2第二次中国临床血脂控制达标率及影响因素多中心协作研究显示文摘目的了解我国临床血脂控制的最新现状,指导临床血脂异常防治实践。方法在全国21家省部级医院和6家地县级医院中,查阅2004年1月1日至2006年2月28日间开始服调脂药物,且同一药物同一剂量维持≥2个月的2237名患者病例资料,依据美国2004年国家胆固醇教育计划(NCEP)成人治疗组第三次报告(ATPBⅢ)及《中国成人血脂异常防治指南》标准计算血脂控制达标率。结果 (1)在符合任一血脂防治建议/指南的药物起始治疗标准的2094例患者中,80%来自省部级医院,60%为60岁以上,57%有胆固醇升高,15%无血脂异常,68%合并冠心病等动脉粥样硬化性疾病,75%合并高血压,80%为高危和极高危患者,84%使用他汀类药物,83%采取了不同程度的饮食治疗。(2)依据美国2004年 NCEP ATPⅢ最新报告,总达标率为34%,低危组、中危组、中高危组、高危组和极高危组达标率分别为85%、78%、61%、3 1%和22%,差异有统计学意义(趋势性检验 P<0.001);依据我国新的《成人血脂异常防治指南》,总达标率为50%,低危组、中危组、高危组和极高危组达标率分别为91%、77%、49%和38%,组间差异及趋势性检验均有统计学意义(P<0.001)。(3)联合用药者达标率为51%,单用他汀类35%,贝特类23%,烟酸类24%,其他类28%,组间差异有统计学意义(P<0.001)。(4)对1808例服用他汀类药物患者的多元 logistic 回归分析表明,他汀类药物剂量(高剂量比低剂量,OR=1.72,95%CI:1.15~2.58)、危险分层(极高危比低危,OR=0.02,95%CI:0.01~0.03)、基线 LDL-C[每升高0.259 mmol/L(10 mg/dl),OR=0.83,95%CI:0.80~0.86]和性别(女性比男性,OR=0.77,95%CI:0.60~0.99)等是影响达标率的主要因素。结论我国目前调脂药物的应用对象发生了很大变化,调脂治疗的目的已不单纯是为了降低胆固醇。临床血脂控制状况与各防治指南要求相距仍甚远,特别是高危和极高危患者。要进一步改善我国临床血脂控制状况,药物种类、药物剂量、联合治疗和治疗性生活方式改变等多方面均需要进一步提高。The Collaborative Research Group for the Second Multi-center Survey of Clinical Management of Dyslipidemia in China 2007中华心血管病杂志2007,35,5:127
3Growth inhibition and apoptosis induction Sulindac on Human gastric cancer cells显示文摘AIM: To evaluate the effects of sulindac in inducing growth inhibition and apoptosis of human gastric cancer cells in comparison with human hepatocellular carcinoma (HCC)cells.METHODS: The human gastric cancer cell lines MKN45 and MKN28 and human hepatocellular carcinoma cell lines HepG2and SMMC7721 were used for the study. Anti-proliferative effect was measured by MTT assay, and apoptosis was determined by Hoechst-33258 staining, electronography and DNA fragmentation. The protein of cyclooxygenase-2 (COX(2) and Bcl-2 were detected by Westem dot blotting.RESULTS: Sulindac could initiate growth inhibition and apoptosis of MKN45, MKN28, HepG2 and SMMC7721 cells in a dose-and time-dependent manner. Growth inhibitory activity and apoptosis were more sensitive in HepG2 cells than in SMMC7721 cells, MKN45 and MKN28 cells. After 24hours incubation with sulindac at 2mmol. L-1 and 4mmol.L-1, the level of COX-2 and Bcl-2 protein were lowered in MKN45, SMMC7721 and HepG2 cells but not in MKN28 cells.CONCLUSION: Sulindac could inhibit the growth of gastric cancer cells and HCC cells effectively in vitro by apoptosis induction, which was associated with regression of COX-2and Bcl-2 expression. The growth inhibition and apoptosis of HCC cells were greater then that of human gastric cancer cells. 4Vascular endothelial growth factor and microvascular density in esophageal and gastric carcinomas显示文摘AIM: To observe the relationship between the expression of vascular endothelial growth factor (VEGF), microvascular density (MVD) and the pathological characteristics of esophageal and gastric carcinomas.METHODS: S-P immunohistochemical staining was used to investigate the expression of VEGF in all the specimens.The antibody against factor VIII-related antigen was used to display vascular endothelial cells, and MVD was examined by counting the factor VIII-positive vascular endothelial cells.RESULTS: The positive rates of VEGF expression in esophageal carcinoma and gastric carcinoma were 81.36 %and 67.5 % respectively, and the MVD averaged 41.81±8.44and 34.36±9.67 respectively, which were higher than those in benign diseases. The expression of VEGF and MVD were closely correlated with the degree of differentiation, lymphatic metastasis, but not related to depth of cancer invasion. In early stage gastric carcinoma, the rate of expression of VEGF and MVD was lower than that in progressive gastric carcinomas. 