1Transjugular intrahepatic portosystemic shunt in cirrhosis: An exhaustive critical update显示文摘More than five decades after it was originally conceptualized as rescue therapy for patients with intractable variceal bleeding,the transjugular intrahepatic portosystemic shunt(TIPS)procedure continues to remain a focus of intense clinical and biomedical research.By the impressive reduction in portal pressure achieved by this intervention,coupled with its minimally invasive nature,TIPS has gained increasing acceptance in the treatment of complications of portal hypertension.The early years of TIPS were plagued by poor long-term patency of the stents and increased incidence of hepatic encephalopathy.Moreover,the diversion of portal flow after placement of TIPS often resulted in derangement of hepatic functions,which was occasionally severe.While the incidence of shunt dysfunction has markedly reduced with the advent of covered stents,hepatic encephalopathy and instances of early liver failure continue to remain a significant issue after TIPS.It has emerged over the years that careful selection of patients and diligent post-procedural care is of paramount importance to optimize the outcome after TIPS.The past twenty years have seen multiple studies redefining the role of TIPS in the management of variceal bleeding and refractory ascites while exploring its application in other complications of cirrhosis like hepatic hydrothorax,portal hypertensive gastropathy,ectopic varices,hepatorenal and hepatopulmonary syndromes,non-tumoral portal vein thrombosis and chylous ascites.It has also been utilized to good effect before extrahepatic abdominal surgery to reduce perioperative morbidity and mortality.The current article aims to review the updated literature on the status of TIPS in the management of patients with liver cirrhosis.Sasidharan Rajesh Tom George Cyriac Abby Philips Rizwan Ahamed Sandeep Kumbar Narain Mohan Meera Mohanan Philip Augustine 2020World Journal of Gastroenterology2020,26,37:26
2Genotype-Phenotype Correlations in Ornithine Transcarbamylase Deficiency: A Mutation Update显示文摘Ornithine transcarbamylase(OTC) deficiency is an X-linked trait that accounts for nearly half of all inherited disorders of the urea cycle.OTC is one of the enzymes common to both the urea cycle and the bacterial arginine biosynthesis pathway; however, the role of OTC has changed over evolution. For animals with a urea cycle, defects in OTC can trigger hyperammonemic episodes that can lead to brain damage and death. This is the fifth mutation update for human OTC with previous updates reported in 1993, 1995, 2002, and 2006. In the2006 update, 341 mutations were reported. This current update contains 417 disease-causing mutations, and also is the first report of this series to incorporate information about natural variation of the OTC gene in the general population through examination of publicly available genomic data and examination of phenotype/genotype correlations from patients participating in the Urea Cycle Disorders Consortium Longitudinal Study and the first to evaluate the suitability of systematic computational approaches to predict severity of disease associated with different types of OTC mutations.Ljubica Caldovic Iman Abdikarim Sahas Narain Mendel Tuchman Hiroki Morizono 2015Journal of Genetics and Genomics2015,42,5:19
3Radiation exposure and reduction in the operating room: Perspectives and future directions in spine surgery显示文摘Intraoperative imaging is vital for accurate placement of instrumentation in spine surgery. However, the use of biplanar fluoroscopy and other intraoperative imaging modalities is associated with the risk of significant radiation exposure in the patient, surgeon, and surgical staff. Radiation exposure in the form of ionizing radiation can lead to cellular damage via the induction of DNA lesions and the production of reactive oxygen species. These effects often result in cell death or genomic instability, leading to various radiation-associated pathologies including an increased risk of malignancy. In attempts to reduce radiation-associated health risks, radiation safety has become an important topic in the medical field. All practitioners, regardless of practice setting, can practice radiation safety techniques including shielding and distance to reduce radiation exposure. Additionally, optimization of fluoroscopic settings and techniques can be used as an effective method of radiation dose reduction. New imaging modalities and spinal navigation systems have also been developed in an effort to replace conventional fluoroscopy and reduce radiation doses. These modalities include Isocentric Three-Dimensional C-Arms, O-Arms, and intraoperative magnetic resonance imaging. 