3HCV-RNA positivity in peripheral blood mononuclear cells of patients with chronic HCV-infection: does it really mean viral replication?显示文摘AIM To analyze the association of HCV-RNA with peripheral blood mononuclear cells (PBMC)and to answer the question whether HCV-RNA positivity in PBMC is due to viral replication,METHODS HCV-RNA was monitored in serumand PBMC preparations from 15 patients with chronic HCV infection before, during and after an IFN-α therapy using a nested RT/ PCRtechnique. In a second approach, PBMC from healthy donors were incubated in HCV positive plasma.RESULTS In the IFN-α responding patients,HCV-RNA disappeared first from total RNApreparations of PBMC and then from serum. In contrast, in relapsing patients, HCV-RNAreappeared first in serum and then in PBMC. A quantitative analysis of the HCV-RNAconcentration in serum was performed before and after transition from detectable to nondetectable HCV-RNA in PBMC-RNA and vice versa. When HCV-RNA was detectable in PBMCpreparations, the HCV concentration in serum was significantly higher than the serum HCV-RNA concentration when HCV-RNA in PBMC was not detectable. Furthermore, at no time during the observation period was HCV specific RNA observed in PBMC, if HCV-RNA in serum was under the detection limit. Incubation of PBMCfrom healthy donors with several dilutions of HCV positive plasma for two hours showed a concentration-dependent PCR-positivity for HCV-RNA in reisolated PBMC.CONCLUSION The detectability of HCV-RNA in total RNA from PBMC seems to depend on the HCV concentration in serum. 4Correlation between in vitro and in vivo immunomodulatory properties of lactic acid bacteria显示文摘AIM: To investigate the correlation between in vitro and in vivo immunomodulation potential of the probiotic strain and its ability to prevent experimental colitis in mice. METHODS: In vitro immunomodulation was assessed by measuring interleukin (IL)-12p70, IL-10, tumor necrosis factor alpha (TNFa) and interferon g (IFNg) release by human peripheral blood mononuclear cells (PBMCs) after 24 h stimulation with 13 live bacterial strains. A murine model of acute TNBS-colitis was next used to evaluate the prophylactic protective capacity of the same set of strains. RESULTS: A strain-specific in vivo protection was observed. The strains displaying an in vitro potential to induce higher levels of the anti-inflammatory cytokine IL-10 and lower levels of the inflammatory cytokine IL-12, offered the best protection in the in vivo colitis model. In contrast, strains leading to a low IL-10/IL-12 cytokine ratio could not significantly attenuate colitis symptoms. CONCLUSION: These results show that we could predict the in vivo protective capacity of the studied lactic acid bacteria (LAB) based on the cytokine profile we established in vitro. 6Epithelial-mesenchymal, mesenchymal-epithelial, and endothelial-mesenchymal transitions in malignant tumors: An update显示文摘Epithelial-to-mesenchymal transition(EMT) represents conversion of an epithelial cell in an elongated cell with mesenchymal phenotype, which can occur in physiologic and pathologic processes such as embryogenesis(type 1 EMT), wound healing and/or fibrosis(type 2 EMT) and malignant tumors(type 3 EMT). The proliferation rate, metastasizing and recurrence capacity, as also the individualized response at chemotherapics, in both epithelial and mesenchymal malignant tumors is known to be influenced by reversible switch between EMT and mesenchymal-to-epithelial transition(MET). Although much research work has already been done in these fields, the specific molecular pathways of EMT, relating to the tumor type and tumor localization, are yet to be elucidated. In this paper, based on the literature and personal experience of the authors, an update in the field of EMT vs MET in epithelial and mesenchymal tumors is presented. The authors tried to present the latest data about the particularities of these processes, and also of the so-called endothelialto-mesenchymal transition, based on tumor location. The EMT-angiogenesis link is discussed as a possible valuable parameter for clinical follow-up and targeted therapeutic oncologic management. The paper begins with presentation of the basic aspects of EMT, its classification and assessment possibilities, and concludes with prognostic and therapeutic perspectives. 