1%的降雨量。林下的土壤溅蚀量 (4.9kg.m- 2 )比裸地上的 (6 .1kg.m- 2 )降低了 19.7%。松林内的土壤流失总量与裸地的5 7.9g.m- 2 比较 ,降低到 32 .9g.m- 2 ,表明松林减少了 43.19%的土壤流失。实验指出 ,降雨强度对这里的土壤流失 ,以及云南松林的侵蚀控制作用有明显的影响。另一方面 ,研究也发现松林能够产生较大直径的叶滴 ,引起林下的叶滴溅蚀 ;而且 ,松林并不能明显地减少地表径流。然而 ,从综合作用来看 ,云南松密林具有土壤侵蚀控制的净效应。与茂密的草丛相比 ,松林侵蚀控制的效果相对较低。周跃 李玉辉 David Watts 2000植物生态学报2000,24,1:9

9Can fecal microbiota transplantation cure irritable bowel syndrome?显示文摘AIM To verify the utility of treatment with fecal microbiota transplantation(FMT) in patients with irritable bowel syndrome(IBS).METHODS We searched EMBASE, Cochrane Library and Pub Med in March, 2017. The reviewed literature was based on two systematic searches in each of the databases. The Me SH terms used were IBS and fecal microbiota transplantation and the abbreviations IBS and FMT. Reference lists from the articles were reviewed to identify additional pertinent articles. RESULTS A total of six conference abstracts, one case report, one letter to the editor, and one clinical review were included. In the final analysis, treatment of 48 patients was evaluated. Treatment revealed an improvement in 58% of cases. The varying structure of the nine included studies must be taken into consideration.CONCLUSION Data on FMT and IBS are too limited to draw sufficientconclusions. 12Marked elevation of B-type natriuretic peptide in patients with heart failure and preserved ejection fraction显示文摘Marked elevations of B-type natriuretic peptide(BNP)are not generally seen in patients with heart failure and preserved ejection fraction(HFpEF).The objective of this study was to examine the clinical and laboratory characteristics of a large cohort of patients with HFpEF and markedly elevated BNP.A retrospective examination of 421inpatients at a university hospital admitted with a diagnosis of HFpEF was performed.Clinical and echocardiographic data in 4 groups of patients with levels of BNP≤100 pg/mL,100-400 pg/mL,400-1,000 pg/mL and>1,000 pg/mL were compared.Patients with HFpEF and BNP>1,000 pg/mL(28%of the population)were characterized by impaired renal function and greater use of anti-hypertensive medications.A subset of these patients with BNP>1,000 pg/mL had normal renal function(21%)and were significantly older,more frequently female,and tended to have lower ejection fractions.Conversely,patients with HFpEF and BNP≤100 pg/mL were younger and had preserved renal function.BNP was inversely related to the likelihood of subsequent admission for heart failure,but not to myocardial infarction or death.In conclusion:BNP>1,000 pg/mL is seen in almost 1/3 of patients hospitalized with HFpEF.This elevation of BNP often reflects impaired renal function,but can also be seen in patients with preserved renal function but relatively impaired systolic function.Samuel Tate Andrea Griem Blythe Durbin-Johnson Clifton Watt Saul Schaefer 2014The Journal of Biomedical Research2014,28,4:7
13Thrombus aspiration during primary percutaneous coronary intervention for acute myocardial infarction:A review of clinical evidence and guidelines显示文摘Acute ST segment elevation myocardial infarction(STEMI) is characterized by complete thrombotic occlusion of a major coronary artery. Early recanalization of the infarct-related artery is most efficiently delivered by primary percutaneous coronary intervention(PPCI),however this does not always restore normal myocardial perfusion,mainly due to distal embolization of the thrombus and microvascular obstruction. Early evidence for manual thrombus aspiration during PPCI was promising and this was once considered an important aspect of the procedure,especially in patients with a high thrombus burden. 16Does progesterone show neuroprotective effects on traumatic brain injury through increasing phosphorylation of Akt in the hippocampus?显示文摘There are currently no federally approved neuroprotective agents to treat traumatic brain injury. Progesterone, a hydrophobic steroid hormone, has been shown in recent studies to exhibit neuroprotective effects in controlled cortical impact rat models. Akt is a protein kinase known to play a role in cell signaling pathways that reduce edema, inflammation, apoptosis, and promote cell growth in the brain. This study aims to determine if progesterone modulates the phosphorylation of Akt via its threonine 308 phosphorylation site. Phosphorylation at the threonine 308 site is one of several sites responsible for activating Akt and enabling the protein kinase to carry out its neuroprotective effects. To assess the effects of progesterone on Akt phosphorylation, C57BL/6 mice were treated with progesterone(8 mg/kg) at 1(intraperitonally), 6, 24, and 48 hours(subcutaneously) post closed-skull traumatic brain injury. The hippocampus was harvested at 72 hours post injury and prepared for western blot analysis. Traumatic brain injury caused a significant decrease in Akt phosphorylation compared to sham operation. However, mice treated with progesterone following traumatic brain injury had an increase in phosphorylation of Akt compared to traumatic brain injury vehicle.
