3Long-term outcomes of a phase Ⅱ randomized controlled trial comparing intensity-modulated radiotherapy with or without weekly cisplatin for the treatment of locally recurrent nasopharyngeal carcinoma显示文摘Background:Salvage treatment for locally recurrent nasopharyngeal carcinoma(NPC) is complicated and relatively limited.Radiotherapy,combined with effective concomitant chemotherapy,may improve clinical treatment outcomes.We conducted a phase Ⅱ randomized controlled trial to evaluate the efficacy of intensity-modulated radiotherapy with concomitant weekly cisplatin on locally recurrent NPC.Methods:Between April 2002 and January 2008,69 patients diagnosed with non-metastatic locally recurrent NPC were randomly assigned to either concomitant chemoradiotherapy group(n = 34) or radiotherapy alone group(n = 35).All patients received intensity-modulated radiotherapy.The radiotherapy dose for both groups was 60 Gy in 27 fractions for 37 days(range 23-53 days).The concomitant chemotherapy schedule was cisplatin 30 mg/m^2 by intravenous infusion weekly during radiotherapy.Results:The median follow-up period of all patients was 35 months(range 2-112 months).Between concomitant chemoradiotherapy and radiotherapy groups,there was only significant difference in the 3-year and 5-year overall survival(OS) rates(68.7%vs.42.2%,P = 0.016 and 41.8%vs.27.5%,P = 0.049,respectively).Subgroup analysis showed that concomitant chemoradiotherapy significantly improved the 5-year OS rate especially for patients in stage rT3-4(33.0%vs.13.2%,P = 0.009),stages Ⅲ-Ⅳ(34.3%vs.13.2%,P = 0.006),recurrence interval >30 months(49.0%vs.20.6%,P = 0.017),and tumor volume >26 cm^3(37.6%vs.0%,P = 0.006).Conclusion:Compared with radiotherapy alone,concomitant chemoradiotherapy can improve OS of the patients with locally recurrent NPC,especially those with advanced T category(rT3-4) and stage(lll-IV) diseases,recurrence intervals >30 months,and tumor volume >26 cm^3.Ying Guan Shuai Liu Han-Yu Wang Ying Guo Wei-Wei Xiao Chun-Yan Chen Chong Zhao Tai-Xiang Lu Fei Han 2016Chinese Journal of Cancer2016,35,4:19
4Insulin-like growth factor binding protein-7 induces activation and transdifferentiation of hepatic stellate cells in vitro显示文摘AIM:To investigate the role of insulin-like growth factor binding protein-7 (IGFBP-7) in the activation and transdifferentiation of hepatic stellate cells (HSC) in vitro.METHODS:Rat HSC-T6 cells were cultured in separate dishes and treated with various concentration of transforming growth factor (TGF)-β1,IGFBP-7 or antiIGFBP-7 antibody for 24 h.The supernatant or a cytoplasm suspension was obtained from cultured HSC,followed by transfer of cells to form cell-coated dishes.Immunocytochemistry and Western blotting were used to analyze the expression of IGFBP-7 induced by TGF-β1 and the level of fibronectin,collagen and α-smooth muscle actin (SMA).The pro-apoptotic effect of antiIGFBP-7 antibody was determined by flow cytometry.RESULTS:Immunocytochemistry and Western blotting revealed that the expression of IGFBP-7 in TGF-β1 treated HSC was significantly up-regulated compared to that in the control group.In addition,fibronectin,collagen and α-SMA also showed enhanced expression in accordance with the transdifferentiation process in a dose-dependent manner to some extent.Moreover,flow cytometry suggested that anti-IGFBP-7 antibody induced apoptosis of activated HSC,which is responsible for the development of liver fibrosis,and may represent a novel pathway and target for therapeutic intervention.CONCLUSION:IGFBP-7 showed increased expression in activated HSC and played an important role in the activation and transdifferentiation process of HSC.AntiIGFBP-7 antibody may ameliorate liver fibrogenesis.Li-Xin Liu Shuai Huang Qian-Qian Zhang Yi Liu Dong-Mei Zhang Xiao-Hong Guo De-Wu Han 2009World Journal of Gastroenterology2009,15,26:16
5Optimal duration of percutaneous microballoon compression for treatment of trigeminal nerve injury显示文摘Percutaneous microballoon compression of the trigeminal ganglion is a brand new operative technique for the treatment of trigeminal neuralgia. However, it is unclear how the procedure mediates pain relief, and there are no standardized criteria, such as compression pressure, compression time or balloon shape, for the procedure. In this study, percutaneous microballoon compression was performed on the rabbit trigeminal ganglion at a mean inflation pressure of 1,005 ± 150 mmHg for 2 or 5 minutes. At 1, 7 and 14 days after percutaneous microballoon compression, the large-diameter myelinated nerves displayed axonal swelling, rupture and demyelination