| 2 | Gastric polyps: Association with Helicobacter pylori status and the pathology of the surrounding mucosa, a cross sectional study显示文摘aim:To assess the endoscopic characteristics of gastric polyps and their association with Helicobacter pylori(H.pylori)status in a predominantly Hispanic population.m ETHODS:We conducted a retrospective study of all esophagogastroduodenoscopies performed at our institution.Demographic,endoscopic and histopathological data were reviewed.Categorization of patients into Hispanic and Non-Hispanic was based on selfidentification.Patients without resection/biopsy were not included in the analysis.Identification of polyps type was based on histological examination.One way analysis of variance was used to compare continuousvariables among different polyp types and Fisher’s exact test was used compare categorical variables among polyp types.Unadjusted and adjusted comparisons of demographic and clinical characteristics were performed according to the H.pylori status and polyp type using logistic regressions.RESULTS:Of 7090 patients who had upper endoscopy,335 patients had gastric polyps(4.7%).Resection or biopsy of gastric polyps was performed in 296 patients(88.4%)with a total of 442 polyps removed or biopsied.Of 296 patients,87(29%)had hyperplastic polyps,82(28%)had fundic gland polyps and 5(1.7%)had adenomatous polyps.Hyperplastic polyps were significantly associated with positive H.pylori status compared with fundic gland polyps(OR=4.621;95%CI:1.92-11.13,P=0.001).Hyperplastic polyps were also found to be significantly associated with portal hypertensive gastropathy compared with fundic gland polyps(OR=6.903;95%CI:1.41-33.93,P=0.0174).Out of 296 patients,30(10.1%)had a followup endoscopy with a mean duration of 26±16.3 mo.Interval development of cancer was not noted in any of the patients during follow up period.CONCLUSi ON:Gastric hyperplastic polyps were significantly associated with positive H.pylori status and portal hypertensive gastropathy as compared with fundic gland polyps. | Sherif Elhanafi Mohammed Saadi Wynee Lou Indika Mallawaarachchi Alok Dwivedi Marc Zuckerman Mohamed O Othman | 2015 | World Journal of Gastrointestinal Endoscopy2015,7,10: | 18 |

| 4 | Combination of Model-based Observer and Support Vector Machines for Fault Detection of Wind Turbines显示文摘Support vector machines and a Kalman-like observer are used for fault detection and isolation in a variable speed horizontalaxis wind turbine composed of three blades and a full converter. The support vector approach is data-based and is therefore robust to process knowledge. It is based on structural risk minimization which enhances generalization even with small training data set and it allows for process nonlinearity by using flexible kernels. In this work, a radial basis function is used as the kernel. Different parts of the process are investigated including actuators and sensors faults. With duplicated sensors, sensor faults in blade pitch positions,generator and rotor speeds can be detected. Faults of type stuck measurements can be detected in 2 sampling periods. The detection time of offset/scaled measurements depends on the severity of the fault and on the process dynamics when the fault occurs. The converter torque actuator fault can be detected within 2 sampling periods. Faults in the actuators of the pitch systems represents a higher difficulty for fault detection which is due to the fact that such faults only affect the transitory state(which is very fast) but not the final stationary state. Therefore, two methods are considered and compared for fault detection and isolation of this fault: support vector machines and a Kalman-like observer. Advantages and disadvantages of each method are discussed. On one hand, support vector machines training of transitory states would require a big amount of data in different situations, but the fault detection and isolation results are robust to variations in the input/operating point. On the other hand, the observer is model-based, and therefore does not require training, and it allows identification of the fault level, which is interesting for fault reconfiguration. But the observability of the system is ensured under specific conditions, related to the dynamics of the inputs and outputs. | 5 | Endothelial progenitor cells, potential biomarkers for diagnosis and prognosis of ischemic stroke: protocol for an observational case-control study显示文摘Ischemic stroke is a devastating,life altering event