2A pediatric non-protein losing Menetrier's disease successfully treated with octreotide long acting release显示文摘Pediatric Menetrier's disease (MD) is an uncommon, acute, self-limited hypertrophic gastropathy characterized by enlarged gastric folds associated with epithelial hyperplasia and usually accompanied by protein losing gastropathy. Gastric cytomegalovirus infection is found in one third of MD children and its treatment is often associated with remission. Diagnosis often requires fullthickness biopsy due to inability to detect typical histological findings with conventional endoscopic biopsy. We report an uncommon case of non self-limited pediatric MD needing endoscopic mucosal resection for diagnosis which was then successfully treated with octreotide long-acting release (LAR). To the best of our knowledge, this is the first pediatric MD case successfully treated with octreotide LAR. 3Superconducting single-photon detectors integrated with diamond nanophotonic circuits显示文摘Photonic quantum technologies hold promise to repeat the success of integrated nanophotonic circuits in non-classical applications.Using linear optical elements,quantum optical computations can be performed with integrated optical circuits and can therefore overcome the existing limitations in terms of scalability.In addition to passive optical devices for realizing photonic quantum gates,active elements,such as single-photon sources and single-photon detectors,are essential ingredients for future optical quantum circuits.Material systems that allow for the monolithic integration of all components are particularly attractive,including III-V semiconductors,silicon and diamond.Here,we demonstrate nanophotonic integrated circuits made from high-quality polycrystalline diamond thin films in combination with on-chip single-photon detectors.By using superconducting nanowires that are coupled evanescently to traveling waves,we achieve high detection efficiencies of up to 66%as well as low dark count rates and a timing resolution of 190 ps.Our devices are fully scalable and hold promise for functional diamond photonic quantum devices.Patrik Rath Oliver Kahl Simone Ferrari Fabian Sproll Georgia Lewes-Malandrakis Dietmar Brink Konstantin Ilin Michael Siegel Christoph Nebel Wolfram Pernice 2015Light(Science & Applications)2015,4,1:3

12Leukocyte-technetium-99m uptake in Crohn's disease:Does it show subclinical disease?显示文摘AIM:To evaluate inflammatory activity in patients with Crohn's disease (CD) using technetium-99m-hexamethylpropyleneamine oxime (99mTc-HMPAO) granulocyte scintigraphy.METHODS: Twenty patients (7 male and 13 female) with CD and five healthy volunteers were selected for 99mTc-HMPAO granulocyte scintigraphy. The Crohn's Disease Activity Index (CDAI), blood tests and C-reactive protein (CRP) of each patient were performed 7 d before the scintigraphic images. The leukocytes were labeled according to the International Society of Radiolabeled Blood Elements (ISORBE) consensus protocol and the scintigraphic images, including single photon emission computed tomography, were obtained 30 min and 2 h after injection of the radiolabeled leukocytes.RESULTS:The labeling yield of the leukocytes with the lipophilic complex 99mTc-HMPAO was 55.0%±10%. Six of the 20 patients (30%) presented congruent results for the three parameters investigated (CDAI, Scintigraphic Index and CRP). On the other hand, 14 patients (70%) did not show congruent results. 18Very low calorie ketogenic diet and common rheumatic disorders:A case report显示文摘BACKGROUND The scientific literature provides evidence that nutritional ketosis can be an important support in the treatment of pathologies in which inflammation is present,as recent studies have shown that ketone bodies have anti-inflammatory activity in numerous diseases,including rheumatic diseases.We report the case of a 22-year-old woman with class I obesity and juvenile idiopathic arthritis who started treatment with a very low calorie ketogenic diet(VLCKD).CASE SUMMARY The patient was a 22-year-old woman diagnosed with juvenile idiopathic arthritis at age 4 years and with a body mass index(BMI)of 30.8 kg/m2,waist circumference(WC)80 cm,fat mass(FM)28.1 kg,free FM 45.7 kg,and visceral adipose tissue(VAT)3.5 kg,assessed on bioimpedance analysis.She was treated using a commercial VLCKD weight-loss program(PNK®method);this program provides high-biological-value protein preparations and natural foods.Each protein preparation contains 15 g protein,4 g carbohydrate,3 g fat,and 50 mg omega-3 docosahexaenoic acid,with an energy content of 90–120 kcal.After four months on the program,the BMI was 28.6 kg/m2,WC 73 cm,FM 23.2 kg,free FM 41.9 kg,and VAT 2.9 kg.CONCLUSION VLCKD enabled the patient to reach her target weight and to reduce her joint pain and headaches.Laboratory inflammatory indices also normalized.Mariangela Rondanelli Zaira Patelli Clara Gasparri Francesca Mansueto Cinzia Ferraris Mara Nichetti Tariq A Alalwan Ignacio Sajoux Roberto Maugeri Simone Perna 2023World Journal of Clinical Cases2023,11,9:0
