1Omental flaps reduces complications after pancreaticoduodenectomy显示文摘BACKGROUND:Major complications after pancreaticoduodenectomy are usually caused by a leaking pancreaticojejunal anastomosis.Omental flaps around various anastomoses were used to prevent the formation of fistula.METHODS:We reviewed 147 patients who had undergone pancreaticoduodenectomy between March 2006 and March2012.The patients were divided into 2 groups according to the application of omental flaps around various anastomoses:group A(101 patients)who underwent omental wrapping procedure;group B(46 patients)who did not undergo the omental wrapping procedure.Perioperative data of the two groups were reviewed to assess the effectiveness of omental flap procedure in the prevention of pancreatic fistula and other complications.RESULTS:No differences were observed in the clinical characteristics between the 2 groups.The incidences of pancreatic fistula(4.0%vs 17.4%),post-pancreatectomy hemorrhage(0vs 6.5%),biliary fistula(1.0%vs 13.0%),and delayed gastric emptying(4.0%vs 17.4%)were significantly less frequent in group A.The overall morbidity(18.8%vs 47.8%)and hospital stay(8.3 vs 9.6 days)were also significantly lower in group A than in group B.CONCLUSIONS:Omental flaps around various anastomoses after pancreaticoduodenectomy can reduce the incidences of pancreatic fistula,biliary fistula,post-pancreatectomy hemorrhage and delayed gastric emptying.This procedure is simple and effective to reduce the overall morbidity after pancreaticoduodenectomy.Omar J Shah Sadaf A Bangri Manmohan Singh Reyaz A Lattoo Mohammad Y Bhat 2015Hepatobiliary & Pancreatic Diseases International2015,14,3:19
2Nanotherapeutic approaches targeting angiogenesis and immune dysfunction in tumor microenvironment显示文摘Tumor microenvironment(TME) comprising cellular and non-cellular components is a major source of cancer hallmarks.Notably, angiogenesis responsible for normal physiological remodeling process can otherwise harness vessel abnormalities during tumorigenesis eliciting severe therapeutic inefficiency. Currently, FDA approved antiangiogenic drugs have only shown modest clinical success owing to tumor hypoxia, antiangiogenic therapeutic resistance, and limited knowledge in understanding TME. In order to overcome these limitations, targeting angiogenesis combined with immunosuppressive TME could offer potential therapeutic opportunities. Indeed, these therapeutic approaches can be further revisited with the advent of nanotechnology that can target the key cellular components of TME and tumor cells more precisely. Synergetic targeting without eliciting systemic toxicity achieved by integration of antiangiogenic and immunotherapy in a single nanoplatform is vital for therapeutic success. 3Complications and survival in patients undergoing colonic stenting for malignant obstruction显示文摘AIM:To investigate whether predicting patients that might be at a higher risk for complications might serve to improve the selection of patients undergoing colonic stenting.METHODS:A retrospective review of consecutive patients who underwent an attempted self-expandable metal stent(SEMS)insertion for malignant colonic obstruction between November 2006 and March 2013.All patients were either referred for preoperative colonic decompression with the intent of a single surgical procedure,or for palliation of the malignant colorectal obstruction for unresectable cancer.Fisher’s test orχ2test was performed on categorical variables,and the t test for continuous variables.Univariable and multivariable logistic regression were used to examine the association between independent variables and the presence of complications from SEMS insertion.RESULTS:SEMS insertion was attempted in 73 patients.Males comprised 55.71%and the mean age was 67.41±12.41 years.Of these,65.15%underwent subsequent surgery,while 34.85%received SEMS as palliation for advanced disease.Extracolonic tumors were only4.76%.The majority of patients had stageⅣdisease(63.83%),while the remainder had stageⅢ(36.17%).SEMS were successfully inserted in 93.85%(95%CI:87.85%-99.85%).Perforations occurred in 4.10%,SEMS migration in 8.21%,and stent re-occlusion from ingrowth occurred in 2.74%of patients.The mean duration of follow up for the patients was 13.52±17.48 mo(range 0-73 mo).None of the variables:age,sex,time between the onset of symptoms to SEMS insertion,time between SEMS insertion and surgery,length of the stenosis,location of the stenosis,albumin level,or receiving neoadjuvant chemotherapy,could predict the development of complications from either SEMS insertion nor prolonged survival.CONCLUSION:None of the variables could predict the development of complications or survival.Further studies are required to identify patients who would benefit the most from SEMS.Majid A Almadi Nahla Azzam Othman Alharbi Alabbas H Mohammed Nazia Sadaf Abdulrahman M Aljebreen 2013World Journal of Gastroenterology2013,19,41:5

