| 1 | TGF-β/BMP signaling and other molecular events: regulation of osteoblastogenesis and bone formation显示文摘Transforming growth factor-beta(TGF-β)/bone morphogenetic protein(BMP) plays a fundamental role in the regulation of bone organogenesis through the activation of receptor serine/threonine kinases. Perturbations of TGF-β/BMP activity are almost invariably linked to a wide variety of clinical outcomes, i.e., skeletal, extra skeletal anomalies, autoimmune, cancer, and cardiovascular diseases. Phosphorylation of TGF-β(I/II) or BMP receptors activates intracellular downstream Smads, the transducer of TGF-β/BMP signals. This signaling is modulated by various factors and pathways, including transcription factor Runx2. The signaling network in skeletal development and bone formation is overwhelmingly complex and highly time and space specific.Additive, positive, negative, or synergistic effects are observed when TGF-β/BMP interacts with the pathways of MAPK, Wnt, Hedgehog(Hh), Notch, Akt/m TOR, and mi RNA to regulate the effects of BMP-induced signaling in bone dynamics. Accumulating evidence indicates that Runx2 is the key integrator, whereas Hh is a possible modulator, mi RNAs are regulators, and b-catenin is a mediator/regulator within the extensive intracellular network. | 2 | Molecular and biochemical trajectories from diabetes to Alzheimer's disease: A critical appraisal显示文摘Diabetes mellitus(DM), a metabolic disorder is a major orchestra influencing brain and behavioral responses via direct or indirect mechanisms. Many lines of evidence suggest that diabetic patients apparently face severe brain complications, but the story is far from being fully understood. Type 2 diabetes, an ever increasing epidemic and its chronic brain complications are implicated in the development of Alzheimer's disease(AD). Evidences from clinical and experimental studies suggest that insulin draws a clear trajectory from the peripheral system to the central nervous system. This review is a spot light on striking pathological, bio-chemical, molecular and behavioral commonalities of AD and DM. Incidence of cognitive decline in diabetic patients and diabetic symptoms in AD patients has brought the concept of brain diabetes to attention. Brain diabetes reflects insulin resistant brain state with oxidative stress, cognitive impairment, activation of various inflammatory cascade and mitochondrial vulnerability as a shared footprint of AD and DM. It has become extremely important for the investigators to understand the patho-physiology of brain complications in diabetes and put intensive pursuits for therapeutic interventions. For most cases, the only chance of cure comes from resection for localised disease, however relapse rates remain high following surgery. Data has emerged over recent years regarding the utility of adjuvant chemotherapy for improving disease-free and overall survival of patients following curative resection. This paper reviews the clinical trials that have been conducted in this area along with the studies integrating radiation therapy in the adjuvant setting. | 4 | Neutrophil-lymphocyte ratio:A prognostic tool in patients with inhospital cardiac arrest显示文摘BACKGROUND In-hospital cardiac arrest(IHCA) portends a poor prognosis and survival to discharge rate. Prognostic markers such as interleukin-6, S-100 protein and high sensitivity C reactive protein have been studied as predictors of adverse outcomes after return of spontaneous circulation(ROSC); however; these variables are not routine laboratory tests and incur additional cost making them difficult to incorporate and less attractive in assessing patient's prognosis. The neutrophil-lymphocyte ratio(NLR) is a marker of adverse prognosis for many cardiovascular conditions and certain types of cancers and sepsis. We hypothesize that an elevated NLR is associated with poor outcomes including mortality at discharge in patients with IHCA.AIM To determine the prognostic significance of NLR in patients suffering IHCA who achieve ROSC.METHODS A retrospective study was performed on all patients who had IHCA with the advanced cardiac life support protocol administered in a large urban community United States hospital over a one-year period. Patients were divided into two groups based on their NLR value(NLR < 4.5 or NLR ≥ 4.5). This cutpoint was derived from receiving operator characteristic curve analysis(area under the curve = 0.66) and provided 73% positive predictive value, 82% sensitivity and42% specificity for predicting in-hospital death after IHCA. The primary outcome was death or discharge at 30 d, whichever came first.RESULTS We reviewed 153 patients with a mean age of 66.1 ± 16.3 years; 48% were female.In-hospital mortality occurred in 65%. The median NLR in survivors was 4.9(range 0.6-46.5) compared with 8.9(0.28-96) in non-survivors(P = 0.001). | 6 | A new approach for real time object detection and tracking on high resolution and multi-camera surveillance videos using GPU显示文摘High resolution cameras and multi camera systems are being used in areas of video surveillance like security of public places, traffic monitoring, and military and satellite imaging. This leads to a demand for computational algorithms for real time processing of high resolution videos. Motion detection and background separation play a vital role in capturing the object of interest in surveillance videos, but as we move towards high resolution cameras, the time-complexity of the algorithm increases and thus fails to be a part of real time systems. Parallel architecture provides a surpass platform to work efficiently with complex algorithmic solutions. In this work, a method was proposed for identifying the moving objects perfectly in the videos using adaptive background making, motion