| 1 | Current surgical strategies for total arthroplasty in valgus knee显示文摘The majority of orthopaedic surgeons even currentlyagree that primary total arthroplasty in valgus knees with a deformity of more than ten degrees may prove challenging. The unique sets of bone and soft tissue abnormalities that must be addressed at the time of the operation make accurate axis restoration, component orientation and joint stability attainment a difficult task. Understanding the specific pathologic anatomic changes associated with the valgus knee is a prerequisite so as to select the proper surgical method, to optimize component position and restore soft-tissue balance. The purpose of this article is to review the valgus knee anatomical variations, to assess the best pre-operative planning and to evaluate how to choose the grade of constraint of the implant. | 2 | Management of patients after recovering from acute severe biliary pancreatitis显示文摘Cholelithiasis is the most common cause of acute pancreatitis,accounting 35%-60% of cases. Around 15%-20% of patients suffer a severe attack with high morbidity and mortality rates. As far as treatment is concerned,the optimum method of late management of patients with severe acute biliary pancreatitis is still contentious and the main question is over the correct timing of every intervention. Patients after recovering from an acute episode of severe biliary pancreatitis can be offered alternative options in their management,including cholecystectomy,endoscopic retrograde cholangiopancreatography(ERCP) and sphincterotomy,or no definitive treatment. Delaying cholecystectomy until after resolution of the inflammatory process,usually not earlier than 6 wk after onset of acute pancreatitis,seems to be a safe policy. ERCP and sphincterotomy on index admission prevent recurrent episodes of pancreatitis until cholecystectomy is performed,but if used for definitive treatment,they can be a valuable tool for patients unfit for surgery. Some patients who survive severe biliary pancreatitis may develop pseudocysts or walled-off necrosis. Management of pseudocysts with minimally invasive techniques,if not therapeutic,can be used as a bridge to definitive operative treatment,which includes delayed cholecystectomy and concurrent pseudocyst drainage in some patients. | 3 | Statins and the risk of colorectal cancer: An updated systematic review and meta-analysis of 40 studies显示文摘AIM:To investigate the association between statin use and colorectal cancer risk,we conducted an updated meta-analysis of published studies.METHODS:We performed a comprehensive search for studies published up to July 2013.Eligible studies for this meta-analysis were either randomized controlled trials(RCTs)or observational studies(casecontrol or cohort)evaluating any exposure to statins and the risk of colorectal cancer.Two reviewers selected studies based on predefined inclusion criteria,and abstracted the data.Pooled relative risk(RR)estimates with their 95%CI were calculated using fixedand random-effects models.Then,we assessed the potential presence of publication bias and betweenstudies heterogeneity.To evaluate the results,we also performed a'leave-one-out'sensitivity analysis.RESULTS:A total of 40 studies,involving more than eight million subjects,contributed to the analysis.They were grouped on the basis of study design and,consequently,three separate meta-analyses were conducted.A similar modest reduction in the risk of colorectal cancer with statin use was observed,which was not statistically significant among RCTs(RR=0.89,95%CI:0.74-1.07;n=8),but reached statistical significance among cohort studies(RR=0.91,95%CI:0.83-1.00;n=13)and case-control studies(RR=0.92,95%CI:0.87-0.98;n=19).While we did not find significant evidence of selective outcome reporting or publication bias,substantial heterogeneity was detected,mainly among the observational studies.The sensitivity analysis confirmed the stability of our results.CONCLUSION:A modest reduction in risk of colorectal cancer among statin users cannot be disproved.Further targeted research is warranted.2014 Baishideng Publishing Group Co.,Limited.All | Theodore Lytras Georgios Nikolopoulos Stefanos Bonovas | 2014 | World Journal of Gastroenterology2014,20,7: | 7 |
| 4 | Investigation of proper modeling of very dense granular flows in the recirculation system of CFBs显示文摘The aim of this paper is the development of new models and/or the improvement of existing numerical models,used for simulating granular flow in CFB(circulating fluidized bed) recirculation systems.Most recent models follow the TFM(two-fluid model) methodology,but they cannot effectively simulate the inter-particle friction forces in the recirculation system,because the respective stress tensor does not incorporate compressibility of flow due to change of effective particle density.As a consequence,the induced normal and shear stresses are not modeled appropriately during the flow of the granular phase in the CFB recirculation system.The failure of conventional models,such as that of von Mises/Coulomb,is mainly caused by false approximation of the yield criterion which is not applicable to the CFB recirculation system.The present work adopts an alternative yield function,used for the first time in TFM Eulerian modeling.The proposed model is based on the Pitman-Schaeffer-Gray-Stiles yield criterion.Both the temporal deformation of the solid granular phase and the repose angle that the granular phase forms are more accurately simulated by this model.The numerical results of the proposed model agree well with experimental data,implying that frictional forces are efficiently simulated by the new model. | Aristeidis Nikolopoulos Nikos Nikolopoulos Nikos Varveris Sotirios Karellas Panagiotis Grammelis Emmanuel Kakaras | 2012 | Particuology2012,10,6: | 7 |
