2他汀、高密度脂蛋白胆固醇与冠状动脉粥样硬化消退显示文摘背景:他汀类药物可以降低低密度脂蛋白胆固醇(low—density lipoprotein cholesterol,LDL—C)水平并减缓冠状动脉粥样硬化的进展。然而,对于他汀类药物所致高密度脂蛋白胆固醇(high—density lipoprotein cholesterol,HDL—C)变化与疾病进展的关系,目前仍无相关数据描述。目的:了解LDL—C和HDL—C水平变化与动脉粥样硬化的相互关系。设计、地点及患者:对4项前瞻性随机试验(1999~2005年在美国、北美、欧洲和澳大利亚进行)的原始数据进行事后分析。其中1455例经血管造影证实的冠心病患者在接受他汀治疗(18个月或24个月)时进行了系列血管内超声扫描。所有超声分析均在相同的核心实验室中进行。主要观测指标:脂蛋白水平变化与冠状动脉粥样斑块体积的相互关系。结果:他汀治疗期间,LDL—C平均(SD)水平从124.0(38.3)mg,/dL(3.2[0.99]mmol/L)降至87.5(28.8)mg,/dL(2.3[0.75]mmol/L)(下降23.5%;P〈0.001),HDL—C水平从42.5(11.0)mg/dL(1.1[0.28]mmol/L)升至45.1(11.4)mg/dL(1.2[0.29]mmol/L)(上升7.5%;P〈0.001)。LDL—C与HDL—C比值从平均(SD)3.0(1.1)降至2.1(0.9)(下降26.7%;P〈0.001)。这些变化同时伴随平均(SD)百分粥样斑块体积的增加(从39.7%[9.8%]增至40.1%[9.7%])(增加0.5%[3.9%];P=0.001)以及平均(SD)总粥样斑块体积的减小(2.4[23.6]mm^3;P〈0.001)。在单变量分析中,LDL—C、总胆固醇、非HDL胆固醇、载脂蛋白B以及载脂蛋白B与载脂蛋白A-1比值平均水平和治疗引起的变化与动脉粥样硬化进展速率显著相关,而治疗所致HDL—C变化则与动脉粥样斑块体积负相关。在多变量分析中,平均LDL—C水平(β系数,0.11[95%可信区间,0.07~0.15])以及HDL—C升高(β系数,-0.26[95%可信区间,-0.41--0.10])依然为动脉粥样硬化消退的独立预测因子。动脉硬化显著消退(动脉粥样斑块体积减少≥5%)见于治疗期间LDL—C水平低于均值(87.5mg/dL)和HDL—C升高百分比大于均值(7.5%;P〈0.001)的患者。临床事件的发生未见显著差异。结论:在LDL—C显著降低以及HDL上升〉7.5%时,他汀治疗与冠状动脉粥样硬化消退相关。这些结果提示他汀治疗的获益源自致动脉粥样硬化脂蛋白的降低以及HDL.C的升高。尽管血脂水平的这些变化与动脉粥样硬化消退相关,但是后者是否可以转化为临床事件的显著减少,并改善临床预后仍有待进一步研究确定。Stephen J. 3Endoscopic ultrasound evaluation in the surgical treatment of duodenal and peri-ampullary adenomas显示文摘AIM:To investigate endoscopic ultrasound(EUS) for predicting depth of mucosal invasion and to analyze outcomes following endoscopic and transduodenal resection.METHODS:Records of 111 patients seen at our institution from November 1999 to July 2011 with the postoperative pathological diagnosis of benign ampullary and duodenal adenomas were reviewed.Records of patients who underwent preoperative EUS for diagnostic purposes were identified.The accuracy of EUS in predicting the absence of muscular invasion was assessed by comparing EUS reports to the final surgical pathological results.In addition,the incidence of the post-operative complications over a period of 30 d and the subsequent long-term outcome(recurrence) over a period of 30 mo associated with endoscopic and transduodenal surgical resection was recorded,compared and analyzed.RESULTS:Among 111 patients with benign ampullary and duodenal adenomas,47 underwent preoperative EUS for 29 peri-ampullary lesions and 18 duodenal lesions.In addition,computed tomography was performed in 18 patients,endoscopic retrograde cholangio-pancreatography in 10 patients and esophagogastroduodenoscopy in 22 patients.There were 43 patients with sporadic adenomas and 4 patients with familial adenomatous polyposis(FAP)/other polyposis syndromes.In 38(81%,P < 0.05) patients,EUS reliably identified absence of submucosal and muscularis invasion.In 4 cases,EUS underestimated submucosal invasion that was proven by pathology.In the other 5 patients,EUS predicted muscularis invasion which could not be demonstrated in the resected specimen.EUS predicted tumor muscularis invasion with a specificity of 88% and negative predictive value of 90%(P < 0.05).Types of resection performed included endoscopic resection in 22 cases,partial duodenectomy in 9 cases,transduodenal ampullectomy with sphincteroplasty in 10 cases and pancreaticoduodenectomy in 6 cases.The main post-operative final pathological results included villous adenoma(n = 5),adenoma(n = 8),tubulovillous adenoma(n = 10),tubular adenoma(n = 20) and hyperplastic polyp(n = 2).Among the 47 patients who underwent resection,8(17%,5 of which corresponded to surgical resection) developed post-procedural complications which included retroperitoneal hematoma,intra-abdominal abscess,wound infection,delayed gastric emptying and prolonged ileus.After median followup of 20 mo there were 6 local recurrences(13%,median follow-up = 20 mo) 4 of which were in patients with FAP.CONCLUSION:EUS accurately predicts the depth of mucosal invasion in suspected benign ampullary and duodenal adenomas.These patients can safely undergo endoscopic or local resection.Lilian C Azih Brett L Broussard Milind A Phadnis Martin J Heslin Mohamad A Eloubeidi Shayam Varadarajulu Juan Pablo Arnoletti 2013World Journal of Gastroenterology2013,19,4:13