5Formalized therapeutic guideline for hyperlipidemic severe acute pancreatitis显示文摘AIM: To investigate a formalized therapeutic guideline for hyperlipidemic severe acute pancreatitis (HL-SAP).METHODS: Thirty-two consecutive patients with severe acute pancreatitis were included in the clinical trial. All of them met the following five criteria for admission to the study, namely the Atlanta classification and stratification system for the clinical diagnosis of SAP, APACHEII score more than 8, time interval for therapeutic intervention less than 72 hours after onset of the disease, serum triglyceride (TG) level 6.8 mmol/l or over, and exdusion of other etiologies.They were divided into severe acute pancreatitis group (SAP,22 patients) and fulminant severe acute pancreatitis group (FSAP, 10 patients). Besides the conventional therapeutic measures, Penta-association therapywas also applied in the two groups, which consisted of blood purification (adsorption of triglyceride and hemofiltration), antihyperlipidemic agents (fluvastatin or lipanthyl), low molecular weight heparin (fragmin), insulin, topical application of Pixiao (a traditional Chinese medicine) over the whole abdomen, serum triglyceride,pro-inflammatory cytokines and anti-inflammatory cytokines were determined before blood purification (PF), at the end of blood purification (AFE) and on the 7th day after onset of the disease (AF7) respectively. Simultaneously, severity of the diseases was assessed by the APACHE Ⅱ system.Prognosis was evaluated by non-operation cure rate,absorption rate of pseudocyst, time interval pseudocyst absorption, hospital stay and survival rate.RESULTS: Serum triglyceride level (mmol/L), TNFα (U/mi) concentration and APACHE Ⅱ score were significantly decreased (P<0.05) at AFE and AF7, as compared with PF.However, serum IL-10 concentration (pg/ml) was increased significantly (P<0.001) at AFE, and decreased significantly (P<0.05) at AF7 when compared with PF. Operations: The First surgical intervention time was 55.8±42.6 days in SAP group (5 patients) and 12.2±6.6 days in FSAP group (7 patients),there was a significant difference between the two groups (P=0.02). The number of operations in the two groups was 1.33±0.5 vs3.5±1.2 (P=0.0037), respectively. Prognosis: Nonoperation cure rate, absorption rate of pseudocyst, hospital stay and survival rate in SAP group and FSAP group were 100 % (22/22) vs11.1% (1/9), 77.3 % (17/22) vs 11.1%(1/9), 54.2±35.9 vs99.1±49.5 days (P=0.008) and 100 %(22/22) vs66.7 % (6/9) (P=0.0044). 6Oncofetal antigen glypican-3 as a promising early diagnostic marker for hepatocellular carcinoma显示文摘BACKGROUND:Hepatocellular carcinoma (HCC) is characterized by a multi-cause,multi-stage and multi-focus process of tumor progression.Its prognosis is poor and early diagnosis is of utmost importance.This study was undertaken to investigate the dynamic expression of oncofetal antigen glypican-3 (GPC-3) and GPC-3 mRNA in hepatocarcinogenesis and to explore their early diagnostic value for HCC.METHODS:A hepatoma model was induced in male Sprague-Dawley rats with 0.05% 2-fluorenylacetamide and confirmed by hematoxylin and eosin staining and gamma-glutamyltransferase (GGT) expression.Total RNA was purified and transcribed into cDNA by reverse transcription.Fragments of the GPC-3 gene were amplified by nested RT-PCR,and confirmed by sequencing.GPC-3 was analyzed by immunohistochemistry,Western blotting or ELISA.RESULTS:Positive GPC-3 expression showed as brown granule-like staining localized in the cytoplasm.Histological examination of