| 1 | Effects of Helicobacter pylori infection on gastric epithelial cell kinetics in patients with chronic renal failure显示文摘AIM: To evaluate the effects of Helicobacter pylori infection on gastric epithelial cell kinetics in patients with chronic renal failure (CRF).METHODS: Forty-four patients were enrolled in this study and divided into four groups with respect to their Helicobacter pylori (H pylori) and CRF status. Groups were labeled as follows: 1a: normal renal function, H pylori negative (n = 12), 1b: normal renal function,H pylori positive (n = 11), 2a: CRF, H pylori negative (n = 10), 2b: CRF, H pylori positive (n = 11). Upper gastrointestinal endoscopy was done in all the patients involved in the study. During endoscopical investigation,antral biopsy specimens were taken from each patient.In order to evaluate the cell apoptosis and proliferation in gastric epithelial cells, Bax and proliferating cell nuclear antigen (PCNA) labeling indexes (LI) were assessed with immunohistochemical staining method.RESULTS: For groups 1a, 1b, 2a, and 2b, mean Bax LI was identified as 34.4±13.7, 44.1±16.5, 46.3±20.5,60.7±13.8, respectively and mean PCNA LI was identified as 36.2±17.2, 53.6±25.6, 59.5±25.6, 67.2±22,respectively. When the one-way ANOVA test was applied,statistically significant differences were detected between the groups for both Bax LI (P = 0.004 <0.01) and PCNA LI (P = 0.009 <0.01). When groups were compared further in terms of Bax LI and PCNA LI with Tukey's HSD test for multiple pairwise comparisons, statistically significant difference was observed only between groups 1a and 2b (P = 0.006 <0.01).CONCLUSION: In gastric epithelial cells, expression of both the pre-apoptotic protein Bax and the proliferation marker PCNA increase with H pylori infection. This increase is more evident in patients with uremia. | 2 | Serum positive cagA in patients with non-ulcer dyspepsia and peptic ulcer disease from two centers in different regions of Turkey显示文摘AIM: To investigate and compare frequencies of serum positive cagA in patients from two separate regions of Turkey who were grouped according to the presence of peptic ulcer disease or non-ulcer dyspepsia.METHODS: One hundred and eighty Helicobacter pyloripositive patients with peptic ulcer disease or non-ulcer dyspepsia were included in the study. One hundred and fourteen patients had non-ulcer dyspepsia and 66 had peptic ulcer disease (32 with gastric ulcers and/or erosions and 34with duodenal ulcers). Each patient was tested for serum antibody to H. pylori cagA protein by enzyme immunoassay.RESULTS: The total frequency of serum positive cagA in the study group was 97.2 %. The rates in the patients with peptic ulcers and in those with non-ulcer dyspepsia were 100% and 95.6%, respectively. These results were similar to those reported in Asian studies, but higher than those that have been noted in other studies from Turkey and Western countries.CONCLUSION: The high rates of serum positive cagA in these patients with peptic ulcer disease and non-ulcer dyspepsia were similar to results reported in Asia. | 3 | Association of ezrin expression in intestinal and diffuse gastric carcinoma with clinicopathological parameters and tumor type显示文摘AIM: To investigate the correlation between ezrin expression and types of gastric carcinoma and clinicopathological variables. METHODS: We examined ezrin protein expression in 75 gastric carcinoma (53 intestinal types of adenocarcinoma, 22 diffuse types of carcinoma) tissues by immunohistochemistry. The results were compared with clinicopathological parameters such as tumor type, grade of tumor, clinical stage, presence of metastatic lymph node, and depth of invasion. RESULTS: Ezrin immunostaining was positive in 43 cases (81.1%) of intestinal type and in 9 (40.9%) cases of diffuse type adenocarcinomas (P < 0.001). In gastric carcinomas, the expression of ezrin protein correlated with the status of H pylori and survival. There was no correlation between expression of ezrin with TNM stage and histological grade of gastric carcinomas (P > 0.05). CONCLUSION: The low expression of ezrin implicates the loss of adhesion in diffuse carcinomas. | 4 | Current and future surgery strategies for spinal cord injuries显示文摘Spinal cord trauma is a prominent cause of mortality and morbidity. In developed countries a spinal cord injury(SCI) occurs every 16 min. SCI occurs due to tissue destruction, primarily by mechanical and secondarily ischemic. Primary damage occurs at the time of the injury. It cannot be improved. Following the primary injury, secondary harm mechanisms gradually result in neuronal death. One of the prominent causesof secondary harm is energy deficit, emerging from ischemia, whose main cause in the early stage, is impaired perfusion. Due to the advanced techniques in spinal surgery, SCI is still challenging for surgeons. Spinal cord doesn't have a self-repair property. The main damage occurs at the time of the injury primarily by mechanical factors that cannot be improved. Secondarily mechanisms take part in the following sections. Spinal compression and neurological deficit are two major