4Cytokine orchestration in post-operative peritoneal adhesion formation显示文摘Peritoneal adhesions are a near inevitable occurrence after laparotomy and a major cause of both patient and physician misery. To date, clinical attempts at their amelioration have concentrated on manipulating the physical factors that affect their development despite a wealth of experimental data elucidating the molecular mechanisms that underlie their initiation, development and maturation. However, the advent of targeted, specific anti-cytokine agents as directed therapy for inflammatory and neoplastic conditions raises the prospect of a new era for anti-adhesion strategies. 5针刺作为辅助手段治疗精神分裂症的探索性病例研究(英文)显示文摘目的:探索个体针刺疗法作为常规治疗的辅助手段在精神分裂症患者中的效果;采用探索性病例研究的方法,使用定量和定性评价工具,以便能够对针刺治疗此类患者的确切疗效作出科学评价,并对未来采用针刺治疗此类患者的试验设计提供方法学建议。方法:共纳入11例精神分裂症患者,在予以10周的针刺治疗前,治疗过程中及治疗后分别采用多种有效的定量和定性评价工具对患者进行多方面的评价。这些评价手段包括对针刺治疗、全科医生及精神科医生病历的整理回顾。分析定性资料以评价受试者向研究者及其临床医生和护理人员报告的内容的可靠性。结果:11名受试者中共有8名完成了1个疗程的针刺治疗,全部11名受试者都报告了针刺治疗的积极作用,包括精神分裂症症状的改善,缓解药物的副作用,对精力、内在动机、睡眠、成瘾症状及其他身体机能的促进作用。但是,受试者向研究者和针刺医生报告的信息在不同治疗阶段有一定差别,且往往不一致;在研究结束后,受试者向研究者报告了更多的信息。结论:本研究表明,在常规疗法之外辅以针刺治疗能够给精神分裂症病人带来一定益处。对数据的三角测量表明了受试者报告内容的某些不一致性,而这些可以通过多种研究方法的混合使用得以克服。通过对数据的比较,推荐在将来的研究设计中使用阳性与阴性症状量表、针刺临床试验干预措施报告标准以及在问卷中增加对药物副作用、运动、睡眠以及日常作息的调查。在将来的研究中还应充分考虑到,恢复期患者容易复发,应该让他们可以方便地获得帮助。Patricia Ronan Nicola Robinson Dominic Harbinson Douglas MacInnes 2011中西医结合学报2011,9,5:6
6It is safe to use transdermal glyceryl trinitrate to lower blood pressure in patients with acute ischaemic stroke with carotid stenosis显示文摘Background There is concern that blood pressure(BP)lowering in acute stroke may compromise cerebral perfusion and worsen outcome in the presence of carotid stenosis.We assessed the effect of glyceryl trinitrate(GTN)in patients with carotid stenosis using data from the Efficacy of Nitric Oxide in Stroke(ENOS)Trial.Methods ENOS randomised 4011 patients with acute stroke and raised systolic BP(140-220 mm Hg)to transdermal GTN or no GTN within 48 hours of onset.Those on prestroke antihypertensives were also randomised to stop or continue their medication for 7 days.The primary outcome was the modified Rankin Scale(mRS)at day 90.Ipsilateral carotid stenosis was split:<30%;30-<50%;50-<70%;≥70%.Data are ORs with 95%CIs adjusted for baseline prognostic factors.results 2023(60.5%)ischaemic stroke participants had carotid imaging.As compared with<30%,≥70%ipsilateral stenosis was associated with an unfavourable shift in mRS(worse outcome)at 90 days(OR 1.88,95%CI 1.44 to 2.44,p<0.001).Those with≥70%stenosis who received GTN versus no GTN had a favourable shift in mRS(OR 0.56,95%CI 0.34 to 0.93,p=0.024).In those with 50-<70%stenosis,continuing versus stopping prestroke antihypertensives was associated with worse disability,mood,quality of life and cognition at 90 days.Clinical outcomes did not differ across bilateral stenosis groups.Conclusions Following ischaemic stroke,severe ipsilateral carotid stenosis is associated with worse functional outcome at 90 days.GTN appears safe in ipsilateral or bilateral carotid stenosis,and might improve outcome in severe ipsilateral carotid stenosis.Jason P Appleton Lisa J Woodhouse Andrew Belcher Daniel Bereczki Eivind Berge Valeria Caso Hui Meng Chang Hanne K Christensen Ronan Collins John Gommans Ann C Laska George Ntaios Serefnur Ozturk Gillian M Sare Szabolcs Szatmari Yongjun Wang Joanna M Wardlaw Nikola Sprigg Philip M Bath for the ENOS investigators 2019Stroke & Vascular Neurology2019,4,1:6

9耐碱产酸菌筛选及其在赤泥碱性调控中的应用(英文)显示文摘赤泥堆场是一种典型矿业废弃地,盐度高,碱性强,对植物生长十分不利。酸碱中和是降低赤泥碱性的主要方法,对堆场植被重建具有重要意义。本研究从赤泥堆场中筛选出1株耐碱产酸细菌EEEL02,经鉴定,该菌株为苏云金芽孢杆菌,从属于芽孢杆菌门。通过单因素试验确定该菌株最佳产酸条件:初始pH为10,盐浓度5%,培养温度25°C;最优发酵培养基组成为葡萄糖2%,蛋白胨0.5%。将EEEL02接种于赤泥中,在最佳培养条件下培养5 d后,赤泥p H从10.26降低至5.62。EEEL02在发酵过程中主要代谢产物为乙酸、丙酸和二氧化碳。微生物发酵产酸过程能有效降低赤泥碱度,为赤泥碱性调控提出一种新思路。吴昊 廖嘉欣 朱锋 Graeme MILLAR Ronan COURTNEY 薛生国 2019Journal of Central South University2019,26,2:4