hepatocytes revealed three morphological stages of granule-like degeneration,atypical hyperplasia (precancerous),and cancer formation,with a progressive increase of liver total RNA and GGT expression.The incidence of liver GPC-3 mRNA and GPC-3,and serum GPC-3 was 100%,100% and 77.8% in the HCC group,100%,100%,and 66.7% in the precancerous group,83.3%,83.3%,and 38.9% in the degeneration group,and no expression in the liver or blood of the control group,respectively.There was a positive correlation between liver GPC-3 mRNA and total RNA level (r=0.475,P<0.05) or liver GPC-3 (r=1.0,P<0.001) or serum GPC-3 (r= 0.994,P<0.001).CONCLUSION:Abnormal oncofetal antigen GPC-3 and GPC-3 mRNA expression in hepatocarcinogenesis may be promising molecular markers for early diagnosis of HCC.Research Center of Clinical Medicine,Affiliated Hospital of Nantong University,Nantong 226001,China (Yao M,Yao DF,Qiu LW,Wu W,Sai WL,Yang JL and Zhang HJ) Department of Oncology,Yancheng First People’s Hospital,Yancheng 224001,China (Bian YZ) Department of Oncology,Second Affiliated Hospital,Nanjing Medical University,Nanjing 210011,China (Zhang CG) 2011Hepatobiliary & Pancreatic Diseases International2011,10,3:51

8Effects of time interval for hemofiltration on the prognosis of severe acute pancreatitis显示文摘AIM: To evaluate the impact of time interval for hemofiltration(HF) on the prognosis of severe acute pancreatitis (SAP).METHODS: Thirty-six consecutive patients with severe acutepancreatitis were included in the study. Atlanta classificationsystem was applied for stratification. They were randomlydivided into short veno-venous HF group, (SWH, Group 1,20 patients); and long veno-venous HF group (LWH, Group2, 16 patients). In group 1, SWH was stopped when theabdominal signs disappeared, and heart rate and breathrate were less than 90 leats/min and 20 times/min,respectively. HF was stopped if SWH was continued, andwhen heart rate and breath rate were more than 90 beats/min and 20 times/min again (Group 2). Except that the timeinterval for HF was different, other parameters for HF werethe same. And conservative curing rate, survival rate, costfor hospital stay and length of hospital stay were observed.RESULTS: Time interval for HF in Group 1 (3.81±1.3 hr)was shorter than that of in Group 2 (9.38+2.9hr), P<0.01.Conservative curing rate (90 %) in Group 1 was much higherthan that in Group 2 (56.3 %) (P<0.05); but cost in Group 1(RMB 56 600±56 400 Yuan) was lower than that in Group 2(RMB 137 000±105 000 Yuan) (P<0.05). And the survival rate(95 %) in Group 1 was higher than that in Group 2 (81.3 %)(P<0.25); however, hospital stay in Group 1 (44.3±41 days)was shorter than that in Group 2 (55.2±39.5 days) (P<0.2).So, the prognosis was not improved through the prolongationof time interval for HF, but side-effects were seen.CONCLUSION: The prognosis was not further improvedby LWH in the treatment of SAP, with side-effects. Timeinterval for HF plays an important role in treatment of SAPin early stage. 11Effects of Chinese Jianpi herbs on cell apoptosis and related gene expression in human gastric cancer grafted onto nude mice显示文摘AIM: To explore the mechanism of the Sijunzi decoction and another Chinese herbal recipe (SRRS) based mainly on the Sijunzi decoction in treatment of gastric cancer.METHODS: A human gastric adenocarcinoma cell line SGC7901 grafted onto nude mouse was used as the animal model. The mice were divided into 3 groups, one control and the two representative experimental conditions. Ahimals in the two experimental groups received either Sijunzi decoction or SRRS over a 