which can severely reduce patient quality of life.Despite years of research there have been minimal therapeutic advances.Endothelial progenitor cells(EPCs),stem cells involved in both vasculogenesis and angiogenesis,may be a potential therapeutic target.After a stroke,EPCs migrate to the site of ischemic injury to repair cerebrovascular damage,and their numbers and functional capacity may determine patients'outcome.This study aims to determine whether the number of circulating EPCs and their functional aspects may be used as biomarkers to identify the type(cortical or lacunar)and/or severity of ischemic stroke.The study will also investigate if there are any differences in these characteristics between healthy volunteers over and under 65 years of age.100 stroke patients(50 lacunar and 50 cortical strokes)will be recruited in this prospective,observational case-controlled study.Blood samples will be taken from stroke patients at baseline(within 48 hours of stroke)and days 7,30 and90.EPCs will be counted with flow cytometry.The plasma levels of pro-and anti-angiogenic factors and inflammatory cytokines will also be determined.Outgrowth endothelial cells will be cultured to be used in tube formation,migration and proliferation functional assays.Primary outcome is disability or dependence on day 90 after stroke,assessed by the modified Rankin Scale.Secondary outcomes are changes in circulating EPC numbers and/or functional capacity between patient and healthy volunteers,between patient subgroups and between elderly and young healthy volunteers.Recruitment started in February 2017,167 participants have been recruited.Recruitment will end in November 2019.West Midlands-Coventry&Warwickshire Research Ethics Committee approved this study(REC number:16/WM/0304)on September8,2016.Protocol version:2.0.The Bayraktutan Dunhill Medical Trust EPC Study was registered in ClinicalTrials.gov(NCT02980354)on November 15,2016.This study will determine whether the number of EPCs can be used as a prognostic or diagnostic marker for ischemic strokes and is a step towards discovering if transplantation of EPCs may aid patient recovery. | Kamini Rakkar Othman Othman Nikola Sprigg Philip Bath Ulvi Bayraktutan | 2020 | Neural Regeneration Research2020,15,7: | 7 |
| 6 | Review of sustainability, pretreatment, and engineering considerations of asphalt modifiers from the industrial solid wastes显示文摘The escalating mass of solid waste at an overwhelming sum requires a global attention to strive for efficient waste management and to outsource the ecological treatments. The asphalt pavement industry that consumes a vast scale of natural resources while contributing to thermal and greenhouse emissions is viewed as a high potential alternative for the application of solid waste as asphalt modifier and substantial waste reduction. In efforts to urge for cleaner and greener asphalt production, a growing trend towards usage of solid waste as a renewable material is paving a sustainable future for the asphalt pavement industry. The economic options of incorporating solid waste into the asphaltic mixture coupled with proven effective performances are a green and cost-effective alternative to mitigate various pavement distresses. Various options either as coarse or fine aggregates and as fillers in powder, ash, or fibre form are to stimulate further research interest to incorporate a diversified range of solid waste into the asphalt binder and asphalt mixture. Prior to incorporating solid waste into the asphaltic mixture through the selected options, prerequisite tests in addressing engineering limitations due to the unknown properties of solid waste are reviewed. The concerns on environmental impact are given heavy metals leaching possibilities into water sources are addressed by Toxicity Characterization Leaching Procedure test, to leverage further the systematic reuse of solid waste in steering towards the cleaner production of the asphalt mixture. The microcosmic traits of solid waste namely surface morphology, mineralogical composition and chemical composition are summarized based on the usage of Scanning Electron Microscope, X-Ray Diffraction, and X-Ray Fluorescence, respectively. | 7 | Thermo-mechanical,Wear and Fracture Behavior of High-density Polyethylene/Hydroxyapatite Nano Composite for Biomedical Applications:Effect of Accelerated Ageing显示文摘The objective of this work is to demonstrate how the viscoelastic,thermal,rheological,hardness,wear resistance and fracture behavior of