1欧洲卒中组织(ESO)关于无活动能力的急性缺血性卒中患者静脉血栓栓塞的预防指南显示文摘背景静脉血栓栓塞(venous thromboembolism, VTE)包括深静脉血栓形成(deep vein thrombosis, DVT)和肺栓塞,是无活动能力的急性缺血性卒中患者的常见并发症。本文阐述了欧洲卒中组织(European Stroke Organisation, ESO)关于无活动能力的急性缺血性卒中患者VTE预防的指南。有关出血性卒中的指南已发表。方法一个多学科小组确定了相关问题并使用推荐意见分级评估、制定和评价系统根据随机对照试验证据制定推荐意见。本文经过ESO内部及外部评审,并得到ESO指南委员会和ESO执行委员会批准。结果我们发现的主要是中等质量证据,包括评估梯度压力弹力袜(graduated compression stocking, GCS)、间歇充气加压装置(intermittent pneumatic compression, IPC)以及使用普通肝素(unfractionated heparin, UFH)、低分子量肝素(low molecular weight heparin, LMWH)和类肝素进行预防性抗凝治疗的随机对照试验和系统评价,但未发现评估神经肌肉电刺激(neuromuscular electrical stimulation, NES)的随机试验。我们推荐临床医生对无活动能力的患者使用IPC,但不应使用GCS。在无活动能力的缺血性卒中患者中,如果降低VTE风险的益处足以抵消其使用导致的颅内外出血风险增高,则应考虑使用UFH(5 000 U,2~3次/d)、LMWH或类肝素进行预防性抗凝治疗。如果确定具有预防性抗凝治疗的指征,应考虑使用LMWH或类肝素而非UFH,因为前两者能更明显地降低DVT风险,使用更加便利,人员成本更低,患者舒适度更高(每天仅需注射1次),但这些优势应与LMWH和类肝素的更高颅外出血风险、更高药物成本以及在肾功能不全老年患者中使用的相关风险进行权衡。结论IPC、UFH或LMWH和类肝素可降低无活动能力的急性缺血性卒中患者的VTE风险,但需要进一步研究来评价NES是否有效。其中证据最强的预防手段是IPC。需要更好的方法在卒中发病后最初数周内根据VTE风险和抗凝药的出血风险对患者进行分层。Martin Dennis Valeria Caso L Jaap Kappelle Aleksandra Pavlovic Peter Sandercock 刘品一 黄丽丽 徐运 2017国际脑血管病杂志2017,25,3:11
2Stents for malignant ureteral obstruction显示文摘Malignant ureteral obstruction can result in renal dysfunction or urosepsis and can limit the physician’s ability to treat the underlying cancer.There are multiple methods to deal with ureteral obstruction including regular polymeric double J stents(DJS),tandem DJS,nephrostomy tubes,and then more specialized products such as solid metal stents(e.g.,Resonance Stent,Cook Medical)and polyurethane stents reinforced with nickel-titanium(e.g.,UVENTA stents,TaeWoong Medical).In patients who require long-term stenting,a nephrostomy tube could be transformed subcutaneously into an extra-anatomic stent that is then inserted into the bladder subcutaneously.We outline the most recent developments published since 2012 and report on identifiable risk factors that predict for failure of urinary drainage.These failures are typically a sign of cancer progression and the natural history of the disease rather than the individual type of drainage device.Factors that were identified to predict drainage failure included low serum albumin,bilateral hydronephrosis,elevated C-reactive protein,and the presence of pleural effusion.Head-to-head studies show that metal stents are superior to polymeric DJS in terms of maintaining patency.Discussions with the patient should take into consideration the frequency that exchanges will be needed,the need for externalized hardware(with nephrostomy tubes),or severe urinary symptoms in the case of internal DJS.This review will highlight the current state of diversions in the setting of malignant ureteral obstruction.Kristina Pavlovic Dirk Lange Ben H.Chew 2016Asian Journal of Urology2016,3,3:9

4Telmisartan induces melanoma cell apoptosis and synergizes with vemurafenib in vitro by altering cell bioenergetics显示文摘Objective: Despite recent advancements in targeted therapy and immunotherapies, prognosis for metastatic melanoma patients remains extremely poor. Development of resistance to previously effective treatments presents a serious challenge and new approaches for melanoma treatment are urgently needed. The objective of this study was to examine the effects of telmisartan, an AGTR1 inhibitor and a partial agonist of PPARγ, on melanoma cells as a potential agent for repurposing in melanoma treatment.Methods: Expression of AGTR1 and PPARγ m RNA in melanoma patient tumor samples was examined in publicly available datasets and confirmed in melanoma cell lines by qRT-PCR. A panel of melanoma cell lines was tested in viability, apoptosis and metabolic assays in presence of telmisartan by flow cytometry and immunocytochemistry. A cytotoxic effect of combinations of telmisartan and targeted therapy vemurafenib was examined using the Chou-Talalay combination index method.Results: Both AGTR1 and PPARγ mRNA were expressed in melanoma patient tumor samples and decreased compared to the expression in the healthy skin. In vitro, we found that telmisartan decreased melanoma cell viability by inducing cell apoptosis.Increased glucose uptake, but not utilization, in the presence of telmisartan caused the fission of mitochondria and release of reactive oxygen species. 5Pentadecapeptide BPC 157 resolves suprahepatic occlusion of the inferior caval vein, Budd-Chiari syndrome model in ratsPentadecapeptide BPC 157 resolves suprahepatic occlusion of the inferior caval vein, Budd-Chiari syndrome model in rats显示文摘BACKGROUND Recently,as a possible therapy resolving solution,pentadecapeptide BPC 157 therapy,has been used in alleviating various vascular occlusion disturbances.BPC 157 was previously reviewed as novel mediator of Robert cytoprotection and endothelium protection in the stomach,and gut-brain axis,beneficial therapy in gastrointestinal tract,with particular reference to vascular recruitment,ulcerative colitis and tumor cachexia,and other tissues healing.Here we raised new hypothesis about BPC 157 therapy in the Budd-Chiari syndrome in rats,rapid bypassing of the suprahepatic inferior caval vein occlusion,and rats recovery with the active and effective pharmacotherapy treatment.AIM To investigate Budd-Chiari syndrome model(inferior caval vein suprahepatic occlusion)resolution,since BPC 157 resolves various rat vascular