1指南2.0:为成功制定指南而系统研发的全面清单显示文摘背景虽然当前已有一些评估卫生保健指南可靠性的工具,但仍缺乏针对制定指南实际步骤的指导。针对指南制定者们所需考虑的相关资源和工具,我们系统研发了一份全面的条目清单,但这并不意味着每篇指南都需遵守该清单的所有条目。方法我们检索了国际指南制定机构的指南制定手册、指南的指南(主要是来自国际和国家机构以及专业学会的方法学报告),以及提供系统指导的最新文章。经过反复评价这些资料,尽可能全面地罗列和提取条目,并制定与指南有关的重要主题。通过反复讨论,我们对条目进行评价以去重和补漏,同时邀请指南制定专家对所增加的条目进行修改并提出建议。结果我们制定了一份包含18个主题、146个条目的清单,并建立了帮助指南制定者应用这些条目的网站。这些主题和条目涵盖了指南从规划、完成、实施和评估的全过程。最终的清单版本也包括了培训所需的资料以及应用这些条目时用到的方法学参考文献的链接。解释本清单将提供给指南制定者用作参考。仔细考虑清单中的条目将有助于指南的制定、实施和评估,我们也将会通过大众反馈来修订并持续更新清单。Holger J. 2Management of pancreatic fluid collections: A comprehensive review of the literature显示文摘Pancreatic fluid collections(PFCs) are a frequent complication of pancreatitis. It is important to classify PFCs to guide management. The revised Atlanta criteria classifies PFCs as acute or chronic, with chronic fluid collections subdivided into pseudocysts and walled-off pancreatic necrosis(WOPN). Establishing adequate nutritional support is an essential step in the management of PFCs. Early attempts at oral feeding can be trialed in patients with mild pancreatitis. Enteral feeding should be implemented in patients with moderate to severe pancreatitis. Jejunal feeding remains the preferred route of enteral nutrition. Symptomatic PFCs require drainage; options include surgical, percutaneous, or endoscopic approaches. With the advent of newer and more advanced endoscopic tools and expertise, and an associated reduction in health care costs, minimally invasive endoscopic drainage has become the preferable approach. An endoscopic ultrasonography-guided approach using a seldinger technique is the preferred endoscopic approach. Both plastic stents and metal stents are efficacious and safe; however, metal stents may offer an advantage, especially in infected pseudocysts and in WOPN. Direct endoscopic necrosectomy is often required in WOPN. Lumen apposing metal stents that allow for direct endoscopic necrosectomy and debridement through the stent lumen are preferred in these patients. Endoscopic retrograde cholangio pancreatography with pancreatic duct(PD) exploration should be performed concurrent to PFC drainage. PD disruption is associated with an increased severity of pancreatitis, an increased risk of recurrent attacks of pancreatitis and long-term complications, and a decreased rate of PFC resolution after drainage. 3Real time endoscopic ultrasound elastography and strain ratio in the diagnosis of solid pancreatic lesions显示文摘AIM To evaluate the accuracy of the elastography score combined to the strain ratio in the diagnosis of solid pancreatic lesions(SPL). METHODS A total of 172 patients with SPL identified by endoscopic ultrasound were enrolled in the study to evaluate the efficacy of elastography and strain ratio in differentiating malignant from benign lesions. The semi quantitative score of elastography was represented by the strain ratio method. Two areas were selected, area(A) representing the region of interest and area(B) representing the normal area. Area(B) was then divided by area(A). Sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), and accuracy were calculated by comparing diagnoses made by elastography, strain ratio and final diagnoses.RESULTS SPL were shown to be benign in 49 patients and malignant in 123 patients. Elastography alone had a sensitivity of 99%, a specificity of 63%, and an accuracy of 88%, a PPV of 87% and an NPV of 96%. The best cut-off level of strain ratio to obtain the maximal area under the curve was 7.8 with a sensitivity of 92%, specificity of 77%, PPV of 91%, NPV of 80% and an accuracy of 88%. Another estimated cut off strain ratio level of 3.8 had a higher sensitivity of 99% and NPV of 96%, but with less specificity, PPV and accuracy 53%, 84% and 86%, respectively. Adding both elastography to strain ratio resulted in a sensitivity of 98%, specificity of 77%, PPV of 91%, NPV of 95% and accuracy of 92% for the diagnosis of SPL. 4Hepato-cardiac disorders显示文摘Understanding the mutual relationship between the liver and the heart is important for both hepatologists and cardiologists. Hepato-cardiac