| 1 | Kallistatin,a new and reliable biomarker for the diagnosis of liver cirrhosis显示文摘Kallistatin,which protects organs and cells against inflammation,fibrosis and oxidative stress,is mainly synthesized and secreted in liver.However,its relationship to human liver disease remains unclear.The purpose of this study was to explore the relationship between serum kallistatin and clinical evidence of both cirrhosis and hepatocellular carcinoma(HCC),and to determine if serum kallistatin levels could be used as a diagnostic indicator of hepatic health status,especially human liver cirrhosis(LC).Our cohort consisted of 115 patients with clinically proven liver fibrosis(LF),LC,or HCC by liver biopsies,and 31 healthy controls(CON).Serum kallistatin levels were quantified by ELISA.Results of the present study demonstrated that irrespective of the underlying etiology,serum kallistatin levels were significantly lower in the LF/LC group when compared with the CON group.A decrease in serum kallistatin levels appeared to reflect the extent of cirrhosis,with the lowest levels associated with higher grades of cirrhosis.Patients with LC had a noticeable correlation between serum kallistatin levels and other serum biochemical indicators.The area under the curve(AUC) for LC,viral liver cirrhosis(VLC)and alcoholic liver cirrhosis(ALC) was 0.845,0.757 and 0.931,respectively.In conclusion,our findingsdemonstrated that kallistatin,a plasma protein produced by the liver,can be a useful and reliable diagnostic indicator of hepatic health status,especially for LC. | Zhiyun Cheng Yinghui Lv Suqiu Pang Ruyu Bai Mingxi Wang Shuyu Lin Tianwen Xu Duncan Spalding Nagy Habib Ruian Xu | 2015 | Acta Pharmaceutica Sinica B2015,5,3: | 7 |
| 2 | No difference in mortality among ALPPS,two-staged hepatectomy,and portal vein embolization/ligation:A systematic review by updated traditional and network meta-analyses显示文摘Background:There is an ongoing debate on the feasibility,safety,and oncological efficacy of the associating liver partition and portal vein ligation for staged hepatectomy(ALPPS)technique.The aim of this study was to compare ALPPS,two-staged hepatectomy(TSH),and portal vein embolization(PVE)/ligation(PVL)using updated traditional meta-analysis and network meta-analysis(NMA).Data sources:Electronic databases were used in a systematic literature search.Updated traditional metaanalysis and NMA were performed and compared.Mortality and major morbidity were selected as primary outcomes.Results:Nineteen studies including 1200 patients were selected from the pool of 436 studies.Of these patients,315(31%)and 702(69%)underwent ALPPS and portal vein occlusion(PVO),respectively.Ninetyday mortality based on updated traditional meta-analysis,subgroup analysis of the randomized controlled trials(RCTs),and both Bayesian and frequentist NMA did not demonstrate significant differences between the ALPPS cohort and the PVE,PVL,and TSH cohorts.Moreover,analysis of RCTs did not demonstrate significant differences of major morbidity between the ALPPS and PVO cohorts.The ALPPS cohort demonstrated significantly more favorable outcomes in hypertrophy parameters,time to operation,definitive hepatectomy,and R0 margins rates compared with the PVO cohort.In contrast,1-year disease-free survival was significantly higher in the PVO cohort compared to the ALPPS cohort.Conclusions:This study is the first to use updated traditional meta-analysis and both Bayesian and frequentist NMA and demonstrated no significant differences in 90-day mortality between the ALPPS and other hepatic hypertrophy approaches.Furthermore,two high quality RCTs including 147 patients demonstrated no significant differences in major morbidity between the ALPPS and PVO cohorts. | Paschalis Gavriilidis Robert P Sutcliffe Keith J Roberts Madhava Pai Duncan Spalding Nagy Habib Long R Jiao Mikael H Sodergren | 2020 | Hepatobiliary & Pancreatic Diseases International2020,19,5: | 6 |

| 6 | Combined administration of platelet rich plasma and autologous bone marrow aspirate concentrate for spinal cord injury: a descriptive case series显示文摘Administration of platelet rich plasma(PRP) and bone marrow aspirate concentrate(BMAC) has shown some promise in the treatment of neurological conditions;however, there is limited information on combined administration. As such, the purpose of this study was to assess safety and functional outcomes for patients administered combined autologous PRP and BMAC for spinal cord injury(SCI). This retrospective case series included seven patients who received combined treatment of autologous PRP and BMAC via intravenous and intrathecal administration as salvage therapy for SCI. Patients were reviewed for adverse reactions and clinical outcomes using the Oswestry Disability Index(ODI) for up to 1 year, as permitted by availability of follow-up data. Injury levels ranged from C3 through T11, and elapsed time between injury and salvage therapy ranged from 2.4 months to 6.2 years. Post-procedure complications were mild and rare, consisting only of self-limited headache and subjective memory impairment in one patient. Four patients experienced severe disability prior to PRP combined with BMAC injection, as evidenced by high(> 48/100) Oswestry Disability Index scores. Longitudinal Oswestry Disability Index scores for two patients with incomplete SCI at C6 and C7, both of whom had cervical spine injuries, demonstrated a decrease of 28–40% following salvage therapy, representing an improvement from severe to minimal disability. In conclusion, intrathecal/intravenous co-administration of PRP and BMAC resulted in no significant complications and may have had some clinical benefits. Larger clinical studies are needed to further test this method of treatment for patients with SCI who otherwise have limited meaningful treatment options.