4CT平扫预测高血压脑出血早期血肿扩大的价值显示文摘目的探讨CT平扫征象预测高血压脑出血(ICH)患者血肿扩大的价值。方法入组80例发病6 h以内的ICH患者,入院行头颅CT扫描,由两位影像科医师评估血肿内“黑洞征”和“卫星征”。根据24 h CT复查结果,分为血肿扩大组和血肿未扩大组。ROC曲线比较“黑洞征”、“卫星征”及其联合征象对血肿扩大的预测能力。结果“黑洞征”、“卫星征”和联合征象在血肿扩大组与血肿未扩大组之间差异有统计学意义(P均<0.05)。联合征象预测早期血肿扩大的敏感性、特异性、阳性预测值、阴性预测值及准确性分别为84.09%、63.89%、74.00%、76.67%和75.30%,其预测的准确性高于“黑洞征”(66.00%)和“卫星征”(69.70%)。结论头颅CT平扫联合征象较单一征象更能预测早期血肿扩大。傅璠 Shakya Milind Ratna 於帆 张苗 孙胜军 卢洁 2020医学影像学杂志2020,30,11:11
5Application of artificial intelligence to forecast hydrocarbon production from shales显示文摘Artificial intelligence(AI)methods and applications have recently gained a great deal of attention in many areas,including fields of mathematics,neuroscience,economics,engineering,linguistics,gaming,and many others.This is due to the surge of innovative and sophisticated AI techniques applications to highly complex problems as well as the powerful new developments in high speed computing.Various applications of AI in everyday life include machine learning,pattern recognition,robotics,data processing and analysis,etc.The oil and gas industry is not behind either,in fact,AI techniques have recently been applied to estimate PVT properties,optimize production,predict recoverable hydrocarbons,optimize well placement using pattern recognition,optimize hydraulic fracture design,and to aid in reservoir characterization efforts.In this study,three different AI models are trained and used to forecast hydrocarbon production from hydraulically fractured wells.Two vastly used artificial intelligence methods,namely the Least Square Support Vector Machine(LSSVM)and the Artificial Neural Networks(ANN),are compared to a traditional curve fitting method known as Response Surface Model(RSM)using second order polynomial equations to determine production from shales.The objective of this work is to further explore the potential of AI in the oil and gas industry.Eight parameters are considered as input factors to build the model:reservoir permeability,initial dissolved gas-oil ratio,rock compressibility,gas relative permeability,slope of gas oil ratio,initial reservoir pressure,flowing bottom hole pressure,and hydraulic fracture spacing.The range of values used for these parameters resemble real field scenarios from prolific shale plays such as the Eagle Ford,Bakken,and the Niobrara in the United States.Production data consists of oil recovery factor and produced gas-oil ratio(GOR)generated from a generic hydraulically fractured reservoir model using a commercial simulator.The Box-Behnken experiment design was used to minimize the number of simulations for this study.Five time-based models(for production periods of 90 days,1 year,5 years,10 years,and 15 years)and one rate-based model(when oil rate drops to 5 bbl/day/fracture)were considered.Particle Swarm Optimization(PSO)routine is used in all three surrogate models to obtain the associated model parameters.Models were trained using 80%of all data generated through simulation while 20%was used for testing of the models.All models were evaluated by measuring the goodness of fit through the coefficient of determination(R2)and the Normalized Root Mean Square Error(NRMSE).Results show that RSM and LSSVM have very accurate oil recovery forecasting capabilities while LSSVM shows the best performance for complex GOR behavior.Furthermore,all surrogate models are shown to serve as reliable proxy reservoir models useful for fast fluid recovery forecasts and sensitivity analyses.Palash Panja Raul Velasco Manas Pathak Milind Deo 2018Petroleum2018,4,1:4