under the electron microscope. Fragmentation of myelin and formation of digestion chambers were more evident after 5 minutes of compression. Image analyzer results showed that the diameter of trigeminal ganglion cells remained unaltered after compression. These experimental findings indicate that a 2-minute period of compression can suppress pain transduction. Immunohistochemical staining revealed that vascular endothelial growth factor expression in the ganglion cells and axons was significantly increased 7 days after trigeminal ganglion compression, however, the changes were similar after 2-minute compression and 5-minute compression. The upregulated expression of vascular endothelial growth factor in the ganglion cells after percutaneous microballoon compression can promote the repair of the injured nerve. 7Deworming of stray dogs and wild canines with praziquantel-laced baits delivered by an unmanned aerial vehicle in areas highly endemic for echinococcosis in China显示文摘Background:Canines,the definitive hosts for the parasites causing alveolar(AE)and cystic echinococcosis(CE),are the main source of this infections playing the key role in the transmission.The ten-year mortality rate of AE is extremely high(94%)if the patients are not given sustained treatment.The aim of this field study is to explore the possibility of delivery of praziquantel-laced baits using unmanned aerial vehicles(UAVs)aimed at deworming wild canines in the endemic areas.Methods:UAVs were compared to manual bait delivery in the 1-km^(2)test areas followed by testing of canine faeces using an Echinococcus coproantigen ELISA test in the ensuing year.The outcomes of the two approaches were compared with respect to time of delivery and overall cost.Findings:Compared to manual bait delivery,delivery by UAVs saved up to 67%of the overall cost.Three times more staff was needed for the former approach compared to the latter and,time wise,UAV bait delivery saved 350%compared to manual bait delivery on average.With regard to investment needed,the use of UAVs showed an efficiency 2.5 times better than manual bait delivery.Compared to the area served by UAVs,the average positive rate for the canine faecal samples was more than 38%higher in the area served manually.Conclusion:The technique of bait delivery with praziquantel using UAVs for canine deworming has a strong potential with regard to savings of manpower,time and overall cost in areas highly endemic for echinococcosis.Qing Yu Ning Xiao Shi-jie Yang Shuai Han 2017Infectious Diseases of Poverty2017,6,1:12
8Cyclin Dl antisense oligodexoyneucleotides inhibits growth and enhances chemosensitivity in gastric carcinoma cells显示文摘AIM: To examine the effects of cyclin D1 antisense oli-godexoyneucleotides (ASODN) on growth and chemosensitivity of gastric carcinoma cell lines SGC7901 and its mechanism. METHODS: Phosphorothioate modified cyclin D1 ASODN was encapsulated by LipofectAMINE2000 (LF2000) and transfected into cells, the dose-effect curves and growth curves were observed. 5-FU, MTX, CDDP of different concentrations were given after transfecting cells with cyclin D1 ASODN for 24 h, the dose-effect responses were observed and IC50s were calculated. The mRNA expression of cyclin D1, thymidylate synthase (TS), thy-midine phosphorylase (TP) and dihydrofolate reductase (DHFR) was detected by reverse transcription-PCR (RT-PCR) at 24 h and 48 h after transfection. RESULTS: Dose-dependent inhibitory effect was caused by cyclin Dl ASODN in SGC7901 cells. Transfecting gastric carcinoma cells with 0.2μmol/L cyclin D1 ASODN for 24 h could inhibit growth significantly and reduce expression of cyclin D1 mRNA. Cyclin D1 ASODN could increase the chemosensitivity to 5-FU, MTX, CDDP in cells, The IC50s of different chemotherapeutic agents in ASODN plus chemotherapy groups were significantly lower than those in controls. Transfection with cyclin D1 ASODN leaded to an increase in TS and DHFR mRNA and a decrease in TP mRNA as determined by RT-PCR at 24 h, the alterations were more significant at 48 h. 9Role of macrophages in peripheral nerve injury and repair显示文摘Resident and inflammatory macrophages are essential effectors of the innate immune system. These cells provide innate immune defenses and regulate tissue and organ homeostasis. In addition to their roles in diseases such as cancer, obesity and osteoarthritis, they play vital roles in tissue repair and disease rehabilitation. Macrophages and other inflammatory cells are recruited to tissue injury sites where they promote changes in the microenvironment. Among the inflammatory cell types, only macrophages have both pro-inflammatory (Ml) and anti-inflammatory (M2) actions, and M2 macrophages have four subtypes. The co-action of Ml and M2 subtypes can create a favorable microenvironment, releasing cytokines for damaged tissue repair. In this review, we discuss the activation of macrophages and their roles in severe peripheral nerve injury. 