1Using old liver grafts for liver transplantation: Where are the limits?显示文摘The scarcity of ideal liver grafts for orthotopic liver transplantation(OLT) has led transplant teams to investigate other sources of grafts in order to augment the donor liver pool. One way to get more liver grafts is to use marginal donors, a not well-defined group which includes mainly donors > 60 years, donors with hypernatremia or macrosteatosis > 30%, donors with hepatitis C virus or hepatitis B virus positive serologies, cold ischemia time > 12 h, non-heart-beating donors, and grafts from split-livers or living-related donations. Perhaps the most practical and frequent measure to increase the liver pool, and thus to reduce waiting list mortality, is to use older livers. In the past years the results of OLT with old livers have improved, mainly due to better selection and maintenance of donors, improvements in surgical techniques in donors and recipients, and intra- and post-OLT management. At the present time, sexagenarian livers are generally accepted, but there still exists some controversy regarding the use of septuagenarian and octogenarian liver grafts. The aim of this paper is to briefly review the aging process of the liver and reported experiences using old livers for OLT. 2Biometric and ICL-related risk factors associated to sub-optimal vaults in eyes implanted with implantable collamer lenses显示文摘Background:To identify biometric and implantable collamer lens(ICL)-related risk factors associated with suboptimal postoperative vault in eyes implanted with phakic ICL.Methods:This study reports a retrospective case series of the first operated eye in 360 patients implanted with myopic spherical or toric ICL.Preoperatively,white-to-white(WTW),central keratometry(Kc)and central corneal thickness(CCT)were measured using the Pentacam.Anterior-segment optical coherence tomography(AS-OCT,Visante)was applied preoperatively for measuring the horizontal anterior-chamber angle-to-angle distance(ATA),internal anterior chamber depth(ACD),crystalline lens rise(CLR),anterior-chamber angle(ACA)and postoperatively the vault.Eyes were divided into three vault groups:low(LVG:≤250μm),optimal(OVG:>250 and<1000μm)and high(HVG:≥1000μm).Multinomial logistic regression(MLR)was used to find the sub-optimal vault predictors.Results:MLR showed that CLR,ICL size minus the ATA(ICL size-ATA),age,ICL spherical equivalent(ICLSE)and ICL size as contributing factors for sub-optimal vaults(pseudo-R2=0.40).Increased CLR(OR:1.01,CI:1.00-1.01)and less myopic ICLSE(OR:1.22,CI:1.07-1.40)were risk factors for low vaults.Larger ICL size-ATA(OR:41.29,CI:10.57-161.22)and the 13.7mm ICL(OR:7.08,CI:3.16-15.89)were risk factors for high vaults,whereas less myopic ICLSE(OR:0.85,CI:0.76-0.95)and older age(OR:0.92,CI:0.88-0.98)were protective factors.Conclusion:High CLR and low ICLSE were the major risk factors in eyes presenting low vaults.In the opposite direction,ICL size-ATA was the major contributor for high vaults.This relationship was more critical in higher myopic ICLSE,younger eyes and when 13.7mm ICL were used.The findings show that factors influencing the vault have differentiated weight of influence depending on the type of vault(low,optimal or high).Santiago Cerpa Manito Angel Sánchez Trancón Oscar Torrado Sierra António Manuel Baptista Pedro Miguel Serra 2021Eye and Vision2021,8,1:5
3Influence of cirrhosis in cardiac surgery outcomes显示文摘Liver cirrhosis has evolved an important risk factor for cardiac surgery due to the higher morbidity and mortality that these patients may suffer compared with general cardiac surgery population.The presence of contributing factors for a poor outcome,such as coagulopathy,a poor nutritional status,an adaptive immune dysfunction,a degree of cirrhotic cardiomyopathy,and a degree of renal and pulmonary dysfunction,have to be taken into account for surgical evaluation when cardiac surgery is needed,together with the degree of liver disease and its primary complications.The associated pathophysiological characteristics that liver cirrhosis represents have a great influence in the development of complications during cardiac surgery and the postoperative course.Despite the population of cirrhotic patients who are referred for cardiac surgery is small and recommendations come from small series,since liver cirrhotic patients have increased their chance of survival in the last 20 years due to the advances in their medical care,which includes liver transplantation,they have been increasingly considered for cardiac surgery.Indeed,there is an expected rise of cirrhotic patients within the cardiac surgical population due to the increasing rates of non-alcoholic fatty liver disease and non-alcoholic steatohepatitis,especially in western countries.In consequence,a more specific approach is needed in the assessment of care of these patients if we want to improve their management.In this article,we review the pathophysiology and outcome prediction of cirrhotic patients who underwent cardiac surgery.Juan C Lopez-Delgado Francisco Esteve Casimiro Javierre Josep L Ventura Rafael Maez Elisabet Farrero Herminia Torrado David Rodríguez-Castro Maria L Carrio 2015World Journal of Hepatology2015,7,5:2