detection and object estimation. The pre-processing part includes an adaptive block background making model and a dynamically adaptive thresholding technique to estimate the moving objects. The post processing includes a competent parallel connected component labelling algorithm to estimate perfectly the objects of interest. New parallel processing strategies are developed on each stage of the algorithm to reduce the time-complexity of the system. This algorithm has achieved a average speedup of 12.26 times for lower resolution video frames(320×240, 720×480, 1024×768) and 7.30 times for higher resolution video frames(1360×768, 1920×1080, 2560×1440) on GPU, which is superior to CPU processing. Also, this algorithm was tested by changing the number of threads in a thread block and the minimum execution time has been achieved for 16×16 thread block. | 7 | The response of stromatolites to seismic shocks: Tomboliths from the Palaeoproterozoic Chaibasa Formation, E India显示文摘It is demonstrated here for the first time how Palaeoproterozoic stromatolites survived seismic disturbance of their substrate.The stromatolites under study could have been cyanobacteria or any other photoautotrophic microbes,which formed mats that covered a substrate of very fine-grained sandstones and mudstones of the Chaibasa Fm.in eastern India.The sediments represent a shelf environment.The local abundance of the stromatolites suggests that the low-energy environment formed a suitable habitat.The common phases of tectonic quiescence were,however,occasionally interrupted by seismic shocks.These were sufficiently strong to deform the mat layers,the lower parts of which might already have been(semi-) consolidated.The mats became partly folded,partly faulted,and already consolidated parts of the stromatolite layers broke off.This can be deduced from the angular shapes of part of the broken-off fragments.It appears,however,that part of these fragments were still sufficiently soft to become rounded and deformed by rolling over the seafloor,probably under the influence of tidal currents.When come to rest,these fragments served as a new substrate for new generations of the microorganisms.These micro-organisms thus survived by continued growth on the reworked fragments and built up new stromatolites that may show an 'angular disconformity' with the stromatolites of their substrate.It thus is shown that stromatolites have an adequate response to a sudden disturbance of their habitat,and that they survive earthquakes by colonization of broken-off fragments.We call the'healed'fragments' tomboliths'(tumbled stones). | A.J. | 11 | Weight loss interventions in living donor liver transplantation as a tool in expanding the donor pool: A systematic review and metaanalysis显示文摘BACKGROUND With increasing rates of liver transplantation and a stagnant donor pool,the annual wait list removals have remained high.Living donor liver transplantation(LDLT)is an established modality in expanding the donor pool and is the primary method of liver donation in large parts of the world.Marginal living donors,including those with hepatic steatosis,have been used to expand the donor pool.However,due to negative effects of steatosis on graft and recipient outcomes,current practice excludes overweight or obese donors with more than 10%macro vesicular steatosis.This has limited a potentially important source to help expand the donor pool.Weight loss is known to improve or resolve steatosis and rapid weight loss with short-term interventions have been used to convert marginal donors to low-risk donors in a small series of studies.There is,however,a lack of a consensus driven standardized approach to such interventions.AIM To assess the available data on using weight loss interventions in potential living liver donors with steatotic livers and investigated the feasibility,efficacy,and safety of using such donors on the donor,graft and recipient outcomes.The principal objective was to assess if using such treated donor livers,could help expand the donor pool.METHODS We performed a comprehensive literature review and meta-analysis on studies examining the role of short-term weight loss interventions in potential living liver donors with hepatic steatosis with the aim of increasing liver donation rates and improving donor,graft,and recipient outcomes.RESULTS A total of 6 studies with 102 potential donors were included.Most subjects were males(71).All studies showed a significant reduction in body mass index postintervention with a mean difference of-2.08(-3.06,1.10,I2=78%).A significant reduction or resolution of hepatic steatosis was seen in 93 of the 102(91.2%).Comparison of pre-and post-intervention liver biopsies showed a significant reduction in steatosis with a mean difference of-21.22(-27.02,-15.43,I2=56%).The liver donation rates post-intervention was 88.5(74.5,95.3,I2=42%).All donors who did not undergo LDLT had either recipient reasons or had fibrosis/steatohepatitis on post intervention biopsies.Post-operative biliary complications in the intervention group were not significantly different compared to controls with an odds ratio of 0.96[(0.14,6.69),I2=0].The overall post-operative donor,graft,and recipient outcomes in treated donors were not significantly different compared to donors with no steatosis.CONCLUSION Use of appropriate short term weight loss interventions in living liver donors is an effective tool in turning marginal donors to low-risk donors and therefore in expanding the donor pool.It is feasible and safe,with comparable donor,graft,and recipient outcomes,to non-obese donors.Larger future prospective studies are needed. | Sushrut Trakroo Nakul Bhardwaj Rajat Garg Jamak Modaresi Esfeh | 2021 | World Journal of Gastroenterology2021,27,24: | 2 |
