| 1 | Effect of preoperative biliary drainage on outcome of classical pancreaticoduodenectomy显示文摘AIM: To investigate the role of preoperative biliary drainage (PBD) in the outcome of classical pancreaticodu odenectomy. METHODS: A 10-year retrospective data analysis was performed on patients (n = 48) undergoing pancreaticoduodenectomy from March 1994 to March 2004 in department of surgery at SMS medical college, Jaipur, India. Demographic variables, details of preoperative stenting, operative procedure and post operative complications were noted. RESULTS: Preoperative biliary drainage was performed in 21 patients (43.5%). The incidence of septic complications was significantly higher in patients with biliary stent placement (P < 0.05, 0 vs 4). This group of patients also had a significantly higher minor biliary leak rate. Mortality and hospital stay in each group was comparable. CONCLUSION: Within this study population the use of PBD by endoscopic stenting was associated with a high incidence of infective complications. | 2 | Use of a Bioaugmented Organic Soil Amendment in Combination with Gypsum for Withania somnifera Growth on Sodic Soil显示文摘Limited availability of organic matter is a problem to sustain crop growth on sodic soil. Organic soil amendments are a costeffective source of nutrients to enhance crop growth. A field study was conducted to evaluate the effect of an organic soil amendment bioaugmented with plant growth-promoting fungi(SF_(OA) ) in combination with gypsum on soil properties and growth and yield attributes of Withania somnifera, one of the most valuable crops of the traditional medicinal system in the world, on a sodic soil at the Aurawan Research Farm of CSIR-National Botanical Research Institute, Lucknow, India. The SF_(OA) used was prepared by pre-enriching farm waste vermicompost with plant growth-promoting fungi before mixing with pressmud and Azadirachta indica seed cake. The application of SF_(OA) at 10 Mg ha^(-1)after gypsum(25.0 Mg ha^(-1)) treatment significantly(P < 0.05) increased root length(by 96%) and biomass(by 125%) of Withania plants compared to the control without SF_(OA) and gypsum. Similarly, the highest withanolide contents were observed in leaves and roots of Withania plants under 10 Mg ha^(-1)SF_(OA) and gypsum. Combined application of SF_(OA) and gypsum also improved physical, chemical and enzymatic properties of the soil, with the soil bulk density decreasing by 25%, water-holding capacity increasing by 121%, total organic C increasing by 90%, p H decreasing by 17% and alkaline phosphatase, β-glucosidase, dehydrogenase and cellulase activities increasing by 54%, 128%, 81% and 96%, respectively, compared to those of the control. | 3 | In-situ observation and transport modelling of arsenic in Gangetic plain,India显示文摘The focus of this study is to investigate the arsenic movement and impacts on the residual concentrations on groundwater pollution load.The Gangetic plain area in the Ballia,Uttar Pradesh is selected as study area,which is also reported to extreme arsenic pollution in soil-water system.A modelling approach is developed to assess the arsenic flux in partially saturated zone using data of soil texture,soil hydraulic properties and stratigraphy.Soil type,slope,and land-use cover is considered for estimating the transient flux at the top boundary from daily precipitation and evapotranspiration data of the study area.Solute transport in the subsurface is predicted by the mass transfer equation,which is derived by integrating Darcy's law with the equation of mass balance.The arsenic profiles of varying hydrogeological conditions associated with different locations in the study area are presented as breakthrough curves.The results shows that the arsenic transport is dominated by the advective flux and strongly depends on the soilmoisture flow conditions.Which may increases the arsenic load to underlaying groundwater resources.The simulated results suggest that mobility plays a vital role arsenic transport as well as on adsorbed arsenic concentration in subsurface.Likewise,the adsorption isotherms show that the high peak curve for Bairai and low at Sikarderpur.A higher pollution risk is observed in the Belthara Road,whereas a lower vulnerability is computed in the north and northeast regions.This study can help in strategising sustainable groundwater management and protection planning of identified regions of India. | Pankaj Kumar Gupta Pranjay Joshi Jahangeer | 2017 | Emerging Contaminants2017,3,4: | 2 |
| 4 | Laying open(deroofing) and curettage under local anesthesia for pilonidal disease: An outpatient procedure显示文摘AIM: To test the efficacy of lay open(deroofing, not excision) with curettage under local anesthesia(LOCULA) for pilonidal sinus as an outpatient procedure. METHODS: LOCULA procedure was done for all types of pilonidal disease. The primary outcome measure was cure rate. The secondary outcome measures were hospital stay, operating time, return to work, healing time and complication rate.RESULTS: Thirty-three(M/F-30/3, mean age-23.4 ± 5.8 years) consecutive patients were operated and followed for 24 mo(6-46 mo). Eleven were pilonidal abscess and 22 were chronic pilonidal disease. Six had recurrent disease. Operating time and the hospital stay was 22.3 ± 5.6 min and 63.8 ± 22.3 min respectively. The patients could resume normal work in 4.3 ± 3.2 d and the healing time was 42.9 ± 8.1 d. Thirty(93.8%)patients had complete resolution of the disease and two(6.2%) had a recurrence. Both the recurrences happened in patients who had complete healing but ignored the prescribed recommendations. One out of these got cured after getting operated again with the same procedure. Thus the overall success rate of this procedure was 96.9%.CONCLUSION: Lay open(deroofing) with curettage procedure under local anesthesia is an effective procedure to treat both simple and complicated pilonidal sinus and abscess. It is a simple procedure, has a high cure