40-day period starting at 1st day after grafting. Control animals received saline on an identical schedule. Animals were killed 41 days after being grafted.The effect of therapy was assessed by two ways: (1)tumor size was periodically measured during the life of the animals; (2) tumor weight was determined by a electron balance immediately after the animals killed. For detection of apoptotic cells, apoptotic indices(AI) were examined by the terminal deoxynucleotidyl transferase-mediated deoxyuddine tdphosphate fluorescence nick end labeling (TUNEL) method.Morphological alterations were observed with electron microscopy. S-P immunohistochemical method was used to detect the expression of Ki-67 in xenografts. Expression of bcl-2 and p53 was semiquantitatively detected using a reverse transcriptase-polymerase chain reaction (RT-PCR)technique.RESULTS: When compared with controls, tumor growth (size and weight) was significantly inhibited by treatment with the Sijunzi decoction (P<0.05) or SRRS (P<0.01). The tumor inhibitory rate in the Sijunzi decoction group was 34.33 % and SRRS group 46.53 %. AI of human gastric cancer xenografts in nude mice was significantly increased to 16.24±3.21% using TUNEL method and 11.38±6.46 % by FACScan in the Sijunzi decoction group compared with the controls (TUNEL: 2.63±1.03 %, P<0.01; FACScan: 7.15± 1.32 %, P<0.05). SRRS group was also found a significantly increased AI by using TUNEL method and flow cytometry analysis compared with the controls (TUNEL: 13.18±3.05 %,P<0.05; FACScan: 11.58±5.71% (P<0.05). Under electron microscope, cell shrinkage, nuclear chromatin condensation,formation of membrane blebs and apoptotic bodies were frequently observed in Sijunzi decoction group and SRRS group. The average labeling index (LI) for Ki-67 in SRRS group was significantly decreased to 8.43±2.22 % compared with the control group (10.37±t.91%) (P<0.05). The average labeling index for Ki-67 in sijunzi decoction group was 7.95±2.54 % which was lower than that of the control group, but showed no significance (P=0.07). The expression level of p53 mRNA was lower in both Sijunzi decoction group and SRRS group than that in control group (P<0.05; P<0.01). 12Abnormal immunity and gene mutation in patients with severe hepatitis-B显示文摘AIM: To evaluate the abnormal immunity and gene mutation at precore 1896 site in patients with severe hepatitis-B.METHODS: This study included 23 patients with severe hepatitis-B, 22 patients with acute hepatitis-B and 20 controls.Mutation at precore 1896 site of HBV gene was confirmed with restriction fragment length polymorphism (RFLP) analysis.Cytokines including TNF-α, IFN-y, IL-6, and IL-8 were measured with ELISA, and T subgroups were detected with alkaline phosphatase anti alkaline phosphatase (APAAP) technique.RESULTS: In patients with severe hepatitis-B, the infective rate of HBV mutant strain was 52.5% (12/23), and only one patient with acute hepatitis-B was infected with the mutant strain. The percentage of CD8+ T lymphocyte was obviously lower (0.16±0.02%) and the ratio of CD4+/CD8+ was obviously higher (2.35±0.89) in mutant group than in wildtype group (0.28±0.05% and 1.31±0.18%, respectively,P<0.01 or P<0.05). The levels of cytokines in patients with severe hepatitis-B were higher (TNF-α 359.0±17.2 ng/L, IFNγ 234.7±16.5 ng/L, IL-6 347.5±31.3 ng/L, IL-8 181.1±19.6ng/L) than those in acute hepatitis-B (TNF-α 220.6±8.9ng/L, IFN-γ 174.9±12.0 ng/L, IL-6 285.8±16.5 ng/L, IL-8118.4±5.1 ng/L, P<0.01 or 0.05). In patients with severe hepatitis-B, the levels of IFN-γ and IL-6 were higher in mutant group (273.4±26.6 ng/L, 387.7±32.5 ng/L) than in wild-type group (207.8±12.8 ng/L, 300.9±16.3 ng/L). The mortality of patients infected with HBV mutant strain was higher (100%)than that with wild-type (0.9%).CONCLUSION: In severe hepatitis-B, the infective rate of HBV mutant strain was high. The mutant strain induces more severe immune disorders in host, resulting in the activation of lymphocyte and release of cytokines. HBV DNA mutates easily in response to the altered immunity. 