diseases can be classified into heart diseases affecting the liver, liver diseases affecting the heart, and conditions affecting the heart and the liver at the same time. Differential diagnoses of liver injury are extremely important in a cardiologist's clinical practice calling for collaboration between cardiologists and hepatologists due to the many other diseases that can affect the liver and mimic haemodynamic injury. Acute and chronic heart failure may lead to acute ischemic hepatitis or chronic congestive hepatopathy. Treatment in these cases should be directed to the primary heart disease. In patients with advanced liver disease, cirrhotic cardiomyopathy may develop including hemodynamic changes, diastolic and systolic dysfunctions, reduced cardiac performance and electrophysiological abnormalities. Cardiac evaluation is important for patients with liver diseases especially before and after liver transplantation. Liver transplantation may lead to the improvement of all cardiac changes and the reversal of cirrhotic cardiomyopathy. There are systemic diseases that may affect both the liver and the heart concomitantly including congenital, metabolic and inflammatory diseases as well as alcoholism. 5Development of an Arabic version of the National Eye Institute Visual Function Questionnaire as a tool to study eye diseases patients in Egypt显示文摘AIM:To develop and test an Arabic version of the National Eye Institute Visual Function Questionnaire-25(NEI-VFQ-25).METHODS:NEI-VFQ-25 was translated into Arabic according to WHO translation guidelines. We enrolled adult consenting patients with bilateral chronic eye diseases who presented to 14 hospitals across Egypt from October to December 2012, and documented their clinical findings. Psychometric properties were then tested using STATA.RESULTS:We recruited 379 patients, whose mean age was(54.5±15)y. Of 46.2% were males, 227 had cataract,31 had glaucoma, 23 had retinal detachment, 37 had diabetic retinopathy, and 61 had miscellaneous visual defects. Non-response rate and the floor and ceiling numbers of the Arabic version(ARB-VFQ-25) were calculated. Internal consistency was high in all subscales(except general health), with Cronbach-α ranging from0.702-0.911. Test-retest reliability was high(intraclass correlation coefficient 0.79).CONCLUSION:RB-VFQ-25 isareliableandvalidtool for assessing visual functions of Arabic speaking patients. However, some questions had high non-response rates and should be substituted by available alternatives. 8稀土氧化物EuO_x对Pt/CeO_2材料在甲苯催化燃烧中的显著促进机制(英文)显示文摘挥发性有机污染物是大气环境中一类重要的污染物质,其污染控制问题已成为21世纪大气污染控制的优先方向之一.甲苯作为一种典型芳香烃在医药、农药、染料合成等多个化工行业中常用作有机化工原料并被排入大气环境,对人类的生产和生活造成极大危害.由于其高致癌性,甲苯在2017年被世界卫生组织国际癌症研究机构列为3类致癌物.在甲苯的催化燃烧降解中,负载型铂基催化剂由于其高效的低温催化活性被学者广泛研究.CeO_2作为一种典型的稀土金属氧化物具有较强的储放氧能力和高温稳定性,被视为负载型贵金属催化剂的优良载体.近期研究表明, EuO_x具有良好的催化氧化活性,并能促进金属氧化物表面氧缺位的形成.为了进一步研究EuO_x与CeO_2之间的协同效应对贵金属基催化剂低温催化性能的影响,我们利用表面模板剂法合成了具有不同Eu负载量的Pt/Eu_2O_3-CeO_2材料并应用于甲苯的深度催化氧化.研究表明, EuO_x的加入显著提高了Pt/CeO_2材料的催化性能,当Eu含量为2.5at%时(Pt/EC-2.5)该材料拥有最佳的低温氧化活性,可在200°C实现对0.09%甲苯的完全降解,在160°C时反应速率和转化频次分别高达0.9 mmolg^(–1)s^(–1)和5.52×10~2s^(–1).相比于Pt/CeO_2催化剂, EuO_x与CeO_2之间的协同作用极大提高了催化剂的氧化还原性能,增大了材料表面晶格氧和Ce3+的含量,并显著提高了贵金属活性相的分散度.通过原位漫反射红外光谱在线分析了甲苯在Pt/Eu_2O_3-CeO_2材料表面的深度降解机理.与甲苯在Pt/CeO_2材料表面的氧化行为相比,甲苯在Pt/EC-2.5催化剂表面的降解遵循典型的Mars-vanKrevelen(MvK)机理.当吸附于稀土金属氧化物载体后,在Pt活性位点、晶格氧及表面酸性位点的联合作用下,甲苯以酮类和醛类为主要的中间产物,被迅速深度氧化为H_2O和CO_2.赵宝明 建艳飞 姜泽宇 Reem Albilali 何炽 2019Chinese Journal of Catalysis2019,40,4:7
