| 2 | 口腔颌面部恶性肿瘤治疗指南显示文摘口腔颌面部恶性肿瘤约占全身恶性肿瘤的3%~5%,其组织病理学类型多样,以鳞状细胞癌最多见,约占90%以上。由于解剖部位特殊,口腔颌面部恶性肿瘤不仅影响外观,而且造成咀嚼、吞咽、呼吸和语音等功能障碍,严重降低患者的生存质量,并危及生命。手术、放疗和化疗是口腔颌面部恶性肿瘤的三大治疗手段,免疫治疗、生物治疗对某些类型的肿瘤和晚期患者是必要的补充和辅助措施。早期患者以手术治疗为主,晚期患者则提倡综合序列治疗,并鼓励患者参加临床试验。口腔颌面部恶性肿瘤患者的5年总存活率在65%左右,晚期患者的5年存活率不足30%。为了规范口腔颌面部恶性肿瘤的治疗,进一步提高患者的远期存活率,中华口腔医学会口腔颌面外科专业委员会肿瘤学组牵头,组织国内从事口腔颌面部恶性肿瘤治疗的知名专家,参考国内外文献,尤其是2009年美国国立综合癌症网络(NCCN)制订的指南中的相关内容,撰写了《口腔颌面部恶性肿瘤治疗指南》,希望对口腔颌面部恶性肿瘤的规范治疗起到指导作用。随着医学科学技术的发展,新的技术、方法、药物会不断出现,本指南将及时予以更新,以反映和纳入最新的研究成果,为广大患者提供最新的治疗方案。 | Division of Oral and Maxillofacial Oncology,Chinese Society of Oral and Maxillofacial Surgery | 2010 | 中国实用口腔科杂志2010，3，7： | 27 |
| 3 | Upregulation of vimentin and aberrant expression of E-cadherin /beta-catenin complex in oral squamous cell carcinomas:correlation with the clinicopathological features and patient outcome显示文摘Oral squamous cell carcinoma is a challenging oncology problem.A reliable biomarker for metastasis or high-risk prognosis in oral cancer patients remains undefined.Using quantitative immunohistochemistry,we examined the expression of vimentin,E-cadherin,and beta-catenin in 83 oral squamous cell carcinoma patients,and the relationships between the expression of these markers and specific clinicopathological features were analysed.The high expression of vimentin was observed in 23 of 43(53%) tumours from patients who eventually developed a recurrent tumour and was associated with recurrence and death(P < 0.001 and < 0.001,respectively).The decreased expression of E-cadherin was observed in 36 of 43(84%) tumours from patients who eventually developed a recurrent tumour and was also associated with recurrence and death(P < 0.001 and < 0.001,respectively).Although no correlation between beta-catenin expression in whole-tumour sections and clinicopathological features was observed,decreased beta-catenin expression at the tumour invasive front was closely associated with recurrence and death(P=0.002 and 0.002,respectively).The expression of vimentin and that of E-cadherin were associated with survival and were independent prognostic factors in univariate and multivariate analyses.Our data show that the overexpression of vimentin was closely associated with recurrence and death in oral squamous cell carcinoma patients.The combination of the upregulation of vimentin and aberrant expression of E-cadherin/beta-catenin complexes at the tumour invasive front may provide a useful prognostic marker in oral squamous cell carcinoma. | Liu,LK Jiang,XY Zhou,XX Wang,DM Song,XL Jiang,HB Nanjing Med Univ,Coll Stomatol,Dept Oral Pathol,Nanjing 210029,Jiangsu,Peoples R China Nanjing Med Univ,Inst Stomatol,Dept Oral Pathol,Nanjing 210029,Jiangsu,Peoples R China Nanjing Med Univ,Coll Stomatol,Dept Oral & Maxillofacial Surg,Nanjing 210029,Jiangsu,Peoples R China | 2010 | 南京医科大学学报(自然科学版)2010，30，5： | 25 |
| 4 | Long-term vision-threatening complications of phakic intraocular lens implantation for high myopia显示文摘AIM:To report the long-term vision-threatening complications in patients who underwent phakic intraocular lens(pIOLs)implantation for high myopia.METHODS:Thisstudywasdesignedfromaconsecutive series of phakic intraocular lens complication and corrective surgeries.Sixteen eyes of 13 patients had implantation of phakic intraocular lens for correction high myopia and developed serious complications have been included in this study.The mean age of patients was 38.6±6.35y(range 32-50y)and the mean time of history of pIOL implantation for high myopia was 6±2y(range 2-10y).Before corrective surgery,best spectaclecorrective visual acuity(BSCVA)ranged from perception to 20/200 in the eyes in which severe complications occurred.RESULTS:Corneal decompensation occurred in 12eyes of 9 high myopic patients after anterior chamber pIOL implantation.Rhegmatogenous retinal detachment(RRD)occurred in 4 eyes of 4 high myopic patients following anterior chamber and posterior chamber pIOL implantation.Patients with corneal decompensation,had combined