rate(up to 97%), doesn't require admission and is associated with minimal morbidity and scarring. | 6 | Clinical outcomes for nasopharyngeal cancer with intracranial extension after taxanebased induction chemotherapy and concurrent chemo-radiotherapy in the modern era显示文摘Objective:To evaluate the survival outcomes for a cohort of nasopharyngeal cancer with intracranial extension(ICE)treated with induction chemotherapy(ICT)followed by chemo-intensity-modulated radiotherapy(CTRT)at a tertiary cancer center.Methods:We retrospectively analyzed 45 patients with histologically proven,non-metastatic NPC with ICE treated at our institute between October 2008 and October 2016.Patients were classified as minor ICE or major ICE,based on the extent of ICE.All the patients received 2-3 cycles of a taxane-based ICT regimen followed by CTRT.Radiotherapy was delivered with'riskadapted'intensity-modulated radiotherapy(IMRT)technique in all patients.Results:After a median follow up of 45 months(range:8-113 months),the estimated 5-year DFS,LRFS,DMFS,and OS of the entire cohort was 58%,82%,67%and 74%respectively.On multivariate analysis,histological subtype was an independent predictor of LRFS,and age was an independent predictor of DFS.The extent of ICE showed only a trend towards worse DFS(P=0.06).None of the factors significantly predicted for DMFS or OS.Gender,N-stage,and response to ICT did not significantly affect any of the outcomes.Grade 2 or worse subcutaneous fibrosis was seen in 22%of patients and grade 2 or worse xerostomia was seen in 24%of patients at last follow up.Thirty-three percent of the patients developed clinical hypothyroidism at last follow up.None of the patients experienced any neurological or vascular complications.Conclusions:Taxane-based induction chemotherapy followed by chemo-intensity modulated radiotherapy resulted in excellent locoregional control and survival with acceptable toxicities in patients of nasopharyngeal cancer with intracranial extension.Distant metastasis continues to be the predominant problem in these patients. | Sarbani Ghosh-Laskar Avinash Pilar Carlton Johnny Kumar Prabhash Amit Joshi Jai Prakash Agarwal Tejpal Gupta Ashwini Budrukkar Vedang Murthy Monali Swain Vanita Noronha Vijay Maruthi Patil Prathamesh Pai Deepa Nair Devendra Arvind Chaukar Shivakumar Thiagarajan Gouri Pantvaidya Anuja Deshmukh Pankaj Chaturvedi Sudhir Nair Anil D’Cruz | 2020 | World Journal of Otorhinolaryngology-Head and Neck Surgery2020,6,1: | 1 |

| 10 | Sustainable remediation of failed slope using helical soil nails显示文摘The performance of a helical soil nailed structure is dependent on the installation torque required and the consequent pullout resistance developed.The present research work aims at proposing theoretical models to estimate the required torque during installation of helical soil nails.Moreover,theoretical models are also developed to predict the pullout capacity of single and group of the helical nail for uniform and staggered arrangements.The proposed model predicts the pure-elastic and elastic-plastic pullout behavior of different helical nails.An equation for estimating the capacity-totorque Ratio(Kt)has also been developed for different nail shaft diameters.The results from the proposed models are validated with experimental results obtained from model testing of both single and group of helical nails.The predicted results are also compared for validation with the published literature.The results for installation torque and pullout load depict that the developed models predict values which are in accordance with the experimental results and are also found in good agreement with the published literature.Thus,the proposed models can effectively be used by the filed engineers for estimating the required installation torque and corresponding pullout capacities for single or double plate helical soil nails in cohesionless soil under surcharge pressure range of 0–50k Pa. | Saurabh RAWAT Ashok Kumar GUPTA Pankaj SHARMA | 2023 | Journal of Mountain Science2023,20,6: | 0 |
| 11 | Radiation dose analysis of computed tomography coronary angiography in Children with Kawasaki disease显示文摘BACKGROUND There is evolving role of computed tomography coronary angiography(CTCA)in non-invasive evaluation of coronary artery abnormalities in children with Kawasaki disease(KD).Despite this,there is lack of data on radiation dose in this group of children undergoing CTCA.AIM To audit the radiation dose of CTCA in children with KD.METHODS Study(December 2013-February 2018)was performed on dual source CT scanner using adaptive prospective electrocardiography-triggering.The dose length product(DLP in milligray-centimeters-mGy.cm)was recorded.Effective radiation dose(millisieverts-mSv)was calculated by applying appropriate age adjusted conversion factors as per recommendations of International Commission on Radiological Protection.Radiation dose was compared across the groups(0-1,1-5,5-10,and>10 years).RESULTS Eighty-five children(71 boys,14 girls)with KD underwent CTCA.The median age was 5 years(range,2 mo-11 years).Median DLP and effective dose was 21 mGy.cm,interquartile ranges(IQR)=15(13,28)and 0.83 mSv,IQR=0.33(0.68,1.01)respectively.Mean DLP increased significantly across the age groups.Mean effective dose in infants(0.63 mSv)was significantly lower than the other age groups(1-5 years 0.85 mSv,5-10 years 1.04 mSv,and>10 years 1.38 mSv)(P<0.05).There was no significant difference in the effective dose between the other groups of children.All the CTCA studies were of diagnostic quality.No child required a repeat examination.CONCLUSION CTCA is feasible with submillisievert radiation dose in most children with KD.Thus,CTCA has the potential to be an important adjunctive imaging modality in children with KD. | Mahesh Chandra Bhatt Manphool Singhal Rakesh Kumar Pilania Subhash Chand Bansal Niranjan Khandelwal Pankaj Gupta Surjit Singh | 2023 | World Journal of Clinical Pediatrics2023,12,4: | 0 |