7Coronary thrombus in patients undergoing primary PCI for STEMI:Prognostic significance and management显示文摘Acute ST-elevation myocardial infarction(STEMI) usually results from coronary atherosclerotic plaque disruption with superimposed thrombus formation. Detection of coronary thrombi is a poor prognostic indicator,which is mostly proportional to their size and composition. Particularly,intracoronary thrombi impair both epicardial blood flow and myocardial perfusion,by occluding major coronary arteries and causing distal embolization,respectively. Thus,although primary percutaneous coronary intervention is the preferred treatement strategy in STEMI setting,the associated use of adjunctive antithrombotic drugs and/or percutaneous thrombectomy is crucial to optimize therapy of STEMI patients,by improving either angiographical and clinical outcomes. 8Albumin liver dialysis as pregnancy-saving procedure in cholestatic liver disease and intractable pruritus显示文摘Progressive familial intrahepatic cholestasis type 3 (PFIC3) is a rare cholestatic liver disease. Such liver disease can get worse by female hormone disorder. Albumin dialysis or Molecular Adsorbent Recirculating System (MARS) has been reported to reverse severe cholestasis-linked pruritus. Here, we report the first use of MARS during a spontaneous pregnancy and its successful outcome in a patient with PFIC3 and intractable pruritus. 10中美急诊道路交通伤患者流行病学研究显示文摘目的阐明中美两国综合性医院急诊医学科道路交通伤患者的异同点,为两国在道路交通伤临床、教学和科研方面的进一步合作提供新思路。方法选取2013年11—12月中国浙江省人民医院急诊医学科和2013年7—9月美国纽约州立大学上州医科大学急诊医学科两家单位的道路交通伤患者作为调查对象。双方合作设计《MVC Survey Questionare》(中英文版),供两家单位开展问卷调查。调查方法为标准流行病学抽样调查。调查内容包括患者到达急诊医学科的方式、交通工具、损伤类型、影像学检查情况、会诊情况、转归等。结果共发放问卷425份,回收合格问卷399份,有效率为93.9%,其中中国有效问卷155份,美国244份。到达急诊医学科的方式:美国道路交通伤患者救护车使用率高于中国,自备车使用率低于中国(P<0.05)。交通工具:美国自行车、电动车、行人交通事故发生率低于中国,汽车交通事故发生率高于中国(P<0.05)。损伤类型:美国患者颅脑、皮肤软组织损伤发生率低于中国,胸部损伤、骨折发生率高于中国(P<0.05)。影像学检查:美国患者X线、CT检查率均高于中国(P<0.05)。两国住院率、病死率、救治成功出院率、入住ICU比例、急诊手术率比较,差异均无统计学意义(P>0.05)。结论与美国相比,中国较少选择救护车转运道路交通伤患者。中国有更多类型的机动车涉及道路交通伤事故,中国道路交通伤患者中骑电动车、摩托车和行人的比例较高,这在一定程度上导致了中国颅脑损伤的比例远高于美国。另外,中国患者影像学检查率明显低于美国。上述差异为未来双方在道路交通伤临床、教学和科研方面开展进一步合作提供了新思路。涂建锋 张可 周晟昂 陈环 蔡文伟 PaulKo Susan Wojcik Sabine Khan William D. 12Herbal Traditional Chinese Medicine and suspected liver injury: A prospective study显示文摘AIM To analyze liver tests before and following treatment with herbal Traditional Chinese Medicine(TCM) in order to evaluate the frequency of newly detected liver injury.METHODS Patients with normal values of alanine aminotransferase(ALT) as a diagnostic marker for ruling out pre-existing liver disease were enrolled in a prospective study of a safety program carried out at the First German Hospital of TCM from 1994 to 2015. All patients received herbal products, and their ALT values were reassessed 1-3 d prior to discharge. To verify or exclude causality for suspected TCM herbs, the Roussel Uclaf Causality Assessment Method(RUCAM) was used.RESULTS This report presents for the first time liver injury data derived from a prospective, hospital-based and largescale study of 21470 patients who had no liver disease prior to treatment with herbal TCM. Among these, ALT ranged from 1 × to < 5 × upper limit normal(ULN) in 844 patients(3.93%) and suggested mild or moderate liver adaptive abnormalities. However, 26 patients(0.12%) experienced higher ALT values of ≥ 5 × ULN(300.0 ± 172.9 U/L, mean ± SD). Causality for TCM herbs was RUCAM-based probable in 8/26 patients, possible in 16/26, and excluded in 2/26 cases. Bupleuri radix and Scutellariae radix were the two TCM herbs most commonly implicated. 15Global kinematics of tectonic plates and subduction zones since the late Paleozoic Era显示文摘Detailed global plate motion models that provide a continuous description of plate boundaries through time are an effective tool for exploring processes both at and below the Earth's surface. A new generation of numerical models of mantle dynamics pre-and post-Pangea timeframes requires global kinematic descriptions with full plate reconstructions extending into the Paleozoic(410 Ma). Current plate models that cover Paleozoic times are characterised by large plate speeds and trench migration rates because they assume that lowermost mantle structures are rigid and fixed through time. When used as a surface boundary constraint in geodynamic models, these plate reconstructions do not accurately reproduce the present-day structure of the lowermost mantle. Building upon previous work, we present a global plate motion model with continuously closing plate boundaries ranging from the early Devonian at 410 Ma to present