1Assessment and management of low anterior resection syndrome after sphincter preserving surgery for rectal cancer显示文摘Many patients develop a variety of bowel dysfunction after sphincter preserving surgeries(SPS)for rectal cancer.The bowel dysfunction usually manifests in the form of low anterior resection syndrome(LARS),which has a negative impact on the patients'quality of life.This study reviewed the LARS after SPS,its mechanism,risk factors,diagnosis,prevention,and treatment based on previously published studies.Adequate history taking,physical examination of the patients,using validated questionnaires and other diagnostic tools are important for assessment of LARS severity.Treatment of LARS should be tailored to each patient.Multimodal therapy is usually needed for patients with major LARS with acceptable results.The treatment includes conservative management in the form of medical,pelvic floor rehabilitation and transanal irrigation and invasive procedures including neuromodulation.If this treatment failed,fecal diversion may be needed.In conclusion,Initial meticulous dissection with preservation of nerves and creation of a neorectal reservoir during anastomosis and proper Kegel exercise of the anal sphincter can minimize the occurrence of LARS.Pre-treatment counseling is an essential step for patients who have risk factors for developing LARS.Ahmad Sakr Fozan Sauri Mohammed Alessa Eman Zakarnah Homoud Alawfi Radwan Torky Ho Seung Kim Seung Yoon Yang Nam Kyu Kim 2020Chinese Medical Journal2020,,15:17
2Review of tumoral calcinosis: A rare clinico-pathological entity显示文摘Tumoral calcinosis(TC) has long been a controversial clinico-pathological entity. Its pathogenesis and genetic background have been gradually unravelled since its first description in 1943. According to the presence or absence of an underlying calcifying disease process, TC has been divided into primary and secondary varieties. Two subtypes of the primary variety exist; a hyperphosphatemic type with familial basis represented by mutations in Gal NAc transferase 3 gene(GALNT3), KLOTHO or Fibroblast growth factor 23(FGF23) genes, and a normo-phosphatemic type with growing evidence of underlying familial base represented by mutation in SAMD9 gene. The secondary variety is mainly associated with chronic renal failure and the resulting secondary or tertiary hyperparathyroidism. Diagnosis of TC relies on typical radiographic features(on plain radiographs and computed tomography) and the biochemical profile. Magnetic resonance imaging can be done in difficult cases, and scintigraphy reflects the disease activity. Treatment is mainly surgical for the primary variety; however, a stage-oriented conservative approach using phosphate binders, phosphate restricted dietsand acetazolamide should be considered before the surgical approach is pursued due to the high rate of recurrences and complications after surgical intervention. Medical treatment is the mainstay for treatment of the secondary variety, with failure warranting subtotal or total parathyroidectomy. 3Post-transplantation lymphoproliferative disorders:Current concepts and future therapeutic approaches显示文摘Transplant recipients are vulnerable to a higher risk of malignancy after solid organ transplantation and allogeneic hematopoietic stem-cell transplant.Posttransplant lymphoproliferative disorders(PTLD)include a wide spectrum of diseases ranging from benign proliferation of lymphoid tissues to frank malignancy with aggressive behavior.Two main risk factors of PTLD are:Firstly,the cumulative immunosuppressive burden,and secondly,the oncogenic impact of the Epstein-Barr virus.The latter is a key pathognomonic driver of PTLD evolution.Over the last two decades,a considerable progress has been made in diagnosis and therapy of PTLD.The treatment of PTLD includes reduction of immunosuppression,rituximab therapy,either isolated or in combination with other chemotherapeutic agents,adoptive therapy,surgical intervention,antiviral therapy and radiotherapy.In this review we shall discuss the prevalence,clinical clues,prophylactic measures as well as the current and future therapeutic strategies of this devastating disorder.Fedaey Abbas Mohsen El Kossi Ihab Sakr Shaheen Ajay Sharma Ahmed Halawa 2020World Journal of Transplantation2020,10,2:6