13Inhibitive effect of cordyceps sinensis on experimental hepatic fibrosis and its possible mechanism显示文摘AIM: To investigate the inhibitive effect and its possible mechanism of Cordyceps Sinensis (CS) on CCl4-plus ethanolinduced hepatic fibrogenesis in experimental rats.METHODS: Rats were randomly allocated into a normal control group, a model control group and a CS group. The latter two groups were administered with CCl4 and ethanol solution at the beginning of the experiment to induce hepatic fibrosis. The CS group was also treated with CS 10 days after the beginning of CCl4 and ethanol administration. All control groups were given corresponding placebo at the same time. At the end of the 9th week, rats in each group were humanely sacrificed. Blood and tissue specimens were taken.Biochemical, radioimmunological, immunohistochemical and molecular biological examinations were used to determine the level change of ALT, AST, HA, LN content in serum and TGFβ1, PDGF, collagen Ⅰ and Ⅲ expression in tissue at either protein or mRNA level or both of them.RESULTS: As compared with the model control group,serum ALr, AST, HA, and LN content levels were markedly dropped in CS group (86.0±34.4 vs224.3±178.9, 146.7±60.2vs272.6±130.1, 202.0±79.3 vs316.5±94.1 and 50.4±3.0vs 59.7±9.8, respectively, P<0.05). Tissue expression of TGFβ1 and its mRNA, collagen I mRNA were also markedly decreased (0.2±0.14 vs1.73±1.40, 1.68±0.47 vs3.17±1.17,1.10±0.84 vs 2.64±1.40, respectively, P<0.05). More dramatical drop could be seen in PDGF expression (0.87±0.43vs1.91±0.74, P<0.01). Although there was no statistical significance, it was still strongly suggested that collagen Ⅲ mRNA expression was also decreased in CS group as compared with model control group (0.36±0.27 vs0.95±0.65,P=0.0615). In this experiment, no significant change could be found in PDGF mRNA expression between two groups (0.35±0.34 vs 0.70±0.46, P>0.05).CONCLUSION: Cordyceps sinensis could inhibit hepatic fibrogenesis derived from chronic liver injury, retard the development of cirrhosis, and notably ameliorate the liver function. PCR and Southern blot analysis wereperformed to determine if U6 cassette integratedinto the genomic DNA of positive clone.Transfected tumor cells were evaluated for RNAexpression by ribonuclease protection assays.The VEGF protein in the supernatant of parentaltumor cells and genetically modified tumor cellswas determined with ELISA. In vitro and in vivogrowth properties of antisense VEGF cell clonein nude mice were analyzed.RESULTS Restriction enzyme digestion andPCR sequencing verified that the antisense VEGFRNA retroviral vector was successfullyconstructed. After G418 selection, resistantSMMC-7721 cell clone was picked up. PCR andSouthern blot analysis suggested that U6cassette was integrated into the cell genomicDNA. Stable SMMC-7721 cell clone transducedwith U6 antisense RNA cassette could express200bp small antisense VEGF RNA and secretereduced levels of VEGF in culture condition.Production of VEGF by antisense transgeneexpressing cells was 65 ± 10 ng / L per 106 cells,420 ± 45 ng/L per 106 cells in sense