| 1 | Lateral lymph node dissection for low rectal cancer: Is it necessary?显示文摘Rectal cancer constitutes a major public health issue.Total mesorectal excision has remained the gold standard treatment for mid and low rectal tumors since its introduction in the late 1980s.Removal of all lymph nodes located in the mesorectum has indeed improved pathological and oncological outcomes.However,when cancer spreads to the lateral lymph nodes(located along the iliac and obturator arteries)Western and Japanese practices differ.Where the Western guidelines consider this condition as an advanced form of the disease and use neoadjuvant radiochemotherapy liberally,the Japanese guidelines define it as a local disease and proceed to lateral lymph node dissection with or without neoadjuvant treatment.Herein,we review the current literature regarding both therapeutic strategies,with the aim of contributing to potential improvements in treatment and outcome for patients with low and mid rectal cancer. | Niki Christou Jeremy Meyer Christian Toso Frédéric Ris Nicolas Christian Buchs | 2019 | World Journal of Gastroenterology2019，25，31： | 11 |
| 2 | Management of hepatocellular adenoma: Solitary-uncomplicated, multiple and ruptured tumors显示文摘AIM: While hepatocellular adenomas (Has) have often been studied as a unique entity, we aimed to better define current management of the various forms of Has.METHODS: Twenty-five consecutive patients operated for solitary-uncomplicated (9), multiple (6), and ruptured (10)Has were reviewed according to management strategies and outcomes.RESULTS: All solitary-uncomplicated Has (ranged 2.2-14 cm in size) were removed. Out of 25 Has, 2 (8%) included foci of carcinoma. In the multiple HA group, previously undiagnosed tumors were identified during surgery in 5/6cases. In three cases with multiple spread HA, several lesions had to be left unresected. They remained unmodified after 4-, 6-, and 6-year radiological follow-up. Patients with ruptured HA (ranged 1.7-10 cm in size) were initially managed with hemodynamic support and angiography,allowing the embolization of actively bleeding tumors in two patients. All ruptured tumors were subsequently removed 5.5 d (range 4-70 d) after admission. CONCLUSION: Tumors suspected of HA, regardless of the size, should be resected, because of high chances of rupture causing bleeding, and/or containing malignant foci.Although it is desirable to remove all lesions of multiple HA, this may not be possible in some patients, for whom long-term radiological follow-up is advised. | 3 | Berberine behind the thriller of marked symptomatic bradycardia显示文摘Berberine is used in traditional Chinese medicine for the treatment of congestive heart failure, hypertension, diabetes, and dyslipidaemia and has a good safety profile. We report a case of a 53-year-old sportsman referred to our hospital for the onset of fatigue and dyspnoea upon exertion after he started berberine to treat hypercholesterolaemia. An electrocardiogram showed sinus bradycardia (45 bpm), first-degree atrioventricular block, and competitive junctional rhythm. An ergometric stress test showed slightly reduced chronotropic competence and the presence of runs of competitive junctional rhythm, atrial tachycardia, and sinus pauses in the recovery. After 10 d of wash-out from berberine, the patient experienced a complete resolution of symptoms, and an ergometric stress test showed good chronotropic competence. An electrocardiogram Holtershowed a latent hypervagotonic state. This is the first case report that shows that berberine could present certain side effects in hypervagotonic people, even in the absence of a situation that could cause drug accumulation. | 10 | Timber-concrete composite bridges:Three case studies显示文摘During the last years, timber-concrete composite(TCC) structures have been extensively used in Europe both in new and existing buildings. Generally speaking, a composite structure combines the advantages of both materials employed: the strength and stiffness of the concrete in compression and the tensile strength, lightweight, low embodied energy, and aesthetical appearance of the timber. The concrete slab provides protection of the timber beams from direct contact with water, which is crucial to ensure the durability of the timber beams, particularly when used for bridges. Different types of connectors can be used to provide force exchange between the concrete slab and the timber beam. The choice of a structurally effective yet cheap shear connection between the concrete topping and the timber joist is crucial to make the TCC structures a viable solution that can compete with reinforced concrete and steel structures. In this paper, the possibilities offered by TCC structures for short-span bridge decks are discussed. The technology of TCC structures and the general design rules are illustrated. Three case studies are reported, including a short-span bridge tested in Colorado, USA, with the timber layer being constructed from recycled utility poles and notch connection; a TCC bridge with glulam beams and triangular notches with epoxyglued rebar connectors built in Portugal; and a TCC bridge with glulam beams and rectangular notches built in Germany. All the solutions were found to be structurally effective and aesthetically pleasing.