11Continuous Positive Airway Pressure Effectively Alleviates Arrhythmias in Patients with Obstructive Sleep Apnea:Possible Relationship with Counteracting Oxidative Stress显示文摘This work is aimed at exploring the clinical efficacy of continuous positive airway pressuie(CPAP)in treatment of patients with arrhythmias combined with obstructive sleep apnea(OSA).Through evaluating serum native thiol,malonaldehyde(MDA)and nicotinamide adenine dinucleotide phosphate oxidase(NADPH oxidase)in these patients and describing the effects on oxidative parameters of CPAP therapy for 3 months,we confirmed the impact of oxidative stress on arrhythmias.A total of 64 patients with OSA combined with arrhythmias were collected from April 2014 to April 2017 with full clinical information.Patients were divided into two groups(paired experiment design):32 patients in group A(control group),who received unchanged anti-arrhythmia treatment and 32 patients in group B,who were subjected to unchanged pharmacological anti-arrhythmia therapy combined with CPAP.OSA related parameters were compared between the two groups after 3-month therapy.And the levels of parameters of oxidative stress in patients were measured before and after CPAP therapy.After 3 months of CPAP therapy,compared with the control group,the percentage of sage N3(NREM 3)and stage R(REM)in total sleep time was significantly increased,while apnea-hypopnea index(AHI)and the Epworth Sleepiness Scale(ESS)score were evidently decreased.Meanwhile,the lowest oxygen saturation(LSpCh)was also elevated after CPAP treatment for 3 months.The CPAP therapy significantly prevented the occurrence of arrhythmias(P<0.05).Both the MDA level and NADPH oxidase levels were significantly lower in the group B than in the group A(P<0.05).But serum native thiol was improved by CPAP treatment(P<0.05).In conclusion,proper use of CPAP therapy provides significant benefits for the treatment of arrhythmia in patients with OSA.Xiao-ting WANG Gang ZHAO Ling TU Zhi-yong YUE Zhen-hua LIU Jie HAN Kun GAO Xuan-chen ZHOU Shuai XU Jian-feng LI 2019Current Medical Science2019,39,1:10
12Very high baseline HIV viremia impairs efficacy of non-nucleoside reverse transcriptase inhibitor-based ART:a longterm observation in treatment-naïve patients显示文摘Background:It is not completely clear whether a very high pre-therapy viral load(≥500000 copies/ml)can impair the virological response.The aim of this study was to examine the influence of very high baseline HIV-RNA levels on long-term virological responses under one type of regimen.Methods:A retrospective study was performed based on data from two multicenter cohorts in China from January to November 2009,and from May 2013 to December 2015.Untreated HIV infected adults between 18 and 65 years old were recruited before receiving non-nucleoside reverse transcriptase inhibitor-based regimen.All patients had baseline HIV-RNA levels over 500 copies/ml,good adherence,and were followed for at least 24 weeks.Virological suppression was defined as the first HIV-RNA<50 copies/ml.Virological failure was defined as any of incomplete viral suppression(HIV-RNA≥200 copies/ml without virological suppression within 24 weeks of treatment)and viral rebound(confirmed HIV-RNA level≥50 copies/ml after virological suppression).Chi-square test,Kaplan–Meier analysis,Cox proportional hazards model and Logistic regression were used to compare virological response between each pretreated viral load stratum.Results:A total of 758 treatment-naïve HIV patients in China were enlisted.Median follow-up time(IQR)was 144(108–276)weeks.By week 48,rates of virological suppression in three groups(<100000,100000–500000 and≥500000 copies/ml)were 94.1,85.0,and 63.8%,respectively(P<0.001).Very high baseline HIV viremia over 500000 copies/ml were found to be associated with delayed virological suppression(≥500000 vs<100000,adjusted relative hazard=0.455,95%CI:0.32–0.65;P<0.001)as well as incomplete viral suppression(≥500000 vs<100000,adjusted odds ratio[aOR]=6.084,95%CI:2.761–13.407;P<0.001)and viral rebound(≥50000 vs<100000,aOR=3.671,95%CI:1.009–13.355,P=0.048).Conclusions:Very high levels of pre-treatment HIV-RNA were related with delayed efficacy of NNRTI-based ART and increased risk of treatment failure.More potent initial regimens should be considered for those with this clinical character.Shuai Chen Yang Han Xiao-Jing Song Yan-ling Li Ting Zhu Hong-Zhou Lu Xiao-Ping Tang Tong Zhang Min Zhao Yun He Sheng-Hua He Min Wang Yong-Zhen Li Shao-Biao Huang Yong Li Jing Liu Wei Cao Tai-Sheng Li 2020Infectious Diseases of Poverty2020,9,3:10