procedures consisting of pIOL removal and penetrating keratoplasty(PKP).Removals of pIOL,phacoemulsification and pars plana vitrectomy(PPV)with silicone oil tamponade were performed in patients with RRD.After corrective surgeries,all patients but one(P+,patient 2,right eye)achieved moderate BSCVA ranged from 20/200 to 20/50 at the last visit.CONCLUSION:Phakic IOLs may be effective for thecorrection of high myopia.Although these IOLs may have severe complications and it affects safety and efficacy of this surgery.As seen here,corneal decompensation and rhegmatogenous retinal detachment are possible postoperative vision-threatening complications of phakic IOLs.Patients must be carefully examined before and after surgery for possible endothelial cell loss and vitreoretinal problems. | Isil Bahar Sayman Muslubas Baran Kandemir Ayse Yesim Aydin Oral Suleyman Kugu Metin Dastan | 2014 | International Journal of Ophthalmology(English edition)2014，7，2： | 14 |
| 5 | Pharmacokinetics of radioimmunotherapeutic agent of direct labeling mAb ^(188)Re-HAb18显示文摘AIM: To labed Anti-hepatoma monoclonal antibody (mAb)fragment HAb18 F (ab')2 was labeled with 188 Re for thepharmacokinetic model of 188 Re-HAb18 F (ab')2 and toevaluate its pharmacokinetic parameters in hepatoma-bearing nude mice.METHODS: HAb18 F(ab')2 was directly labeled with 188Reusing 2-mercaptoethanol (2-ME) as reducing agents.Labeling efficiency and immunoreactivity of 188 Re-HAb18 F( ab')2 were evaluated by Whatman 3MM paperchromatography and live cell assay, respectively.Biodiatribution analysis was also conducted in nude micebearing human hepatoma in which animals were sacrificed atdifferent time points(1, 4, 18, 24 and 24h) after 188Re-HAb18F(ab' )2 was injected through tail-vein into hepatoma-bearingnude mice. The blood and radioactivity of organs and masswere measured. The concentrations of 188 Re-HAb18 F(ab')2were evaluated with a pharmacokinetic 3P97 software.RESULTS: The optimum labeling efficiency andimmunoreactive fraction were 91.7% and 0.78%,respectively. The parameters of 188Re-HAb18 F(ab')2 were:T1/2, 2.29h; Vd, 1.49 × 10-9L@ Bq- 1; AUC, 20.49 × 109Bq@ h@L-1 ;CL, 0.45 × 10-3L@ h- 1. 188Re- HAb18 F(ab')2 could locatespecially in hepatoma with high selective reactivity of HAb18F(ab')2. 188 Re-HAb18 F(ab')2 was mainly eliminated bykidney. | 7 | Neoadjuvant hyperfractionated accelerated radiotherapy plus concomitant 5-fluorouracil infusion in locally advanced rectal cancer: A phase Ⅱ study显示文摘AIM To evaluate the efficacy and tolerability of neoadjuvant hyperfractionated accelerated radiotherapy(HART)and concurrent chemotherapy in patients with locally advanced infraperitoneal rectal cancer. METHODS A total of 30 patients with histopathologically confirmed T2-3/N0+ infraperitoneal adenocarcinoma of rectum cancer patients received preoperative 42 Gy/1.5 Gy/18 days/bid radiotherapy and continuous infusion of 5-fluorouracil(325 mg/m^2). All patients were operated 4-8 wk after neoadjuvant concomitant therapy. RESULTS In the early phase of treatment, 6 patients had grade Ⅲ-Ⅳ gastrointestinal toxicity, 2 patients had grade Ⅲ-Ⅳ hematologic toxicity, and 1 patient had grade Ⅴ toxicity due to postoperative sepsis during chemotherapy. Only 1 patient had radiotherapy-related late side effects, i.e., grade Ⅳ tenesmus. Complete pathological response was achieved in 6 patients(21%), while near-complete pathological response was obtained in 9(31%). After a median follow-up period of 60 mo, the local tumor control rate was 96.6%. In 13 patients, distant metastasis occurred. Disease-free survival rates at 2 and 5 years were 63.3% and 53%, and corresponding overall survival rates were 70% and 53.1%, respectively.CONCLUSION Although it has excellent local control and complete pathological response rates, neoadjuvant HART concurrent chemotherapy appears to not be a feasible treatment regimen in locally advanced rectal cancer, having high perioperative complication and intolerable side effects.