| 5 | Protective and defensive roles of non-glandular trichomes against multiple stresses:structure-function coordination显示文摘As superficial structures,non-glandular trichomes,protect plant organs against multiple biotic and abiotic stresses.The protective and defensive roles of these epidermal appendages are crucial to developing organs and can be attributed to the excellent combination of suitable structural traits and chemical reinforcement in the form of phenolic compounds,primarily fl avonoids.Both the formation of trichomes and the accumulation of phenolics are interrelated at the molecular level.During the early stages of development,non-glandular trichomes show strong morphological similarities to glandular ones such as the balloon-like apical cells with numerous phenolics.At later developmental stages,and during secondary wall thickening,phenolics are transferred to the cell walls of the trichomes.Due to the diff use deposition of phenolics in the cell walls,trichomes provide protection against UV-B radiation by behaving as optical fi lters,screening out wavelengths that could damage sensitive tissues.Protection from strong visible radiation is also aff orded by increased surface light refl ectance.Moreover,the mixtures of trichome phenolics represent a superfi-cial chemical barrier that provides protection against biotic stress factors such as herbivores and pathogens.Although the cells of some trichomes die at maturity,they can modulate their quantitative and qualitative characteristics during development,depending on the prevailing conditions of the external biotic or abiotic environment.In fact,the structure and chemical constituents of trichomes may change due to the particular light regime,herbivore damage,wounding,water stress,salinity and the presence of heavy metals.Hence,trichomes represent dynamic protective structures that may greatly aff ect the outcome of many plant–environment interactions. | George Karabourniotis Georgios Liakopoulos Dimosthenis Nikolopoulos Panagiota Bresta | 2020 | Journal of Forestry Research2020,31,1: | 5 |
| 6 | Vertebral fracture assessment:Current research status and application in patients with kyphoplasty显示文摘Imaging of the spine is of paramount importance for the recognition of osteoporotic vertebral fractures(VFs), and standard radiography(SR) of the spine is the suggested diagnostic method but is not routinely used because of the cost and radiation exposure considerations. VF assessment(VFA) is an efficient, low radiation method for identifying VFs at the time of bone mineral density(BMD) measurement. Prediction models used to indicate the need for VFA may have little predictive power in subspecialty referral populations such as rheumatologic patients or patients who underwent kyphoplasty. Rheumatologic patients are frequently at increased risk for VFs, and VFA should be performed on an individual basis, also taking in account the guidelines for the general population. Kyphoplasty is a new minimal invasive procedure for the treatment of VFs and is being performed with increasing frequency. Following kyphoplasty, there may be a risk of new VFs in adjacent vertebrae. The assessment and follow-up of patients who underwent kyphoplasty requires repetitive X-ray imaging with the known limitations of SR. Thus, VFA may facilitate the evaluation of VFs in these patients because most of the kyphoplasty patients would fulfill the criteria. In a pilot study, we measured the BMD and performed VFA in 28 patients treated with kyphoplasty. Ratios of anterior to posterior(A/P) and middle to posterior(M/P) height were measured, and Genant's method was used to classify vertebrae accordingly. Intraobserver and interobserver reliability for A/P, M/P and the Genant's method were determined. Only 1 patient did not meet the criteria for VFA. Of the 364 available vertebrae, 295 could be analyzed. Most missing data(concerning 69 vertebrae) occurred in the upper thoracic region. Three of the 69 non-eligible vertebrae were lumbar vertebrae with cement leakage from the kyphoplasty procedure. In our hands, VFA was highly reproducible, demonstrating very good agreement in terms of intraobserver and interobserver reliability. Agreement was very good onthe vertebral level, 'vertebrae with kyphoplasty' level and '2 above and 1 below the kyphoplasty vertebrae'level. The application of Genant's method to these patients also resulted in perfect agreement. We believe that the potential value of VFA in patients treated with kyphoplasty requires further evaluation, particularly comparing VFA with SR and performing a longitudinal follow-up.