factors used to decide on surgery. According to advanced imaging techniques the classifications systems for spinal injury has been changed in time. Aim of the surgery is to decompress the spinal channel and to restore the spinal alinement and mobilize the patient as soon as possible. | 5 | Clinical presentation and management of Fasciola hepatica infection:Single-center experience显示文摘AIM:To identify the characteristic clinical,laboratory and radiological findings and response to treatment in patients with fascioliasis.METHODS:Patients who were diagnosed with Fasciola hepatica infection were included in this prospective study.Initial clinical,laboratory and radiological findings were recorded.All patients were followed until a complete response was achieved or for 6 mo after treatment discontinuation.RESULTS:Fasciola hepatica infection was diagnosed in 30 patients(24 females;mean age:42.6 years) between January 2008 and February 2011.Twentytwo(73%) patients had hepatic phase fascioliasis,5 patients had biliary phase,and 3 patients had biliary phase associated with acute pancreatitis.Of the 8 patients with biliary phase fascioliasis,2 patients displayed features that overlapped with both hepatic and biliary phase.Abdominal pain and right upper abdominal tenderness were the most prominent signs and symptoms in all patients.Eosinophilia was the most prominent laboratory abnormality in both patients with hepatic and biliary phase(100% and 50%,respectively).Multiple nodular lesions like micro-abscesses on abdominal computerized tomography were the main radiological findings in patients with hepatic phase.Small linear filling defects in the distal choledochus were the main endoscopic retrograde cholangiopancreatography(ERCP) findings in patients with biliary phase.Patients with hepatic phase were treated with triclabendazole alone,and patients with biliary phase were treated with triclabendazole and had live Fasciola hepatica extracted from the bile ducts during ERCP.CONCLUSION:Fasciola hepatica infection should be considered in the differential diagnosis of patients with hepatic or biliary disease and/or acute pancreatitis associated with eosinophilia. | Muhsin Kaya Remzi Bestas Sedat Cetin | 2011 | World Journal of Gastroenterology2011,17,44: | 6 |
| 6 | Effects of glutamine and curcumin on bacterial translocation in jaundiced rats显示文摘AIM: To investigate the effect of curcumin on bacterial translocation and oxidative damage in an obstructive jaundice model and compare the results to glutamine, an agent known to be effective and clinically used. METHODS: Twenty-four female Wistar-Albino rats, weighing 200-250 g, were randomly divided into three groups (8 in each group). After ligation of the common bile duct in all animals, GroupⅠ received oral normal saline, Group Ⅱ received oral glutamine and Group Ⅲ received oral curcumin for seven days. Blood samples via cardiac puncture, tissue samples (terminal ileum, liver and mesenteric lymph node) and peritoneal fluid were obtained from the animals at the time of death to investigate bacterial translocation and oxidative damage. RESULTS: We observed that both glutamine and curcumin reduced bacterial translocation in blood, hepatocellular damage, plasma cytokine levels, oxidative tissue damage and apoptosis significantly compared to the control group. Additionally, glutamine showed protective effects on ileal epithelium and reduced villus atrophy. | 7 | He/icobacter py/ori infection in hemodialysis patients: Susceptibility to amoxicillin and clarithromycin显示文摘AIM: To evaluate susceptibility of Helicobacter pylori to amoxicillin and clarithromycin in end-stage renal disease (ESRD) patients and non-uremic controls.METHODS: The subjects with dyspeptic complaints were 33 ESRD patients and 46 age- and sex-matched nonuremic controls who exhibited H pylori on antral biopsy specimens. The two groups were age and sex matched.The H pylori strains' pattern of susceptibility to amoxicillin and clarithromycin was investigated with the agar dilution technique.RESULTS: None of the H pylori strains from either group showed resistance to amoxicillin with the agar dilution method. Twelve (36.4%) of the ESRD group strains and 7 (15.2%) of the control group strains showed resistance to clarithromycin, and this difference was statistically significant (P<0.05).CONCLUSION: Resistance to amoxicillin does not appear to be an important problem in H pylori-infected ESRD and non-uremic patients in our region. | 8 | Artificial neural network models predicting the leaf area index:a case study in pure even-aged Crimean pine forests from Turkey显示文摘Background: Leaf Area Index(LAI) is an important parameter used in monitoring and modeling of forest ecosystems. The aim of this study was to evaluate performance of the artificial neural network(ANN) models to predict the LAI by comparing the regression analysis models as the classical method in these pure and even-aged Crimean pine forest stands.Methods: One hundred eight temporary sample plots were collected from Crimean pine forest stands to estimate stand parameters. Each sample plot was imaged with hemispherical