9New approaches for diagnosis of dry eye disease显示文摘We reviewed the literature for different diagnostic approaches for dry eye disease(DED) including the most recent advances, contradictions and promising diagnostic tools and technique. We performed a broad literature search for articles discussing different methods for diagnosis of DED including assessment of tear osmolarity, tear film stability, ocular biomarkers and others. Articles indexed in PubMed and google scholar were included. With the growing cosmetic industry, environmental pollution, and booming of digital screens, DED is becoming more prevalent. Its multifactorial etiology renders the diagnosis challenging and invites the emergence of new diagnostic tools and tests. Diagnostic tools can be classified, based on the parameter they measure, into tear film osmolarity, functional visual acuity, tear volume, tear turnover, tear film stability, tear film composition, ocular biomarkers and others. Although numerous methods exist, the most accurate diagnosis can be reached through combining the results of more than one test. Many reported tests have shown potential as diagnostic/screening tools, however, require more research to prove their diagnostic power, alone or in combination. 10Transcranial magnetic stimulation in animal models of neurodegeneration显示文摘Brain stimulation techniques offer powerful means of modulating the physiology of specific neural structures. In recent years, non-invasive brain stimulation techniques, such as transcranial magnetic stimulation(TMS) and transcranial direct current stimulation, have emerged as therapeutic tools for neurology and neuroscience. However, the possible repercussions of these techniques remain unclear, and there are few reports on the incisive recovery mechanisms through brain stimulation. Although several studies have recommended the use of non-invasive brain stimulation in clinical neuroscience, with a special emphasis on TMS, the suggested mechanisms of action have not been confirmed directly at the neural level. Insights into the neural mechanisms of non-invasive brain stimulation would unveil the strategies necessary to enhance the safety and efficacy of this progressive approach. Therefore, animal studies investigating the mechanisms of TMSinduced recovery at the neural level are crucial for the elaboration of non-invasive brain stimulation. Translational research done using animal models has several advantages and is able to investigate knowledge gaps by directly targeting neuronal levels. 14Nutritional assessment in patients with liver cirrhosis显示文摘Malnutrition is a liver cirrhosis complication affecting more than 20%-50%of patients.Although the term can refer to either nutrient deficiency or excess,it usually relates to undernutrition in cirrhosis settings.Frailty is defined as limited physical function due to muscle weakness,whereas sarcopenia is defined as muscle mass loss and an advanced malnutrition stage.The pathogenesis of malnutrition in liver cirrhosis is multifactorial,including decreased oral intake,maldigestion/malabsorption,physical inactivity,hyperammonemia,hypermetabolism,altered macronutrient metabolism and gut microbiome dysbiosis.Patients with chronic liver disease with a Body Mass Index of<18.5 kg/m2 and/or decompensated cirrhosis or Child-Pugh class C are at the highest risk of malnutrition.For patients at risk of malnutrition,a detailed nutritional assessment is required,typically including a history and physical examination,laboratory testing,global assessment tools and body composition testing.The latter can be done using anthropometry,cross-sectional imaging including computed tomography or magnetic resonance,bioelectrical impedance analysis and dual-energy X-ray absorptiometry.A multidisciplinary team should screen for and treat malnutrition in patients with cirrhosis.Malnutrition and sarcopenia are associated with an increased risk of complications and a poor prognosis in patients with liver cirrhosis;thus,it is critical to diagnose these conditions early and initiate the appropriate nutritional therapy.In this review,we describe the prevalence and pathogenesis of malnutrition in liver cirrhosis patients and discuss the best diagnostic approach to nutritional assessment for them.Sara Haj Ali Awni Abu Sneineh Reem Hasweh 2022World Journal of Hepatology2022,14,9:3