4Myxedema ascites with high CA-125:Case and a review of literature显示文摘Ascites appearing in a previously healthy female patient is usually ascribed to a variety of causes,among which,is a cancerous process,especially if it comes with a raised CA-125 level.Although the CA-125 antigen is present on more than 80% of malignant epithelial ovarian tissue of non-mucinous type,it is also found on both healthy and malignant cells of mesothelial and non-mesothelial origin.Myxedema ascites which is caused by hypothyroidism is a rare entity,but on the other hand is easy to treat.It is one of the differential diagnoses when the ascites is refractory to treatment and no other obvious cause can be identified.If the diagnosis is delayed,patients will frequently receive unnecessary procedures,while treatment has very good response rates and ascites resolve with serum CA-125 normalization after adequate hormonal treatment.Bou Khalil Roula El Rassi Patrick Chammas Nizar Obeid Jean Ghabach Maroun Sakr Charles Itani Tarek Farhat Said 2013World Journal of Hepatology2013,5,2:4
5Effect of mesenchymal stem cells on anti-Thy1,1 induced kidney injury in albino rats显示文摘Objective:To evaluate the effect of mesenchymal stem cells(MSCs)in rats with anti-Thy 1,1nephritis.Methods:Female albino rats were divided into three groups,control group,anti-Thy 1,1group and treatment with i.v.MSCs group.MSCs were derived from bone marrow of male albino rats,Y—chromosome gene was detected by polymerase chain reaction in the kidney.Serum urea and creatinine were estimated for all groups.Kidney of all studied groups was examined histologically and histochemically(total carbohydrates and total proteins).DNA fragmentation and expression ofα-SMA were delected.Results:Kidney of animals injected with anti-Thy 1,1showed inflammatory leucocytic infiltration,hypertrophied glomeruli,tubular necrosis and congestion in the renal blood vessels.The kidney tissue also showed reduction of carbohydrates and total proteins together with increase in apoptosis and in expression ofα-SMA.Moreover,the levels of urea and creatinine were elevated.Treating animals with MSCs revealed that kidney tissue displayed an improvement in the histological and histochemical changes.Apoptosis andα-SMA expression were decreased,and the levels of urea and creatinine decreased.Conclusions:The obtained results demonstrated the potential of MSCs to ameliorate the structure and function of the kidney in rats with anti-Thy 1,1 nephritis possibly through the release of paracrine growth factor(s).Saber Sakr Laila Rashed Waheba Zarouk Rania El-Shamy 2013Asian Pacific Journal of Tropical Biomedicine2013,3,3:3
6Outcome of non surgical hepatic decompression procedures in Egyptian patients with Budd-Chiari显示文摘AIM: To evaluate outcome of patients with Budd-Chiari syndrome after balloon angioplasty ± stenting or transjugular intrahepatic portosystemic shunt (TIPS). METHODS: Twenty five patients with Budd-Chiari syndrome admitted to Ain Shams University Hospitals, Tropical Medicine Department were included. Twelve patients (48%) with short segment occlusion were candidates for angioplasty; with stenting in ten cases and without stenting in two. Thirteen patients (52%) had Transjugular Intrahepatic Portosystemic Shunt. Patients were followed up for 12-32 mo. RESULTS: Patency rate in patients who underwent angioplasty ± stenting was 83.3% at one year and at end of follow up. The need of revision was 41.6% with one year survival of 100%, dropped to 91.6% at end of follow up. In patients who had Transjugular Intrahepatic Portosystemic Shunt, patency rate was 92.3% at one year, dropped to 84.6% at end of follow up. The need of revision was 38.4% with one year and end of follow up survival of 100%. Patients with patent shunts showed marked improvement compared to those with occluded shunts. CONCLUSION: Morbidity and mortality following angioplasty ± stenting and TIPS are low with satisfactory outcome. 