13Theoretical and experimental investigation on a liquid-gas ejector power cycle using ammonia-water显示文摘The purpose of this paper is to investigate a novel power cycle using low-temperature heat sources such as oceanic-thermal, biomass as well as industrial waste heat. Both a reheater and a liquid-gas ejector are used in this ammonia-water based cycle. Energy analysis and parametric analysis are performed to guide the theoretical performance and experimental investigation is done to verify the theoretical results. The results show that the generator pressure, heating source temperature and turbine outlet depressurization made by the ejector can affect the cycle performances. Besides, the experimental thermal efficiency is much lower than the theoretical one on account of the heat losses and irreversibility. 14Petrogenesis, Tectonic Evolution and Geothermal Implications of Mesozoic Granites in the Huangshadong Geothermal Field, South China显示文摘Mesozoic multi-stage tectono-magmatic events produced widely distributed granitoids in the South China Block. Huangshadong(HSD) is located in south-eastern South China Block, where closely spaced hot springs accompany outcrops of Mesozoic granites. New data on whole-rock geochemistry, zircon U-Pb geochronology, and zircon Lu-Hf isotopes are presented, to study the petrogenesis and tectonic evolution of the granites, and to explore the relationship between granites and geothermal anomalies. Zircon U-Pb isotopes display three periods of granites in the HSD area: Indosinian(ca. 253 Ma, G4) muscovite-bearing monzonitic granite, early Yanshanian(ca. 175–155 Ma, G5 and G3) monzonitic granite and granodiorite, and late Yanshanian(ca. 140 Ma, G1 and G2) biotite monzonitic granite. In petrogenetic type, granites of the three periods are I-type granite. Among them, G1, G2, G3, and G4 are characterized by high fractionation, with high values of SiO2, alkalis, Ga/Al, and Rb/Sr, and depletion in Sr, Ba, Zr, Nb, Ti, REEs, with low(La/Yb)N, Nb/Ta, and Zr/Hf ratios and negative Eu anomalies. In terms of tectonic setting, 253 Ma G4 may be the product of partial melting of the ancient lower crust under post-orogenic extensional tectonics, as the closure of the Paleo-Tethys Ocean resulted in an intracontinental orogeny. At 175 Ma, the subduction of the Pacific Plate became the dominant tectonic system, and low-angle subduction of the Paleo-Pacific Plate facilitated partial melting of the subducted oceanic crust and basement to generate the hornblende-bearing I-type granodiorite. As the dip angle of the subducting plate increased, the continental arc tectonic setting was transformed to back-arc extension, inducing intense partial melting of the lower crust at ca. 158 Ma and resulting in the most frequent granitic magmatic activity in the South China hinterland. When slab foundering occurred at ca. 140 Ma, underplating of mantle-derived magmas caused melting of the continental crust, generating extensive highly fractionated granites in HSD. Combining the granitic evolution of HSD and adjacent areas and radioactive heat production rates, it is suggested that highly fractionated granites are connected to the enrichments in U and Th with magma evolution. The high radioactive heat derived from the Yanshanian granites is an important part of the crustal heat, which contributes significantly to the terrestrial heat flow. 15Multi-functional stretchable and flexible sensor array to determine the location, shape, and pressure: Application in a smart robot显示文摘The rapid increase in the aging population prompts the development of wearable devices and sophisticated robots. With their ability to collect complex information about their surroundings via e-skins, robots could perform more dynamic and variable tasks such as rescue missions or caring for the elderly. In this paper, we present a new concept of utilizing a very simple, highly flexible and stretchable capacitor sensor array, that can be attached on the surface of a retractable robot hand to realize three functions: determining the location, shape, and pressure of an object. This adaptive sensing system is accomplished using capacitors connected by aligned carbon nanotube(CNT) films constructed on an elastomer dielectric material, which can reduce the requirement on the accuracy of the machine vision system. 