16Tumor necrosis factor-α-G308A polymorphism is associated with liver pathological changes in hepatitis C virus patients显示文摘AIM To investigate the association of tumor necrosis factor alpha(TNFα)-G308 A polymorphism with different liver pathological changes in treatment-na?ve Egyptian patients infected with hepatitis C virus(HCV) genotype 4.METHODS This study included 180 subjects,composed of 120 treatment-na?ve chronic HCV patients with different fibrosis grades(F0-F4) and 60 healthy controls. The TNFα-G308 A region was amplified by PCR and the different genotypes were detected by restriction fragment length polymorphism analysis. The TNFα protein was detected by enzyme-linked immunosorbent assay. The influence of different TNFα-G308 A genotypes on TNFα expression and liver disease progression were statistically analyzed. The OR and 95%CI were calculated to assess the relative risk confidence.RESULTS Current data showed that the TNFα-G308 A SNP frequency was significantly different between controls and HCV infected patients(P = 0.001). Both the AA genotype and A allele were significantly higher in late fibrosis patients(F2-F4,n = 60) than in early fibrosis patients(F0-F1,n = 60)(P = 0.05,0.04 respectively). Moreover,the GA or AA genotypes increased the TNFα serum level greater than the GG genotype(P = 0.002). The results showed a clear association between severe liver pathological conditions(inflammation,steatosis and fibrosis) and(GA + AA) genotypes(P = 0.035,0.03,0.04 respectively). The stepwise logistic regression analysis showed that the TNFα genotypes(GA + AA) were significantly associated with liver inflammation(OR = 3.776,95%CI: 1.399-10.194,P = 0.009),severe steatosis(OR = 4.49,95%CI: 1.441-14.0,P = 0.010) and fibrosis progression(OR = 2.84,95%CI: 1.080-7.472,P = 0.034). Also,the A allele was an independent risk factor for liver inflammation(P = 0.003),steatosis(P = 0.003) and fibrosis(P = 0.014). 17Potential role of killer immunoglobulin receptor genes among individuals vaccinated against hepatitis B virus in Lebanon显示文摘AIM To explore the role of killer immunoglobulin receptor(KIR) genes in responsiveness or non-responsiveness to vaccination against hepatitis B virus.METHODS We recruited 101 voluntary participants between March 2010 and December 2011. Sera samples from vaccinated and non-vaccinated participants were tested for the presence of anti-HBs antibodies as a measure of protection against hepatitis B, hepatitis B surface antigen and hepatitis B core antibody as indicators ofinfection by enzyme-linked immunosorbent assay. KIR gene frequencies were determined by polymerase chain reaction.RESULTS Sera samples from 99 participants were tested for the levels of anti-HBs as an indicator of protection(≥ 10 mI U/ml) following vaccination as defined by the World Health Organization international reference standard. Among the vaccinated participants, 47%(35/74) had anti-HBs titers above 100 mI U/ml, 22%(16/74) had antiHBs ranging between 10-100 mI U/ml, and 20%(15/74) had values of less than 10 mI U/ml. We report the lack of significant association between the number of vaccine dosages and the titer of antibodies among our vaccinated participants. The inhibitory KIR2Dl1, KIR2Dl4, KIR3Dl1, KIR3Dl2, and KIR3 Dl were detected in more than 95%, whereas KIR2Dl2, KIR2Dl3, KIR2Dl5(KR2Dl5A and KIR2Dl5B) were expressed in 56%, 84% and 42%(25% and 29%) of participants, respectively. The observed frequency of the activating KIR genes ranged between 35% and 55% except for KIR2DS4, detected in 95% of the study participants(40.6% 2DS4*001/002; 82.2% 2DS4*003/007). KIR2DP1 pseudogene was detected in 99% of our participants, whereas KIR3DP*001/02/04 and KIR3DP1*003 had frequencies of 17% and 100%, respectively. No association between the frequency of KIR genes and anti-HBs antibodies was detected. When we compared the frequency of KIR genes between vaccinated individuals with protective antibodies titers and those who lost their protective antibody levels, we did not detect a significant difference. KIR2Dl5 B was significantly different among different groups of vaccinated participants(group Ⅰ > 100 mI U/ml, group Ⅱ 10-100 mI U/ml, group Ⅲ < 10 mI U/ml and group Ⅳ with undetectable levels of protective antibodies). CONCLUSION To our knowledge, this is the first study screening for the possible role of KIR genes among individuals vaccinated against hepatitis B virus(HBV). 