6Natural history of Barrett's esophagus显示文摘The natural history of Barrett's esophagus (BE) is difficult to quantify because,by definition,it should describe the course of the condition if left untreated.Pragmatically,we assume that patients with BE will receive symptomatic treatment with acid suppression,usually a proton pump inhibitor,to treat their heartburn.This paper describes the development of complications of stricture,ulcer,dysplasia and adenocarcinoma from this standpoint.Controversies over the definition of BE and its implications in clinical practice are presented.The presence of intestinal metaplasia and its relevance to cancer risk is discussed,and the need to measure the extent of the Barrett's epithelium (long and short segments) using the Prague guidelines is emphasized.Guidelines and international consensus over the diagnosis and management of BE are being regularly updated.The need for expert consensus is important due to the lack of randomized trials in this area.After searching the literature,we have tried to collate the important studies regarding progression of Barrett's to dysplasia and adenocarcinoma.No therapeutic studies yet reported show a clear reduction in the development of cancer in BE.The effect of pharmacological and surgical intervention on the natural history of Barrett's is a subject of ongoing research,including the Barrett's Oesophagus Surveillance Study and the aspirin and esomeprazole cancer chemoprevention trial with interesting results.The geographical variation and the wide range of outcomes highlight the difficulty of providing an individualized risk profile to patients with BE.Future studies on the interaction of genome wide abnormalities in Barrett's and their interaction with environmental factors may allow individualization of the risk of cancer developing in BE.Rao Milind Stephen E Attwood 2012World Journal of Gastroenterology2012,18,27:3

8Graphene-wrapped Ag3PO4/LaCO3OH heterostructures for water purification under visible light显示文摘We demonstrated a unique synthesis approach of graphene(GR)-wrapped Ag_3PO_4/LaCO_3OH(APO/LCO)heterostructures by an in-situ wet chemical method. FESEM analysis reveals the formation of rhombic dodecahedrons of APO decorated with LCO and later wrapped with GR flakes. Optical studies shows two absorption edges corresponding to the band gap energies of APO(2.41 eV) and LCO(4.1 eV). Considering the absorption edge of the heterostructures in the visible region, the photocatalytic activities of photocatalysts containing different APO/LCO mass ratios were evaluated by the degradation of MB. GR-decorated composite with 20% LCO(APO/LCO20/GR) exhibited the highest photocatalytic activity for MB degradation, with a rate constant, k of 0.541 min^(-1). The photocatalytic activity of APO/LCO20/GR more greatly enhanced than those of the individual constituents(APO, LCO, APO/LCO20). The enhanced photocatalytic activity of the heterostructure can be attributed to the co-catalytic effect of LCO as well as intriguing physicochemical properties of GR. To understand the enhanced photocatalytic activity of the heterostructures the photocatalytic reaction mechanism is proposed in detail. 10MEMS-in-the-lens architecture for a miniature high-NA laser scanning microscope显示文摘Laser scanning microscopes can be miniaturized for in vivo imaging by substituting optical microelectromechanical system(MEMS)devices in place of larger components.The emergence of multifunctional active optical devices can support further miniaturization beyond direct component replacement because those active devices enable diffraction-limited performance using simpler optical system designs.In this paper,we propose a catadioptric microscope objective lens that features an integrated MEMS device for performing biaxial scanning,axial focus adjustment,and control of spherical aberration.The MEMS-in-the-lens architecture incorporates a reflective MEMS scanner between a low-numerical-aperture back lens group and an aplanatic hyperhemisphere front refractive element to support high-numerical-aperture imaging.We implemented this new optical system using a recently developed hybrid polymer/silicon MEMS three-dimensional scan mirror that features an annular aperture that allows it to be coaxially aligned within the objective lens without the need for a beam splitter.The optical performance of the active catadioptric system is simulated and imaging of hard targets and human cheek cells is demonstrated with a confocal microscope that is based on the new objective lens design.Tianbo Liu Milind Rajadhyaksha David L.Dickensheets 2019Light(Science & Applications)2019,8,1:2