19Epidemiological survey of echinococcosis in Tibet Autonomous Region of China显示文摘Background:The echinococcosis is prevalent in 10 provinces/autonomous region in western and northern China.Epidemiological survey of echinococcosis in China in 2012 showed the average prevalence of four counties in Tibet Autonomous Region(TAR)is 4.23%,much higher than the average prevalence in China(0.24%).It is important to understand the transmission risks and the prevalence of echinococcosis in human and animals in TAR.Methods:A stratified and proportionate sampling method was used to select samples in TAR.The selected residents were examined by B-ultrasonography diagnostic,and the faeces of dogs were tested for the canine coproantigen against Echinococcus spp.using enzyme-linked immunosorbent assay.The internal organs of slaughtered domestic animals were examined by visual examination and palpation.The awareness of the prevention and control of echinococcosis among of residents and students was investigated using questionnaire.All data were inputted using double entry in the Epi Info database,with error correction by double-entry comparison,the statistical analysis of all data was processed using SPSS 21.0,and the map was mapped using ArcGIS 10.1,the data was tested by Chi-square test and Cochran-Armitage trend test.Results:A total of 80384 people,7564 faeces of dogs,and 2103 internal organs of slaughtered domestic animals were examined.The prevalence of echinococcosis in humans in TAR was 1.66%,the positive rate in females(1.92%)was significantly higher than that in males(1.41%),(X2=30.31,P<0.01),the positive rate of echinococcosis was positively associated with age(X2trend=-423.95,P<0.01),and the occupational populations with high positive rates of echinococcosis were herdsmen(3.66%)and monks(3.48%).The average positive rate of Echinococcus coproantigen in TAR was 7.30%.The positive rate of echinococcosis in livestock for the whole region was 11.84%.The average awareness rate of echinococcosis across the region was 33.39%.Conclusions:A high prevalence of echinococcosis is found across the TAR,representing a very serious concern to human health.Efforts should be made to develop an action plan for echinococcosis prevention and control as soon as possible,so as to control the endemic of echinococcosis and reduce the medical burden on the population.Bin Li Gongsang Quzhen Chui-Zhao Xue Shuai Han Wei-Qi Chen Xin-Liu Yan Zhong-Jie Li MLinda Quick Yong Huang Ning Xiao Ying Wang Li-Ying Wang Gesang Zuoga Bianba Gangzhu Bing-Cheng Ma Gasong Xiao-Gang Wei Niji Can-Jun Zheng Wei-Ping Wu Xiao-Nong Zhou 2019Infectious Diseases of Poverty2019,8,2:8
20Prognostic value of lymph node metastasis in patients with T1-stage colorectal cancer from multiple centers in China显示文摘AIM To explore the features and prognostic value of lymph node metastasis in patients with T1-stage colorectal cancer(CRC).METHODS In all,321 cases of T1-stage CRC were selected from 10132 patients with CRC who received surgical therapy in six large-scale hospitals in China and were retrospectively analyzed. Univariate and multivariate analyses were performed to analyze the risk factors for lymphatic metastasis. A survival analysis was then performed to analyze the prognostic value of lymph node metastasis.RESULTS The occurrence rate of T1 stage was 3.17%(321/10132);of these patients,the lymph node metastasis rate was 8.41%(27/321),and the non-lymph node metastasis rate was 91.59%(294/321). Univariate analysis showed that preoperative serum CEA,preoperative serum CA199,preoperative serum CA724,vascular invasion,and degree of differentiation were associated with lymph node metastasis in T1-stage CRC(P < 0.05 for all). Multivariate analysis indicated that preoperative serum CA724,vascular invasion,and degree of differentiation were closely related to lymph node metastasis(P < 0.05 for all). Log-rank survival analysis showed that age,preoperative serum CEA,preoperative serum CA199,vascular invasion,degree of differentiation,and lymph node metastasis(χ2 = 24.180,P < 0.001) were predictors of 5-year overall survival(OS)(P < 0.05 for all). COX regression analysis demonstrated that preoperative serum CA199 and lymph node metastasis(HR = 5.117;P < 0.05;95%CI: 0.058-0.815) were independent prognostic indicators of 5-year OS in patients with T1-stage CRC(P < 0.05 for both). CONCLUSION The morbidity of T1-stage CRC was 3.17% for all CRC cases. Preoperative serum CA724,vascular invasion,and degree of differentiation are independent risk factors for lymph node metastasis.
