1Role of artificial intelligence in hepatobiliary and pancreatic surgery显示文摘Over the past decade,enhanced preoperative imaging and visualization,improved delineation of the complex anatomical structures of the liver and pancreas,and intra-operative technological advances have helped deliver the liver and pancreatic surgery with increased safety and better postoperative outcomes.Artificial intelligence(AI)has a major role to play in 3D visualization,virtual simulation,augmented reality that helps in the training of surgeons and the future delivery of conventional,laparoscopic,and robotic hepatobiliary and pancreatic(HPB)surgery;artificial neural networks and machine learning has the potential to revolutionize individualized patient care during the preoperative imaging,and postoperative surveillance.In this paper,we reviewed the existing evidence and outlined the potential for applying AI in the perioperative care of patients undergoing HPB surgery.Hassaan Bari Sharan Wadhwani Bobby V M Dasari 2021World Journal of Gastrointestinal Surgery2021,13,1:4
2Comparative study of the efficacy of gentamicin-coated intramedullary interlocking nail versus regular intramedullary interlocking nail in Gustilo type I and II open tibia fractures显示文摘Purpose:Open tibia fracture is prone to infection,consequently causing significant morbidity and increasing the hospital stay,occupational loss and onset of chronic osteomyelitis.Intramedullary nailing is one choice for treating tibia shaft fractures.To improve the delivery of antibiotics at the tissue-implant interface,many methods have been proposed as a part of prophylaxis against infection.This study was conducted to study the role of gentamicin-impregnated intramedullary interlocking(IMIL)nail in the prevention of infection in Gustilo typeⅠandⅡopen tibia fractures and to compare the results with regular intramedullary nail.Methods:The study included 28 patients with open tibia fractures(Gustilo type 1 or type 2);of them 14 underwent regular IMIL nailing and the other 14 were treated with gentamicin-coated nailing.Randomization was done by alternate allocation of the patients.Follow-up was done postoperatively(day 1),1 week,6 weeks,and 6 months for bone union,erythrocyte sedimentation rate(ESR),hemoglobin and C?reactive protein(CRP).Statistical significance was tested using unpaired t-test.A p value less than 0.05 was considered significant.Results:There were 4 cases of infection in controls(regular IMIL nail)and no infection among patients treated with gentamicin-coated nail during the follow up(Х^2=4.66,p=0.031).At 6 months postoperatively,CRP(p=0.031),ESR(p=0.046)and hemoglobin level(p=0.016)showed significant difference between two groups.The bone healing rate was better with gentamicin-coated nail in comparison to regular IMIL nail at 6 months follow-up(p=0.016).Conclusion:Gentamicin-coated IMIL nail has a positive role in preventing infection in Gustilo typeⅠandⅡopen tibia fractures.Deepak Pinto K.Manjunatha Amarnath D.Savur Naufal Rizwan Ahmed Sharan Mally V.Ramya 2019Chinese Journal of Traumatology2019,22,5:4
3Who benefits from percutaneous closure of patent foramen ovale vs medical therapy for stroke prevention? In-depth and updated metaanalysis of randomized trials显示文摘BACKGROUND A few randomized clinical trials(RCT) and their meta-analyses have found patent foramen ovale closure(PFOC) to be beneficial in prevention of stroke compared to medical therapy. Whether the benefit is extended across all groups of patients remains unclear.AIM To evaluate the efficacy and safety of PFOC vs medical therapy in different groups of patients presenting with stroke, we performed this meta-analysis of RCTs.METHODS Electronic search of PubMed, EMBASE, Cochrane Central, CINAHL and ProQuest Central and manual search were performed from inception through September 2018 for RCTs. Ischemic stroke(IS), transient ischemic attack(TIA), a composite of IS, TIA and systemic embolism(SE), mortality, major bleeding,atrial fibrillation(AF) and procedural complications were the major outcomes.Random-effects model was used to perform analyses.RESULTS Meta-analysis of 6 RCTs including 3560 patients showed that the PFOC,compared to medical therapy reduced the risk of IS [odds ratio: 0.34; 95%confidence interval: 0.15-0.78; P = 0.01] and the composite of IS, TIA and SE [0.55(0.32-0.93); P = 0.02] and increased the AF risk [4.79(2.35-9.77); P < 0.0001]. No statistical difference was observed in the risk of TIA [0.86(0.54-1.38); P = 0.54],mortality [0.74(0.28-1.93); P = 0.53] and major bleeding [0.81(0.42-1.56); P = 0.53]between two strategies. 7MicroRNA-6744-5p promotes anoikis in breast cancer and directly targets NAT1 enzyme显示文摘Objective:Anoikis is apoptosis that is induced when cells detach from the extracellular matrix and neighboring cells.As anoikis serves as a regulatory barrier,cancer cells often acquire resistance towards anoikis during tumorigenesis to become metastatic.MicroRNAs(miRNAs)are short strand RNA molecules that regulate genes post-transcriptionally by binding to mRNAs and reducing the expression of its target genes.This study aimed to elucidate the role of a novel miRNA,miR-6744-5 p,in regulating anoikis in breast cancer and identify its target gene.Methods:An anoikis resistant variant of the luminal A type breast cancer MCF-7 cell line(MCF-7-AR)was generated by selecting and amplifying surviving cells after repeated exposure to growth in suspension.Mi RNA microarray analysis identified a list of dysregulated miRNAs from which miR-6744-5 p was chosen for overexpression and knockdown studies in MCF-7.Additionally,the miRNA was also overexpressed in a triple-negative breast cancer cell line,MDA-MB-231,to evaluate its ability to impair the metastatic potential of breast cancer cells.Results:This study showed that overexpression and knockdown of miR-6744-5 p in MCF-7 increased and decreased anoikis sensitivity,respectively.Similarly,overexpression of miR-6744-5 p in MDA-MB-231 increased anoikis and also decreased tumor cell invasion in vitro and in vivo.Furthermore,NAT1 enzyme was identified and validated as the direct target of miR-6744-5 p.Conclusions:This study has proven the ability of miR-6744-5 p to increase anoikis sensitivity in both luminal A and triple negative breast cancer cell lines,highlighting its therapeutic potential in treating breast cancer.Sharan Malagobadan Chai San Ho Noor Hasima Nagoor 2020Cancer Biology & Medicine2020,17,1:2