4Autochthonous halotolerant plant growth-promoting rhizobacteria promote bacoside A yield of Bacopa monnieri(L.)Nash and phytoextraction of salt-affected soil显示文摘Phytoremediation is a promising approach for reclamation of salt-affected soil.Phytoextraction is the most commonly used process,which exploits plants to absorb,immobilize,and accumulate salt in their shoots.In this study,halotolerant plant growth-promoting rhizobacteria(PGPR)were isolated from the rhizosphere of wild grasses growing naturally in salt-affected areas of Lucknow,Uttar Pradesh(India)and were tested for their efficacies of salt-tolerance and plant growth-promoting(PGP)abilities.Based on 16S rRNA sequences,the most efficient halotolerant isolates possessing PGP traits were identified as Pseudomonas plecoglossicida(KM 233646),Acinetobacter calcoaceticus(KM 233647),Bacillus flexus(KM 233648),and Bacillus safensis(KM 233652).Application of these isolates as bio-inoculants significantly(P<0.05)increased the growth and bacoside A yield of a medicinal plant,Bacopa monnieri(L.)Nash,grown on natural salt-affected soil.The phytoremediation of salt-affected soil was evident by the substantial increase in shoot Na^+:K^+ratio of bio-inoculant-treated plants.When compared to un-inoculated control plants,the soil physico-chemical properties of bio-inoculant-treated plants were improved.The shoot and root biomass(fresh and dry weights),soil enzymes,and soil nutrient parameters showed significant positive correlations with the shoot Na+:K+ratio.Consequently,the halotolerant PGPR screened in this study could be useful for the reclamation of saline soils concomitant with improved plant growth and bacoside A yield.Umesh PANKAJ Durgesh Narain SINGH Pooja MISHRA Pooja GAUR C.S.Vivek BABU Karuna SHANKER Rajesh Kumar VERMA 2020Pedosphere2020,30,5:9

6DG指数在定量多样性时的缺陷及其内涵解析显示文摘生物多样性通常使用物种丰富度、Simpson指数、Shannon-Wiener多样性指数等来进行度量,但是在土壤动物群落研究中,由于使用了粗水平的分类方法,因此即使生境变化很大,这些多样性指数在评估群落多样性变化时仍然是不适当的。为了克服这种限制,廖崇惠(1990,2009)提出用DG指数来代替这些标准的多样性指数,并在土壤动物生态学领域得到了广泛应用。然而笔者分析发现DG指数与Pielou均匀度指数呈显著的负相关关系(r=–0.534,P=0.000),即,高的均匀度反而有低的多样性。另外,DG指数与类群数(r=0.648,P=0.000)和类群密度(r=0.487,P=0.000)呈明显的正相关,类群数的下降可以通过部分类群密度的上升而获得补偿,群落的类群丢失后却可以获得一个不变的甚至更高的多样性值。因此,笔者不支持DG指数用于测度生物多样性,提议使用各类群实际群势与潜在群势比值的平均值来估计群落潜在多度的实现程度。如果继续使用DG指数作为实际生境条件的一个指标,那么与以往不同,DG指数测度的是该生境群落多度增长的一种潜力。颜绍馗 Anand Narain Singh 邱红兵 张伟东 汪思龙 崔洋 2009生物多样性2009,17,5:8

8Effect of sample size on the fluid flow through a single fractured granitoid显示文摘Most of deep geological engineered structures,such as rock caverns,nuclear waste disposal repositories,metro rail tunnels,multi-layer underground parking,are constructed within hard crystalline rocks because of their high quality and low matrix permeability.In such rocks,fluid flows mainly through fractures.Quantification of fractures along with the behavior of the fluid flow through them,at different scales,becomes quite important.Earlier studies have revealed the influence of sample size on the confining stress-permeability relationship and it has been demonstrated that permeability of the fractured rock mass decreases with an increase in sample size.However,most of the researchers have employed numerical simulations to model fluid flow through the fracture/fracture network,or laboratory investigations on intact rock samples with diameter ranging between 38 mm and 45 cm and the diameter-to-length ratio of 1:2 using different experimental methods.Also,the confining stress,σ_3,has been considered to be less than 30 MPa and the effect of fracture roughness has been ignored.In the present study,an extension of the previous studies on 'laboratory simulation of flow through single fractured granite' was conducted,in which consistent fluid flow experiments were performed on cylindrical samples of granitoids of two different sizes(38 mm and 54 mm in diameters),containing a 'rough walled single fracture'.These experiments were performed under varied confining pressure(σ_3=5-40 MPa),fluid pressure(f_p ≤25 MPa),and fracture roughness.The results indicate that a nonlinear relationship exists between the discharge,Q,and the effective confining pressure,σ_(eff),and Q decreases with an increase in σ_(eff).Also,the effects of sample size and fracture roughness do not persist when σ_(eff)≥ 20 MPa.It is expected that such a study will be quite useful in correlating and extrapolating the laboratory scale investigations to in-situ scale and further improving theoretical/numerical models associated with fluid flow through rock masses.Kunal Kumar Singh Devendra Narain Singh Ranjith Pathegama Gamage 2016Journal of Rock Mechanics and Geotechnical Engineering2016,8,3:5
