20经皮经导管射频消融去肾交感神经术后血管和肾血流动力学的改变显示文摘经皮经导管射频消融去肾交感神经术(RDN)能有效降低难治性高血压患者的血压,但其对患者的肾灌注、肾血管阻力(RVR)和中心动脉血流动力学的影响尚未明确。2011-01-12,入选接受RDN的难治性高血压患者19例[服用≥3种降压药物,包括利尿剂,血压仍≥140/90mmHg(1mmHg=0.133kPa)]。在RDN前1天,RDN后1天和3月,通过无创动脉血质子自旋标记磁共振成像评估患者肾灌注和RVR,通过估算的肾小球滤过率评估肾功能;RDN前1天,RDN后6月,通过脉冲波分析评估中心动脉血流动力学变化情况。研究结果显示,RDN后6月,外周诊室血压[收缩压(142±23)比(158±26)mmHg,P<0.01和舒张压(76±9)比(83±13)mmHg,P<0.05]、24h平均收缩压[(152±17)比(159±17)mmHg,P<0.05]、中心动脉血压[收缩压(131±28)比(145±31)mmHg,P<0.01和舒张压(80±14)比(85±18)mmHg,P<0.05]、中心动脉脉压[(52±18)比(61±18)mmHg,P<0.05]、中心动脉反射波增强指数[(20±8)%比(24±8)%,P<0.05]明显降低。与RDN前1天比较,RDN后1天和3月肾灌注差异无统计学意义;RDN后3月,RVR明显降低,而肾功能任何时间点间差异均无统计学意义。结论:RDN后,外周和中心血压明显降低,但肾灌注和肾功能没有改变。Ott C Janka R Schmid A Titze S Ditting T Sobotka PA Veelken R Uder M Schmieder RE 林东杰 叶鹏 2013中华高血压杂志2013,21,5:0