day.We analyse the model in terms of surface kinematics and predicted lower mantle structure. The magnitude of global plate speeds has been greatly reduced in our reconstruction by modifying the evolution of the synthetic Panthalassa oceanic plates, implementing a Paleozoic reference frame independent of any geodynamic assumptions, and implementing revised models for the Paleozoic evolution of North and South China and the closure of the Rheic Ocean. Paleozoic(410-250 Ma) RMS plate speeds are on average ~8 cm/yr, which is comparable to Mesozoic-Cenozoic rates of ~6 cm/yr on average.Paleozoic global median values of trench migration trend from higher speeds(~2.5 cm/yr) in the late Devonian to rates closer to 0 cm/yr at the end of the Permian(~250 Ma), and during the Mesozoic-Cenozoic(250-0 Ma) generally cluster tightly around ~1.1 cm/yr. Plate motions are best constrained over the past 130 Myr and calculations of global trench convergence rates over this period indicate median rates range between 3.2 cm/yr and 12.4 cm/yr with a present day median rate estimated at~5 cm/yr. For Paleozoic times(410-251 Ma) our model results in median convergence rates largely~5 cm/yr. Globally,~90% of subduction zones modelled in our reconstruction are determined to be in a convergent regime for the period of 120-0 Ma. Over the full span of the model, from 410 Ma to 0 Ma,~93% of subduction zones are calculated to be convergent, and at least 85% of subduction zones are converging for 97% of modelled times. Our changes improve global plate and trench kinematics since the late Paleozoic and our reconstructions of the lowermost mantle structure challenge the proposed fixity of lower mantle structures, suggesting that the eastern margin of the African LLSVP margin has moved by as much as ~1450 km since late Permian times(260 Ma). 17Serum levels of soluble Fas, nitric oxide and cytokines in acute decompensated cirrhotic patients显示文摘AIM: To evaluate plasma levels of nitrite/nitrate (NOx), soluble Fas (sFas) antigen, tumor necrosis factor alpha (TNF-α) and interleukin-6 (IL-6) in patients with com- pensated and acute decompensated cirrhosis and to evaluate mediators causing acute decompensation in liver cirrhosis. METHODS: This prospective study was conducted in the medical intensive care unit of an academic tertiary center. Fifty-five patients with acute decompensation (gastrointestinal hemorrhage, encephalopathy, hydropic decompensation) and twenty-five patients with compen- sated liver cirrhosis were included. Blood samples were taken for analyses of sFas, Nox, IL-6, TNF-α. Liver en- zymes and kidney functions were also tested. RESULTS: In patients with acute decompensation, plas- ma sFas levels were higher than in non-decompensated patients (15 305 ±4646 vs 12 458 ±4322 pg/mL,, P < 0.05). This was also true for the subgroup of patients with alcoholic liver cirrhosis (P < 0.05). The other media- tors were not different and none of the parameters pre- dicted survival, except for ALT (alanine-aminotransfer- ase). In patients with portal-hypertension-induced acute hemorrhage, NOx levels were significantly lower than in patients with other forms of decompensation (70.8 ± 48.3 vs 112.9 ± 74.9 pg/mL, P < 0.05). When NOx lev- els were normalized to creatinine levels, the difference disappeared. IL-6, TNF-α and sFas were not different between bleeders and non-bleeders. In decompensated patients sFas, IL-6 and NOx levels correlated positively with creatinine levels, while IL-6 levels were dependent on Child class. CONCLUSION: In acute decompensated cirrhotic pa- tients sFas is increased, suggesting a role of apoptosis in this process and patients with acute bleeding have lower NOx levels. 18血清乙型肝炎病毒DNA阳性与原发性肝癌危险性关系的前瞻性队列研究显示文摘目的探讨血清HBV DNA阳性与原发性肝癌的关系。方法以分层抽样方法对广西隆安县12个乡镇30~55岁农村居民抽样,采静脉血,酶联免疫吸附试验检测HBsAg,套式聚合酶链反应检测HBV DNA。根据HBsAg和DNA检测结果,将研究对象分为HBsAg(+)/HBvDNA(+)组(A组)和HBsAg(+)/HBVDNA(-)组(B组);根据1:1匹配原则,从本村屯HBsAg(-)者中为前两组观察对象挑选对照,组成HBsAg阴性对照组(C组),对这3组人群进行4年的前瞻性跟踪随访。卡方检验对各组及各因素的发病率进行统计分析,然后用Cox比例风险模型分析与PLC有关的危险因素,用后退法对数值进行迭代分析。结果30~55岁人群HBsAg阳性率为14.52%(3975/27379),HBsAg阳性者HBV DNA阳性率为40.35%(1604/3975)。A、B两组总的肝癌发病率为672.45/10万人年,明显高于C组的17.19/10万人年(P〈0.01),相对危险度为39.123,95%可信因司为9.018~159.146。A组肝癌发病率为984.03/10万人年,显明高于C组的324.38/10万人年(P〈0.01),相对危险度为3.034,95%可信区间为1.795~5.125。对A、B两组进行肝癌危险因素的多因素Cox模型分析,结果表明性别、年龄、血清HBV DNA阳性、肝癌家族史和以玉米为主食均为肝癌的危险因素。结论血清HBV DNA阳性可增加HBsAg阳性者的肝癌危险性。陈钦艳 董柏青 杨进业 韦少超 方孔雄 王学燕 方钟燎 Caroline Sabin Tim J Harrison 2009中华肝脏病杂志2009,17,12:10
