4Upper-gastrointestinal bleeding secondary to peptic ulcer disease:Incidence and outcomes显示文摘AIM:To evaluate the incidence,surgery,mortality,and readmission of upper gastrointestinal bleeding(UGIB)secondary to peptic ulcer disease(PUD).METHODS:Administrative databases identified all hospitalizations for UGIB secondary to PUD in Alberta,Canada from 2004 to 2010(n=7079)using the International Classification of Diseases Codes(ICD-10).A subset of the data was validated using endoscopy reports.Positive predictive value and sensitivity with 95%confidence intervals(CI)were calculated.Incidence of UGIB secondary to PUD was calculated.Logistic regression was used to evaluate surgery,in-hospital mortality,and 30-d readmission to hospital with recurrent UGIB secondary to PUD.Co-variants accounted for in our logistic regression model included:age,sex,area of residence(i.e.,urban vs rural),number of Charlson comorbidities,presence of perforated PUD,undergoing upper endoscopy,year of admission,and interventional radiological attempt at controlling bleeding.A subgroup analysis(n=6356)compared outcomes of patients with gastric ulcers to those with duodenal ulcers.Adjusted estimates are presented as odds ratios(OR)with95%CI.RESULTS:The positive predictive value and sensitivity of ICD-10 coding for UGIB secondary to PUD were85.2%(95%CI:80.2%-90.2%)and 77.1%(95%CI:69.1%-85.2%),respectively.The annual incidence between 2004 and 2010 ranged from 35.4 to 41.2 per100000.Overall risk of surgery,in-hospital mortality,and 30-d readmission to hospital for UGIB secondary to PUD were 4.3%,8.5%,and 4.7%,respectively.Interventional radiology to control bleeding was performed in 0.6%of patients and 76%of these patients avoided surgical intervention.Thirty-day readmission significantly increased from 3.1%in 2004 to 5.2%in 2010(OR=1.07;95%CI:1.01-1.14).Rural residents(OR rural vs urban:2.35;95%CI:1.83-3.01)and older individuals(OR≥65 vs<65:1.57;95%CI:1.21-2.04)were at higher odds of being readmitted to hospital.Patients with duodenal ulcers had higher odds of dying(OR=1.27;95%CI:1.05-1.53),requiring surgery(OR=1.73;95%CI:1.34-2.23),and being readmitted to hospital(OR=1.54;95%CI:1.19-1.99)when compared to gastric ulcers.CONCLUSION:UGIB secondary to PUD,particularly duodenal ulcers,was associated with significant morbidity and mortality.Early readmissions increased over time and occurred more commonly in rural areas.Samuel Quan Alexandra Frolkis Kaylee Milne Natalie Molodecky Hong Yang Elijah Dixon Chad G Ball Robert P Myers Subrata Ghosh Robert Hilsden Sander Veldhuyzen van Zanten Gilaad G Kaplan 2014World Journal of Gastroenterology2014,20,46:21
5Proton accumulation accelerated by heavy chemical nitrogen fertilization and its long-term impact on acidifying rate in a typical arable soil in the Huang-Huai-Hai Plain显示文摘Cropland productivity has been significantly impacted by soil acidification resulted from nitrogen(N) fertilization, especially as a result of excess ammoniacal N input. With decades' intensive agricultural cultivation and heavy chemical N input in the Huang-Huai-Hai Plain, the impact extent of induced proton input on soil p H in the long term was not yet clear. In this study, acidification rates of different soil layers in the soil profile(0–120 cm) were calculated by p H buffer capacity(p HBC) and net input of protons due to chemical N incorporation. Topsoil(0–20 cm) p H changes of a long-term fertilization field(from 1989) were determined to validate the predicted values. The results showed that the acid and alkali buffer capacities varied significantly in the soil profile, averaged 692 and 39.8 mmolc kg–1 p H–1, respectively. A significant(P<0.05) correlation was found between p HBC and the content of calcium carbonate. Based on the commonly used application rate of urea(500 kg N ha–1 yr–1), the induced proton input in this region was predicted to be 16.1 kmol ha–1 yr–1, and nitrification and plant uptake of nitrate were the most important mechanisms for proton producing and consuming, respectively. The acidification rate of topsoil(0–20 cm) was estimated to be 0.01 unit p H yr–1 at the assumed N fertilization level. From 1989 to 2009, topsoil p H(0–20 cm) of the long-term fertilization field decreased from 8.65 to 8.50 for the PK(phosphorus, 150 kg P2O5 ha–1 yr–1; potassium, 300 kg K2 O ha–1 yr–1; without N fertilization), and 8.30 for NPK(nitrogen, 300 kg N ha–1 yr–1; phosphorus, 150 kg P2O5 ha–1 yr–1; potassium, 300 kg K2 O ha–1 yr–1), respectively. Therefore, the apparent soil acidification rate induced by N fertilization equaled to 0.01 unit p H yr–1, which can be a reference to the estimated result, considering the effect of atmospheric N deposition, crop biomass, field management and plant uptake of other nutrients and cations. 