5Difficult-to-culture bacteria in the rhizosphere:The underexplored signature microbial groups显示文摘Microorganisms represent a substantial portion of the earth’s biodiversity and biomass, and the plant rhizosphere is an innate reservoir teeming with heterogeneous microbes predominated by bacterial communities. Rhizospheric microbial diversity (genetic, phenotypic, and metabolic) has been extensively studied to understand the key ecological roles played by the microbial members, including plant growth promotion. The application of 16S rRNA gene sequencing and next-generation sequencing (NGS) technologies has revolutionized the discovery of novel bacterial groups that have remained undetected by traditional cultivation-based approaches. Such technological advancements have opened new vistas in our current understanding of predominant but concealed and missed bacterial diversity referred to as difficult-to-culture bacterial lineages, especially the predominant phyla Acidobacteria, Verrucomicrobia,Planctomycetes, and Gemmatimonadetes. Regardless of their ubiquity and prevalence, little is known about their ecophysiology because of the non-availability of culturable members. More recently, there has been increased interest in understanding the cosmopolitan distribution and diversity of the difficult-to-culture bacteria, focusing on their role in driving complex plant-microbial interactions and mobilizing nutrients in soil and their potential as sources of novel bioactive metabolites. As an initial step, we review the distribution and significance of such bacterial phyla in soil, their ecophysiological roles, and their hidden plant growth promoting potential. 7Should dopamine be the first line inotrope in the treatment of neonatal hypotension? Review of the evidence显示文摘AIM: To determine if dopamine is effective in treating neonatal hypotension and safe to use comparing to other inotropes. METHODS: This is a review of evidence on inotropic treatment of neonatal hypotension. Databases searched were MEDLINE and the Cochrane Library, a total of 134 studies were identified. Only studies with high quality evidence(level 1a and b and 2a) were included. After review, only eight studies were included in the final analysis. Pooled risk ratios derived for each outcome [Mantel-Haenzel(M-H) fixed effect] with CI, as reported in the Cochrane reviews were plotted in forest plot form. RESULTS: Eight articles met inclusion criteria, which all included treatment in preterm infants. Dopamine increased mean arterial blood pressure(BP)(n = 163; r = 0.88, 95%CI: 0.76 to 0.94) and systolic BP(n = 142; r = 0.81, 95%CI: 0.42 to 0.94) comparing to placebo. Dopamine has been shown overall to be statistically more effective in increasing BP than dobutamine(n = 251, r = 0.26, 95%CI: 0.20-0.32). However there were no differences in short term outcomes(periventricular leucomalacia, periventricular haemorrhage) and mortality between both drugs. There is no statistical evidence of dopamine being more effective than adrenaline or corticosteroids. There was no difference in morbidity and mortality outcomes when dopamine was compared to hydrocortisone(RR 1.81, 95%CI: 0.18 to 18.39) or adrenaline. 9A selective approach to the surgical management of periampullary cancer patients and its outcome显示文摘BACKGROUND: Pancreaticoduodenectomy is a high risk, complex, technically challenging operation associated with significant perioperative morbidity and mortality. This study on the surgical management of periampullary cancer patients is based on our experience in a period of nearly 13 years.METHODS: The study was conducted on two groups of patients: group A included 42 patients who were treated between January 2000 and September 2005 and group B included 134 patients who were treated between October 2005 to October 2012. Preoperative, intraoperative and postoperative details of all these patients were collected, tabulated and analyzed to assess the impact of the selective approach introduced in the department with effect from October 2005.RESULTS: Intraoperative details revealed highly significant differences in the management of the two groups of patients in respect of operative time (250.4 vs 126.6 minutes; P<0.001), operative blood loss (1070.2 vs 414.9 m L; P<0.001) and intraoperative blood transfusion (1.4 vs 0.2 units; P<0.001). Variations between the two groups in the frequency of complications were found to be statistically insignificant. 11Recent advancements in surface plasmon polaritons-plasmonics subwavelength structures in microwave and terahertz regimes显示文摘This paper presents a review of recent investigational studies on exciting Surface Plasmon Polaritous (SPPs)in MicroWave (MW)and TeraHertZ (THz)regimes by using subwavelength corrugated patterns on conductive or metal surfaces.This article also describes SPP Microstrip (MS)structures at microwave and terahertz frequencies, and compares their significance with that of conventional MS Transmission Lines (TL),in order to tackle the key challenges of high gain,bandwidth size,compactness,TL losses,and signal integrity in high-end electronic de- vices.Because they have subwavelength properties,surface plasmon polaritous are gaining attention for their improved performance and ability for miniaturization in high-speed dense circuits.They possess comparably minuscule wavelength compared to incident light (photons).Consequently,they can demonstrate stronger spatial confinement and higher local field intensity at optical frequencies.In addition to engineering spoof SPP waveguides,which are created by engraving grooves and slits on metal surfaces to allow operation on at low frequencies (microwave and terahertz),semiconductors with smaller permittivity values and thus lower free charge carrier concentration have been demonstrated as a potential candidate in plasmonic devices.If necessary,further tuning of semiconductor-based SPP structures is aided by controlling the charge carrier concentration through doping,or by external stimuli such as optical illumination or thermal excitation of charge carriers from valence to conduction bands.This article conclusively covers previously elucidated perspectives on manipulating SSPPs in the MW and THz ranges,and emphasizes how these could steer next-generation plasmonic devices.Rana Sadaf Anwar Huansheng Ning Lingfeng Mao 2018Digital Communications and Networks2018,4,4:3