occlusion.METHODS We assessed the activated bypassing pathways between the inferior and superior caval veins and portocaval shunt,counteracted caval/portal hypertension,aortal hypotension,venous/arterial thrombosis,electrocardiogram disturbances,liver and gastrointestinal lesions(i.e.,stomach and duodenum hemorrhages,in particular,congestion).Rats with suprahepatic occlusion of the inferior vena cava by ligation were medicated at 1 min,15 min,24 h,or 48 h post-ligation.Medication consisted of 10μg/kg BPC 157,10 ng BPC 157 or 5 m L/kg saline,administered once as an abdominal bath or intragastric application.Gross and microscopic observations were made,in addition to assessments of electrical activity of the heart(electrocardiogram),portal and caval hypertension,aortal hypotension,thrombosis,hepatomegaly,splenomegaly and venography.Furthermore,levels of nitric oxide,malondialdehyde in the liver and serum enzymes were determined.RESULTS BPC 157 counteracted increased P wave amplitude,tachycardia and ST-elevation,i.e.,right heart failure from acute thrombotic coronary occlusion.The bypassing pathway of the inferior vena cava-azygos(hemiazygos)vein-superior vena cava and portocaval shunt occurred rapidly.Even with severe caval portal hypertension,BPC 157 antagonized portal and caval hypertension and aortal hypotension,and also reduced refractory ascites.Thrombosis of portal vein tributaries,inferior vena cava,and hepatic and coronary arteries was attenuated.In addition,there was reduced pathology of the lungs(severe capillary congestion)and liver(dilated central veins and terminal portal venules),decreased intestine hemorrhagic lesions(substantial capillary congestion,submucosal edema and architecture loss),and increased liver and spleen weight.During the period of ligation,nitric oxide-and malondialdehyde-levels in the liver remained within normal healthy values,and increases in serum enzymes were markedly reduced.CONCLUSION BPC 157 counteracts Budd Chiari syndrome in rats.Slaven Gojkovic Ivan Krezic Borna Vrdoljak Dominik Malekinusic Ivan Barisic andreja Petrovic Katarina Horvat Pavlov Marijan Kolovrat Antonija Duzel Mario Knezevic Katarina Kasnik Kovac Domagoj Drmic Lovorka Batelja Vuletic Antonio Kokot Alenka Boban Blagaic Sven Seiwerth Predrag Sikiric 2020World Journal of Gastrointestinal Pathophysiology2020,11,1:2

8Splenectomy with chemotherapy vs surgery alone as initial treatment for splenic marginal zone lymphoma显示文摘AIM:To evaluate the clinical characteristics of splenic marginal-zone lymphoma (SMZL) following antigen expression and the influence of therapeutic approaches on clinical outcome and overall survival (OS).METHODS: A total of 30 patients with typical histological and immunohistochemical SMZL patterns were examined. Splenectomy plus chemotherapy was applied in 20 patients, while splenectomy as a single treatment-option was performed in 10 patients. Prognostic factor and overall survival rate were analyzed.RESULTS: Complete remission (CR) was achieved in 20 (66.7%), partial remission (PR) in seven (23.3%), and lethal outcome due to disease progression occurred in three (10.0%) patients. Median survival of patients with a splenectomy was 93.0 mo and for patients with splenectomy plus chemotherapy it was 107.5 mo (Log rank=0.056, P>0.05). Time from onset of first symptoms to the beginning of the treatment (mean 9.4 mo) was influenced by spleen dimensions, as measured by computerized tomography and ultra-sound (t=2.558, P=0.018). Strong positivity (+++) of CD20 antigen expression in splenic tissue had a positive influence on OS (Log rank=5.244, P<0.05). The analysis of factors interfering with survival (by the Kaplan-Meier method) revealed that gender, general symptoms, clinical stage, and spleen infiltration type (nodular vs diffuse) had no significant (P>0.05) effects on the OS. 9Sustainable development of lignite production on open cast mines in Serbia显示文摘Energy sector represents a key industrial branch for national, environmental and economic success. With its exclusive access to domestic deposits, lignite industry represents a guarantor of reliable raw materials, offering long-term supply security based on verified reserves. Currently operated coalmines in Serbia (Kolubara and Kostolac) have production around 36 million tons of lignite, and over 108 million m3 of overburden. Consequently, sustainability of lignite production requires cost reduction and environmental protection, as well as capacity increase. In order to rationalise, and increase efficiency of Serbian lignite mines, it is necessary to focus the activities on major issues shown within the triangle of energy policy objectives (security of supply, competi- tive prices and environmental protection). Production process optimisation singled out several special programs. Equipment revi- talization and modernization is necessary taking into account that majority of the currently operated machinery has a life up to 25 years. Production process automation would enable high level of technical operation in the field of open cast mines management. Lack of coal quality uniformity is the permanent problem resulting by great amounts of coal reserves to be used uneconomically. Planning and training at all levels and finally cooperative software for business procedures and work order management.
