| 1 | Bariatric surgery in patients with non-alcoholic fatty liver disease-from pathophysiology to clinical effects显示文摘Non-alcoholic fatty liver disease(NAFLD) is increasingly recognized as a significant liver disease,and it covers the disease spectrum from simple steatosis with a risk of development of non-alcoholic steatohepatitis(NASH) to fibrosis,subsequent cirrhosis,end-stage liver failure,and liver cancer with a potential need for liver transplantation.NAFLD and NASH are closely related to obesity,metabolic syndrome,and type 2 diabetes(T2 D).The role of gut hormones,especially glucagon-like peptide 1(GLP-1),is important in NAFLD.Bariatric surgery has the potential for inducing great weight loss and may improve the symptoms of metabolic syndrome and T2 D.Recent data demonstrated significant effects of bariatric surgery on GLP-1 and other gut hormones and important lipid metabolic and inflammatory abnormalities in the pathophysiology of NAFLD.Therefore,bariatric surgery may reverse the pathological liver changes in NAFLD and NASH patients.In the present review,we describe NAFLD and NASH pathophysiology and the primary effects of bariatric surgery on metabolic pathways.We performed a systematic review of the beneficial and harmful effects and focused on changes in liver disease severity in NAFLD and NASH patients.The specific focus was liver histopathology as assessed by the invasive liver biopsy.Additionally,we reviewed several non-invasive methods used for the assessment of liver disease severity following bariatric surgery. | Tea L Laursen Christoffer A Hagemann Chunshan Wei Konstantin Kazankov Karen L Thomsen Filip K Knop Henning Grønbæk | 2019 | World Journal of Hepatology2019,11,2: | 11 |

| 3 | Cervical cellulitis and mediastinitis following esophageal perforation:A case report显示文摘Chicken bone is one of the most frequent foreign bodies (FB) associated with upper esophageal perforation. Upper digestive tract penetrating FB may lead to life threatening complications and requires prompt management. We present the case of a 52-year-old man who sustained an upper esophageal perforation associated with cervical cellulitis and mediastinitis. Following CT-scan evidence of FB penetrating the esophagus, the impacted FB was successfully extracted under rigid esophagoscopy. Direct suture was required to close the esophageal perforation. Cervical and mediastinal drainage were made immediately. Naso- gastric tube decompression, broad-spectrum intravenous antibiotics, and parenteral hyperalimentation were administered for 10 d postoperatively. An esophagogram at d 10 revealed no leak at the repair site, and oral alimentation was successfully reinstituted. Conclusion: Rigid endoscope management of FB esophageal penetration is a simple, safe and effective procedure. | 7 | Stratigraphic Definition and Correlation of Middle Triassic Volcaniclastic Facies in the External Dinarides: Croatia and Bosnia and Herzegovina显示文摘Middle Triassic volcaniclastic depositsin the External Dinarides of Croatia and Bosnia and Herzegovina are related to the rifting of the Tethyan Ocean. Three localities in the External Dinarides: Donje Pazari?te, Bosansko Grahovo and Zelovo were biostratigraphically analysed in this study. The Middle Triassic carbonate deposits with volcaniclastic interlayers in Donje Pazari?te were defined by means of conodonts. Rare ammonoid specimens were collected.Recovered conodont and ammonoid taxa suggest these sections are of Early Illyrian to Early Fassanian Age. The section studied in Bosansko Grahovo is dominantly composed of volcanic and volcaniclastic rocks. Limestone peperites were collected for conodont analysis. Two conodont zones were defined, suggesting volcanic activity in the same, from Lower Illyrian to Fassanian, time interval. The Zelovo Section was biostratigraphically investigated by several authors. The pietra verde deposits from Zelovo are referred as late Fassanian to Early Longobardian Age. | 8 | Targeting the Wnt Signaling Pathway in Liver Fibrosis for Drug Options:An Update显示文摘morbidity and mortality for healthcare systems worldwide.It imparts an enormous economic burden to societies,making continuous research and informational updates about its pathogenesis and treatment crucial.This review′s focus is on the current knowledge about the Wnt signaling path-way,serving as an important pathway in liver fibrosis development and activation of hepatic stellate cells(HSCs).Two types of Wnt pathways are distinguished,namely the ß-catenin-dependent canonical and non-canonical Ca^(2+) or planar cell polarity(PCP)-dependent pathway.The dynamic balance of physiologically healthy liver and hepatocytes is disturbed by repeated liver injuries.Activation of theß-catenin Wnt pathway prevents the regeneration of hepatocytes by the replacement of extracellular matrix(ECM),leading to the appearance of scar tissue and the formation of regenerated nodular hepatocytes,lacking the original function of healthy hepatocytes.Therefore,liver function is reduced due to the severely advanced disease.Selective inhibition ofß-catenin inhibits inflammatory processes(since chemokines and pro-inflammatory cytokines are produced during Wnt activation),reduces growth of activated HSCs and reduces collagen synthesis and angiogenesis,thereby reducing the progression of liver fibrosis in vivo.While the canonical Wnt pathway is usually inactive in a physiologically healthy liver,it shows activity during cell regeneration or renewal and in certain pathophysiological conditions,such as liver diseases and cancer.Targeted blocking of some of the basic components of the Wnt path-way is a therapeutic approach.These include the frizzled transmembrane receptor(Fz)receptors using the secreted frizzled-related protein family(sFRP),Fz-coreceptors low-density LRP 5/6 through dickkopf-related protein 1(DKK1)or niclosamide,glycogen kinase-3 beta(GSK-3β)using SB-216763,cyclic-AMP response element-binding protein(CBP)using PRI-724 and ICG-001,the lymphoid enhancer binding factor(LEF)/T cell-specific transcription factor(TCF)system as well as Wnt inhibitory factor 1(WIF1)and miR-17-5p using pinostilbene hydrate(PSH).Significant progress has been made in inhibiting Wnt and thus stopping the progression of liver fibrosis by diminishing key components for its action.Comprehending the role of the Wnt signaling pathway in liver fibrosis may lead to discovery of novel targets in liver fibrosis therapeutic strategies’development. | Kristina Duspara Kristina Bojanic Josipa Ivanusic Pejic Lucija Kuna Tea Omanovic Kolaric Vjera Nincevic Robert Smolic Aleksandar Vcev Marija Glasnovic Ines Bilic Curcic Martina Smolic | 2021 | Journal of Clinical and Translational Hepatology2021,9,6: | 2 |