bioinert high-density polyethylene(HDPE) can be changed by the addition of hydroxyapatite(HAP) nano particles.Also the effects of accelerated thermal ageing on the composite properties have been investigated.Different weight fractions of HAP nano particles up to 30 wt%have been incorporated in HDPE matrix by using melt blending in co-rotating intermeshing twin screw extruder.The fracture toughness results showed a remarkable decrease in proportion to the HAP content.The differential scanning calorimetry results indicated that the melting temperature and crystallinity were affected by the addition of HAP nano particles into the matrix.The complex viscosity increased as the percentage of HAP increased due to the restriction of the molecular mobility.The dynamic mechanical analysis results revealed that higher storage modulus(8.3 10^(11) Pa) could be obtained in the developed HDPE/HAP in 30 wt% compared to neat HDPE(5.1 10^(11) Pa).Finally,the hardness and wear resistance of HDPE were improved significantly due to the addition of HAP nano particles.The changes in the HDPE and its nano composite properties due to ageing showed that the HDPE and its nano composites crystallinity increased while the fracture toughness,hardness,wear resistance,storage and loss modulus decreased. | H.Fouad R.Elleithy Othman Y.Alothman | 2013 | Journal of Materials Science & Technology2013,29,6: | 6 |

| 9 | Complications and survival in patients undergoing colonic stenting for malignant obstruction显示文摘AIM:To investigate whether predicting patients that might be at a higher risk for complications might serve to improve the selection of patients undergoing colonic stenting.METHODS:A retrospective review of consecutive patients who underwent an attempted self-expandable metal stent(SEMS)insertion for malignant colonic obstruction between November 2006 and March 2013.All patients were either referred for preoperative colonic decompression with the intent of a single surgical procedure,or for palliation of the malignant colorectal obstruction for unresectable cancer.Fisher’s test orχ2test was performed on categorical variables,and the t test for continuous variables.Univariable and multivariable logistic regression were used to examine the association between independent variables and the presence of complications from SEMS insertion.RESULTS:SEMS insertion was attempted in 73 patients.Males comprised 55.71%and the mean age was 67.41±12.41 years.Of these,65.15%underwent subsequent surgery,while 34.85%received SEMS as palliation for advanced disease.Extracolonic tumors were only4.76%.The majority of patients had stageⅣdisease(63.83%),while the remainder had stageⅢ(36.17%).SEMS were successfully inserted in 93.85%(95%CI:87.85%-99.85%).Perforations occurred in 4.10%,SEMS migration in 8.21%,and stent re-occlusion from ingrowth occurred in 2.74%of patients.The mean duration of follow up for the patients was 13.52±17.48 mo(range 0-73 mo).None of the variables:age,sex,time between the onset of symptoms to SEMS insertion,time between SEMS insertion and surgery,length of the stenosis,location of the stenosis,albumin level,or receiving neoadjuvant chemotherapy,could predict the development of complications from either SEMS insertion nor prolonged survival.CONCLUSION:None of the variables could predict the development of complications or survival.Further studies are required to identify patients who would benefit the most from SEMS. | Majid A Almadi Nahla Azzam Othman Alharbi Alabbas H Mohammed Nazia Sadaf Abdulrahman M Aljebreen | 2013 | World Journal of Gastroenterology2013,19,41: | 5 |
| 10 | Antimicrobial activity and essential oils of Curcuma aeruginosa,Curcuma mangga,and Zingiber cassumunar from Malaysia显示文摘Objective:To analyze the chemical composition of the essential oils of Curcuma aeruginosa (C.aeruginosa),Curcuma mangga(C.mangga),and Zingiber cassumunar(Z.cassumunar). and study their antimicrobial activity.Methods:Essential oils obtained by steam distillation were analyzed by gas chromatography-mass speclrometry(GC-MS).The antimicrobial activity of the essential oils was evaluated against four bacteria:Bacillus cereus(H.cereus).Staphylococcus aureus(S.aureus).Escherichia coli(E.coli).and Pseudomonas aeruginosa(P.aeruginosa);and two fungi:Candida albicans(C.albicans) and Cyptococcus neoformans(C.neoformans),using disc-diffusion and broth microdilution methods.Results:Cycloisolongifolene,8.9-dehydro-9- formyl(35.29%) and dihydrocoslunolide(22.51%) were the major compounds in