photographs to detect the LAI. The partial correlation analysis was used to assess the relationships between the stand LAI values and stand parameters, and the multivariate linear regression analysis was used to predict the LAI from stand parameters. Different artificial neural network models comprising different number of neuron and transfer functions were trained and used to predict the LAI of forest stands.Results: The correlation coefficients between LAI and stand parameters(stand number of trees, basal area, the quadratic mean diameter, stand density and stand age) were significant at the level of 0.01. The stand age, number of trees, site index, and basal area were independent parameters in the most successful regression model predicted LAI values using stand parameters(/?;adj = 0.5431). As corresponding method to predict the interactions between the stand LAI values and stand parameters, the neural network architecture based on the RBF 4-19-1 with Gaussian activation function in hidden layer and the identity activation function in output layer performed better in predicting LAI(SSE(12.1040), MSE(0.1223), RM5 E(0.3497), AIC(0.1040), BIC(-777310) and R2(0.6392)) compared to the other studied techniques.Conclusion: The ANN outperformed the multivariate regression techniques in predicting LAI from stand parameters. | 10 | Low eradication rate of Helicobacterpyloriwith triple 7-14 days and quadriple therapy in Turkey显示文摘AIM:The eradication rate of Helicobacter pylori (H pylon) shows variation among countries and regimens of treatment.We aimed to study the eradication rates of different regimens in our region and some factors affecting the rate of eradication.METHODS:One hundred and sixty-four Hpylori positivepatients (68 males, 96 females; mean age:48±12 years)with duodenal or gastric ulcer without a smoking history were included in the study. The patients were divided into three groups according to the treatment regimens. Omeprazole 20mg, clarithromycin 500mg, amoxicillin 1g were given twice daily for 1 week (Group I) and 2 weeks (Group Ⅱ).Patients in Group Ⅲ received bismuth subsitrate 300mg,tetracyline 500 mg and metronidazole 500mg four times daily in addition to Omeprazole 20mg twice daily.Two biopsies each before and after treatment were obtained from antrum and corpus, and histopathologically evaluated.Eradication was assumed to be successful if no Hpylorus was detected from four biopsy specimens taken after treatment. The effects of factors like age, sex, Hpyloridensity on antrum and corpus before treatment, the total Hpylori density, and the inflammation scores on the rate of Hpylori eradication were evaluated.RESULTS:The overall eradication rate was 42%. The rates in groups Ⅱ and Ⅲ were statistically higher than that in group I (P<0.05). The rates of eradication were 24.5%,40.7% and 61.5% in groups Ⅰ, Ⅱ and Ⅲ, respectively. The eradication rate was negatively related to either corpus Hpylori density or total Hpyloridensity (P<0.05).The median age was older in the group in which the eradication failed in comparison to that with successful eradication (55yr vs 39yr, P<0.001). | 12 | Helicobacter pylori eradication lowers serum homocysteine level in patients without gastric atrophy显示文摘AIM: To determine whether Helicobacter pylori ( H pylori)infection caused hyperhomocysteinemia by altering serum vitamin B12, serum folate and erythrocyte folate levels and whether eradication of this organism decreased serum homocysteine level.METHODS: The study involved 73 dyspeptic H pylori-positive patients, none of them had gastric mucosal atrophy based on rapid urease test and histology. Out of 73 patients, 41 (56.2%) showed a successful eradication of Hpylori4 wk after the end of treatment. In these 41 patients, fasting serum vitamin B12, folate and homocysteine levels, and erythrocyte folate levels before and 4 wk after Hpylorieradication therapy were compared.RESULTS: The group with a successful eradication of Hpylori had significantly higher serum vitamin B12 and erythrocyte folate levels in the post-treatment period compared to those in pre-treatment period (210±97 pg/mL vs237±94pg/mL, P<0.O01 and 442±212 ng/mL vs 539±304 ng/mL,P = 0.024, respectively), but showed no significant change in serum folate levels (5.6±2.6 ng/mL vs 6.0±2.4 ng/mL,P= 0.341). Also, the serum homocysteine levels in this group were significantly lower after therapy (13.1±5.2 μmol/L vs 11.9±6.2 μmol/L, P = 0.002). Regression analysis showed that serum homocysteine level was positively correlated with age (P = 0.01) and negatively with serum folate level before therapy (P = 0.003).CONCLUSION: Eradication of H pylori decreases serum homocysteine even in patients who do not exhibit gastric mucosal atrophy. | 13 | Reaching proficiency in laparoscopic splenectomy显示文摘AIM:To investigate the proficiency level reached in laparoscopic splenectomy using the learning curve method.METHODS:All patients in need of splenectomy for benign causes in whom laparoscopic splenectomy was attempted by a single surgeon during a time period of 6 years were included in the study (n=33). Besides demographics, operation-related