10NOTES:The question for minimal resection and sentinel node in early gastric cancer显示文摘Surgical therapy for gastric cancer involves both removal of the cancer lesion and complete lymph node dissection.Natural orifice transluminal endoscopic surgery(NOTES) is considered to represent the next revolution in surgery.Many surgeons and endoscopists believe that NOTES may be a superior alternative for early gastric cancer treatment.Sentinel node(SN) navigation surgery for gastric cancer:Single institution results of SN mapping for early gastric cancer are increasingly being considered acceptable.Furthermore,a major large-scale clinical trial of SN mapping for gastric cancer has recently been completed by The Japan Society of SN Navigation Surgery study group.They reported false negative rate of 7.0 while the sensitivity of metastasis detection based on SN status was 93.Combination of SN biopsy and NOTES:This concept was first described by Cahill et al who proved the feasibility of lymphatic mapping and SN biopsy by NOTES.Lymphatic channel filling was immediately observable via the intraperitoneal optics.Partial resection of the stomach by hybrid NOTES:Several centers have already reported gastrectomy assisted by NOTES using the transvaginal route.However,the main problem of full-thickness resection of gastric wall remains endoscopic gastric closure.Establishing an endoscopic suturing method would be an important step toward expanding potential indications.NOTES is met with both enthusiasm and skepticism but will gain its own place as human creativity eventually provides solutions to its technical limitations.In the near future,NOTES can evolve the capacity to complement the existing armamentarium for gastric cancer surgery.Mitsuhiro Asakuma Ronan A Cahill Sang-Woong Lee Eiji Nomura Nobuhiko Tanigawa 2010World Journal of Gastrointestinal Surgery2010,2,6:3
11What's wrong with sentinel node mapping in colon cancer?显示文摘Despite near-universal embrace of the concept and clinical relevance of lymphatic mapping for sentinel node identification and analysis for cancers of the breast and integument,the same technique has struggled to a find a role in gastrointestinal cancers in general and,perhaps,in colon cancer in particular. Despite many studies demonstrating its feasibility in malignancies of the large bowel,concern is continually aroused by the variable and often unacceptably low sensitivity rates. Additionally,many confess uncertainty as to what benefit it could ever confer to patients even if it were proven sufficiently accurate given that standard surgical resection incorporates mesenteric resection anyway. 18Efficient mid-infrared single-photon detection using superconducting NbTiN nanowires with high time resolution in a Gifford-McMahon cryocooler显示文摘Shortly after their inception, superconducting nanowire single-photon detectors(SNSPDs) became the leading quantum light detection technology. With the capability of detecting single-photons with near-unity efficiency,high time resolution, low dark count rate, and fast recovery time, SNSPDs outperform conventional single-photon detection techniques. However, detecting lower energy single photons(<0.8 eV) with high efficiency and low timing jitter has remained a challenge. To achieve unity internal efficiency at mid-infrared wavelengths, previous works used amorphous superconducting materials with low energy gaps at the expense of reduced time resolution(close to a nanosecond), and by operating them in complex milli Kelvin(mK) dilution refrigerators. In this work,we provide an alternative approach with SNSPDs fabricated from 5 to 9.5 nm thick NbTiN superconducting films and devices operated in conventional Gifford-McMahon cryocoolers. By optimizing the superconducting film deposition process, film thickness, and nanowire design, our fiber-coupled devices achieved >70% system detection efficiency(SDE) at 2 μm and sub-15 ps timing jitter. Furthermore, detectors from the same batch demonstrated unity internal detection efficiency at 3 μm and 80% internal efficiency at 4 μm, paving the road for an efficient mid-infrared single-photon detection technology with unparalleled time resolution and without mK cooling requirements. We also systematically studied the dark count rates(DCRs) of our detectors coupled to different types of mid-infrared optical fibers and blackbody radiation filters. This offers insight into the trade-off between bandwidth and DCRs for mid-infrared SNSPDs.
