2Role of cardiac magnetic resonance imaging in the diagnosis and management of COVID-19 related myocarditis: Clinical and imaging considerations显示文摘There is a growing evidence of cardiovascular complications in coronavirusdisease 2019 (COVID-19) patients. As evidence accumulated of COVID-19 mediatedinflammatory effects on the myocardium, substantial attention has beendirected towards cardiovascular imaging modalities that facilitate this diagnosis.Cardiac magnetic resonance imaging (CMRI) is the gold standard for thedetection of structural and functional myocardial alterations and its role inidentifying patients with COVID-19 mediated cardiac injury is growing. Despiteits utility in the diagnosis of myocardial injury in this population, CMRI’s impacton patient management is still evolving. This review provides a framework for theuse of CMRI in diagnosis and management of COVID-19 patients from theperspective of a cardiologist. We review the role of CMRI in the management ofboth the acutely and remotely COVID-19 infected patient. 3Endoscopic sinus surgery and aspirin desensitization improve otologic-specific SNOT-22 scores显示文摘Objective:There are few studies evaluating the impact of Aspirin-exacerbated respiratory disease(AERD)treatment on otologic symptoms.The aim of this study is to evaluate the effects of endoscopic sinus surgery(ESS)and aspirin desensitization(AD)on otologic symptoms in subjects with AERD.Methods:Retrospective chart review of adult patients diagnosed with AERD at our tertiary Care Academic Medical Center-Otorhinolaryngology Department.Charts of adult patients diagnosed with AERD who underwent ESS and ASA desensitization at our institution’s AERD Center from 2016 to 2019 were reviewed.Sino-Nasal Outcomes Test 22-item survey(SNOT-22)scores were evaluated for patients at various time points including:pre-surgery,post-surgery/pre-aspirin desensitization,and various times post-desensitization up to>12 months.Within the SNOT-22,otologic-specific subdomain scores were evaluated at similar time points.Patients on immunomodulatory medications other than corticosteroids were excluded from analysis.Results:SNOT-22 scores were analyzed for 121 patients.There was a significant improvement in overall SNOT scores from pre-surgery(44.62)to post surgery/pre-desensitization(23.34)(P<0.0005).Similarly,SNOT-22 otologic-specific scores also improved after surgery prior to desensitization(3.19-2.04)(P=0.005).Following AD,the improvement in the overall SNOT-22 continued to improve for up to 12 months(P<0.005).While the otologic-specific SNOT-22 scores remained stable after surgery and ASA desensitization.Conclusion:ESS and AD reduce otologic-specific SNOT-22 scores and parallel trends in overall SNOT-22 scores.The effect of treatment is durable over the course of 12 months.Future work should aim to correlate otologic SNOT-22 scores with objective otologic data.James G.Naples Andrew Corr Siddhant Tripathi Morgan Berman Jason A.Brant Michael J.Ruckenstein John Bosso 2020World Journal of Otorhinolaryngology-Head and Neck Surgery2020,6,4:1
4COVID-19 vaccine-associated myocarditis显示文摘Myocarditis is now recognized as a rare complication of coronavirus disease 2019(COVID-19)mRNA vaccination,particularly in adolescent and young adult males.Since the authorization of the Pfizer-BioNTech^(TM)and Moderna^(TM)mRNA vaccines targeting the severe acute respiratory syndrome coronavirus-2(SARSCoV-2)spike protein,the Centers for Disease Control and Prevention(CDC)has reported 1175 confirmed cases of myocarditis after COVID-19 vaccination in individuals ages 30 years and younger as of January 2022.According to CDC data in June 2021,the incidence of vaccine-mediated myocarditis in males ages 12-29 years old was estimated to be 40.6 cases per million second doses of COVID-19 mRNA vaccination administered.Individuals with cases of COVID-19 vaccinemediated myocarditis typically present with acute chest pain and elevated serum troponin levels,often within one week of receiving the second dose of mRNA COVID-19 vaccination.Most cases follow a benign clinical course with prompt resolution of symptoms.Proposed mechanisms of COVID-19 vaccine myocarditis include molecular mimicry between SARS-CoV-2 spike protein and self-antigens and the triggering of preexisting dysregulated immune pathways in predisposed individuals.The higher incidence of COVID-19 vaccine myocarditis in young males may be explained by testosterone and its role in modulating the immune response in myocarditis.There is limited data on long-term outcomes in these cases given the recency of their occurrence.The CDC continues to recommend COVID-19 vaccination for everyone 5 years of age and older given the greater risk of serious complications related to natural COVID-19 infection including hospitalization,multisystem organ dysfunction,and death.Further study is needed to better understand the immunopathology and long-term outcomes behind COVID-19 mRNA vaccine-mediated myocarditis.Michael C Morgan Lavannya Atri Sean Harrell Wael Al-Jaroudi Adam Berman 2022World Journal of Cardiology2022,14,7:1