variables and the response to surgery were recorded. The patients were allocated to groups of five, ranked according to the date of the operation. Operation duration, complications, postoperative length of stay, conversion to laparotomy and splenic weight were then compared between these groups.RESULTS: There was a significant difference regarding operation times between the groups (P = 0.001). An improvement was observed after the first 5 cases. The learning curve was flat up to the 25th case. Following the 25th case the operation times decreased still further. There was no difference between the groups regarding the other parameters.CONCLUSION: Unlike the widely accepted 'L' shape, the learning curve for laparoscopic splenectomy is a horizontal lazy 'S' with two distinct slopes. Privileges may be granted after the first 5 cases. | 15 | Deep learning for non-parameterized MEMS structural design显示文摘The geometric designs of MEMS devices can profoundly impact their physical properties and eventual performances.However,it is challenging for researchers to rationally consider a large number of possible designs,as it would be very time-and resource-consuming to study all these cases using numerical simulation.In this paper,we report the use of deep learning techniques to accelerate the MEMS design cycle by quickly and accurately predicting the physical properties of numerous design candidates with vastly different geometric features.Design candidates are represented in a nonparameterized,topologically unconstrained form using pixelated black-and-white images.After sufficient training,a deep neural network can quickly calculate the physical properties of interest with good accuracy without using conventional numerical tools such as finite element analysis.As an example,we apply our deep learning approach in the prediction of the modal frequency and quality factor of disk-shaped microscale resonators.With reasonable training,our deep learning neural network becomes a high-speed,high-accuracy calculator:it can identify the flexural mode frequency and the quality factor 4.6×10^(3)times and 2.6×10^(4)times faster,respectively,than conventional numerical simulation packages,with good accuracies of 98.8±1.6%and 96.8±3.1%,respectively.When simultaneously predicting the frequency and the quality factor,up to~96.0%of the total computation time can be saved during the design process.The proposed technique can rapidly screen over thousands of design candidates and promotes experience-free and data-driven MEMS structural designs. | Ruiqi Guo Fanping Sui Wei Yue Zekai Wang Sedat Pala Kunying Li Renxiao Xu Liwei Lin | 2022 | Microsystems & Nanoengineering2022,8,4: | 2 |
| 16 | Hepatitis C infection in hemodialysis patients: A review显示文摘Hepatitis C virus(HCV)-related liver disease is a significant cause of morbidity and mortality in patients with end-stage renal disease(ESRD) who is treated with dialysis or kidney transplantation(KT). The survival rate for HCV-infected renal transplant recipients is better than that for HCV-infected hemodialysis patients on transplant waiting lists. Early diagnosis and treatment HCV infection prior to KT prevents complications posttransplantation and reduces mortality. In addition to screening for anti-HCV antibodies and detecting HCV RNA, percutaneous liver biopsy is particularly valuable for assessing the stage of liver damage in HCV-infected patients, because the stage of fibrosis is importantdetermining optimal treatment for HCV. Studies have been demonstrated that with conventional interferon(IFN) monotherapy or pegylated IFN monotherapy are similar efficacy and safety in HCV-infected hemodialysis patients. Sustained viral responses(SVRs) with these monotherapies have ranged approximately 30% to 40%. Limited reports support the use of IFN and ribavirin combination therapy as antiviral treatment for ESRD patients or patients on hemodialysis. Ribavirin can be started at low dose and careful monitoring for side effects. Patients that show SVR after treatment are strong candidates for KT. | 17 | Evaluation of mixed mode-Ⅰ/Ⅱ criteria for fatigue crack propagation using experiments and modeling显示文摘In this study, in-plane mixed mode-Ⅰ/Ⅱ fatigue crack growth simulations and experiments are performed for the Al 7075-T651 aluminum alloy which is widely used in the aerospace industry. Tests are carried out under different mode mixity ratios to evaluate the applicability of a fracture criterion developed in a previous study to mixed mode-Ⅰ/Ⅱ fatigue crack growth tests.Results obtained from the analyses and experiments are compared with existing and developed criteria in terms of crack growth lives. Compact Tension Shear(CTS) specimens, which enable mixed mode loading with loading devices under different loading angles, are used in the simulations and experiments. In an effort to model and simulate the actual conditions in the experiments, crack surfaces of fractured specimens are scanned, crack paths are modeled exactly, and contacts are defined between the contact surfaces of a specimen and the loading device for each crack propagation step in the analyses.