7Ameliorative effect of grapefruit juice on amiodarone-induced cytogenetic and testicular damage in albino rats显示文摘Objective:To evaluate the ameliorative role of grapefruit juice on the cytogenetic and testicular damage induced by the antiarrythmic drug amiodarone in albino rats.Methods:Animals were divided into four groups.Group I was considered as control.Group II was given grapefruit juice at a dose level of 27 mL/kg body weight.Group III was orally administered amiodarone(18 mg/kg body weight)daily for 5 weeks.Animals were sacrificed after 5 weeks of treatment.Bone marrow was collected from the femurs for analysis of chromosomal aberrations and mitotic indices.Testes were removed and stained with H&E for histological examination.Sperms were collected from epidedymis for detection of sperm head abnormalities.Comet assay was used to detect DNA damage.Results:Amiodarone treatment caused a significant increase in the percentage of chromosomal aberrations,decreased the mitotic index and increased DNA damage.The testis showed many histopathological alterations,inhibition of spermatogenesis and morphometric changes.The number of sperm head abnormalities was increased.Treating animals with amiodarone and grapefruit juice caused a reduction in chromosomal aberrations,mitotic index,DNA damage and testicular alterations caused by amiodarone.Conclusions:The results of this study indicated that grapefruit juice ameliorates the cytotoxicty and testicular alterations induced by amiodarone in albino rats and this is may be due to the potent antioxidant effects of its components.Saber Abdelruhman Sakr Mohamed El-said Zoil Samraa Samy El-shafey 2013Asian Pacific Journal of Tropical Biomedicine2013,3,7:2
8Living donor liver transplantation for high model for endstage liver disease score:What have we learned?显示文摘AIM: To assess the impact of model for end-stage liver disease(MELD) score on patient survival and morbidity post living donor liver transplantation(LDLT). METHODS: A retrospective study was performed on 80 adult patients who had LDLT from 2011-2013. Nine patients were excluded and 71 patients were divided into two groups; Group 1 included 38 patients with a MELD score < 20, and Group 2 included 33 patients with a MELD score > 20. Comparison between both groups was done regarding operative time, intra-operative blood requirement, intensive care unit(ICU) and hospital stay, infection, and patient survival.RESULTS: Eleven patients died(15.5%); 3/38(7.9%)patients in Group 1 and 8/33(24.2%) in Group 2 with significant difference(P = 0.02). Mean operative time, duration of hospital stay, and ICU stay were similar in both groups. Mean volume of blood transfusion and cell saver re-transfusion were 8 ± 4 units and 1668 ± 202 m L, respectively, in Group 1 in comparison to 10 ± 6 units and 1910 ± 679 m L, respectively, in Group 2 with no significant difference(P = 0.09 and 0.167, respectively). The rates of infection and systemic complications(renal, respiratory, cardiovascular and neurological complications) were similar in both groups. 9Novel non-invasive biological predictive index for liver fibrosis in hepatitis C virus genotype 4 patients显示文摘AIM To investigate the diagnostic ability of a non-invasive biological marker to predict liver fibrosis in hepatitis C genotype 4 patients with high accuracy.METHODS A cohort of 332 patients infected with hepatitis C genotype 4 was included in this cross-sectional study. Fasting plasma glucose, insulin, C-peptide, and angiotensinconverting enzyme serum levels were measured. Insulin resistance was mathematically calculated using the homeostasis model of insulin resistance(HOMA-IR).RESULTS Fibrosis stages were distributed based on Metavir score as follows: F0 = 43, F1 = 136, F2 = 64, F3 = 45 and F4 = 44. Statistical analysis relied upon reclassification of fibrosis stages into mild fibrosis(F0-F) = 179, moderate fibrosis(F2) = 64, and advanced fibrosis(F3-F4) = 89. Univariate analysis indicated that age, log aspartate amino transaminase, log HOMA-IR and log platelet count were independent predictors of liver fibrosis stage(P < 0.0001). A stepwise multivariate discriminant functional analysis was used to drive a discriminative model for liver fibrosis. Our index used cut-off values of ≥ 0.86 and ≤-0.31 to diagnose advanced and mild fibrosis, respectively, with receiving operating characteristics of 0.91 and 0.88, respectively. 