group and 485± 30 ng/L per 106 cells in the negative control group, (P<0.05). The antisense-VEGF cell clone appeared phenotypically indistinguishable from SMMC-7721 cells and SMMC-7721 cells transfected sense VEGF. The growth rate of the antisense-VEGF cell clone was the same as the control cells. When S. 18Clinical short-term results of radiofrequency ablation in liver cancers显示文摘AIM: To study local therapeutic efficacy, side effects, andcomplications of radiofrequency ablation (RFA), whichis emerging as a new method for the treatment ofpatients with hepatocellular carcinoma (HCC) withcirrhosis or chronic hepatitis and metastatic liver cancer.METHODS: Thirty-six patients with primary andsecondary liver cancers (21 with primaryhepatocellular carcinoma, 12 with colorectal cancerliver metastases and 3 with other malignant livermetastases), which were considered not suitable forcurative resection, were include in this study. Theywere treated either with RFA (RITA2000, MountainView, California, USA) percutaneously (n=20)orintraoperatively (n=16).The procedures wereperformed using the ultrasound guidance. The qualityof RFA were based on monitoring of equipments andsubject feeling of the practitioners. Patients treatedwith RFA was followed according to clinical findings,radiographic images, and tumor markers.RESULTS: Thirty-six patients underwent RFA for 48nodules. RFA was used to treat an average 1.3 lesionsper patient, and the median size of treated lesions was2.5 cm (range, 0.5-9 cm). The average hospital staywas 5.6 days overall (2.8 days for percutaneous casesand 7.9 days for open operations). Seven patientsunderwent a second RFA procedure (sequentialablations). Sixteen HCC patients with a high level ofalpha fetoprotein (AFP) and 9 colorectal cancer livermetastases patients with a high level of serumcarcinoembryonic antigen (CEA) have a great reductionbenefited from RFA. Four (11.1 %) patients hadcomplications: one skin burn; one postoperativehemorrhage; one cholecystitis and one hepatic abscessassociated with percutaneous ablations of a largelesion. There were 4 deaths: 3 patients died from localand system diseases (1 at 7 month, 1 at 9 month, and 1at 12 month), 1 patients died from cardiovascular shock,but no RFA-related death. At a median follow-up of 10months (range, 1-24 months), 6 patients (16.7 %) hadrecurrences at an RFA site, and 20 patients (56.7 %)remained clinically free of disease.CONCLUSION: RF ablation appears to be an effective,safe, and relatively simple procedure for the treatmentof unresectable liver cancers. The rate and severity ofcomplications appear acceptable. 19Significance of cyclooxygenase-2 expression in human primary hepatocellular carcinoma显示文摘AIM: To clarify the significance of cyclooxygenase-2 (COX-2) expression in human primary hepatocellular carcinoma(HCC) and adjacent nontumorous tissues.METHODS: The COX-2 protein and mRNA were investigatedin 27 HCC tissues with adjacent nontumorous tissues, and 5histologically normal liver tissues, using immunohistochemistryand in situ hybridization.RESULTS: The well-differentiated HCC expressed COX-2protein (5.68±1.19) more strongly than moderated HCC(3.43± 1.98) and poor differentiated HCC (3.33± 1.50)(P<0.05 respectively), adjacent nontumorous tissues (4.93±1.05) and normal liver tissues (3.20±1.g2) (P<0.01respectively); More intensive staining of COX-2 in adjacentnontumorous tissues was observed than that in normal livertissues (P<0.05). There was no significant difference amongadjacent nontumorous tissues, moderately differentiated HCCand poorly differentiated HCC (P>0.05).