18Maternal vitamin D level and vitamin D receptor gene polymorphism as a risk factor for congenital heart diseases in offspring;An Egyptian case-control study显示文摘Vitamin D&vitamin D receptor(VDR)signaling play a very crucial role in early embryonic heart development.We construct this case-control study to investigate the association between maternal serum vitamin D level&VDR gene Fok1 polymorphism and risk of congenital heart defects(CHD)in offspring.Fifty mothers who had term neonates with CHD were considered as cases.Fifty age-comparable healthy mothers who had neonates without CHD were contemplated as controls.Maternal serum 25 hydroxyvitamin D[25(OH)D]level was tested using ELISA.Maternal VDR gene Fok1 polymorphism was analyzed using PCR-based RFLP-assay.There was a significant decrease in maternal vitamin D level(P=0.002)and a significant increase in vitamin D deficient status(P=0.007)among cases when compared to controls.VDR gene Fok1 genotypes distribution frequency were in accordance with Hardy Weinberg equilibrium(HW)among controls.A significant increase in VDR gene Fok1 F/f&f/f genotypes and f allele were observed in cases compared to controls with estimated odds ratio(95%confidence interval)&P-value of 3(1e8)&P=0.006,11(1e97)&P=0.01 and 3(2e6)&P=0.001 respectively.There was a significant decrease in maternal vitamin D level in neonates with cyanotic CHD(P=0.000)compared to those with a cyanotic CHD while there was no significant difference in VDR Fok1 genotype(P=0.18)&allele(P=0.05)distribution between two groups.We concluded that maternal vitamin D deficiency and VDR gene Fok1 F/f,f/f genotype and f allele were associated with increased risk of CHD in offspring.Wesam A.Mokhtar Amal Fawzy Reem M.Allam Rania M.Amer Mona S.Hamed 2019Genes & Diseases2019,6,2:2

20High omega arachidonic acid/docosahexaenoic acid ratio induces mitochondrial dysfunction and altered lipid metabolism in human hepatoma cells显示文摘BACKGROUND Non-alcoholic fatty liver disease(NAFLD) is a common cause of liver disease worldwide and is a growing epidemic. A high ratio of omega-6 fatty acids to omega-3 fatty acids in the diet has been implicated in the development of NAFLD. However, the inflicted cellular pathology remains unknown. A high ratio may promote lipogenic pathways and contribute to reactive oxygen species(ROS)-mediated damage, perhaps leading to mitochondrial dysfunction.Therefore, these parameters were investigated to understand their contribution to NAFLD development.AIM To examine the effect of increasing ratios of omega-6:3 fatty acids on mitochondrial function and lipid metabolism mediators.METHODS Hep G2-derived VL-17 A cells were treated with normal(1:1, 4:1) and high(15:1,25:1) ratios of omega-6: omega-3 fatty acids [arachidonic acid(AA):docosahexaenoic acid(DHA)] at various time points. Mitochondrial activity and function were examined via MTT assay and Seahorse XF24 analyzer, respectively.Triglyceride accumulation was determined by using Enzy Chrom? and levels of ROS were measured by fluorescence intensity. Protein expression of the mediators of lipogenic, lipolytic and endocannabinoid pathways was assessed by Western blotting.RESULTS High AA:DHA ratio decreased mitochondrial activity(P < 0.01;up to 80%) and promoted intracellular triglyceride accumulation(P < 0.05;40%-70%).Mechanistically, it altered the mediators of lipid metabolism;increased the expression of stearoyl-Co A desaturase(P < 0.05;22%-35%), decreased the expression of peroxisome proliferator-activated receptor-alpha(P < 0.05;30%-40%) and increased the expression of cannabinoid receptor 1(P < 0.05;31%).Furthermore, the high ratio increased ROS production(P < 0.01;74%-115%) and reduced mitochondrial respiratory functions such as basal and maximal respiration, ATP production, spare respiratory capacity and proton leak(P < 0.01;35%-68%).CONCLUSION High AA:DHA ratio induced triglyceride accumulation, increased oxidative stress and disrupted mitochondrial functions.