C.aeruginosa oil; whereas caryophyllene oxide(18.71%) and caryophyllene(12.69%) were the major compounds in C.mangga oil:and 2,6.9,9-tetramethyl-2.6.10-cycloundecatrien-1-one(60.77%) andα-caryophyllene(23.92%) were abundant in Z.cassumunar oil.The essential oils displayed varying degrees of antimicrobial activity against all lested microorganisms.C.mangga oil had the highest and most broad-spectrum activity by inhibiting all microorganisms tested,with C.neoformans being the most sensitive microorganism by having the lowest minimum inhibitory concentration(MIC) and minimum fungicidal concentration(MFC) values of 0.1μL/mL. | 12 | Outgrowth endothelial cells form a functional cerebral barrier and restore its integrity after damage显示文摘Breakdown of blood-brain barrier,formed mainly by brain microvascular endothelial cells(BMECs),represents the major cause of mortality during early phases of ischemic strokes.Hence,discovery of novel agents that can effectively replace dead or dying endothelial cells to restore blood-brain barrier integrity is of paramount importance in stroke medicine.Although endothelial progenitor cells(EPCs)represent one such agents,their rarity in peripheral blood severely limits their adequate isolation and therapeutic use for acute ischemic stroke which necessitate their ex vivo expansion and generate early EPCs and outgrowth endothelial cells(OECs)as a result.Functional analyses of these cells,in the present study,demonstrated that only OECs endocytosed DiI-labelled acetylated low-density lipoprotein and formed tubules on matrigel,prominent endothelial cell and angiogenesis markers,respectively.Further analyses by flow cytometry demonstrated that OECs expressed specific markers for sternness(CD34),immaturity(CD133)and endothelial cells(CD31)but not for hematopoietic cells(CD45).Like BMECs,OECs established an equally tight in vitro model of human BBB with astrocytes and pericytes,suggesting their capacity to form tight junctions.Ischemic injury mimicked by concurrent deprivation of oxygen and glucose(4 hours)or deprivation of oxygen and glucose followed by reperfusion(20 hours)affected both barrier integrity and function in a similar fashion as evidenced by decreases in transendothelial electrical resistance and increases in paracellular flux,respectively.Wound scratch assays comparing the vasculoreparative capacity of cells revealed that,compared to BMECs,OECs possessed a greater proliferative and directional migratory capacity.In a triple culture model of BBB established with astrocytes,pericytes and BMEC,exogenous addition of OECs effectively repaired the damage induced on endothelial layer in serum-free conditions.Taken together,these data demonstrate that OECs may effectively home to the site of vascular injury and repair the damage to maintain(neuro)vascular homeostasis during or after a cerebral ischemic injury. | Rais Reskiawan Abdulkadir Mansour Alwjwaj Othman Ahmad Othman Kamini Rakkar Ulvi Bayraktutan | 2020 | Neural Regeneration Research2020,15,6: | 5 |
| 13 | Neutrophil-lymphocyte ratio:A prognostic tool in patients with inhospital cardiac arrest显示文摘BACKGROUND In-hospital cardiac arrest(IHCA) portends a poor prognosis and survival to discharge rate. Prognostic markers such as interleukin-6, S-100 protein and high sensitivity C reactive protein have been studied as predictors of adverse outcomes after return of spontaneous circulation(ROSC); however; these variables are not routine laboratory tests and incur additional cost making them difficult to incorporate and less attractive in assessing patient's prognosis. The neutrophil-lymphocyte ratio(NLR) is a marker of adverse prognosis for many cardiovascular conditions and certain types of cancers and sepsis. We hypothesize that an elevated NLR is associated with poor outcomes including mortality at discharge in patients with IHCA.AIM To determine the prognostic significance of NLR in patients suffering IHCA who achieve ROSC.METHODS A retrospective study was performed on all patients who had IHCA with the advanced cardiac life support protocol administered in a large urban community United States hospital over a one-year period. Patients were divided into two groups based on their NLR value(NLR < 4.5 or NLR ≥ 4.5). This cutpoint was derived from receiving operator characteristic curve analysis(area under the curve = 0.66) and provided 73% positive predictive value, 82% sensitivity and42% specificity for predicting in-hospital death after IHCA. The primary outcome was