11Recent advances in the management of hemorrhoids显示文摘Hemorrhoids are considered one of the most common anorectal diseases with a prevalence of 4.4% up to 36.4% of the general population, and a peak incidence between 45 and 65 years. Hemorrhoidal disease presents with a prolapsed lump, painless bleeding, discomfort, discharge, hygiene problems, soiling, and pruritus. Sliding anal canal lining theory is the most accepted theory as a cause of hemorrhoidal disease; however, it is also associated with hyper-vascularity, and, recently, with several enzymes or mediators involved in the disintegration of the tissues supporting the anal cushions, such as matrix metalloproteinase. A comprehensive search in published English-language literature till 2013 involving hemorrhoids was performed to construct this review article, which discusses advances in the management of hemorrhoids. This includes conservative treatment(life style modification, oral medications, and topical treatment), office procedures(rubber band ligation, injection sclerotherapy, infrared and radiofrequency coagulation, bipolar diathermy and direct-current electrotherapy, cryosurgery, and laser therapy), as well as surgical procedures including diathermy hemorrhoidectomy, Liga Sure hemorrhoidectomy, Harmonic scalpel hemorrhoidectomy, hemorrhoidal artery ligation, stapled hemorrhoidopexy(SH), and double SH. 12Epidemiological aspects of Budd-Chiari in Egyptian patients:A single-center study显示文摘AIM:To describe the socio-demographic features,etiology,and risk factors for Budd-Chiari syndrome(BCS) in Egyptian patients.METHODS:Ninety-four Egyptian patients with confirmed primary Budd-Chiari syndrome were presented to the Budd-Chiari Study Group(BCSG) and admitted tothe Tropical Medicine Department of Ain Shams University Hospital(Cairo,Egypt) . Complete clinical evaluation and laboratory investigations,including a throm bophiliaworkup and full radiological assessment,were performed to determine underlying disease etiologies.RESULTS:BCS was chronic in 79.8% of patients,acute or subacute in 19.1%,and fulminant in 1.1%.Factor V Leiden mutation(FVLM) was the most common etiological cause of disease(53.1%) ,followed by mutation of the gene encoding methylene tetra-hydrofolate reductase(MTHFR) (51.6%) . Current orrecent hormonal treatment was documented in 15.5%of females,and BCS associated with pregnancy was present in 17.2% of females. Etiology could not be determined in 8.5% of patients. Males had significantly higher rates of MTHFR gene mutation and Beh et's disease,and females had significantly higher ratesof secondary antiphos pholipid antibody syndrome.A highly significant positive relation ship was evident between the presence of Beh et's disease and inferiorven a caval occlusion,either alone or combined with occlusion of the hepatic veins(P < 0.0001) .CONCLUSION:FVLM is the most common disease etiology and MTHFR the second most common in Egyptian BCS patients. 13The use of a standardized Chinese herbal formula in patients with advanced lung cancer:a feasibility study显示文摘Objective: Increasing numbers of cancer patients are using Chinese herbs(CHs). However, differences among prior studies make it difficult to draw firm conclusions about the clinical usefulness of any specific CH formula. The primary objective of this study was to establish the acceptability of taking a standardized CH formula for patients with advanced lung cancer. The secondary objective was to identify any toxicities attributable to this CH formula and to measure changes in quality of life.Methods: A single-arm, prospective study of a 6-week intervention with a selected CH formula in15 patients with stage 4 nonsmall-cell lung cancer(NSCLC, Seventh American Joint Committee on Cancer TNM staging system).Results: Patients with advanced lung cancer were interested in using the CH formula. Completion(93%)and adherence(98%) levels were very high and most patients perceived the CH treatment as easy to take and were willing to take the CHs used in the study again if it was available. About half of the patients reported adverse events, all of which were mild(Grade 1 or 2) and only a small minority(8%) were potentially related to CHs. No biochemical or hematological evidence of toxicity was observed.
