1非高密度脂蛋白胆固醇、载脂蛋白A-I及B100、标准脂质测量、脂质比值以及CRP作为女性心血管病危险因素的临床用途显示文摘背景:关于不同脂质测定、非高密度脂蛋白胆固醇(non-HDL-C)、脂质比值、载脂蛋白和C-反应蛋白(CRP)的临床应用,现行心血管危险检测指南仍有争议。 目的:直接比较总胆固醇(totalcholesterol,TC)、低密度脂蛋白胆固醇(LDL-C)、HDL-C、non-HDL-C、载脂蛋白A-I(apoA-I)及B100(apoB100)、高敏C-反应蛋白(高敏CRP)以及TC与HDL-C、LDL-C与HDL-C、apoB100与apoA-I、apoB100与HDL-C比值,在妇女作为将来心血管事件预测指标的临床用途。 设计、地点及参试者:于15 632例起始健康美国妇女进行的前瞻性队列研究。1992年11月至1995年7月人组时年龄≥45岁(4分位范围48~59岁)。所有参试者均经10年随访,观察将来心血管事件的发生情况。 主要观测指标:按照各项生化指标不同基线值而确定的首次主要心血管事件(n=4 764)的风险比(hazard ratios,HRs)及95%可信区间(confidence intervals,CIs)。 结果:在校正年龄、吸烟状况、血压、糖尿病及体重指数后,测量结果在最高5分位者将来发生心血管事件的风险比LDL-C为1.62(95%CI,1.17~2.25),apoA-I为1.75(95%CI,1.30~2.38),TC为2.08(95%CI,1.45~2.97),HDL-C为2.32(95%CI,1.64~3.33),apoB100为2.50(95%CI,1.68~3.72),non-HDL-C为2.51(95%CI,1.69~3.72),高敏CRP为2.98(95%CI,1.90~4.67)(P<0.001,趋势贯穿所有5分位)。脂质比值风险比,apoBl00与apoA-I为3.01(95%CI,2.01~4.50),LDL-C与HDL-C为3.18(95%CI,2.12~4.75),apoB100与HDL-C为3.56(95%CI,2.31~5.47),TC与HDL-C为3.81(95%CI,2.47~5.86)(P<0.001,趋势贯穿所有5分位)。高敏CRP与脂质参数的相关系数范围为-0.33到0.15,以CRP<1、1~3和>3 ms/L为临床分割点,可对每项脂质测量及脂质比值水平增加提供预后危险信息。 结论:non-HDL-C以及TC与HDL-C比值对将来心血管事件的预测效果与载脂蛋白相同或者更好。校正年龄、血压、吸烟、糖尿病和肥胖后,高敏CRP提供的预后信息超越各项脂质测量。Paul M Ridker Nader Rifai Nancy R.Cook Gary Bradwin Julie E Buring 徐成斌(译) 2006美国医学会杂志(中文版)2006,25,3:3

4Stratification of Patients with Diabetes Using Continuous Glucose Monitoring Profiles and Machine Learning显示文摘Background.Continuous glucose monitoring(CGM)offers an opportunity for patients with diabetes to modify their lifestyle tobetter manage their condition and for clinicians to provide personalized healthcare and lifestyle advice.However,analytic toolsare needed to standardize and analyze the rich data that emerge from CGM devices.This would allow glucotypes of patients tobe identified to aid clinical decision-making.Methods.In this paper,we develop an analysis pipeline for CGM data and applyit to 148 diabetic patients with a total of 8632 days of follow up.The pipeline projects CGM data to a lower-dimensional spaceof features representing centrality,spread,size,and duration of glycemic excursions and the circadian cycle.We then useprincipal components analysis and k-means to cluster patients’records into one of four glucotypes and analyze clustermembership using multinomial logistic regression.Results.Glucotypes differ in the degree of control,amount of time spent inrange,and on the presence and timing of hyper-and hypoglycemia.Patients on the program had statistically significantimprovements in their glucose levels.Conclusions.This pipeline provides a fast automatic function to label raw CGM datawithout manual input.Yinan Mao Kyle Xin Quan Tan Augustin Seng Peter Wong Sue-Anne Toh Alex R.Cook 2022Health Data Science2022,,1:0
5EpiMix:A novel method to estimate effective reproduction number显示文摘Transmission potential of a pathogen,often quantified by the time-varying reproduction number R t,provides the current pace of infection for a disease and indicates whether an emerging epidemic is under control.In this study,we proposed a novel method,EpiMix,for R t estimation,wherein we incorporated the impacts of exogenous factors and random effects under a Bayesian regression framework.Using Integrated Nested Laplace Approx-imation,EpiMix is able to efficiently generate reliable,deterministic R t estimates.In the simulations and case studies performed,we further demonstrated the method's robust-ness in low-incidence scenarios,together with other merits,including its flexibility in selecting variables and tolerance of varying reporting rates.All these make EpiMix a potentially useful tool for real-time R t estimation provided that the serial interval distri-bution,time series of case counts and external influencing factors are available.Shihui Jin Borame Lee Dickens Jue Tao Lim Alex R.Cook 2023Infectious Disease Modelling2023,8,3:0
6Urban-Rural Disparities for COVID-19:Evidence from 10 Countries and Areas in the Western Pacific显示文摘Background.Limited evidence on the effectiveness of various types of social distancing measures,from voluntary physical distancing to a community-wide quarantine,exists for the Western Pacific Region(WPR)which has large urban and rural populations.Methods.We estimated the time-varying reproduction number(Rt)in a Bayesian framework using district-level mobility data provided by Facebook(i)to assess how various social distancing policies have contributed to the reduction in transmissibility of SARS-COV-2 and(ii)to examine within-country variations in behavioural responses,quantified by reductions in mobility,for urban and rural areas.Results.Social distancing measures were largely effective in reducing transmissibility,with Rt estimates decreased to around the threshold of 1.Within-country analysis showed substantial variation in public compliance across regions.Reductions in mobility were significantly lower in rural and remote areas than in urban areas and metropolitan cities(p<0:001)which had the same scale of social distancing orders in place.Conclusions.Our findings provide empirical evidence that public compliance and consequent intervention effectiveness differ between urban and rural areas in the WPR.Further work is required to ascertain the factors affecting these differing behavioural responses,which can assist in policy-making efforts and increase public compliance in rural areas where populations are older and have poorer access to healthcare.Minah Park Jue Tao Lim Lin Wang Alex R.Cook Borame L.Dickens 2021Health Data Science2021,,1:0
7Predicting flows through microfluidic circuits with fluid walls显示文摘The aqueous phase in traditional microfluidics is usually confined by solid walls;flows through such systems are often predicted accurately.As solid walls limit access,open systems are being developed in which the aqueous phase is partly bounded by fluid walls(interfaces with air or immiscible liquids).Such fluid walls morph during flow due to pressure gradients,so predicting flow fields remains challenging.We recently developed a version of open microfluidics suitable for live-cell biology in which the aqueous phase is confined by an interface with an immiscible and bioinert fluorocarbon(FC40).Here,we find that common medium additives(fetal bovine serum,serum replacement)induce elastic no-slip boundaries at this interface and develop a semi-analytical model to predict flow fields.We experimentally validate the model’s accuracy for single conduits and fractal vascular trees and demonstrate how flow fields and shear stresses can be controlled to suit individual applications in cell biology.Cyril Deroy Nicholas Stovall-Kurtz Federico Nebuloni Cristian Soitu Peter R.Cook Edmond J.Walsh 2021Microsystems & Nanoengineering2021,7,6:0
