1Computed tomography attenuation values of ascites are helpful to predict perforation site显示文摘AIM:To evaluate the effect of computed tomography(CT) attenuation values of ascites on gastrointestinal(GI) perforation site prediction.METHODS:The CT attenuation values of the ascites from 51 patients with GI perforations were measured by volume rendering to calculate the mean values.The effect of the CT attenuation values of the ascites on perforation site prediction and postoperative complications was evaluated.RESULTS:Of 24 patients with colorectal perforations,the CT attenuation values of ascites were significantly higher than those in patients with perforations at other sites [22.5 Hounsfield units(HU) vs 16.5 HU,respectively,P = 0.006].Colorectal perforation was significantly associated with postoperative complications(P = 0.038).The prediction rate of colorectal perforation using attenuation values as an auxiliary diagnosis improved by 9.8% compared to that of CT findings alone(92.2% vs 82.4%).CONCLUSION:The CT attenuation values of ascites could facilitate the prediction of perforation sites and postoperative complications in GI perforations,particularly in cases in which the perforation sites are difficult to predict by CT findings alone.Ryo Seishima Koji Okabayashi Hirotoshi Hasegawa Masashi Tsuruta Hiroki Hoshino Toru Yamada Yuko Kitagawa 2015World Journal of Gastroenterology2015,21,5:3
2Chemoprevention of colorectal cancer by targeting obesity-related metabolic abnormalities显示文摘Obesity and its related metabolic disorders, including insulin resistance and chronic inflammation, increase the risk of colorectal cancer(CRC). This observation suggests that the metabolic abnormalities associated with obesity can be effective targets for preventing the development of CRC in obese individuals. In recent years, many studies using obese and diabetic animal models have been conducted to investigate the chemoprevention of CRC using pharmaceutical or nutritional interventions. Pitavastatin, a medicine used to treat hyperlipidemia, prevents the development of obesityrelated colorectal carcinogenesis by attenuating chronic inflammation. Anti-hypertensive medicines, such as captopril and telmisartan, also suppress the formation of colonic preneoplastic lesions in obese and diabetic mice. In addition, several phytochemicals, including green tea catechins, have been reported to improve metabolic disorders and prevent the development of various cancers, including CRC. Moreover, the administration of branched-chain amino acids, which improves protein malnutrition and prevents the progression of hepatic failure, is effective for suppressing obesityrelated colon carcinogenesis, which is thought to be associated with improvements in insulin resistance. In the present article, we summarize the detailed relationship between metabolic abnormalities and the development of CRC.