12Clinical epidemiology of ulcerative colitis in Arabs based on the Montréal classification显示文摘AIM:To determine the clinical,epidemiological and phenotypic characteristics of ulcerative colitis(UC)in Saudi Arabia by studying the largest cohort of Arab UC patients.METHODS:Data from UC patients attending gastroenterology clinics in four tertiary care centers in three cities between September 2009 and September 2013were entered into a validated web-based registry,inflammatory bowel disease information system(IBDIS).The IBDIS database covers numerous aspects of inflammatory bowel disease.Patient characteristics,disease phenotype and behavior,age at diagnosis,course of the disease,and extraintestinal manifestations were recorded.RESULTS:Among 394 UC patients,males comprised51.0%and females 49.0%.According to the Montréal classification of age,the major chunk of our patients belonged to the A2 category for age of diagnosis at17-40 years(68.4%),while 24.2%belonged to the A3category for age of diagnosis at>40 years.According to the same classification,a majority of patients had extensive UC(42.7%),35.3%had left-sided colitis and29.2%had only proctitis.Moreover,51.3%were in remission,16.6%had mild UC,23.4%had moderate UC and 8.6%had severe UC.Frequent relapse occurred in17.4%patients,infrequent relapse in 77%and 4.8%had chronic disease.A majority(85.2%)of patients was steroid responsive.With regard to extraintestinal manifestations,arthritis was present in 16.4%,osteopenia in 31.4%,osteoporosis in 17.1%and cutaneous involvement in 7.0%.CONCLUSION:The majority of UC cases were young people(17-40 years),with a male preponderance.While the disease course was found to be similar to that reported in Western countries,more similarities were found with Asian countries with regards to the extent of the disease and response to steroid therapy.Othman R Alharbi Nahla A Azzam Ahmed S Almalki Majid A Almadi Khalid A Alswat Nazia Sadaf Abdulrahman M Aljebreen 2014World Journal of Gastroenterology2014,20,46:2
13Diagnostically untypable hepatitis C virus variants:It is time to resolve the problem显示文摘Pakistan is a low income country with more than 10million hepatitis C virus(HCV)infections and the burden is on continuous raise.Accurate viral genotyping is very critical for proper treatment of the infected individuals as the sustained virological response of the standard antiviral interferon therapy is genotype dependent.We observed at our diagnostic center that15.6%of HCV patient’s samples were not genotypeable by using Ohno et al method.The genotyped samples showed that 3a(68.3%)is the major prevalent genotype in Pakistan followed by 2a(10.3%),3b(2.6%),1b(1.5%),2b(1.2%)and 1a(0.5%).Presence of large number of untypable HCV variants in the current study highlights an important issue of health care setup in Pakistan.Untypable HCV cases create difficulties in treatment of these patients.The problem of routine diagnostics setup of Pakistan should be addressed on priority basis to facilitate the medical professionals in patient’s treatment and to help in achieving the maximum sustained virological response.Muhammad Sohail Afzal Muhammad Yousaf Khan Muhammad Ammar Sadia Anjum Najm us Sahar Sadaf Zaidi 2014World Journal of Gastroenterology2014,20,46:2

15Cooperative Sensor Anomaly Detection Using Global Information显示文摘Sensor networks are deployed in many application areas nowadays ranging from environment monitoring, industrial monitoring, and agriculture monitoring to military battlefield sensing. The accuracy of sensor readings is without a doubt one of the most important measures to evaluate the quality of a sensor and its network. Therefore, this work is motivated to propose approaches that can detect and repair erroneous (i.e., dirty) data caused by inevitable system problems involving various hardware and software components of sensor networks. As information about a single event of interest in a sensor network is usually reflected in multiple measurement points, the inconsistency among multiple sensor measurements serves as an indicator for data quality problem. The focus of this paper is thus to study methods that can effectively detect and identify erroneous data among inconsistent observations based on the inherent structure of various sensor measurement series from a group of sensors. Particularly, we present three models to characterize the inherent data structures among sensor measurement traces and then apply these models individually to guide the error detection of a sensor network. First, we propose a multivariate Gaussian model which explores the correlated data changes of a group of sensors. Second, we present a Principal Component Analysis (PCA) model which captures the sparse geometric relationship among sensors in a network. The PCA model is motivated by the fact that not all sensor networks have clustered sensor deployment and clear data correlation structure. Further, if the sensor data show non-linear characteristic, a traditional PCA model can not capture the data attributes properly. Therefore, we propose a third model which utilizes kernel functions to map the original data into a high dimensional feature space and then apply PCA model on the mapped linearized data. All these three models serve the purpose of capturing the underlying phenomenon of a sensor network from its global view, and then guide the error detection to discover any anomaly observations.