11Responses of Plant Community and Soil Properties to Inter-Annual Precipitation Variability and Grazing Durations in a Desert Steppe in Inner Mongolia显示文摘Grazing can dramatically affect arid grassland communities that are very vulnerable to environmental changes due to its relatively short and sparse ground coverage, low biomass, sandy soil and inter-annual precipitation found in the desert steppe. The study investigates the effects of different grazing durations on vegetation and soil properties of a desert steppe community. The experiment was conducted in Xisu Banner in Inner Mongolia with five treatments: CG(continuous grazing), 40UG(40 d ungrazed), 50UG(50 d ungrazed), 60UG(60 d ungrazed) and UG(ungrazed). The biomass of both shrub and annual-biennial plant communities were significantly decreased by CG. Continuous grazing and 40UG significantly reduced the ANPP(aboveground net primary productivity) by the end of the three year study. 60UG treatment increased soil organic carbon(OC), total nitrogen concentration(TN) and total phosphorus concentration(TP) concentrations and 50UG increased the TN and total phosphorus concentration(TK) concentrations, whereas CG, 40UG and 50UG decreased soil OC, TP and available phosphorus concentration(AP) concentrations. The perennial plant species of the desert steppe were generally tolerant for grazing. The annual-biennial plant species had large variability in ANPP because of the inter-annual precipitation. 13URBAN COMPETITIVE PATTERN AND ITS CHANGES IN CHINA显示文摘With the national economy’s transition from a planning one to a market one, the factors affecting regional economic pattern has changed greatly. In this paper we first construct an index system for evaluating competitiveness of Chinese cities based on modern competitive advantage theory. With principal component analysis and cluster analysis methods, we compare the factors affecling cities’ integrated competitiveness in 1990 and 1997. We find that four major factors are capital (including investment, FDI and urban financial revenue ), urban ithestructure (transporiation and communication, urbanization level, education etc. ), industrial performance and structure, degree of market openness and that these factors have become more advanced and complicated since 1990. Most Chinese cities are transforming from productive factors-oriented type to investment-oriented type and a few are becoming innovation oriented. The integrated competitiveness is closely related to cities’ scale, urban function, regional policy and degree of market openness and displays an uneven spatial pattern. The difference between the south and the north and the difference between the west and the east co-exist bul the former has been more larger. 14连柱框架结构基于性能的塑性设计与评估(英文)显示文摘抗弯钢框架结构(MRF)是目前研究最广泛、最流行的抗震系统,具有建筑灵活度高和施工效率高等优点。然而,这种结构常常缺乏足够的侧向刚度来限制结构的位移角,会导致在强震下结构和非结构构件产生严重的破坏。相反,偏心支撑钢框架结构(EBF)却有很大的侧向刚度来限制结构的位移角和有专属的抗震耗能构件。但是,支撑结构并不为建筑师所广泛接纳。为了结合抗弯钢框架结构和偏心支撑钢框架结构的优点,于2007年首次提出一种新型的抗震系统——连柱框架结构(LCF)。这种双抗震系统有相似于偏心支撑钢框架结构的窄连梁框架作为主要的抗震系统,而抗弯钢框架结构则作为次要的抗震系统。连梁作为结构保险丝可以在中震到大震中消耗地震能量,而且在震后很容易就可以被替代和修复。另一方面,抗弯钢框架结构可以提供附加的侧向抵抗力来防止结构在大震下倒塌。采用2008年提出的基于性能的塑性设计方法(PBPD)对建筑原型位于美国强震地区——加州洛杉矶的连柱框架结构进行设计。最后,使用先进的基于性能的评估步骤对原型结构的抗震性能进行了研究。研究结果表明,连柱框架结构(LCF)是一种具有良好抗震性能的结构。YANG T Y TUNG D P 李元杰 2015建筑钢结构进展2015,17,1:5
15Power management unit chip design for automobile active-matrix organic light-emitting diode display module显示文摘A power management unit (PMU) chip supplying dual panel supply voltage, which has a low electro-magnetic interference (EMI) characteristic and is favorable for miniaturization, is designed. A two-phase charge pump circuit using external pumping capacitor increases its pumping current and works out the charge-loss problem by using bulk-potential biasing circuit. A low-power start-up circuit is also proposed to reduce the power consumption of the band-gap reference voltage generator. And the ring oscillator used in the ELVSS power circuit is designed with logic devices by supplying the logic power supply to reduce the layout area. The PMU chip is designed with MagnaChip’s 0.25 μm high-voltage process.