death or discharge at 30 d, whichever came first.RESULTS We reviewed 153 patients with a mean age of 66.1 ± 16.3 years; 48% were female.In-hospital mortality occurred in 65%. The median NLR in survivors was 4.9(range 0.6-46.5) compared with 8.9(0.28-96) in non-survivors(P = 0.001). Animals in group 2(carnitine-supplemented group) were given L-carnitine(200 mg/kg per day)in drinking water for 8 wk.Animals in group 3(carnitine-depleted group)were given D-carnitine(200 mg/kg per day)and mildronate(200 mg/kg per day)in drinking water for 8 wk.Rats in group 4(DENA group)were injected with a single dose of DENA(200 mg/kg,i.p.)and 2 wk later received a single dose of carbon tetrachloride(2 mL/kg) by gavage as 1:1 dilution in corn oil.Animals in group 5(DENA-carnitine depleted group)received the same treatment as group 3 and group 4.Rats in group 6 (DENA-carnitine supplemented group)received the same treatment as group 2 and group 4.RESULTS:Administration of DENA resulted in a significant increase in alanine transaminase(ALT), gamma-glutamyl transferase(G-GT),alkaline phosphatase(ALP),total bilirubin,thiobarbituric acid reactive substances(TBARS)and total nitrate/ nitrite(NOx)and a significant decrease in reduced glutathione(GSH),glutathione peroxidase(GSHPx), catalase(CAT)and total carnitine content in liver tissues.In the carnitine-depleted rat model,DENA induced a dramatic increase in serum ALT,G-GT,ALP and total bilirubin,as well as a progressive reduction in total carnitine content in liver tissues.Interestingly, L-carnitine supplementation resulted in a complete reversal of the increase in liver enzymes,TBARS and NOx,and a decrease in total carnitine,GSH,GSHPx, and CAT induced by DENA,compared with the control values.Histopathological examination of liver tissues confirmed the biochemical data,where L-carnitine prevented DENA-induced hepatic carcinogenesis while D-carnitine-mildronate aggravated DENA-induced hepatic damage. | 15 | Corneal sensitivity,ocular surface health and tear film stability after punctal plug therapy of aqueous deficient dry eye显示文摘·AIM: To evaluate the effect of punctal occlusion using thermosensitive(smart plug) versus silicone plug for management of aqueous deficient dry eye on cornea sensitivity, ocular surface health and tear film stability.·METHODS: A comparative prospective interventiona case study included 45 patients with bilateral severe form of aqueous deficient dry eye. In each patient, the smar plug was inserted in the lower punctum of the right eye which was considered as study group 1 and silicone plug was inserted in the lower punctum of the left eye o the same patient which was considered as study group2. All patients were subjected to careful history taking and questionnaire for subjective assessment of severity of symptoms. Corneal sensitivity, corneal fluorescein rose bengal staining, Schirmer's I test, tear film break up time and conjunctival impression cytology were performed pre and 1, 3 and 6mo post plug insertion.·RESULTS: A statistically significant improvement in subjective and objective manifestations occurred following treatment with both types of plugs(P <0.01)The thermosensitive plug caused significant overal improvement, decrease in frequency of application o tear substitutes and improvement of conjunctiva impression cytology parameters in the inserted side(P <0.01). Canaliculitis was reported in two eyes(4.4% following punctal occlusion using thermosensitive plug(study group 1). Spontaneous plug loss occurred in 21eyes(46.6%) in the silicone plug group(study group 2).·CONCLUSION: Improvement of subjective and objective manifestations of aqueous deficient dry eye occurs following punctal plug occlusion. In this study, the dispersion issue was addressed by sonication and non-covalent surface functionalization of graphite nanoplatelets(GNPs) using two types of surfactant: anionic(sodium dodecyl benzene sulfate(SDBS)) and non-ionic polymeric(ethyl cellulose(EC)). After colloidal mixing with Al powder, consolidation was performed at two sintering temperatures(550 and 620°C). The structure, density, mechanical and wear properties of the nanocomposite samples were investigated and compared with a pure Al and a pure GNPs/Al nanocomposite sample. Noticeably, EC-based 0.5 wt% GNPs/Al samples showed the highest increment of 31% increase in hardness with reduced wear rate of 98.25% at 620°C, while a 22% increase in hardness with reduced wear rate of 96.98% at 550°C was observed, as compared to pure Al.
